In recognition, context effects often manifest as higher hit and false-alarm rates to probes tested in an old context compared with probes tested in a new context; sometimes, this concordant effect is accompanied by a discrimination advantage. According to the cue-overload account of context effects (Rutherford, 2004), context acts like any other cue, and thus context effects should be larger with lighter context loads. Conversely, the Item, Associated Context, and Ensemble (ICE) account (Murnane et al., 1999) attributes context effects to two factors: subjects erroneously attributing context familiarity to the probe, and the formation of ensembles (mnemonic combinations of item and context). Context familiarity increases as exposure at study increases, and thus ICE predicts larger effects of context as context load increases. Relatedly, ICE predicts larger effects of context as context meaningfulness increases, as meaningful contexts are more likely to be bound to the target in an ensemble. In Experiments 1 and 2, rather than manipulate context load during the study phase, we relied on subjects' preexperimental context exposure to manipulate context load. Subjects studied words superimposed on photographs of their university campus or another university campus. At test, targets and distractors were evenly divided between study and novel contexts and between familiar and unfamiliar contexts. In Experiment 3, we manipulated context familiarity within the experimental session. Results supported ICE, suggesting that context does not act as a retrieval cue in recognition.

The mirror effect is the finding that in recognition tests, a manipulation that increases the hit rate also decreases the false alarm rate. For example, low frequency words have a higher hit rate and a lower false alarm rate than high frequency words. Because the mirror effect is held to be a regularity of memory, it has had a pronounced influence on theories of recognition. We took advantage of the recent increase in the number of linguistic databases to create sets of stimuli that differed on one dimension (contextual diversity, frequency, or concreteness) but were more fully equated on other dimensions known to affect memory. Experiment 1 (contextual diversity), Experiment 3 (frequency), and Experiment 5 (concreteness) found no evidence of a mirror effect. We also conducted parallel experiments which used previously published stimuli that could not avail of the new databases and which therefore contained confounds. Experiment 2 (contextual diversity), Experiment 4 (frequency), and Experiment 6 (concreteness) all resulted in mirror effects. If this pattern of results is replicable, it has broad implications for theories of recognition, which typically view the mirror effect as a benchmark finding. Unfortunately, few articles on the mirror effect include the stimuli, rendering the past literature of little use in testing this hypothesis. We encourage researchers to create and assess other pools of highly controlled stimuli to establish whether the stimulus-based mirror effect obtains when confounds are eliminated or whether it is due to the presence of these confounds.

Objective: We evaluated the impressions and perceived workflow consequences following installation of a touch-free aseptic stethoscope barrier dispenser in the clinical environment. Methods: Beginning in 2020, we conducted a volunteer survey of aseptic stethoscope diaphragm barrier (AseptiScope, San Diego, CA) users in multiple departments at 7 US healthcare facilities. A 10-question survey was presented on an iPad near the aseptic barrier dispenser, which was usually located in the patient exam room, to be available immediately after the practitioner completed their examination, which included the use of the stethoscope barrier. This evaluation was considered a quality improvement project and was exempt from institutional review board approval. For this analysis, only 1 survey per practitioner was included. Results: Overall, 147 surveys were obtained from 7 institutions geographically distributed across the United States, immediately after placement of the DiskCover system in the patient care environment. Responses were generally positive and included ease of use (95.2% rated easy or very easy), comparison to a disposable stethoscope (97.9% as similar to, improved over, or significant improvement), workflow changes (53.7% improvement, 97.3% no impact, or improved), and perceived effect on patient safety (90.3% felt that patient safety was improved or significantly improved). Conclusions: The use of a touch-free aseptic stethoscope barrier system was reported to be easy to use, superior to a disposable stethoscope, and an improvement to practitioner workflow and perceived patient safety.

The word length effect, better recall of lists of short (fewer syllables) than long (more syllables) words has been termed a benchmark effect of working memory. Despite this, experiments on the word length effect can yield quite different results depending on set size and stimulus properties. Seven experiments are reported that address these 2 issues. Experiment 1 replicated the finding of a preserved word length effect under concurrent articulation for large stimulus sets, which contrasts with the abolition of the word length effect by concurrent articulation for small stimulus sets. Experiment 2, however, demonstrated that when the short and long words are equated on more dimensions, concurrent articulation abolishes the word length effect for large stimulus sets. Experiment 3 shows a standard word length effect when output time is equated, but Experiments 4-6 show no word length effect when short and long words are equated on increasingly more dimensions that previous demonstrations have overlooked. Finally, Experiment 7 compared recall of a small and large neighborhood words that were equated on all the dimensions used in Experiment 6 (except for those directly related to neighborhood size) and a neighborhood size effect was still observed. We conclude that lexical factors, rather than word length per se, are better predictors of when the word length effect will occur.

Abstract Background Phenylketonuria (PKU) is an autosomal recessive inherited disorder characterised by a deficiency in phenylalanine hydroxylase. Untreated, PKU is associated with a wide range of cognitive and psychiatric sequelae. Contemporary management guidelines recommend lifetime dietary control of phenylalanine (Phe) levels, however many individuals who discontinue dietary control subsequently suffer symptoms of anxiety, depression and disturbances to cognition. We undertook a prospective cohort study of patients with early-treated phenylketonuria who had ceased dietary control to test the hypothesis that resumption of dietary control of PKU is associated with improvements in measures of psychiatric morbidity and cognitive functioning. Methods We re-initiated dietary control for early-treated patients with PKU and monitored cognitive and psychiatric outcomes over a twelve-month period. Assessments included objective cognitive function (measured by cognitive proficiency index (CPI)), anxiety and depression scales. General linear mixed model (GLMM) analyses were performed to assess change in psychometric variables from baseline over twelve months after resumption of dietary control. Results A total of nine patients were recruited. Mean age was 33 years (SD = 8.75), five were female. Mean time off dietary control was 19.1 years (SD = 11.3), and mean baseline phenylalanine (Phe) levels were 1108 µmol/L (SD = 293). GLMM analysis demonstrated a positive relationship between CPI and time on diet (b = 0.56 [95% CI = 0.17, 0.95]). Age, time off diet, Phe levels and depression scores were not associated with cognitive function. There was a negative relationship between time on diet and anxiety (b = − 0.88 95% CI = [− 1.26, − 0.50]) and depression ratings (b = − 0.61, 95% CI = [− 0.95, − 0.26]). Conclusions This study demonstrated improvements in cognitive function, anxiety, and depression ratings associated with resumption of dietary control of PKU. Raw Phe levels were not strongly associated with psychiatric or cognitive scores in this cohort. These findings support the importance of lifelong treatment for PKU in improving the cognitive and psychiatric sequelae of the disease.

Background/aim To determine willingness to pay for children’s spectacles, and barriers to purchasing children’s spectacles in Cambodia.Methods We conducted vision screenings, and eye examinations as indicated, for all consenting children at 21 randomly selected secondary schools. We invited parents/guardians of children found to have refractive problems to complete a willingness to pay for spectacles survey, using a binary-with-follow-up technique.Results We conducted vision screenings on 12 128 secondary schoolchildren, and willingness to pay for spectacles surveys with 491 parents/guardians (n=491) from Kandal and Phnom Penh provinces in Cambodia. We found 519 children with refractive error, 7 who had pre-existing spectacles and 14 recommended spectacles for lower ametropias. About half (53.2%; 95% CI 44.0% to 62.1%) of parents/guardians were willing to pay KHR70 000 (US$17.5; average market price) or more for spectacles. Mean willingness-to-pay price was KHR74 595 (US$18.6; 95% CI KHR64 505 to 86 262; 95% CI US$16.1 to US$21.6) in Phnom Penh and KHR55 651 (US$13.9; 95% CI KHR48 021 to 64 494; 95% CI US$12.0 to US$16.1) in Kandal province. Particle size is a significant factor in determining the dispersal and inhalation risk from bioaerosols. Green-waste composting is a significant source of bioaerosols (including pathogens), but little is known about the distribution of specific taxa across size fractions. To characterise size fractionated bioaerosol emissions from a compost facility, we used a Spectral Intensity Bioaerosol Sensor (SIBS) to quantify total bioaerosols and qPCR and metabarcoding to quantify microbial bioaerosols. Overall, sub-micron bioaerosols predominated, but molecular analysis showed that most (>75%) of the airborne microorganisms were associated with the larger size fractions (>3.3 µm da). The microbial taxa varied significantly by size, with Bacilli dominating the larger, and Actinobacteria the smaller, size fractions. The human pathogen Aspergillus fumigatus dominated the intermediate size fractions (>50% da 1.1–4.7 µm), indicating that it has the potential to disperse widely and once inhaled may penetrate deep into the respiratory system. Serial position functions with marked primacy and recency effects are ubiquitous in episodic memory tasks. The demonstrations reported here explored whether bow-shaped serial position functions would be observed when people ordered exemplars from various categories along a specified dimension. The categories and dimensions were: actors and age; animals and weight; basketball players and height; countries and area; and planets and diameter. In all cases, a serial position function was observed: People were more accurate to order the youngest and oldest actors, the lightest and heaviest animals, the shortest and tallest basketball players, the smallest and largest countries, and the smallest and largest planets, relative to intermediate items. The results support an explanation of serial position functions based on relative distinctiveness, which predicts that serial position functions will be observed whenever a set of items can be sensibly ordered along a particular dimension. The serial position function arises because the first and last items enjoy a benefit of having no competitors on 1 side and therefore have enhanced distinctiveness relative to mid-dimension items, which suffer by having many competitors on both sides.

Bidirectional Inductive power transfer (IPT) systems behave as high order resonant networks and hence are highly sensitive to changes in system parameters. Traditional PID controllers often fail to maintain satisfactory power regulation in the presence of parametric uncertainties. To overcome these problems, this paper proposes a robust controller which is designed using linear matrix inequality (LMI) techniques. The output sensitivity to parametric uncertainty is explored and a linear fractional transformation of the nominal model and its uncertainty is discussed to generate a standard configuration for μ-synthesis and LMI analysis. An H∞ controller is designed based on the structured singular value and LMI feasibility analysis with regard to uncertainties in the primary tuning capacitance, the primary and pickup inductors and the mutual inductance. Robust stability and robust performance of the system is studied through μ-synthesis and LMI feasibility analysis. Simulations and experiments are conducted to verify the power regulation performance of the proposed controller.

The word length effect, the finding that lists of short words are better recalled than lists of long words, has been termed one of the benchmark findings that any theory of immediate memory must account for. Indeed, the effect led directly to the development of working memory and the phonological loop, and it is viewed as the best remaining evidence for time-based decay. However, previous studies investigating this effect have confounded length with orthographic neighborhood size. In the present study, Experiments 1A and 1B revealed typical effects of length when short and long words were equated on all relevant dimensions previously identified in the literature except for neighborhood size. In Experiment 2, consonan-vowel-consonant (CVC) words with a large orthographic neighborhood were better recalled than were CVC words with a small orthographic neighborhood. In Experiments 3 and 4, using two different sets of stimuli, we showed that when short (1-syllable) and long (3-syllable) items were equated for neighborhood size, the word length effect disappeared. Experiment 5 replicated this with spoken recall. We suggest that the word length effect may be better explained by the differences in linguistic and lexical properties of short and long words rather than by length per se. Reply to comments on an article "Issues With the SIMPLE Model: Comment on Brown, Neath, and Chater" (2007) by Bennet Murdock on the current authors' original article "A temporal ratio model of memory" by Brown, Neath, and Chater. Does a single mechanism underpin serial and free recall? B. Murdock (2008) argued against the claim, embodied in the SIMPLE model of memory (G. D. A. Brown, I. Neath, & N. Chater, 2007), that data from serial and free recall can be accounted for within a unitary model. Here, the many similarities between serial and free recall are noted, and it is suggested that differences may result not from the use of different mechanisms but from task demands and the involvement of rehearsal processes. Rehearsal has not previously been accommodated by temporal distinctiveness models because of their simplifying assumption that items occupy point locations along a temporal dimension in memory. Here, it is suggested that use of representations that acknowledge items' temporal extension allows the scale-similar temporal distinctiveness approach to account for several additional phenomena that lie outside the scope of other models.

Many models of immediate memory predict the presence or absence of various effects, but none have been tested to see whether they predict an appropriate distribution of effect sizes. The authors show that the feature model (J. S. Nairne, 1990) produces appropriate distributions of effect sizes for both the phonological confusion effect and the word-length effect. The model produces the appropriate number of reversals, when participants are more accurate with similar items or long items, and also correctly predicts that participants performing less well overall demonstrate smaller and less reliable phonological similarity and word-length effects and are more likely to show reversals. These patterns appear within the model without the need to assume a change in encoding or rehearsal strategy or the deployment of a different storage buffer. The implications of these results and the wider applicability of the distribution modeling approach are discussed.

A model of memory retrieval is described. The model embodies four main claims: (a) temporal memory--traces of items are represented in memory partly in terms of their temporal distance from the present; (b) scale-similarity--similar mechanisms govern retrieval from memory over many different timescales; (c) local distinctiveness--performance on a range of memory tasks is determined by interference from near psychological neighbors; and (d) interference-based forgetting--all memory loss is due to interference and not trace decay. The model is applied to data on free recall and serial recall. The account emphasizes qualitative similarity in the retrieval principles involved in memory performance at all timescales, contrary to models that emphasize distinctions between short-term and long-term memory.

Single-predictor and stepwise multinomial logistic regression analyses and an external validation were completed on 3,082 allegations of employment discrimination by adults with multiple sclerosis. Women filed two thirds of the allegations, and individuals between 31 and 50 made the vast majority of discrimination charges (73%). Allegations occurred most frequently in industries with the largest number of employees (501 or more). Results of the single-predictor multinomial logistic regression analyses identified gender and company size as the two predictors, findings that were mirrored in the main effects model developed through the stepwise multinomial logistic regression analyses. Women were more likely than men to allege discrimination related to harassment and intimidation rather than to discharge. Workers in the smallest companies were less likely than workers in the largest companies to allege discrimination regarding reasonable accommodation, working conditions, or employment benefits and more likely to allege discrimination related to discharge. Cross-validation results provided mixed support for the main effects model.

Because employment is a significant predictor of the quality of life of people with disabilities (Rumrill, Roessler, & Fitzgerald, 2004; Viermo & Krause, 1998), discrimination in the workplace that interferes with successful job acquisition or retention is a serious matter. Unfortunately, this type of discrimination is all too prevalent. In a survey conducted by the National Organization on Disability (National Organization on Disability, 2004), one in five employed individuals with disabilities reported that they experienced discrimination in their efforts to work. This finding of one in five workers with disabilities translates into substantial numbers of Americans who do not have fair access to employment opportunities. For example, estimates based on data from the U.S. Census Bureau (Head & Baker, 2005) indicate that approximately 50 million Americans of workforce age have disabilities.

In the SIMPLE model (Scale Invariant Memory and Perceptual Learning), performance on memory tasks is determined by the locations of items in multidimensional space, and better performance is associated with having fewer close neighbors. Unlike most previous simulations with SIMPLE, the ones reported here used measured, rather than assumed, dimensional values. The data to be modeled come from an experiment in which younger and older adults recalled lists of acoustically confusable and nonconfusable items. A multidimensional scaling solution based on the memory confusions was obtained. SIMPLE accounted for the overall difference in performance both between the two age groups and, within each age group, the overall difference between acoustically confusable and nonconfusable items in terms of the MDS coordinates. Moreover, the model accounted for the serial position functions and error gradients. Finally, the generality of the model's account was examined by fitting data from an already published study. The data and the modeling support the hypothesis that older adults' memory may be worse, in part, because of altered representations due to age-related auditory perceptual deficits.

The authors report 2 experiments that compare the serial recall of pure lists of long words, pure lists of short words, and lists of long or short words containing just a single isolated word of a different length. In both experiments for pure lists, there was a substantial recall advantage for short words; the isolated words were recalled better than other words in the same list, and there was a reverse word-length effect: Isolated long words were recalled better than isolated short words. These results contradict models that seek to explain the word-length effect in terms of list-based accounts of rehearsal speed or in terms of item-based effects (such as difficulty of assembling items).

Examines predictive relationships between assessed functional limitations and vocational rehabilitation (VR) services provided for five homogeneous client groups. Results indicate that specific functional limitations were determinants of some VR services but were largely irrelevant to provision of other services. Reports on a multisource identification and an in-depth analysis of exemplary state agency rehabilitation counselors so as to describe the characteristics associated with being exemplary. Results indicate no differences between the two types of rehabilitation counselors -- therapeutic and case manager -- on-job satisfaction or on-task demands. (RJM)

Genetic variations of paraoxonase (PON) correlate with HDL cholesterol and apolipoprotein A-I (apoA-I), suggesting antiatherogenic properties. Atherosclerosis occurs naturally in humans and rabbits but not in mice. We compared variations of PON arylesterase activity (PON AEase, phenylacetate substrate) in humans, rabbits, and mice. In humans and rabbits, >95% of PON AEase is HDL associated. In mice, about 30% of PON AEase is lipid poor. In the absence of apoA-I in mice, total PON AEase is reduced and >60% is lipid poor. PON AEase level and distribution is restored in apoA-I−/− mice injected with adenoviruses encoding human apoA-I and in transgenic mice expressing human apoA-I at a steady-state level. Thus, while apoA-I is not required for the HDL association of PON AEase, induced variations in apoA-I correlate with changes in HDL-associated, but not lipid-poor, PON AEase. PON AEase associates only with apoA-I- or apoE-containing HDL but not VLDL. In the absence of both apoA-I and apoE, PON AEase is all-lipid-poor. PON AEase is displaced from HDL by ultracentrifugation and following incubation with serum amyloid A.Variations in the PON distribution between HDL and lipid-poor fractions may have important consequences in its antioxidant activity and in atherogenesis.

We previously showed that meat from crossbred water buffalo had significantly higher tenderness than beef from crossbred Brahman cattle of the same age, gender, and diet. Extensive studies on meat tenderness have indicated that proteases degrade muscle fibre proteins during postmortem storage, leading to weakening of the myofibrillar structure and an increase in tenderness. Thus, we investigated the difference in protease activity immediately postmortem, in order to explain the difference in tenderness between buffalo meat and beef. Five female crossbred water-buffalo (Philippine Carabao x Bulgarian Murrah) and five female crossbred cattle (Brahman x Philippine Native) were slaughtered at 30 months of age, and Longissimus thoracis muscle was sampled immediately post-slaughter. Protease activity at different pH levels and the effect of various inhibitors on protease activity were examined. Results showed that buffalo meat had significantly higher protease activity compared to beef, and calpain inhibitor 1 was the most effective inhibitor. As calpain inhibitor 1 is a specific inhibitor of calpain 1 and 2, the results suggest that higher calpain activity in buffalo meat was responsible for the higher tenderness of buffalo meat compared to Brahman beef.

• Nearly 1/3 of pregnant women had signs or symptoms of preeclampsia. • A small proportion of suspected cases ultimately received a preeclampsia diagnosis. • Care costs were higher in patients with suspected preeclampsia versus preeclampsia. • Clinical uncertainty around diagnosis may lead to increased resource utilization. Most patients with signs or symptoms (s/s) of suspected preeclampsia are not diagnosed with preeclampsia. We sought to determine and compare the prevalence of s/s, pregnancy outcomes, and costs between patients with and without diagnosed preeclampsia. This retrospective cohort study analyzed a large insurance research database. Pregnancies with s/s of preeclampsia versus a confirmed preeclampsia diagnosis were identified using International Classification of Diseases codes. S/s include hypertension, proteinuria, headache, visual symptoms, edema, abdominal pain, and nausea/vomiting. Pregnancies were classed as 1) s/s of preeclampsia without a confirmed preeclampsia diagnosis (suspicion only), 2) s/s with a confirmed diagnosis (preeclampsia with suspicion), 3) diagnosed preeclampsia without s/s recorded (preeclampsia only), and 4) no s/s, nor preeclampsia diagnosis (control). Of 1,324,424 pregnancies, 29.2 % had ≥1 documented s/s of suspected preeclampsia, and 14.2 % received a preeclampsia diagnosis. Hypertension and headache were the most common s/s, leading 20.2 % and 9.2 % pregnancies developed to preeclampsia diagnosis, respectively. Preeclampsia, with or without suspicion, had the highest rates of hypertension-related severe maternal morbidity (HR [95 % CI]: 3.0 [2.7, 3.2] and 3.6 [3.3, 4.0], respectively) versus controls. A similar trend was seen in neonatal outcomes such as preterm delivery and low birth weight. Cases in which preeclampsia was suspected but not confirmed had the highest average total maternal care costs ($6096 [95 % CI: 602, 6170] over control). There is a high prevalence but poor selectivity of traditional s/s of preeclampsia, highlighting a clinical need for improved screening method and cost-effectiveness disease management. This study aimed to evaluate the effectiveness of an education session in improving the accuracy and efficiency of computed tomography (CT) and magnetic resonance (MR) image registration for brain stereotactic radiosurgery (SRS) patients at the researcher's institution. It also assessed the impact the education session had on GTV position using target registration error (TRE), and image registration variability among radiation therapists (RTs). All RTs working in CT simulation were invited to participate, using a pre- and post-test study design. RTs performed image registrations on five SRS patients before and after an online education session. The session demonstrated point registration tools to aid in the image registration process. Accuracy was measured using transformation data (translation and rotation) and the TRE of the centre coordinates of the GTV, compared to the gold standard (radiation oncologist approved image registration for treatment). Efficiency was assessed by the time taken for each registration. All data were collected and analyzed, reporting their mean values, standard deviations, calculating the difference and determining if there was any improvement between the pre- and post-education session image registrations relative to the gold standard. Pairwise differences in registration accuracy, in terms of the TRE, and efficiency before and after the education session was assessed using the Wilcoxon signed-rank test. These metrics were also used to determine the variability amongst RTs in image registration following the education session, as well as the correlation of accuracy and time. Eleven RTs participated, and 110 image registrations were analyzed. The education session demonstrated no statistical significance on image registration accuracy, reporting a p-value of 0.21 for the TRE. The mean and standard deviation of difference in the pre- and post- education session translation, relative to the gold standard, for the X, Y, and Z coordinates were 0.02 ± 0.05 cm, 0.02 ±0.06 cm, and 0.02 ±0.07 cm, respectively. The mean and standard deviation of the difference in the pre- and post- education session rotation, relative to the gold standard, for the X, Y, and Z coordinates were 0.06° ±0.88°, 0.38° ± 0.67°, and 0.26° ± 1.05°, respectively. The mean difference in TRE from the pre- and post- education image registrations was 0.03 cm with a standard deviation of +/- 0.06 cm. The education session demonstrated no statistical significance on image registration efficiency, reporting a p-value of 0.33. The education session reduced the overall image registration time by 1.9 minutes. There was a slight decrease in interobserver variability for RTs amongst all metrics, and there were some improvements in metrics for some RTs and some image registrations. Although the implementation of the education session resulted in no statistically significant difference in SRS brain image registrations, the improvements for some users in translation, rotation, TRE and efficiency showed some benefit of its implementation.
