Simple Summary: Immune checkpoint inhibitors are standard treatment for patients with metastatic melanoma, and treatment response is most optimally evaluated with FDG-PET/CT. Pseudoprogression describes the difficulty of distinguishing between real cancer growth and growth occurring due to active infiltrating immune cells that mistakenly look like tumor growth on scans. The aim of our study was to investigate whether dual-time point (DTP) FDG-PET/CT scan and modified response criteria (PERCIMT) could be helpful in detecting pseudoprogression. Our findings suggest limited efficacy of DTP FDG-PET/CT whereas PERCIMT criteria could be included in the clinical decision making ensuring correct treatment choices. The activity of immune checkpoint inhibitors (ICIs) in patients with metastatic melanoma is often monitored using fluorine-18-fluorodeoxyglucose-positron emission tomography/computed tomography (FDG-PET/CT) scans. However, distinguishing disease progression (PD) from pseudoprogression (PsPD), where increased FDG uptake might reflect immune cell activity rather than tumor growth, remains a challenge. This prospective study compared the efficacy of dual-time point (DTP) FDG-PET/CT with modified response criteria (PERCIMT) in differentiating PsPD from PD. From July 2017–January 2021, 41 patients suspected to have PsPD on an evaluation scan were prospectively included (29 evaluable). A subsequent DTP FDG-PET/CT scan was conducted within 14 days, followed by a confirmatory FDG-PET/CT scan. Additionally, PERCIMT were applied. DTP FDG-PET/CT identified 24% with PsPD and 76% with PD. Applying PERCIMT criteria, 69% showed PsPD, while 31% had PD. On follow-up, 10 patients (34%) demonstrated confirmed PsPD, while 19 (66%) exhibited PD. The sensitivity and specificity of DTP FDG-PET/CT were 20% and 74%, respectively, and for PERCIMT this was 80% and 37%, respectively. Our findings suggest limited efficacy of DTP FDG-PET/CT in distinguishing PsPD from PD in ICI-treated patients with metastatic melanoma. The use of PERCIMT could complement clinical assessment and be incorporated in multidisciplinary team conferences for enhanced decision-making. [ABSTRACT FROM AUTHOR]

8. A New Method for Measuring Free-Ranging Fish Swimming Speed in Commercial Marine Farms Using Doppler Principle.

- Abstract
A novel Doppler shift based technique for measurement of free-swimming fish speed in marine farms using acoustic telemetry tags was developed and evaluated in this study. The proposed method can potentially augment current telemetry systems with a new biologically relevant measurement without significantly changing the size and energy constrained tag-side of the telemetry systems. For speeds in the range of 20cm s−1-110cm s−1 an overall relative rms error of less than 10% in measured speed based on the proposed Doppler method was achieved in the tests conducted at a fully stocked commercial fish cage, with an rms error of 7.85cm s−1 (std. dev. 7.5cm s−1). The study thus demonstrates the feasibility of measuring the swimming speeds of individual free-ranging fish using this method. [ABSTRACT FROM AUTHOR]

Adoptive cell therapy (ACT) with autologous tumor-infiltrating lymphocytes (TILs) can induce durable complete tumor regression in patients with advanced melanoma. Efforts are currently underway to expand this treatment modality to other cancer types. In the microenvironment of ovarian cancer, the engagement of co-inhibitory immune checkpoint molecules such as CTLA-4 can lead to the inactivation of TILs. Thus, approaches that directly manipulate co-inhibitory pathways within the tumor microenvironment might improve the expansion of tumor-reactive TILs. The initial expansion of TILs for ACT from tumor fragments provides a window of opportunity to manipulate an intact tumor microenvironment and improve CD8
+ T-cell output and TIL tumor reactivity. To exploit this, we used a CTLA-4-blocking antibody, added during the initial TIL culture, and found that the blockade of CTLA-4 favored the propagation of CD8+ TILs from ovarian tumor fragments. Interestingly, adding the CTLA-4 blocking antibody in the initial phase of the TIL culture resulted in more potent anti-tumor TILs in comparison to standard TIL cultures. This phenotype was preserved during the rapid expansion phase. Thus, targeting CTLA-4 within the intact tumor microenvironment of tumor fragments enriches tumor-reactive TILs and may improve clinical outcome of TIL-based ACT in ovarian cancer. Chemokines are essential mediators of cellular trafficking, interactions and tumor development. Though adoptive cell therapy (ACT) has been a tremendous success in the treatment of metastatic melanoma (MM), a major obstacle for successful ACT, is limited homing of effector T cells to immune suppressive tumor sites. We hypothesized that equipping T cells with chemokine receptors matching the chemokines of the tumor microenvironment, could improve tumor homing of T cells. T cells from malignant ascites (n D 13);blood from ovarian cancer (OC) patients (n d 14);and healthy donors (n d 13) were analyzed by flow cytometry. We found that FoxP3C regulatory T cells accumulation in patients with OC associates with CCR4 expression. We characterized a chemokine profile of ascites chemokines, and expression of corresponding receptors on circulating T cells and tumor ascites lymphocytes (TALs). CCL22, CXCL9, CXCL10 and CXCL12 associated with enrichment of CCR4c, CCR5c, CXCR3c and CXCR4c T cells in ascites. Circulating T cells and TALs however did not express CXCR2, identifying CXCR2 as candidate for chemokine receptor transduction. TALs readily expressed IFNg and TNFa upon stimulation despite the frequency decreasing with in vitro expansion. Lentiviral transduction of TALs (n d 4) with chemokine receptor CXCR2 significantly increased transwell migration of TALs towards rhIL8 and autologous ascites. The majority of expanded and transduced TALs were of a T effector memory subtype. This proof of concept study shows that chemokine receptor engineering with CXCR2 is feasible and improves homing of transduced TALs towards the OC microenvironment. [ABSTRACT FROM AUTHOR]

Patients with preexisting active autoimmune disorders were excluded from clinical trials of immune checkpoint inhibitors. However, patients with autoimmune disorders are diagnosed with cancer at least as frequently as the global population, and clinicians treating patients outside clinical trials have generally been reluctant to offer cancer immunotherapy to this patient group. In this brief article, we review the most recent literature on the efficacy and safety of CTLA-4- and PD-1-blocking antibodies in patients with preexisting autoimmune disorders. Simple Summary: Immunotherapy has been a successful treatment for many cancers. However, no immunotherapy treatment has been approved for ovarian cancer due to low efficacy in this patient group. This study investigated the cellular and molecular changes from primary ovarian tumors, at the time of diagnosis, to recurrence, where the disease returns after surgery and chemotherapies. Here we examined the immune contexture to better understand subdued responses to immunotherapy and identify additional, potentially complimentary, therapeutic targets. Indications of the development of adaptive immune resistance during disease progression were observed, with increases in immune and stromal cell infiltration accompanied by the expression of immune suppressive markers. We observed high gene expression of the immune checkpoint genes LAG3 and HAVCR2 (TIM3) in most tumors and increased expression of the immune checkpoint genes TIGIT and CTLA4 in recurrent tumors, compared to the primaries. These markers may be potential candidates for immunotherapy targeting in ovarian cancer. Anti-PD1/PDL1 therapy has proven efficacious against many cancers but only reached modest objective response rates against recurrent ovarian cancer. A deeper understanding of the tumor microenvironment (TME) may reveal other immunosuppressive mechanisms that warrant investigation as immunotherapeutic targets for this challenging disease. Matched primary and recurrent tumors from patients with high-grade serous ovarian carcinoma (HGSC) were analyzed by multicolor immunohistochemistry/immunofluorescence for the presence of T cells, B cells, macrophages, and for the expression of immunosuppressive and HLA molecules. Cancer- and immune-related gene expression was assessed by NanoString analysis. Recurrent tumors showed increased infiltration by immune cells, displayed higher expression of PDL1, IDO, and HLA molecules, and contained more stromal tissue. NanoString analysis demonstrated increased expression of gene signatures related to chemokines and T cell functions in recurrent tumors. The ovarian tumors showed high gene expression of LAG3 and HAVCR2 (TIM3) and enhanced levels of TIGIT and CTLA4 in recurrent tumors compared to primary tumors. The majority of HGSC developed into a more inflamed phenotype during progression from primary to recurrent disease, including indications of adaptive immune resistance. Objective:Ovarian cancer (OC) is often diagnosed at an advanced stage with two thirds of patients experiencing recurrent disease with a poor prognosis. Adoptive cell therapy (ACT) with tumor-infiltrating lymphocytes (TIL) has shown curative potential in malignant melanoma, but has only been investigated scarcely in other cancers. In this pilot study, we tested TIL based ACT in patients with metastatic OC. Methods:Six patients with progressive platinum-resistant metastatic OC were treated with an infusion of TIL preceded by standard lymphodepleting chemotherapy and followed by decrescendo intravenous interleukin-2 (IL-2). Primarily, the feasibility and tolerability of the treatment was assessed. Secondarily, disease control rate was described and immune responses against tumor cells were monitored. Results:Treatment was well tolerated with manageable toxicities. Four patients had stable disease for three months and two patients for five months with five patients having a decrease in target lesions. Progression was primarily due to new lesions while target lesions in general remained stable or in regression. Antitumor reactivity was observed in TIL infusion products from five patients but no antitumor reactivity was detectable in peripheral blood lymphocytes collected after treatment. High numbers of infused TIL expressed exhaustion markers including LAG3 and PD-1, and immunostaining of tumor tissue demonstrated substantial MHCII and PD-L1 expression. Conclusions:ACT with TIL in combination with decrescendo IL-2 is feasible in patients with metastatic OC. Early indications of clinical activity were found. However, TIL ACT efficacy was incomplete with possible involvement of the inhibitory immune checkpoint pathways LAG3/MHCII and PD1/PD-L1. [ABSTRACT FROM AUTHOR]

The article presents a study on the use of over-the-counter drugs (OTC) before hospitalization. According to the authors, the junior physicians on call interviewed all patients admitted to the medical admission unit at Sydvestjysk Sygehus in Esbjerg, Denmark on the use of OTC-drugs using a modified chart template designed for the purpose. The results indicate that there is a need for more focus on the intake of OTC-drugs to avoid potential side-effects and medicine-interactions.

The study showed a significant lower hands-off time when comparing SOWS to the 2010 ERC guidelines.

21. Early fish colonization and community development in a shallow re-established lake.

The establishment of new lakes has gained momentum throughout Europe and North America as a way to remove nutrients, manage extreme rain events, establish recreational areas, and increase biodiversity. We compared freshwater fish colonization and community development of re-established Lake Fil, a 900-ha lake in Denmark, with fish communities in lakes in close proximity as well as those throughout the region. We found that fish migrated quickly into the lake from nearby refugia with fast initial colonization followed by saturation of the species pool. A total of 14 fish species migrated into the lake during the first five years. Over this period, the lake showed increasing similarity not only to the two directly connected lakes, but also to other lakes in the region. This could be seen in terms of species occurrence similarity, catch per unit of effort (CPUE) and biomass per unit of effort (BPUE). The first immigrant was the three-spined stickleback (Gasterosteus aculeatus), followed by roach (Rutilus rutilus) and pike (Esox lucius), which both came to dominate the fish biomass in the following years. However, the pike population showed no reproduction and perch (Perca fluviatilis) had low immigration and reproduction, pointing to a limited success, overall. One solution might be to stock Lake Fil and other new lakes with predatory fish, which might ensure a more balanced food web structure which positively influence the turbidity of the lake water and provide recreational value for anglers. • The species composition in the new lake resembled that in the closely connected lakes. • Three-spined stickleback was dominant to begin with, but then almost disappeared. • Perch was rare and only a single cohort of pike was observed. • Roach became established in large number.
