Background and Purpose Cerebral edema (CED) in ischemic stroke can worsen prognosis and about 70% of patients who develop severe CED die if treated conservatively. We aimed to describe incidence, risk factors and outcomes of CED in patients with extensive ischemia. Methods Oservational study based on Safe Implementation of Treatments in Stroke-International Stroke Treatment Registry (2003–2019). Severe hemispheric syndrome (SHS) at baseline and persistent SHS (pSHS) at 24 hours were defined as National Institutes of Health Stroke Score (NIHSS) >15. Outcomes were moderate/severe CED detected by neuroimaging, functional independence (modified Rankin Scale 0–2) and death at 90 days. Results Patients (n=8,560) presented with SHS and developed pSHS at 24 hours; 82.2% received intravenous thrombolysis (IVT), 10.5% IVT+thrombectomy, and 7.3% thrombectomy alone. Median age was 77 and NIHSS 21. Of 7,949 patients with CED data, 3,780 (47.6%) had any CED and 2,297 (28.9%) moderate/severe CED. Background and Purpose The influence of stroke etiology on outcomes after endovascular thrombectomy (EVT) is not well understood. We aimed to investigate whether stroke etiology subgrouped as large artery atherosclerosis (LAA) and cardiac embolism (CE) influences outcomes in large artery occlusion (LAO) treated by EVT. Methods We included EVT treated LAO stroke patients registered in the Safe Implementation of Treatment in Stroke (SITS) thrombectomy register between January 1, 2014 and September 3, 2019. Primary outcome was successful reperfusion (modified Treatment in Cerebral Infarction 2b-3). Secondary outcomes were symptomatic intracranial hemorrhage (SICH), 3-month functional independence (modified Ranking Scale 0–2) and death. Multivariable logistic regression models were used for comparisons. In addition, a meta-analysis of aggregate data from the current literature was conducted (PROSPERO, ID 167447). Introduction: Since the publication of endovascular treatment trials and European Stroke Guidelines, Portugal has re-organized stroke healthcare. The nine centers performing endovascular treatment are not equally distributed within the country, which may lead to differential access to endovascular treatment. Our main aim was to perform a descriptive analysis of the main treatment metrics regarding endovascular treatment in mainland Portugal and its administrative districts. Material and Methods: A retrospective national multicentric cohort study was conducted, including all ischemic stroke patients treated with endovascular treatment in mainland Portugal over two years (July 2015 to June 2017). All endovascular treatment centers contributed to an anonymized database. Demographic, stroke-related and procedure-related variables were collected. Crude endovascular treatment rates were calculated per 100 000 inhabitants for mainland Portugal, and each district and endovascular treatment standardized ratios (indirect age-sex standardization) were also calculated. Patient time metrics were computed as the median time between stroke onset, first-door, and puncture. Results: A total of 1625 endovascular treatment procedures were registered. The endovascular treatment rate was 8.27/100 000 inhabitants/year. We found regional heterogeneity in endovascular treatment rates (1.58 to 16.53/100 000/year), with higher rates in districts closer to endovascular treatment centers. When analyzed by district, the median time from stroke onset to puncture ranged from 212 to 432 minutes, reflecting regional heterogeneity. Conclusion: The overall national rate of EVT in the first two years after the organization of EVT-capable centers is one of the highest among European countries, however, significant regional disparities were documented. Moreover, stroke-onset-to-first-door times and in-hospital procedural times in the EVT centers were comparable to those reported in the randomized controlled trials performed in high-volume tertiary hospitals.

Resumo: A aliança terapêutica (AT) é a relação estabelecida durante o tratamento psicoterápico entre a dupla terapeuta-paciente. A construção da AT é influenciada pelos fatores do psicoterapeuta e do paciente e elementos do próprio enquadre, e está associada à efetividade do tratamento. Objetivou-se investigar os elementos que constituem a AT, dentre os quais se destacam os fatores do psicoterapeuta e do paciente que influenciam na construção da AT. Trata-se de um estudo qualitativo, de caráter exploratório e descritivo. Participaram 18 pessoas, com idades entre 18 e 57 anos, sendo 9 pacientes e 9 terapeutas. Foi realizada entrevista semiestruturada, submetida à análise qualitativa de conteúdo. Os resultados foram discutidos à luz da literatura por meio de três categorias: fatores do psicoterapeuta na construção da aliança terapêutica, fatores do paciente na construção da aliança terapêutica e impacto da aliança terapêutica nas psicoterapias de orientação analítica e cognitivo-comportamental, destacando a participação da dupla na construção da AT e a importância de seu estabelecimento para obtenção dos resultados em psicoterapia. Evidenciou-se o estabelecimento da AT como um requisito para efetivar a psicoterapia e alcançar as mudanças almejadas. Ressalta-se que outros estudos sobre a temática devem ser realizados, visto sua relevância para o tratamento como um todo.

O estudo avaliou a composição e riqueza florística do banco de sementes em área de caatinga. Foram arranjadas sistematicamente 35 parcelas 10 x 10 m e coletadas, aleatoriamente, amostras de solo. Cada amostra foi homogeneizada e colocada para germinar em dois ambientes: pleno sol e sombra a 50%. Foram encontrados 237 indivíduos, pertencentes a 13 famílias botânicas, 22 gêneros e 26 espécies. A densidade total do banco de sementes foi de 43 sementes/m2. O tratamento sombra apresentou densidade de 33,2 sem/m2, com maior diversidade de indivíduos. Já o tratamento a pleno sol, obteve densidade de 9,28 sem/m2. O ambiente sob sombrite proporcionou maior germinação e sobrevivência inicial de plântulas.

Background and purpose: The best reperfusion treatment for patients with mild acute ischaemic stroke harbouring proximal anterior circulation large vessel occlusion (LVO) is unknown. The aim was to compare the safety and efficacy of intravenous thrombolysis (IVT) plus endovascular thrombectomy (EVT) versus IVT alone in LVO patients with mild symptoms. Methods: From the Safe Implementation of Treatment in Stroke–International Stroke Thrombolysis and Thrombectomy Register (SITS‐ISTR), were included: (i) consecutive acute ischaemic stroke patients, (ii) treated within 4.5 h from symptoms onset, (iii) baseline National Institutes of Health Stroke Scale (NIHSS) score ≤5 and (iv) intracranial internal carotid artery [ICA], M1 or T occlusion [defined as occlusion of ICA terminal bifurcation]. After propensity score matching, 3‐month functional outcomes (modified Rankin Scale [mRS] 0–1 and 0–2) and safety outcomes (symptomatic intracerebral haemorrhage and death) were compared (via univariable and multivariable logistic [and ordinal] regression analyses) in patients treated with IVT + EVT versus IVT alone. Results: In all, 1037 patients were included. After propensity score matching (n = 312 per group), IVT + EVT was independently associated with poor functional outcomes (adjusted odds ratio [aOR] 0.46 for mRS 0–1, 95% confidence interval [CI] 0.30–0.72, p = 0.001; aOR 0.52 for mRS 0–2, 95% CI 0.32–0.84, p = 0.007; aOR 1.61 for 1‐point shift in mRS score, 95% CI 1.12–2.32, p = 0.011), with no significant differences in safety outcomes compared to IVT alone, despite numerically higher rates of symptomatic intracerebral haemorrhage (3.3% vs. 1.1%; p = 0.082), a higher rate of any haemorrhagic transformation (17.6% vs. 7.3%; p < 0.001) and subarachnoid haemorrhage (7.9% vs. 1.5%; p = 0.002) in the IVT + EVT group. Discussion: In anterior circulation LVO patients presenting with NIHSS score ≤5, IVT + EVT (vs. IVT alone) was associated with poorer 3‐month functional outcome. Este estudo objetivou apreender os conhecimentos de adolescentes nuligestas acerca do parto e do nascimento, mediante uma pesquisa qualitativa com doze adolescentes de idade entre 12 e 16 anos, residentes na área de uma unidade de saúde de Mossoró - RN. Os resultados mostraram que as adolescentes possuem conhecimentos precários acerca do parto, sendo as informações permeadas por mitos e desarticuladas de conceitos capazes de transformar os saberes vigentes. Assim, evidencia-se a necessidade da aproximação das adolescentes com a temática da sexualidade, e em particular, da gravidez e parto, para que possam analisar e escolher a forma ideal de parir, identificando possibilidades para um parto mais humanizado e saudável. Para isto, sugere-se que os profissionais de saúde, em especial os enfermeiros, desenvolvam atividades socioeducativas de preparação para o parto em articulação com a escola e a família, com vista a esclarecer as adolescentes quanto à dinâmica da parturição e assim contribuir para a desmistificação de estigmas ainda presentes na sociedade.

Objectivo: estudar determinantes cardiovasculares condicionantes do tempo de ventilação, mortalidade e gravidade de doença em doentes admitidos numa unidade de cuidados intensivos para ventilação mecânica por exacerbação de insuficiência respiratória crónica. Desenho e local: Estudo prospectivo, com duração de 30 meses numa unidade de cuidados intensivos médico-cirúrgica com 14 camas. Material e métodos: Estudados 59 doentes com idade média de 74,7 +/- 9,7 anos, tempo médio de ventilação de 10,8 +/- 12,6 dias, APACHE II médio de 23 +/- 8,3. Avaliaram-se parâmetros ecocardiográficos (dimensões das cavidades, débito cardíaco, estudo Doppler do fluxo transvalvular mitral, estudo da veia cava inferior) e electrocardiográficos (presença de ritmo sinusal ou fibrilhação auricular) nas primeiras 24 horas de internamento na Unidade e parâmetros gasimétricos à saída. Resultados: Um tempo de ventilação mais prolongado associou-se à presença de fibrilhação auricular (p=0,027), à presença conjunta de fibrilhação auricular e uma veia cava inferior dilatada (> 20mm p=0,004) e com níveis séricos de bicarbonato> 35mEq/l na gasimetria obtida à saída (p=0,04). Verificaram-se 12 óbitos. A mortalidade associou-se à presença de dilatação do ventrículo direito (p=0,03) e a uma relação entre o ventrículo direito e o esquerdo> 0,6 (p=0,04). Conclusão: Nos doentes submetidos a ventilação mecânica por exacerbação de insuficiência respiratória crónica, a presença de fibrilhação auricular indica a possibilidade de um período de ventilação mais prolongado, em especial se houver concomitantemente uma veia cava inferior com diâmetro> 20mm. Blood gases were evaluated at dis charge. Results: Greater length of ventilation was observed in patients presenting atrial fibrillation (p=0.027), particularly when a dilated IVC was also present (>20mm, p=0.004). A high level of serum bicarbonate (>35mEq/l), was also related with longer ventilation (p=0. 04). Twelve patients died. Mortality was related to the presence of a dilated right ventricle (p=0. 03) and a ratio between right and left ventricle> 0. 6 (p=0.04). Conclusion: Patients submitted to mechanical ventilation due to exacerbation of chronic respiratory failure which present atrial fibrillation require a longer ventilation period, particularly if a dilated IVC is also present. Patients with dilated right cardiac chambers are at an increased risk of a fatal outcome.

OBJETIVO: Descrever a atividade docente assistencial, cujo objetivo é proporcionar experiência de promoção da saúde bucal coletiva a estudantes concluintes do curso de odontologia. MÉTODOS: Tal experiência fundamenta-se na avaliação do desempenho do aluno, como educador em saúde bucal, à medida que ele tem, entre outras tarefas, a de motivar pacientes internados e seus acompanhantes, na geração de hábitos saudáveis, visando à assistência integral e mais humanizada do paciente hospitalizado. RESULTADOS: Os resultados mostraram que as metas desejadas foram atingidas, visto que a higiene bucal dos pacientes já se tornou uma tarefa incorporada à rotina hospitalar, considerando-se os índices de biofilme dentário: inicial, 1,72; e final, 1,17, respectivamente. Essa diferença, de acordo com a estatística U-Mann-Whitney, mostrou-se significativa, pois o valor de pOBJECTIVE: To describe a teaching aid activity aimed at providing experience for promoting collective oral health to graduating dental students. Fresh and salt water samples analyzed in Rio de Janeiro city showed the presence of Plesiomonas shigelloides. Forty-six strains were isolated from both environments. A high incidence of P. shigelloides was achieved in polluted fresh and salt waters as well as in samples from non-polluted streams. P. shigelloides isolates had biochemical characteristics similar to those already described in the literature. None of the isolates analyzed produced enterotoxin in the suckling mouse assay. Hemolytic activity against sheep and human type A erythrocytes was detected in the strains tested. The results of the antibiotic susceptibility tests indicated that all the isolates were susceptible to the cephalosporins, penicillins combined with a beta-lactamase inhibitor, aminoglycosides, imipenem, norfloxacin, tetracycline, chloramphenicol and trimethoprim-sulfamethoxazole. All the isolates were resistant to the penicillins.

Members of the 14-3-3 protein family have been identified as regulatory molecules in intracellular signaling pathways and cell cycle control. Previously, the first Echinococcus 14-3-3 isoform (E14-3-3.1) was isolated from E. granulosus and E. multilocularis metacestode stages. Hyperexpression of this isoform was claimed to be associated with non-restricted tumor-like growth of the E. multilocularis metacestode. In this report, we describe the characterization of a 14-3-3 cDNA from E. granulosus and E. multilocularis corresponding to a second isoform of this family, E14-3-3.2. The characterized 14-3-3 gene was interrupted by two introns whose sequence and positions were conserved in both Echinococcus species. The deduced amino acid sequence of E14-3-3.2 showed 88% identity to the E14-3-3.1 isoform and 52% identity to a third Echinococcus isoform (E14-3-3.3) described by other authors. These findings, coupled to Southern blot analysis, suggest the presence of more than one 14-3-3 gene in Echinococcus. Phylogenenetically, the Echinococcus 14-3-3.1 and 14-3-3.2 isoforms appeared to cluster with zeta-type (“pro-tumorigenic”) 14-3-3 isoforms from closely related organisms, whereas the E14-3-3.3 isoform grouped with 14-3-3 epsilon isoforms. The presence of more than one 14-3-3 isoform might indicate isoform-specific roles in the different parasite stages of Echinococcus. [ABSTRACT FROM AUTHOR]

Aim: to identify the complications of pressure ulcers in severely ill patients from the perspective of the nurses. Method: This is a descriptive-exploratory study which adopts a qualitative approach involving a questionnaire to nurses working in an intensive care unit of Onofre Lopes Hospital, in Natal, Brazil. The data were submitted to the content analysis topics. Results: From the comments of the professionals, we generated a category "Complications of pressure ulcers in severely ill patients", and the subcategories "infection", "period of hospitalization", "hospital costs", "mortality" and "suffering". Discussion: Ulcers lead to infections and to an increase in microbiological resistance. They increase the period of hospitalization and the costs of treatment due to the resulting complications. Mortality is related to the worsening of clinical conditions. Suffering is a result of pain and dependency. Conclusion: For the nurses, pressure ulcers are complications that generate a worsening of clinical conditions of severely ill patients, which makes it necessary to adapt the treatment.
