Augmentative and alternative communication (AAC) requires on-going support but few studies describing AAC training for healthcare professionals were found. We developed a short-term education program on amyotrophic lateral sclerosis communication support for undergraduate students in multiple healthcare disciplines. The program comprised lectures plus practice of three AAC methods. Participants were 58 students from four courses at four Japanese universities who completed the program twice, at 6-month intervals. Effects of the program on 'Beginners' (taking the program for the first time) and 'Experienced' (repeating the course 6 months later) were evaluated using pre-test/post-test scores, number of letters transmitted in 5 min, subjective burden indicated on visual analogue scale (VAS) before and after each AAC trial, and free-text comments. An increase in scores was shown from the first to the second pre-test. Number of letters generated in 5 min was higher for the Experienced group than for Beginners with the exception of one AAC device. VAS change rate for the Kuchimoji method was lower in the Experienced group than for Beginners. Text-mining of participants' comments showed lower eyestrain reported in the Experienced group. This study suggests that participants' knowledge of AAC for communication support of people with ALS was retained 6 months after completing the short training program, and hands-on AAC experience helped them develop their skills and reduce the perceived subjective burden. Abstract Background We constructed a concise multidimensional care burden scale that reflects circumstances unique to Japan, with a focus on intractable neurological diseases. We surveyed 646 family caregivers of patients with intractable neurological diseases or stroke using 28 preliminary care burden scale items obtained from qualitative research. The results were used to finalize the feeling of care burden scale (BIC: burden index of caregivers), and verify its reliability and validity. Methods The survey was conducted among caregivers providing home health care to patients with intractable neurological diseases (PD [Parkinson's disease], SCD [spinocerebellar degeneration], MSA [multiple system atrophy], and ALS [amyotrophic lateral sclerosis]) or CVA (cerebrovascular accident) using a mailed, self-administered questionnaire between November, 2003 and May, 2004. Results Response rates for neurological and CVA caregivers were 50% and 67%, respectively, or 646 in total (PD, 279; SCD, 78; MSA, 39; ALS, 30; and CVA, 220). Item and exploratory factor analyses led to a reduction to 11 items, comprising 10 items from the 5 domains of time-dependent burden, emotional burden, existential burden, physical burden, and service-related burden; and 1 item on total burden. Examination of validity showed a moderate correlation between each domain of the BIC and the SF-8 (Health related quality of life scale, Short Form-8), while the correlation coefficient of the overall BIC and CES-D was 0.62. Correlation between the BIC and ZBI, a preexisting care burden scale, was high (r = 0.84), while that with the time spent on providing care was 0.47. The ICC (Intraclass correlation coefficient) by test-retest reliability was 0.83, and 0.68 to 0.80 by individual domain. Conclusion These results show that the BIC, a new care burden scale comprising 11 items, is highly reliable and valid.

Introduction: Mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) is reported to be a secondary cause of chronic intestinal pseudo-obstruction (CIPO). Although few case reports have described CIPO in MELAS, effective treatment for CIPO has not been established. Here, we present a case report of amelioration of CIPO symptoms using acotiamide in a patient with MELAS. Case Presentation: A 51-year-old Japanese female with a mitochondrial disorder with m.3243A>G mutation and a history of anorexia for 2 years presented to our hospital with a left temporal headache and acute paraphasia. A stroke-like episode of MELAS was suspected and combined therapy with arginine, edaravone, and levetiracetam was initiated. Although her symptoms improved, she presented with nausea and vomiting and abdominal distension 6 days following admission. Abdominal contrast-enhanced computed tomography revealed dilatation from the stomach to the intestine, particularly marked the stomach, with neither obstruction nor impaired blood flow in the intestine. CIPO exacerbation with MELAS was suspected, and the patient’s symptoms gradually improved with acotiamide. Conclusion: Patients with MELAS could possibly experience stroke-like episodes during CIPO. Since acetylcholine possibly plays an important role in the pathophysiology of CIPO and acotiamide possesses prokinetic activity by inhibiting acetylcholinesterase, acotiamide could possibly improve CIPO symptoms. We aimed to review the incidence of pressure ulcers in patients with amyotrophic lateral sclerosis (ALS) on admission at a teaching hospital in Mie, Japan. 592 patients admitted to the neurological ward of Mie University Hospital from 1 April 2004 to 31 March 2006 were reviewed. A retrospective analysis was conducted based on medical and nursing records about pressure ulcers among patients with ALS, parkinsonism (Parkinson's disease, multisystem atrophy, corticobasal degeneration and progressive supranuclear palsy) and other neurological diseases. 16 patients (12 males and four females) aged 36 to 87 years, 71.2 +/- 14.2 years old (mean +/- SD), were identified as having one or more pressure ulcers on admission. No patients developed a new pressure ulcer after admission. The number of patients with pressure ulcers on admission was two in 41 ALS patients, five in 126 parkinsonism patients and nine in 425 patients with other neurological diseases. A proportional analysis by χ2 test for the groups did not show a lower incidence of pressure ulcers in ALS patients. In conclusion, no differences were found in the incidence of pressure ulcers on admission among three neurological groups at a teaching hospital in Mie, Japan. [ABSTRACT FROM AUTHOR]

Long-term intake of potential zinc-chelating drugs may cause zinc deficiency. We postulated that zinc deficiency in Parkinson's disease (PD) patients was related to the intake of drugs such as levodopa. We investigated the relationship between zinc levels and levodopa administration period, dosage, and symptoms of zinc deficiency in PD patients. We measured serum zinc levels and analyzed correlations between serum zinc levels, the levodopa oral administration period, dosage, dosing frequency, and zinc deficiency symptoms including taste disorders. Data analyses were performed using Spearman's rank correlation coefficient. The mean serum zinc level was 60.5 ± 11.6 μg/dL. The mean administration period for levodopa was 8.0 ± 5.5 years, mean administration frequency 3.4 ± 0.9 times/d, and mean administration dose 420.6 ± 237.1 mg/d. Negative correlations between zinc levels and levodopa dosage and dosing frequency were found. Multiple regression analysis showed a significant correlation with the frequency of levodopa (β = −0.360, p = 0.007). No significant change in clinical symptoms was observed after zinc administration, but anxiety tended to improve. Our results indicated that frequent levodopa administration strongly influenced serum zinc levels which may have alleviating effects on psychiatric symptoms; therefore, preventing zinc deficiency can be important during PD treatment. A letter to the editor about the report of an adult case of megalencephalic leukoencephalopathy with subcortial cysts (MLC) with S93L mutation in MLC1 gene is presented.

Objective: To report neuropathologic features of argyrophilic inclusions in the anterior horn cells, motor cortex Betz cells, and neurons of the medullary reticular formation, spinal posterior horn, and Clarke column in a Japanese case of familial amyotrophic lateral sclerosis with I113T substitution in exon 4 of the copper-zinc superoxide dismutase (SOD1) gene. Methods and Results: These inclusions were stained pale pink on the hematoxylin-eosin stain and dark on the Bielschowsky stain. They were positive for antibodies to phosphorylated neurofilaments, ubiquitin, and SOD1. On electron microscopy, they consisted of abundant intermediate filaments of 10 to 20 nm in diameter with disordered array indicating neurofilaments. Conclusion: These findings suggest that the I113T mutation induces accumulation of neurofilaments and SOD1 in the central nervous system neurons. [ABSTRACT FROM AUTHOR]

Background: Sialorrhea, the excessive flow of saliva from the mouth, causes distress in about half of patients with amyotrophic lateral sclerosis (ALS). Treatments of sialorrhea in ALS include systemic anticholinergic drugs, amitriptyline, botulinum toxin injection, and salivary gland radiotherapy, although each has limitations. Scopolamine transdermal patches have been used to prevent motion sickness since the 1980s but have also been used to treat sialorrhea in oropharyngeal disease, cerebral palsy, and Parkinson’s disease. To date, no blinded, controlled studies of sialorrhea in ALS have been reported. Methods: A crossover, double-blind comparative study was conducted by randomly assigning patients to receive scopolamine or placebo patches for 1 week. Results: A total of 10 patients (three males and seven females; mean age 71.6 years) were enrolled. The mean volume of daily oral suction was decreased with scopolamine treatment. However, there were no significant differences between scopolamine and placebo in terms of a visual analogue scale of sialorrhea severity and difficulty, and the saliva item of the ALS Functional Rating Scale-Revised (ALSFRS-R). Conclusions: Our findings suggest that scopolamine patches might decrease saliva production and relieve sialorrhea in some patients with ALS.
