Abstract

Introduction: Diabetes is prevalent in 10% of the population worldwide. 2.5% of diabetic patients develop diabetic foot ulcers every year. Negative dressing therapy, foam dressing and conventional dressing are common modalities for managing diabetic foot. The purpose of this study was to compare the outcomes of regular dressing vs. negative dressing in diabetic foot patients. Materials and Methods: This study was conducted with 100 patients between the age of 10 and 85 years as per the inclusion and exclusion criteria. Fifty patients underwent regular dressing, and 50 patients underwent negative dressings. Results: In 82% of patients, complete coverage of the wound occurs by negative dressing, and 52% of patients reported that complete coverage of the wound occurs by regular dressing after 4 weeks. Major post-operative complications were significantly less in patients treated with negative dressing. Conclusion: Negative dressing outperforms conventional dressing regarding wound covering, post-operative complications and cost-effectiveness in diabetic diet therapy. [ABSTRACT FROM AUTHOR]

Abstract

Background: There has been heightened interest in performing percutaneous lumbar interbody fusions (percLIFs) through Kambin's triangle, an anatomic corridor allowing entrance into the disc space. However, due to its novelty, there are limited data regarding the long-term benefits of this procedure. Our objective was to determine the long-term efficacy and durability of the percutaneous insertion of an expandable titanium cage through Kambin's triangle without facetectomy. Methods: A retrospective review of patients undergoing percLIF via Kambin's triangle using an expandable titanium cage was performed. Demographics, visual analog scale (VAS) scores, Oswestry Disability Index (ODI), radiographic measurements, perioperative variables, and complications were recorded. VAS, ODI, and radiographic measurements were compared with baseline using the generalized estimating equations assuming normally distributed data. Fusion was assessed with computed tomography (CT) at 1 and 2 years after the procedure. Results: A total of 49 patients were included. Spondylolisthesis, lumbar lordosis (LL), sacral slope, pelvic tilt, and anterior/posterior disc space height were all significantly improved postoperatively at each time point of 3, 6, 12, and 24 months (P < 0.001). Pelvic incidence--LL mismatch decreased significantly at each follow-up (P < 0.001) with a mean reduction of 4° by 24 months. VAS back scores reduced by >2 points at the 6, 12, and 24 month follow-ups. ODI scores reduced by >15 points at the 12- and 24-month follow-ups. Of the patients who had 1- and 2-year CT images, fusion rates at those time points were 94.4% (17/18) and 87.5% (7/8), respectively. The mean annual rate of surgically significant adjacent segment disease was 2.74% through an average follow-up of 2.74 years. Conclusion: These results highlight that percLIF, a procedure done without an endoscope or facetectomy, can be performed using an expandable titanium cage through Kambin's triangle with excellent radiographic and clinical results. Abstract

The emergence of ubiquitous computing (UbiComp) environments has increased the risk of undesired access to individuals’ physical space or their information, anytime and anywhere, raising potentially serious privacy concerns. Individuals lack awareness and control of the vulnerabilities in everyday contexts and need support and care in regulating disclosures to their physical and digital selves. Existing GUI-based solutions, however, often feel physically interruptive, socially disruptive, time-consuming and cumbersome. To address such challenges, we investigate the user interaction experience and discuss the need for more tangible and embodied interactions for effective and seamless natural privacy management in everyday UbiComp settings. We propose the Privacy Care interaction framework, which is rooted in the literature of privacy management and tangible computing. Keeping users at the center, Awareness and Control are established as the core parts of our framework. This is supported with three interrelated interaction tenets: Direct, Ready-to-Hand, and Contextual . Direct refers to intuitiveness through metaphor usage. Ready-to-Hand supports granularity, non-intrusiveness, and ad hoc management, through periphery-to-center style attention transitions. Contextual supports customization through modularity and configurability. Together, they aim to provide experience of an embodied privacy care with varied interactions that are calming and yet actively empowering. The framework provides designers of such care with a basis to refer to, to generate effective tangible tools for privacy management in everyday settings. Through five semi-structured focus groups, we explore the privacy challenges faced by a sample set of 15 older adults (aged 60+) across their cyber-physical-social spaces. The results show conformity to our framework, demonstrating the relevance of the facets of the framework to the design of privacy management tools in everyday UbiComp contexts.

Abstract

Intravitreal anti-vascular endothelial growth factor (VEGF) agents have obtained acceptance as the mainstay in the management strategy of subfoveal choroidal neovascular membranes (CNVM) due to varying etiologies. Few drawbacks include need for repeated intravitreal injections, with its adjunct risks, and the lack of a predefined treatment end point, which can cause doubts and uncertainty in the mind of the patient. Furthermore, it remains a significant financial burden for the patient. Herein we report our data of three patients who were reluctant for further re-injections of anti-VEGF agents and were therefore offered surgical removal of the CNVM by submacular surgery as an alternative treatment plan.

Abstract

Background: The global population is aging, leading to shifts in health care needs. In addition to developing technology to support physical health, there is an increasing recognition of the need to consider how technology can support emotional health. This raises the question of how to design devices that older adults can interact with to log their emotions. Objective: We designed and developed 2 novel tangible devices, inspired by existing paper-based scales of emotions. The findings from a field trial of these devices with older adults are reported. Methods: Using interviews, field deployment, and fixed logging tasks, we assessed the developed devices. Results: Our results demonstrate that the tangible devices provided data comparable with standardized psychological scales of emotion. The participants developed their own patterns of use around the devices, and their experience of using the devices uncovered a variety of design considerations. We discuss the difficulty of customizing devices for specific user needs while logging data comparable to psychological scales of emotion. We also highlight the value of reflecting on sparse emotional data. Conclusions: Our work demonstrates the potential for tangible emotional logging devices. It also supports further research on whether such devices can support the emotional health of older adults by encouraging reflection of their emotional state. [ABSTRACT FROM AUTHOR]
