Summary: Thrush is the term commonly used to describe nonproliferative pododermatitis of the frog epidermis caused by a bacterial infection. Thrush is commonly encountered in veterinary and farrier practices yet information on the pathogenesis, treatment and prevention of the disease in the equine veterinary literature is lacking. There is a plethora of proposed products and treatments on the market for thrush, yet most lack clinical evidence of efficacy. Clinical signs of thrush can range from black exudate in the sulci of the frog accompanied by a foul odour to bacterial invasion of the frog corium and digital cushion resulting in lameness. Thrush is primarily encountered in a conformationally compromised hoof capsule that is generally associated with an unhealthy frog. As thrush is rarely encountered in a healthy frog, it is important to focus on the farrier practices that may play a role in both the pathogenesis of thrush and its prevention. Summary: White line disease (WLD) is a significant pathological condition that affects the equine hoof. White line disease continues to frustrate veterinarians and farriers due to the diversity regarding the aetiology, diagnosis and especially treatment. Furthermore, WLD lacks a definitive definition as the disease becomes apparent only when the hoof wall is compromised with an extensive separation, a hoof capsule distortion is present or when lameness exists. Clinical signs can range from a minor hoof wall separation to an extensive disruption of the external laminar bond resulting in displacement of the distal phalanx within the hoof capsule. There has been a myriad of treatments and topical preparations proposed for treating WLD, but most remain controversial with few having any scientific documentation or evidence of efficacy. This review of WLD is based on the sparse information available in the veterinary literature, the large number of WLD cases treated successfully in the authors' combined practices using conventional farriery and the relevant questions that hopefully can be answered in the future. Summary: The extensive nature of this topic warrants this review paper to be divided into two parts: 'Basic trimming in foals' and 'Therapeutic farriery in foals'. Management of the feet and limbs during this juvenile period will often dictate the success of the foal as a sales yearling or mature sound athlete. Overall hoof care in the foal is often a joint venture between the veterinarian and the farrier. The orthopaedic disorders discussed in this paper that require input from the two professions are flexural limb deformities (FLD) and angular limb deformities (ALD). The concept of protecting the foot from the deleterious effects of mal‐loading created by many FLDs and ALDs is just as important as using the symptomatology as an instrument to correct the deformity. This paper presents a review of the current information regarding the farriery for these two limb deformities while dispelling some of the anecdotal methodology, such as the use of toe extensions to treat flexural deformities, that presently exists. Considering the deficiency of information in the literature, segments of this text will be based on the author's extensive clinical practice, comprehensive clinical records and comparisons of case outcomes. Hoof care in the first few months of life is serious business and should never be taken lightly. Good farriery is vital for the development of the hoof capsule and the conformation of the limb. Management of the feet and limbs during the juvenile period will often dictate the success of the foal as a sales yearling or mature sound athlete. Overall hoof care of a foal is often a joint venture between the veterinarian and the farrier. Part 1 of this paper will outline the concept of a footcare programme, examination of the foal's feet and limbs along with what constitutes good basic farriery to properly apply the trim. It will also emphasise the importance of maintaining a good veterinarian‐farrier relationship ‐ the farrier being responsible for basic trimming with veterinary oversight when necessary, and if orthopaedic disorders develop, the farrier will have significant input with therapeutic farriery. Summary: The wooden shoe provides the clinician with an alternative option to conventional farriery when treating a variety of foot problems such as acute/chronic laminitis, white line disease, distal phalanx fractures and poor‐quality hoof capsules. The wooden shoe provides a simplified method to apply many of the principles of therapeutic farriery which include redistributing the load or forces on the foot, repositioning breakover and providing heel elevation when necessary. Understanding the biomechanics of the wooden shoe along with understanding good basic farriery which include the appropriate foot trim, proper size, fit and placement of the wooden shoe on the foot combined with the appropriate application are essential for consistent success. Background Interleukin ( IL)-33 is implicated in the pathophysiology of asthma and allergic diseases. However, our knowledge is limited regarding how IL-33 release is controlled. The transcription factor nuclear factor-erythroid-2-related factor 2 (Nrf2) plays a key role in antioxidant response regulation. Objective The goal of this project was to investigate the role of cellular oxidative stress in controlling IL-33 release in airway epithelium. Methods Complementary approaches were used that included human bronchial epithelial cells and mouse models of airway type-2 immunity that were exposed to fungus Alternaria extract. The clinically available Nrf2 activator 2-cyano-3,12-dioxooleana-1,9-dien-28-oic acid methyl ester ( CDDO-Me) was used to evaluate the role of Nrf2-induced antioxidant molecules. Results Human bronchial epithelial cells produced reactive oxygen species ( ROS) when they were exposed to Alternaria extract. ROS scavengers, such as glutathione ( GSH) and N-acetyl cysteine, prevented extracellular secretion of ATP and increases in intracellular calcium concentrations that precede IL-33 release. Administration of CDDO-Me to mice enhanced expression of a number of antioxidant molecules in the lungs and elevated lung levels of endogenous GSH. Importantly, CDDO-Me treatment reduced allergen-induced ATP secretion and IL-33 release by airway epithelial cells in vitro and protected mice from IL-33 release and asthma-like pathological changes in the lungs. Conclusions The balance between oxidative stress and antioxidant responses plays a key role in controlling IL-33 release in airway epithelium. The structures of the equine foot have the unique ability to adapt, change shape and restore. There are multiple benefits in shod vs. barefoot or in allowing the horse to be without shoes for a given time period to improve the palmar section of the foot. However, it requires a transition period, a change in the manner in which the foot is trimmed, a commitment from the owner/trainer and, in the case of leaving the horse without shoes permanently, it depends whether the horse can perform the desired function without shoes. The effects of several sulfamoyl benzoic acid derivatives on Na-K-Cl cotransport were investigated in winter flounder intestine. The relative efficacy (IC50 values) and order of potency of these derivatives were benzmetanide, 5 X 10(-8) M greater than bumetanide 3 X 10(-7) M greater than piretanide 3 X 10(-6) M greater than furosemide 7 X 10(-6) M greater than amino piretanide 1 X 10(-5) 3-amino-4-penoxy-5-sulfamoyl benzoic acid. Binding of [3H] bumetanide was studied in microsomal membranes from winter flounder intestine and compared to that in bovine kidney outer medulla. Binding was also studied in brush-border membranes from winter flounder intestine. The estimated values for Kd and number of binding sites (n) were: bovine kidney, Kd = 1.6 X 10(-7), n = 10.5 pmol/mg protein; winter flounder intestine, Kd 1.2 X 10(-7), n = 7.3 pmol/mg protein, and brush-border membranes from winter flounder, Kd = 5.3 X 10(-7), n = 20.4 pmol/mg protein. The estimated Kd for bumetanide binding to winter flounder brush-border membranes derived from association and dissociation kinetics was 6.8 X 10(-7) M. The similarity in magnitudes of IC50 and Kd for bumetanide suggests that the brush-border cotransporter is ordinarily rate-limiting for transmural salt absorption and that bumetanide specifically binds to the cotransporter. Na-K-Cl cotransport stoichiometry and affinities for Na, K and Cl were determined in flounder intestine. Measurement of simultaneous NaCl and RbCl influxes resulted in ratios of 2.2 for Cl/Na and 1.8 for Cl/Rb. The effect of Na and Rb on Rb influx showed first order kinetics with K1/2 values of 5 and 4.5 mM and Hill coefficients of 0.9 and 1.2, respectively. The effect of Cl on rubidium influx showed a sigmoidal relationship with K1/2 of 20 mM and a Hill coefficient of 2.0. The effects of variations in Na and Cl concentration on short-circuit current (Isc) were also determined. The K1/2 for Na was 7 mM with a Hill coefficient of 0.9 and the K1/2 for Cl was 46 mM with a Hill coefficient of 1.9. Based on the simultaneous influx measurements, a cotransport stoichiometry of 1Na:1K:2Cl is concluded. The Hill coefficients for Cl suggest a high degree of cooperativity between Cl binding sites. Measurements of the ratio of net Na and Cl transepithelial fluxes under short-circuit conditions (using a low Na Ringer solution to minimize the passive Na flux) indicate that the Cl/Na flux ratio is approximately 2:1. Therefore, Na recycling from serosa to mucosa does not significantly contribute to the Isc. Addition of serosal ouabain (100 microM) inhibited Rb influx, indicating that Na-K-Cl cotransport is inhibited by ouabain. Summary A true clubfoot results from a flexural deformity of the distal interphalangeal joint that is characterised by a shortening of the deep digital flexor tendon musculotendinous unit. Flexural deformities are a problem not only in foals but are also responsible for the clubfoot conformation seen in mature horses. Treatment is most successful when the cause is investigated and therapy initiated as early as possible, and when the biomechanical properties of the foot are thoroughly understood. Exposure to intrauterine growth restriction (IUGR) is an important risk factor for impaired learning and memory, particularly in males. Although the basis of IUGR-associated learning and memory dysfunction is unknown, potential molecular participants may be insulin-like growth factor 1 (Igf1) and its receptor, IGF1r. We hypothesized that transcript levels and protein abundance of Igf1 and IGF1r in the hippocampus, a brain region critical for learning and memory, would be lower in IUGR male rats than in age-matched male controls at birth (postnatal day 0, P0), at weaning (P21) and adulthood (P120). We also hypothesized that changes in messenger Ribonucleic acid (mRNA) transcript levels and protein abundance would be associated with specific histone marks in IUGR male rats. Lastly, we hypothesized that IUGR male rats would perform poorer on tests of hippocampal function at P120. IUGR was induced by bilateral ligation of the uterine arteries in pregnant dams at embryonic day 19 (term is 21 days). Hippocampal Igf1 mRNA transcript levels and protein abundance were unchanged in IUGR male rats at P0, P21 or P120. At P0 and P120, IGF1r expression was increased in IUGR male rats. At P21, IGF1r expression was decreased in IUGR male rats. Increased IGF1r expression was associated with more histone 3 lysine 4 dimethylation (H3K4Me2) in the promoter region. In addition, IUGR male rats performed poorer on intermediate-term spatial working memory testing at P120. Summary White line disease occurs secondary to a hoof wall separation. Clinical signs may vary from not being lame to severe lameness with rotation of the distal phalanx depending on the extent of the disease affecting the inner hoof wall. Summary The strong association between sheared heels and a spontaneous quarter crack is hard to ignore. Although inappropriate farriery may play a role, limb conformation and the landing pattern of the horse appear to be the dominant factors causing this type of hoof capsule deformation. The importance of determining the underlying cause and implementing the appropriate farriery cannot be over emphasised when managing a quarter crack associated with a sheared heel. Stimulation of adult rat alveolar epithelial cells with terbutaline was previously shown to activate Cl- channels in the apical membrane. In this study, we show that terbutaline stimulates net transepithelial (apical-to-basolateral) Cl- absorption from 0.19 ± 0.13 to 1.43 ± 0.31 mmol × cm-2 × hr-1. Terbutaline also increases net Cl- efflux across the basolateral membrane under conditions where an outward [K+] gradient exists and the membrane voltage is clamped at zero mV. When the [K+] gradient is eliminated, the effect of terbutaline on net Cl- efflux is inhibited to the extent that no significant Cl- efflux can be detected across the basolateral membrane. RT-PCR experiments detected mRNA for three KCl cotransport isoforms (KCC1, KCC3 and KCC4) in monolayer cultures of alveolar epithelial cells. Western blot analysis using antibodies to the four cloned isoforms of KCl cotransporters revealed the presence of KCC1 and KCC4 isoforms in monolayer cultures of these cells. The role of phosphodiesterase (PDE) isoforms in regulation of transepithelial Cl secretion was investigated using cultured monolayers of T84 cells grown on membrane filters. Identification of the major PDE isoforms present in these cells was determined using ion exchange chromatography in combination with biochemical assays for cGMP and cAMP hydrolysis. The most abundant PDE isoform in these cells was PDE4 accounting for 70-80% of the total cAMP hydrolysis within the cytosolic and membrane fractions from these cells. The PDE3 isoform was also identified in both cytosolic and membrane fractions accounting for 20% of the total cAMP hydrolysis in the cytosolic fraction and 15-30% of the total cAMP hydrolysis observed in the membrane fraction. A large portion of the total cGMP hydrolysis detected in cytosolic and membrane fractions of T84 cells was mediated by PDE5 (50-75%). Treatment of confluent monolayers of T84 cells with various PDE inhibitors produced significant increases in short-circuit current (Isc). The PDE3-selective inhibitors terqinsin, milrinone and cilostamide produced increases in Isc with EC50 values of 0.6 nM, 8.0 nM and 0.5 mM respectively. These values were in close agreement with the IC50 values for cAMP hydrolysis. The effects of the PDE1-(8-MM-IBMX) and PDE4-(RP-73401) selective inhibitors on Isc were significantly less potent than PDE3 inhibitors with EC50 values of >7 mM and >50 mM respectively. However, the effects of 8-MM-IBMX and terqinsin on Cl secretion were additive, suggesting that inhibition of PDE1 also increases Cl secretion. The effect of PDE inhibitors on Isc were significantly blocked by apical treatment with glibenclamide (an inhibitor of the CFTR Cl channel) and by basolateral bumetanide, an inhibitor of Na-K-2Cl cotransport activity. These results indicate that inhibition of PDE activity in T84 cells stimulates transepithelial Cl secretion and that PDE1 and PDE3 are involved in regulating the rate of secretion. In this article we report the results of a feasibility study examining the effects of speech-language, occupational, and physical therapy services provided to children with physical or communication needs in the regular school setting. This feasibility study was conducted in preparation for a full program evaluation study. We examined: (a) the utility of goal attainment scaling for evaluating therapy services in the school setting, (b) the utility of several standardized measures in capturing change (the Arizona Articulation Proficiency Scale, the Vineland Adaptive Behavior Scales-Classroom Edition, and the School Function Assessment), (c) the utility of two satisfaction questionnaires In assessing parent and teacher satisfaction with the services, and (d) issues related to teachers', parents', and therapists' participation. The study involved collaborative goal setting between therapists, parents, and teachers of 16 children with three types of special needs: articulation difficulties, developmental coordination disorder, or cerebral palsy. The children received an average of 13 therapy sessions over a four- to five-month period during the school year. The study provides useful information about the nature of important outcomes for school-based therapy services (i.e., functional outcomes in the areas of articulation, productivity, and mobility in the school environment) and methods by which to measure these outcomes. It also provides preliminary evidence of the utility of school-based, functional therapy services for children with special needs and indicates that focusing on functional goals is a worthwhile approach that leads to improvements in children's functioning in the school setting. Enterocytes from the intestinal epithelium of the winter flounder were isolated by collagenase digestion and incubated in flounder Ringer solution. Conventional whole-cell and amphotericin-perforated whole-cell recording techniques were used to characterize the properties of a voltage-activated K current present in dissociated cells. Resting membrane potentials and series resistances were significantly lower (from -23 to -39 mV and 29 to 13 M omega, respectively) when amphotericin was used to achieve the whole-cell configuration. When cells were placed in flounder Ringer solution, held at -80 mV and subsequently stepped to a series of depolarizing voltages (from -70 to 0 mV), an outward current was observed that exhibited inactivation at voltages above -20 mV. This current was sensitive to holding potential and was not activated when the cells were held at -40 mV or above. When cells were bathed in symmetric K Ringer solution and the same voltage protocol was applied to the cell, inward currents were observed in response to the negative intracellular potentials. Reversal potentials at two different extracellular K concentrations were consistent with K as the current-carrying ion. BaCl2 (2 mM) and CsCl (0.5 mM) both produced voltage-dependent blockade of the current when added to the bathing solution. Charybdotoxin (300 nM extracellular concentration) completely blocked the current. The IC50 for charybdotoxin was 50 nM. Cyclic GMP inhibited the voltage-activated current in flounder Ringer and in symmetric K Ringer solution. Abstract Background This qualitative study examines parent and child experiences of participation in a multi-component community-based program aimed at reducing offending behaviour, and increasing social competence in boys 6 to 11 years old in Hamilton, Ontario, Canada. The program builds on the concept of crime prevention through social development, and includes structured groups for the identified boy, parents, and siblings. Methods A sample of 35 families participating in the multi-component program took part in the qualitative study. Individual interviews with the boys, parents and siblings asked about changes in themselves, relationships with family and peers, and school after the group. Interviews were taped, transcribed and content analysis was used to code and interpret the data. Results Parents reported improvement in parenting skills and attainment of more effective communication skills, particularly with their children. Parents also found the relationships they formed with other parents in the program and the advice that they gained to be beneficial. Boys who participated in the program also benefited, with both parents and boys reporting improvements in boys' anger management skills, social skills, impulse control, and ability to recognize potentially volatile situations. Both parents and boys described overall improvement in family relationships and school-related success. Conclusions The qualitative data revealed that parents and boys participating in the multi-component program perceived improvements in a number of specific areas, including social competence of the boys. This has not been demonstrated as clearly in other evaluations of the program.
