Abstract

Background: Allergic rhinitis (AR) is a highly prevalent disease. We aimed to assess the symptoms that physicians who see patients with AR perceive as the most bothersome in their patients. Methods: We performed a cross-sectional study based on an online questionnaire sent to all members of the Allergic Rhinitis and its Impact on Asthma (ARIA) initiative. The survey included questions on the physicians' perceptions of patients’ AR symptoms as well as of their own AR symptoms. Results: Among 401 respondents, 155 (38.7%) reported having AR. ARIA members reported nasal symptoms to be the most frequent (89.7%) and bothersome (80.0%) symptoms experienced by themselves. Likewise, nasal symptoms were reported by ARIA members as the most frequent (94.8% in members with AR vs 96.0% in members without AR) and bothersome (57.0% in members with AR vs 67.9% in members without AR) in their patients. We found a significant association (p = 0.001) between physicians’ own symptoms and those perceived as the most bothersome in their patients. Conclusion: Physicians perceive nasal symptoms to be the most frequent and the most bothersome symptoms in AR patients. The physicians' personal experiences with AR may influence their perception of patients’ symptoms.

Abstract

Obstructive sleep apnea in children is most commonly caused by adenotonsillar hypertrophy. Chronic longstanding hypoxia at night can lead to cardiovascular complications such as pulmonary hypertension and cor pulmonale. We reported a case of a four-year-old boy who had suffered from adenotonsillar hypertrophy and obstructive sleep disturbance for 3 years, resulting in right ventricular hypertrophy. Adenotonsillectomy was performed. Pre-operative awareness of potential complications, good co-operation between pediatric cardiologist and pediatric anesthesiologist with post-operative monitoring in intensive care unit were essential for the success of surgical treatment.

4. Chronic Granulomatous Disease with Disseminated Trichosporonosis : A Case Report

Abstract

A case of a two year and three month old Thai boy with a history of recurrent bacterial infections since aged 2 months. A definite absence of superoxide activity in the patient' s granulocytes detected by nitroblue tetrazolium test indicated the diagnosis of chronic granulomatous disease. He suffered from septicemia and systemic fungal infection, resulting in death despite rigorous medical care. The postmortem findings showed extensive granulomatous inflammation in several internal organs. Tissue culture led to diagnosis of disseminated trichosporonosis, an uncommon infection in immunocompromised patients. This is the first case report of disseminated trichosporonosis in a chronic granulomatous disease patient.

Abstract

History of the World Allergy Organization: In 1951, the leaders in allergy from all over the world came together to form the International Association of Allergology and Clinical Immunology (IAACI). For the next 60 years, the allergy world converged at the IAACI triennial meetings, which became biennial in 2003. The international meetings, originally named the International Congress of Allergology and Clinical Immunology (ICACI), are now the World Allergy Congress (WAC) hosted by the World Allergy Organization (WAO). Everyone who has aspired to have worldwide recognition has played a part in IAACI-WAO. The History of the World Allergy Organization traces the global arc of the allergy field over the past 60 years. The current officers of WAO elected to focus on this rich history, inviting prominent leaders who are interested in being part of this history project to write about their time with IAACI-WAO. This series will be presented in Cancún, México, as part of the XXII World Allergy Congress (December 4-8, 2011). Background: Chronic rhinosinusitis is a common problem in patients with allergic rhinitis or antibody deficiency. Topical antibiotic therapy for chronic sinusitis has been shown to improve symptoms and quality of life in adults. The objective of this study was to describe the clinical outcomes after using gentamicin nasal irrigation in children with recalcitrant rhinosinusitis. Methods: This retrospective cohort was performed in children with chronic rhinosinusitis who received gentamicin nasal irrigation from January 2005 through February 2011. The clinical symptoms, frequency of sinusitis, hospitalization and antibiotic treatment due to rhinosinusitis between pre- and during treatment of gentamicin nasal irrigation were compared. Results: Thirty-eight patients (23 males) with the mean age of 12.7±1.1 years were recruited. The most common comorbidities were allergic rhinitis and antibody deficiency. We have prepared this document, “Sublingual Immunotherapy: World Allergy Organization Position Paper 2013 Update”, according to the evidence-based criteria, revising and updating chapters of the originally published paper, “Sublingual Immunotherapy: World Allergy Organization Position Paper 2009”, available at http://www.waojournal.org. Namely, these comprise: “Mechanisms of sublingual immunotherapy;” “Clinical efficacy of sublingual immunotherapy” – reporting all the data of all controlled trials published after 2009; “Safety of sublingual immunotherapy” – with the recently published Grading System for adverse reactions; “Impact of sublingual immunotherapy on the natural history of respiratory allergy” – with the relevant evidences published since 2009; “Efficacy of SLIT in children” – with detailed analysis of all the studies; “Definition of SLIT patient selection” – reporting the criteria for eligibility to sublingual immunotherapy; “The future of immunotherapy in the community care setting”; “Methodology of clinical trials according to the current scientific and regulatory standards”; and “Guideline development: from evidence-based medicine to patients' views” – including the evolution of the methods to make clinical recommendations.Additionally, we have added new chapters to cover a few emerging crucial topics: “Practical aspects of schedules and dosages and counseling for adherence” – which is crucial in clinical practice for all treatments; “Perspectives and new approaches” – including recombinant allergens, adjuvants, modified allergens, and the concept of validity of the single products. Abstract

Mite allergens Derpl and Derfl were tested in 200 house dust samples using a nitrocellulose membrane- based dot-blot assay. The assay enables the mite allergens in a dust extract to be detected with results that can be visually read semiquantitatively within 30 min. Positive results obtained with this method were easily observed as brown dots. Abstract

The quantity and concentration of major allergens in mite allergenic extracts are crucial for skin testing, which is the recommended standard method for the diagnosis of house dust mite allergic disease. The purposes of this study were 1) To compare the constituents of major mite allergens in 3 types of mite extracts, i.e., extracts from mite reared in-house (Dermatophagoides pteronyssinus and Dermatophagoides farinae), extracts from commercial mite products and commercial ready-made mite extracts. These in-house extracts were prepared either with or without the preservative, glycerine. The concentrations of the major constituents of group 1 and 2 allergens of the extracts were determined by a two-site monoclonal based ELISA. 2 ) Biological assays were also carried out to determine the relative potency of the extracts in 120 allergic patients by skin prick test. It was found that the mean concentrations of Der p1, Der f1 and group 2 mite allergens in extracts from mites reared in-house were 102, 195 and 94 μg/ml, respectively, compared to 169, 238 and 91 μg/ml, respectively in commercial mite extracts. The commercial with product had the lowest concentrations of mite allergens (1 μg/ml). Comparison of mite extracts with and without glycerine preservative showed no significant difference in concentrations of major allergens. Reduction of allergens concentration from 10,000 to 1,000 PNU/ml also reduced the concentration of mite allergen proportionately. Siriraj mite extracts were stable for at least 1 year at 4°C without any significant change in composition or concentration.    In conclusion, mite reared in-house can be used as raw material for preparation of mite allergenic vaccine.
