Thus, the present study aimed to assess the accuracy of diaphragm ultrasound compared to spirometry in the screening of restrictive pulmonary disorders in radiographic SpA patients.Methods: We conducted a cross-sectional study of 50 patients with radiographic SpA, over six months. Sociodemographic data, clinical characteristics of the disease, as well as biological, radiological, and therapeutic data, were collected. Spirometry and diaphragm ultrasound were performed.Results: The mean age of the study participants (N= 50) was 42.7± 11 years [range: 25– 66] with male predominance (N= 41). Spirometry showed a restrictive disorder in 32% of cases. The mean chest expansion (CE) value was 3.9± 1.81cm [range: 1– 9] with a median of 4 cm. A pathological value (< 5cm) was observed in 72% of cases. A significant positive correlation was found between the right inspiratory diaphragmatic thickness and forced vital capacity (FVC) (r= 0.36; p = 0.02) and the supine FVC (r=0.29; p = 0.04). The left inspiratory diaphragmatic thickness was correlated with the percentage of the FVC decrease (r= 0.35; p = 0.01) defined as the percentage of difference between FVC and supine FVC. The right expiratory diaphragmatic thickness was associated with the FVC (r=0.32; p = 0.02). A significant positive correlation was found between the CE and the presence of B lines (r=0.32; p = 0.02), but not between the CE and the FVC.Conclusion: The present study showed that diaphragm ultrasonography is correlated with spirometric findings in radiographic SpA patients. Allergic respiratory diseases are very common worldwide with an increased socioeconomic burden. Allergic immunotherapy (AIT) remains the only curative treatment of these patients. We aimed to highlight the impact of the AIT on the quality of life and on the mental state of allergic patients to house dust mites (HDM). It is a prospective study including stable HDM-allergic patients confirmed with a prick test. All the included patients had received sublingual immunotherapy (SLIT) during 3 years. We used a symptom score to assess the clinical improvement. Rhinitis quality of life questionnaire and Asthma quality of life questionnaire were used. We also screened for the psychological disorders based on the Health Anxiety and Depression (HAD) questionnaire. We enrolled 32 patients; with a mean age of 21.26 years. Thirty cases had an allergic rhinitis and 22 cases had an allergic asthma. Most of them reported a clinical improvement (80%). We noticed a significant reduction in the total symptoms score (P < 0.001). Their daily activity, practical problems and emotional scores were improved (P < 0.001). Patients with an allergic rhinitis had less sleep disorders (P < 0.001). Twenty-nine patients reported a considerable improvement of their quality of life (P < 0.001). We also noticed a reduction in their depression and anxiety levels. In fact, the total mean HAD score decreased (from 20.5 up to 11.13) (P < 0.001); with a diminished anxiety (from 10.42 up to 5.37) and depression scores (from 10.08 up to 5.75) (P < 0.001). Allergenic immunotherapy (AIT) remains the only curative treatment for IgE-mediated allergic diseases. The actual trend is to seek for accurate biomarkers to predict the clinical response. We aimed to highlight the efficiency of the sublingual immunotherapy (SLIT) in house dust mites-allergic patients. It is a prospective study including stable house dust mites (HDM)-allergic patients confirmed with a prick skin test. They were randomized in 2 groups. The first group (G1) had received SLIT with the symptomatic treatment and (G2) was put under pharmacological treatment only. We assessed SLIT efficiency using a symptom score and a medication score. The total serum specific HDM IgE and IgG4 levels were quantified. We performed a Basophil Activation Test (BAT) using the CD63 surface marker by cytometry. We included 32 patients in G1 patients with a mean age of 21 years and 13 patients with a mean age of 26.7 years in G2. A significant clinical improvement was reported in most cases (80.6%) with a reduced mean medication total score (from 6 up to 4.5). A successful interruption of the treatment was reported in 11 cases among patients with allergic rhinitis and in 2 asthma patients. The total serum specific HDM IgG4 level increased in G1 (0.33 g/L) vs. (0.004 g/L) in G2 (P < 0.001). We also observed a significant reduction of the basophiles response,with a mean of (35%) in G1. No change was observed in G2 (62%). Our study showed that the SLIT is an efficient and a safe treatment for HDM-induced allergic diseases. The Ig G4 blocking antibodies and the BAT seem to be very interesting biomarkers. [ABSTRACT FROM AUTHOR]

Introduction La prise en charge des patients asthmatiques doit permettre de concilier la meilleure efficacité thérapeutique et la meilleure acceptabilité possible du traitement par le patient. L’objectif du traitement au long cours dans l’asthme est le contrôle de la maladie asthmatique. L’association d’un β 2 mimétique à longue durée d’action inhalée (LABA) à un corticoïde inhalé (CSI) est recommandée du fait de leurs actions complémentaires. Méthodes Étude rétrospective comparative entre deux groupes de patients hospitalisés au service de pneumo-allergologie de l’HMPIT sur une période de 10 ans s’étalant entre 2005 et 2015 : un groupe traité par une association fixe LABA-CSI et l’autre groupe traité par une association libre. Résultats Au total, 114 patients asthmatiques ont été hospitalisés pendant la période d’étude. Uniquement les patients atteints d’un asthme allergique ont été inclus dans notre étude (50 cas). L’âge moyen était de 32,98 ± 10,76 ans avec une nette prédominance masculine (H = 32, F = 18 ; sex-ratio = 1,77). Une corrélation statistiquement significative n’a pas été retrouvée entre le traitement par l’association fixe et l’observance thérapeutique. En revanche, le groupe traité par l’association fixe, présentait moins de risque de switch thérapeutique ( p = 0,033). Il n’existait pas une corrélation significative entre le traitement par l’association fixe et le degré de sévérité de l’asthme ainsi que le contrôle de l’asthme. De l’autre côté, concernant le groupe traité par l’association libre, l’asthme était plus sévère ( p = 0,029) et moins contrôlé ( p = 0,016, r = −0,389). Une corrélation statistiquement significative n’a pas été retrouvée entre le traitement par l’association libre et l’observance thérapeutique ou le switch thérapeutique. Conclusion Le traitement par l’association fixe LABA-CSI garantit un risque moindre de switch thérapeutique et un meilleur contrôle de l’asthme comparativement au traitement par l’association libre. La spondylarthrite ankylosante (SpA) est un rhumatisme inflammatoire chronique fréquent qui peut donner une atteinte respiratoire restrictive évaluée classiquement par l'exploration fonctionnelle respiratoire. L'objectif de notre travail était d'étudier l'apport de l'échographie thoracique dans l'exploration du trouble ventilatoire restrictif associé à la SpA par rapport à la spirométrie. Il s'agissait d'une étude prospective, transversale, menée sur une période de 6 mois, incluant 50 patients suivis pour une spondyloarthrite axiale au service de rhumatologie. Les caractéristiques épidémiologiques, cliniques et biologiques ont été recueillies. L'échographie thoracique a été effectuée au service de pneumologie et réalisée par le même opérateur formé à cette technique. La spirométrie a été réalisée au service d'exploration fonctionnelle de pneumologie et interprétée par un investigateur non informé aux données de l'échographie. Les deux examens ont été réalisés le même jour. L'âge moyen des patients était de 42,7 ans avec une prédominance masculine. Uniquement 20 % des patients avait une symptomatologie respiratoire. La spirométrie avait montré un syndrome restrictif dans 32 % des cas. Les anomalies échographiques chez nos patients étaient: une course diaphragmatique droite diminuée à 43 mm chez un seul patient. L'épaisseur diaphragmatique en fin d'expiration était abaissée du côté droit dans 54 % des cas et du côté gauche dans 48 % des cas. Une atrophie diaphragmatique du côté droit était notée chez un seul patient (Emin D à 1,4 mm). L'épaisseur diaphragmatique inspiratoire et expiratoire à droite était plus élevée chez les hommes de façon significative. La fraction d'épaississement était pathologique dans 56 % des cas à droite et 60 % des cas à gauche. Une corrélation positive significative a été retrouvée entre l'épaisseur diaphragmatique inspiratoire droite et la CVF (p = 0,02) et la CVF couchée (p = 0,04). Une corrélation positive significative a été retrouvée entre l'épaisseur diaphragmatique inspiratoire gauche et le pourcentage de baisse de la CVF (p = 0,04). Une corrélation positive significative a été retrouvée entre l'épaisseur diaphragmatique expiratoire droite et la CVF (p = 0,02). Les corrélations entre les données cliniques, échographiques et spirométriques avaient montré que la limitation de la flexion lombaire, l'atteinte sévère du rachis cervical ainsi que l'hypercyphose dorsale entrainent une diminution de la course et de l'épaisseur diaphragmatiques ce qui entraine une diminution de la CVF et en conséquence une atteinte restrictive chez les patients SpA. Peu d'études se sont intéressées à valider la place de l'échographie diaphragmatique dans l'atteinte respiratoire au cours de la SpA. À ce jour la réalisation de l'échographie diaphragmatique dans cette indication ne figure dans aucun algorithme décisionnel validé par les sociétés savantes rhumatologiques ou pneumologiques. Sa validité, spécificité, sensibilité, valeurs prédictives positives ou négatives sont encore à étudier. [ABSTRACT FROM AUTHOR]
