A field experiment was undertaken to identify profitable land use based on productivity, net present value, biomass and carbon stocks in litchi based land uses (LBLUs) during 1995–2015 established on degraded riverbed lands of western Himalaya, India. Results of LBLUs (20 years) revealed that LCT + LH + LPG land use produced 6.10–25.27%, 5.82–14.92% and 3.45–13.62% more fruit yield, 14.61–25.22%, 10.18–16.38% and 9.10–16.62% more total biomass, 2.95–9.51%, 0.03–4.54% and 1.60–8.26% higher carbon in soil and 6.93–14.76%, 4.60–10.03% and 6.29–13.14% more total carbon stocks as compared to sole litchi land use of 1st, 2nd and 3rd phase, respectively. Similarly, 80.39% and 78.56% more NPV, 11.74% and 19.77% higher BCR with shortest payback period (5 years) were realized in LCT + LH + LPG land use for 20 and 50 years, respectively. Sensitivity analysis also revealed that changes in discount rate will not affect its performance ranking.

Background Quantitative magnetization transfer imaging (qMTI) 1 is an efficient technique that is sensitive to the density of the macromolecular protons which are found mainly in white matter myelin. Objective The aim of this study is to investigate the statistical significance of complementary qMTI parameters including magnetization transfer rate (Ksat) 2 and longitudinal relaxation time under MT saturation pulse (T1sat) 3 compared to magnetization transfer ratio (MTR) 4 and longitudinal relaxation time (T1) 5 in relapsing remitting multiple sclerosis (RRMS) 6 patients. Methods In this study, conventional MRI protocols as well as MT-3DSPGR imaging were performed on twelve clinically definite RRMS patients and twelve age-matched controls. Two general methodologies were utilized for qMTI calculations, including global volumetric measurement of MTI-T1 parameters using a pixel-based analysis method throughout the brain white matter and the regional assessments within classified MS lesions with respect to their hypointensities on T1-weighted scans. MTI-T1 parameters of segmented lesions and normal appearing white matter (NAWM) 7 in RRMS patients were evaluated in comparison with normal white matter (NWM) 8 of control subjects. Results Significant reductions of MTR and Ksat values in isointense ( p 0.0001 ), mild ( p 0.0001 ) and severe hypointense lesions in comparison with NAWM on T1-weighted scans, were associated with considerable increases in both tissue relaxation times with and without MT pulse, T1sat ( p 0.0001 ) and T1 ( p 0.0001 ) respectively. Similar significant changes of quantitative MT imaging parameters and T1 values were also seen in non-lesion areas of NAWM in MS patients compared to those of NWM in healthy volunteers. Conclusion Complementary qMT imaging parameter contributes to the magnetization transfer phenomenon including Ksat and T1sat can provide more reliable information about the magnitudes and mechanisms of myelin destruction in multiple sclerosis compared to the conventional MTR and T1 relaxation time values.

Erosion by water reduces soil quality and hence crop yield. Understanding the response of crop yields to soil erosion is vital in assessing agriculture’s vulnerability to erosion. However, these effects are difficult to quantify. The study presents a quantitative relationship between soil erosion and soil quality and productivity of rainfed wheat (Triticum aestivum) by comparing field plots with different degrees of erosion in some sub-tropical alfisols in the Doon Valley region of India. By comparing the topsoil depth with the reference site as the control, erosion severity was classified into different phases such as slight, moderate, severe, and very severe. A quantitative, weighted additive model was used to evaluate soil quality for different phases of erosion using soil clay content, water holding capacity, soil aggregate, soil organic carbon, pH, CEC, total N, available P, and available K. The synthesis of long-term experimental data revealed that the mean soil erosion rate varied from 5.5 Mg ha−1 yr−1 in slightly eroded plots to 33.4 Mg ha−1 yr−1 in very severely eroded plots. Compared with the reference forest, the soil organic carbon (SOC) declined by 81.4% and water holding capacity by 31% in severely eroded soils. A substantial loss of total N, extractable P, and available K was also observed. Water stable aggregates (WSA) decreased from 86% to 12.6%, and the cation exchange capacity (CEC) from 25 to 12.6 c mol(+) kg−1. The soil quality index was 0.7 in slightly eroded compared with 0.4 in severely eroded soil. Similarly, the sustainable yield index for wheat was 0.9 and 0.6 for slightly and severely eroded soils, respectively. Thus, there is a strong need to identify land management systems that reduce erosion risks, restore eroded soils, and enhance soil quality.

The impacts of the resource conservation practices can be evaluated in short-term through carbon management index (CMI) that conceptualizes the carbon lability and soil aggregation. To test this hypothesis, we investigated the labile organic carbon (LOC) pool and CMI in relation to runoff, soil loss and maize-wheat system productivity on 2% sloping crop lands of Indian Himalayan region in a study (2009–2014) where different nutrient management practices were adopted. Results showed that all integrated nutrient management (INM) practices (mineral fertilizers with different organic sources like farmyard manure (FYM), vermi-compost (VC), green manure (GM) and poultry manure (PM) enhanced soil aggregation compared with only mineral fertilization (NPK). Among all the treatments, the highest water-stable macroaggregates (+25%) in the 0–5 cm soil layer were recorded in 50% NPK + 50% FYM (7.5 t ha−1) treatment. Land area is dwindling due to degradation, both natural and anthropogenic, therefore, it is reasonable to save this important resource and put focus on achieving land degradation neutrality (LDN). An Investigation was taken to know the impact of erosion on soil physico-chemical and biological properties both in fertilized and unfertilized conditions in four phases of erosion viz., slightly eroded (0.5% slope), moderately eroded (2.5% slope), severely eroded (4.5% slope) and very severely eroded (9.5% slope). Productivity index (PI), vulnerability curve, soil loss tolerance limits (STL), soil quality index (SQI) and relative soil quality index (RSQI) were estimated for all phases of erosion. As the severity of erosion increases the soil quality and productivity index (PI) decreases. Very severely eroded soils have lower PI, SQI and RSQI as compared to slightly eroded soils. However, land use under fertilized condition have better soil quality with SQI value of 143.00 and 118.00 in slightly eroded and very severely eroded soils, respectively, compared to 126.00 and 106.00 in non-fertilized condition. Similarly, the productivity index ranged 0.748 in slightly eroded phase to 0.651 in severely eroded phase under fertilized condition. Compared to slightly eroded soil, severely and very severely eroded soils have lower microbial biomass carbon (MBC), CO2 evolution and dehydrogenase activities. Assessment of soil loss tolerance limit based on PI revealed that it ranged from 6.67 to 12.92 Mg ha−1 year−1 which implies that land degradation took place in the more eroded soils which needs higher priority in order to bringing these eroded land to degradation neutrality (LDN), and also requires suitable soil conservation interventions in different phases of erosion.

Humankind faces the need to achieve sustainable agriculture production, meanwhile increasing crop yields and reducing soil and water losses. Soil conservation through intercropping or crop canopy management is widely accepted as one of the ways of diversifying crop yields in rainfed agriculture in sloping landscapes. Field experiments were conducted between 2009 and 2014 to evaluate the effects of one or two rows of cowpea/okra intercropped with maize (planted either in 90 or 150 cm row spacing) on productivity, profitability, and resource conservation on 4% sloping crop land in the Himalayas. During five years of experimentation, a total of 110 runoff events were observed in the maize crop grown in rainy months of June to September. The results showed that by growing one row of cowpea in between two rows of maize (90 × 20 cm), no effect was observed on the productivity of rainfed maize. Productivity of the succeeding wheat crop was enhanced by 13% which resulted in a higher net return (117 US$ ha−1) than in a maize-wheat system. This system also reduced runoff and soil loss by 26% and 43%, respectively, compared to only a maize cropping system. Regression analysis revealed as runoff in maize crop increases, grain yield of succeeding rainfed wheat crop decreases due to the less availability of soil moisture.

Markov chain and 3-dimensional log-linear models were attempted to model drought class transitions derived from the newly developed drought index the Standardized Precipitation Evapotranspiration Index (SPEI) at a 12 month time scale for six major drought prone areas of India. Log-linear modelling approach has been used to investigate differences relative to drought class transitions using SPEI-12 time series derived form 48 yeas monthly rainfall and temperature data. In this study, the probabilities of drought class transition, the mean residence time, the 1, 2 or 3 months ahead prediction of average transition time between drought classes and the drought severity class have been derived. Seasonality of precipitation has been derived for non-homogeneous Markov chains which could be used to explain the effect of the potential retreat of drought. Quasi-association and Quasi-symmetry log-linear models have been fitted to the drought class transitions derived from SPEI-12 time series. The estimates of odds along with their confidence intervals were obtained to explain the progression of drought and estimation of drought class transition probabilities. For initial months as the drought severity increases the calculated odds shows lower value and the odds decreases for the succeeding months. This indicates that the ratio of expected frequencies of occurrence of transition from drought class to the non-drought class decreases as compared to transition to any drought class when the drought severity of the present class increases. From 3-dimensional log-linear model it is clear that during the last 24 years the drought probability has increased for almost all the six regions. The findings from the present study will immensely help to assess the impact of drought on the gross primary production and to develop future contingent planning in similar regions worldwide.

Tree canopies partition rainfall into throughfall (TF), stemflow (SF), and interception loss (I) which have very significant effect on water balance and the nutrient cycle. In this study, we compared throughfall, stemflow and interception in Morus alba managed under different canopy management practices/treatments viz., coppicing, pollarding and lopping. TF was measured using manual gauges randomly placed beneath the canopies of each treatment. SF was collected from selected trees in each canopy management practices using SF collection collars. Measurements were made on a rainfall event basis. During the study period, 30 rainfall events were identified in the year 2014 and 25 in the year 2015 which delivered a total rain of 685.1 mm and 467.9 mm, respectively in the two consecutive years. Average TF ranged from 80.8% in lopping to 85.1% in coppicing. Relationship of gross rainfall versus throughfall in different canopy management practices indicates that high TF production was measured during high rainfall events and low TF production was measured during low rainfall events. Average SF values showed significant difference between lopping (10%) and pollarding (4.5%) treatments. Both the treatments transmitted less SF as GR event depths increased. As the size of rainfall events increased, intercepted GR by the canopies, decreased. Allometric model was used for fitting the gross rainfall versus SF depth. Total rainfall interception was 9.2% for lopping, 13.0% for pollarding and 14.9% for coppicing. Interception values were higher during small sized GR events.

Pinus gerardiana Wall. is an important ecological and economic forestry species having restricted distribution in India. It has been observed that natural regeneration of the species is extremely poor or entirely lacking. This species has erratic and infrequent seed years and dormancy related problems that also reduce regeneration process in natural habitats. Therefore, different treatments of gibberellic acid and temperature were tried to enhance the germination in the species by subjecting seeds to two gibberellic acid concentrations (75 and 150 ppm) along with control, following four soaking periods (Control, 8, 16 and 24 h) and two incubation temperatures (15 and 25 °C) for assessing their impact on seed germination. It was observed that among different gibberellic acid concentrations highest germinability parameters were observed, when seeds were treated with 75 ppm GA3. Among different soaking periods highest germination was observed when seed were soaked for 24 h. The incubation temperature showed better results at 15 °C in comparison to 25 °C. The present study identified gibberellic acid treatment of 75 ppm, seed soaking for 24 h at 15 °C incubation temperature best treatment for enhancing germination.

On farm bio-resource recycling has been given greater emphasis with the introduction of conservation agriculture specifically withclimate change scenarios in the mid-hills of the north-west Himalaya region (NWHR). Under this changing scenario, elevation, slope aspect and integrated nutrient management (INM) may affect significantly soil quality and crop productivity. A study was conducted during 2009–2010 to 2010–2011 at the Ashti watershed of NWHR in a rainfed condition to examine the influence of elevation, slope aspect and integrated nutrient management (INM) on soil resource and crop productivity. Two years of farm demonstration trials indicated that crop productivity and soil quality is significantly affected by elevation, slope aspect and INM. Results showed that wheat equivalent yield (WEY) of improved technology increased crop productivity by ∼20%–37% compared to the conventional system. Intercropping of maize with cowpea and soybean enhanced yield by another 8%–17%. North aspect and higher elevation increased crop productivity by 15%–25% compared to south aspect and low elevation (except paddy). Intercropping of maize with cowpea and soybean enhanced yield by another 8%–15%. Irrespective of slope, elevation and cropping system, the WEY increased by ∼30% in this region due to INM technology. The influence of elevation, slope aspect and INM significantly affected soil resources (SQI) and soil carbon change (SCC). SCC is significantly correlated with SQI for conventional (R2 = 0.65*), INM technology (R2 = 0.81*) and for both technologies (R2 = 0.73*). It is recommended that at higher elevation. (except for paddy soils) with a north facing slope, INM is recommended for higher crop productivity; conservation of soil resources is recommended for the mid hills of NWHR; and single values of SCC are appropriate as a SQI for this region.

Grewia optiva Drummond is one of important agroforestry tree species grown by the farmers in the lower and mid-hills of western Himalaya. Different models viz., monomolicular, logistic, gompetz, allometric, rechards, chapman and linear were fitted to the relationship between total biomass and diameter at breast height (DBH) as independent variable. The adjusted R2 values were more than 0.924 for all the seven models implying that all models are apparently equally efficient. Out of the six non-linear models, allometric model (Y = a × DBH b ) fulfils the validation criterion to the best possible extent and is thus considered as best performing. Biomass in different tree components was fitted to allometric models using DBH as explanatory variable, the adjusted R2 for fitted functions varied from 0.872 to 0.965 for different biomass components. The t values for all the components were found non-significant (p > 0.05), thereby indicating that model is valid. Using the developed model, the estimated total biomass varied from 6.62 Mg ha−1 in 4 year to 46.64 Mg ha−1 in 23 year old plantation. MAI in biomass varied from 1.66–2.05 Mg ha−1 yr−1. The total biomass carbon stocks varied from 1.99 Mg ha−1 in 4 year to 15.27 Mg ha−1 in 23 year old plantation. Rate of carbon sequestration varied from 0.63–0.81 Mg ha−1 yr−1. Carbon storage in the soil up to 30 cm soil depth varied from 25.4 to 33.6 Mg ha−1.

Despite the wide application of gadolinium as a contrast agent for magnetic resonance imaging (MRI), there is a serious lack of information on its toxicity. Gadolinium and gadolinium oxide (Gd-oxide) are used as contrast agents for magnetic resonance imaging (MRI). There are methods for reducing toxicity of these materials, such as core nanoparticles coating or conjugating. Therefore, for toxicity evaluation, we compared the viability of commercial contrast agents in MRI (Gd-DTPA) and three nanoparticles with the same core Gd2O3 and small particulate gadolinium oxide or SPGO (< 40 nm) but different coatings of diethyleneglycol (DEG) as Gd2O3-DEG and methoxy polyethylene glycol-silane (mPEG-silane: 550 and 2000 Dalton) as SPGO-mPEG-silane550 and SPGO-mPEG-silane2000, respectively, in the SK-MEL3 cell line, by light microscopy, MTT assay using 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyl tetrazolium bromide, and the LDH assay detecting lactate dehydrogenase activity. The viability values were not statistically different between the three nanoparticles and Gd-DTPA. The MTT and LDH assay results showed that Gd2O3-DEG nanoparticles were more toxic than Gd-DTPA and other nanoparticles. Also, SPGO-mPEG-silane2000 was more biocompatible than other nanoparticles. The obtained results did not show any significant increase in cytotoxicity of the nanoparticles and Gd-DTPA, neither dose-dependent nor time-dependent. Therefore, DEG and PEG, due to their considerable properties and irregular sizes (different molecular weights), were selected as the useful surface covering materials of nanomagnetic particles that could reveal noticeable relaxivity and biocompatibility characteristics.

Purpose: The accurate prediction of x-ray spectra under typical conditions encountered in clinical x-ray examination procedures and the assessment of factors influencing them has been a long-standing goal of the diagnostic radiology and medical physics communities. In this work, the influence of anode surface roughness on diagnostic x-ray spectra is evaluated using MCNP4C-based Monte Carlo simulations. Methods: An image-based modeling method was used to create realistic models from surface-cracked anodes. An in-house computer program was written to model the geometric pattern of cracks and irregularities from digital images of focal track surface in order to define the modeled anodes into MCNP input file. To consider average roughness and mean crack depth into the models, the surface of anodes was characterized by scanning electron microscopy and surface profilometry. It was found that the average roughness (R{sub a}) in the most aged tube studied is about 50 {mu}m. The correctness of MCNP4C in simulating diagnostic x-ray spectra was thoroughly verified by calling its Gaussian energy broadening card and comparing the simulated spectra with experimentally measured ones. The assessment of anode roughness involved the comparison of simulated spectra in deteriorated anodes with those simulated in perfectly plain anodes considered as reference. From thesemore » comparisons, the variations in output intensity, half value layer (HVL), heel effect, and patient dose were studied. Results: An intensity loss of 4.5% and 16.8% was predicted for anodes aged by 5 and 50 {mu}m deep cracks (50 kVp, 6 deg. target angle, and 2.5 mm Al total filtration). The variations in HVL were not significant as the spectra were not hardened by more than 2.5%; however, the trend for this variation was to increase with roughness. By deploying several point detector tallies along the anode-cathode direction and averaging exposure over them, it was found that for a 6 deg. anode, roughened by 50 {mu}m deep cracks, the reduction in exposure is 14.9% and 13.1% for 70 and 120 kVp tube voltages, respectively. For the evaluation of patient dose, entrance skin radiation dose was calculated for typical chest x-ray examinations. It was shown that as anode roughness increases, patient entrance skin dose decreases averagely by a factor of 15%. Conclusions: It was concluded that the anode surface roughness can have a non-negligible effect on output spectra in aged x-ray imaging tubes and its impact should be carefully considered in diagnostic x-ray imaging modalities.« less

Background and aim: Recently, a new oral rehydration solution (ORS) called Resomal has been designed specifically for children with severe malnutrition. The aim of this study was to assess the effect of malnutrition on renal and intestinal responses to dehydration, and to compare intestinal water and electrolyte absorption from Resomal and from the standard World Health Organization-Oral Rehydration Solution (WHO-ORS). Methods: Malnutrition was achieved in a rabbit model by feeding the animals daily for 30 days with half the amount of food that a well-nourished group of control animals had consumed on the previous day. Dehydration was achieved by water deprivation for 46 hours in both control and malnourished rabbits. At 46 hours, dehydration was assessed by changes in body weight, urinary volume and osmolarity, and blood urea nitrogen concentration. At that time active colonic and jejunal mucosal electrolyte transport in Ussing chambers was also measured. Small intestinal absorption of water, sodium, and potassium was measured in vivo during intestinal perfusion of the two ORSs and in vitro by measurement of mucosal electrogenic glucose-stimulated sodium absorption across intestinal patches. Results: Compared to controls (C), well-nourished but dehydrated (C+D) animals lost 12% of their body weight, with an 87% reduction in urine volume, a 110% increase in urine osmolality, and a 94% increase in blood urea nitrogen. In the colon of C+D animals, short-circuit current (Isc) and net sodium transepithelial flux (JNa + net) were increased. Almost identical results were obtained in malnourished and dehydrated (M+D) animals. In the jejunum, net in vivo absorption of water (JWater), sodium (JNa + ), and potassium (JK + ) were increased during standard ORS infusion in both dehydrated groups. During Resomal infusion, water absorption was the same as seen with WHO-ORS, but sodium absorption was reduced, and potassium absorption was increased in both well-nourished and malnourished dehydrated animals. In vitro, compared to controls, the glucose-stimulated short-circuit current (ΔIsc), JNa + net and G were increased in both dehydrated groups. Conclusion: During experimental dehydration, the kidney and large intestine salvage water and electrolytes, thus reducing the consequences of dehydration. These findings indicate that jejunal water absorption from Resomal and WHO-ORS is increased during dehydration, but Resomal allows for less sodium and more potassium to be absorbed, both in well-nourished and malnourished dehydrated rabbits.

Exonuclease 1 (EXO1) is a candidate gene for colorectal tumor susceptibility because it is believed to play a role in mismatch repair. There have been several studies investigating the role of EXO1 in mismatch repair but few investigating its role in causing clinical disease. In one recent study, germline variants of EXO1 were reported to be associated with predisposition to colorectal cancer in families with phenotypes similar to hereditary nonpolyposis colon cancer (HNPCC). We recently identified nine individuals from two British families with multiple cutaneous and uterine leiomyomatosis with independently arising heterozygous germline deletions of 1q42.3 approximately q43 encompassing not only FH, the multiple leiomyomatosis-associated gene, but also several flanking genes, including EXO1. We investigated these families for any indication of predisposition to colorectal cancer or other HNPCC spectrum cancers by means of detailed questionnaires, interviews, and examination of EXO1-null skin leiomyomata for microsatellite instability (MSI). No individual in these families had developed colorectal cancer or known colorectal adenomas, and none had any symptoms warranting gastrointestinal or other investigation. EXO1-null tumors showed no evidence of MSI. This study questions the functional significance of previously reported variants of EXO1 reported in HNPCC-like families and suggests that in humans there may be other as yet undiscovered proteins that have exonuclease function overlapping with that of EXO1 in DNA mismatch repair. Also of interest is the absence of phenotypic abnormality apart from multiple leiomyomatosis in any deletion carrier even though the adjacent genes RGS7, KMO, CHML, and OPN3 were also deleted.

Germline mutations of the fumarate hydratase (FH, fumarase) gene are found in the recessive FH deficiency syndrome and in dominantly inherited susceptibility to multiple cutaneous and uterine leiomyomatosis (MCUL). We have previously reported a number of germline FH mutations from MCUL patients. In this study, we report additional FH mutations in MCUL and FH deficiency patients. Mutations can readily be found in about 75% of MCUL cases and most cases of FH deficiency. Some of the more common FH mutations are probably derived from founding individuals. Protein-truncating FH mutations are functionally null alleles. Disease-associated missense FH changes map to highly conserved residues, mostly in or around the enzyme's active site or activation site; we predict that these mutations severely compromise enzyme function. The mutation spectra in FH deficiency and MCUL are similar, although in the latter mutations tend to occur earlier in the gene and, perhaps, are more likely to result in a truncated or absent protein. We have found that not all mutation-carrier parents of FH deficiency children have a strong predisposition to leiomyomata. We have confirmed that renal carcinoma is sometimes part of MCUL, as part of the variant hereditary leiomyomatosis and renal cancer (HLRCC) syndrome, and have shown that these cancers may have either type II papillary or collecting duct morphology. We have found no association between the type or site of FH mutation and any aspect of the MCUL phenotype. Biochemical assay for reduced FH functional activity in the germline of MCUL patients can indicate carriers of FH mutations with high sensitivity and specificity, and can detect reduced FH activity in some patients without detectable FH mutations. We conclude that MCUL is probably a genetically homogeneous tumour predisposition syndrome, primarily resulting from absent or severely reduced fumarase activity, with currently unknown functional consequences for the smooth muscle or kidney cell.

Effective utilization of land and water resources is attempted in the present study through an integrated farming system and multi-objective optimization management framework model using goal programming algorithm in a coastal waterlogged paddy area in Odisha, India. A methodology is developed to identify the water harvesting structure locations in the study area using spatial science tool. Due to the uncertainty of parameters and control variables, development of management framework was considered with 85% and 75% probability of rainfall occurrence and runoff generation. To incorporate the uncertainties, a multi-objective linear goal programming optimization model is developed considering the objective of maximizing the net annual return and production subject to optimal allocation of land. While evaluating the model for different water resources scenarios, the net annual return is found to be Rs 4,343,474 and maximum production is 10,424 q from scenario I, whereas maximum production of 10,980 q is obtained in scenario II. Tomato and rice cultivation area increased from 11.47 to 21.43 ha and 8.82 to 10.48 ha respectively in scenario II. The developed methodology shows the potential applicability in similar farming situations in other areas.

Dominant transmission of multiple uterine and cutaneous smooth-muscle tumors is seen in the disorder multiple leiomyomatosis (ML). We undertook a genomewide screen of 11 families segregating ML and found evidence for linkage to chromosome 1q42.3-q43 (maximum multipoint LOD score 5.40). Haplotype construction and analysis of recombinations permitted the minimal interval containing the locus, which we have designated "MCUL1," to be refined to an approximately 14-cM region flanked by markers D1S517 and D1S2842. Allelic-loss studies of tumors indicated that MCUL1 may act as a tumor suppressor. Identification of MCUL1 should have wide interest, since this gene may harbor low-penetrance variants predisposing to the common form of uterine fibroids and/or may undergo somatic mutation in sporadic leiomyomata.

The concentration and the sources of heavy metals (Pb, Cd, Zn and Cu) in house dust samples of nine selected houses of Jalil Town, Gujranwala, Pakistan are determined and a comparison with the concentration of these metals in respective street dust samples is given. Sources, exterior as well as interior are identified. The extent of contribution of lead in house dust from exterior sources and interior sources is calculated by determining the isotopic ratios in house dust, street dust and paint used in the houses. It is noticed in the case of well ventilated houses, that most of the heavy metal contribution is from the exterior sources. However, in less ventilated houses, contribution from the interior sources is also significant.

Recently, it has been showed that gadolinium oxide nanoparticles can provide high-contrast enhancement in magnetic resonance imaging (MRI). Moreover, liposomes due to high biocompatibility have shown unique model systems, with the most successful application being the drug delivery system. As a suitable cell-tracking contrast agent (CA) in molecular MRI (mMRI), the synthesis and optimisation characteristic of a novel paramagnetic liposomes (PMLs) based on gadolinium nanoparticles, essentially composed of a new complex of gadolinium oxide-diethylene glycol (Gd₂O₃-DEG) loaded in liposomes have been determined in this research. Gd₂O₃-DEG was prepared by a new supervised polyol method and was encapsulated with liposome by the film hydration method. The paramagnetic liposome nanoparticle (PMLN) sizes ranged from 65 to 170 nm. The r₁ of PMLNs and Gd₂O₃-DEG were much higher than that of Gd-diethylenetriamine penta-acetic acid (Gd-DTPA). In MC/9 cell lines, the experiments showed similar results as in water. PMLNs with lower T₁ than Gd-DTPA are sensitive, positive MRI CA that could be attractive candidates for cellular and molecular lipid content targets such as diagnostic applications.

Context: Leydig cell tumors (LCTs) are the most common non-germ-cell neoplasms of the testis. LCTs are often hormonally active and can result in precocious virilization or in adult feminization. We identified an LCT in an affected individual from a kindred with hereditary leiomyomatosis and renal cell cancer (HLRCC) and a germline fumarate hydratase (FH) mutation (N64T). Objective: Our objective was to investigate the role of FH mutations in predisposition to LCTs. Design: We tested for pathogenic effects of the N64T mutation and screened an additional 29 unselected adult LCTs for FH alterations. We also tested these LCTs for mutations in two genes, the LH/choriogonadotropin receptor (LHCGR) and the guanine nucleotide-binding protein α (GNAS) that had been implicated in LCT tumorigenesis. Results: No mutations were found in GNAS, and one tumor had a LHCGR somatic substitution. In addition to the HLRCC case with the N64T germline FH mutation, we identified one other LCT with a previously unreported FH mutation (M411I). Both LCTs from these patients showed loss of the wild-type FH allele. Immunohistochemical and in situ hybridization analyses demonstrated activation of the hypoxia/angiogenesis pathway not only in the tumors belonging to the FH mutation carriers but also in several other mutation-negative LCTs. Conclusions: Our study shows that some LCTs are caused by FH mutations and represents one of the first reports of germline mutations in any type of adult testicular tumor.

To compare the efficacy and safety of low osmolar oral rehydration salts solution (ORS-75) (mmol/L: Na+ 75, osmolarity 245) with that of World Health Organization-recommended ORS (ORS-90) (mmol/L: Na+ 90, osmolarity 311 ) in the treatment of acute watery diarrhoea in neonates and very young infants, a randomized double-blind, controlled clinical trial was carried out at the Clinical Research and Service Centre of ICDDR,B: Centre for Health and Population Research, Dhaka, Bangladesh, during January 1998-December 1999. Infants, agedor = 2 months, presenting with a history of watery diarrhoea ofor = 72 hours, with no or some dehydration and without any systemic illness, were randomly assigned to receive either ORS-75 or ORS-90 for the correction and subsequent prevention of dehydration. Infants were studied for a maximum of five days. Total stool output, stool frequency, and requirement for ORS were outcome measures. Serum electrolytes were measured at 24 hours after admission to monitor serum sodium imbalance. Seventy-three infants received ORS-75, and 71 received ORS-90. Both the groups were comparable in their baseline characteristics. Diarrhoea resolved within five days in 53% and 66% of infants receiving ORS-75 and ORS-90 respectively (p = 0.3). Total stool volume [median (inter-quartile range) 132 (65-280) vs 139 (70-259) g/kg, p = 0.9], during the study period, was not significantly different between the two groups. Total stool frequency [31 (16-51) vs 35 (16-53), p = 0.9] and total ORS intake [192 (96-374) vs 209 (134-317) mL/kg, p = 0.7] were similar between the groups. No infants developed late evidence of hypernatraemia, irrespective of treatment. The results of the study indicate that ORS-75 is as safe as standard ORS-90 in the treatment of acute watery diarrhoea in neonates and very young infants and is effective in correcting and preventing dehydration.

The study analyzed data from a systematic sample of children, aged less than five years, who presented with persistent diarrhoea (diarrhoea of more than 14 days duration). It aims to differentiate (a) non-severe persistent diarrhoea (with no or mild dehydration) and (b) severe persistent diarrhoea (with moderate or severe dehydration), and to identify individual characteristics associated with severe persistent diarrhoea. In total, 7,505 patients, who represented a 4% systematic sample of the patient population, were seen during January 1993-December 1995. Of them, 297 (4%) presented with persistent diarrhoea. The male:female ratio was 2:1. Eighty-three percent of them had mild or no dehydration, and 17% had moderate or severe dehydration. Severe malnutrition of the study patients defined as weight-for-age z-score-3, weight-for-length z-score-3 and length-for-age z-score-3 were 33.9%, 9.7%, and 22.7% respectively. Only 3% had oedematous malnutrition, and 11% had xerophthalmia. Factors independently associated with severe persistent diarrhoea by logistic regression analyses were: number of watery stool10 times during the last 24 hours prior to admission (OR, 10.0; CI, 1.2-87, p = 0.03), lower respiratory tract infection (OR, 111; CI, 4.2-2955, p = 0.004), and lack of mothers' education (OR, 7.8; CI, 1.4-41.9, p = 0.016) after controlling for confounders. Awareness and health education of mothers or caregivers and better case management during acute diarrhoeal episode might prevent the development of severe persistent diarrhoea in young children. In addition, children with severe persistent diarrhoea might need special attention to have adequate rehydration and control of extraintestinal infections, including respiratory tract infection.

Background Antiretroviral therapy (ART) has resulted in increasing median survival times in HIV-infected individuals by sustaining viral load suppression. Since children and adolescents are likely to have long-term exposure to ART, it is important to understand patterns of drug use to investigate the emergence of unintended sequelae. Aim To investigate patterns of ART in children and adolescents. Study Design Cross-sectional analysis of HIV-infected subjects aged 2–22 years across 15 clinical sites in Belgium, Italy and Poland. Method Prevalence of both “ever-use” and “current-use” (at recruitment) of ART drug classes: nucleoside analogue reverse transcriptase inhibitors (NRTIs); non-nucleoside analogue reverse transcriptase inhibitors (NNRTIs); and protease inhibitors (PIs), and individual drugs were investigated. Results Among 468 participants (51% female) the median age was 13.5 years (IQR 9.9–17.0) with 320(68%) of white ethnicity and 104 (22%) Black African. Overall, 291(62%) were virologically suppressed (HIV-RNA ≤50copies/ml) at enrolment and 35(7%) had severe immuno-suppression (age-stratified CD4%). Only 23 (5%) subjects were ART-naive (median age=10.0 years, IQR; 6.1–14.3); 15 had experienced moderate or severe immuno-suppression/HIV-disease, with 15 having detectable viral load at recruitment. Of the ever-treated subjects, 98% (n=436) had received zidovudine or lamivudine in the past and 72% (n=324) currently; 82% (n=368) and 72% (n=323) had been ever-exposed to PIs and NNRTIs, respectively. Median and modal lifetime number of drugs was six. Over their lifetime, 119 (27%) subjects had been exposed to ≥8 drugs (median age=15.7 years, IQR; 12.3–18.0). Age was associated with duration of total drug use (p Conclusions The majority of subjects had been managed with ART, with first exposure occurring at an early age. At least a quarter of participants had been treated with multiple individual drugs suggesting cumulative exposure and switching between regimens. The most prevalent treatment approach at recruitment was cART. However, there is evidence of continued use of suboptimal management strategies, and hypercholesterolaemia being associated with PI use.
