Cet article explore la tendance à l'élan utopique dans ses romans récents de Léonora Miano. À la lecture des deux tomes de Crépuscule du tourment (2016-2017), de La Saison de l'ombre (2013) et de Rouge impératrice (2019), l'article montre que dans chaque cas des constructions fictionnelles pouvant être vues comme des hétérotopies (Foucault) compliquent les représentations sociales dans les romans. Bien que le masculin et le féminin soient deux principes distincts dans la pensée de Miano, cette complexité ouvre des possibilités pour une certaine fluidité de genre.

4. Where Does I Stand? Reflections on Home and Identity Ensnared in a Cultural Narrative.

In this paper the author explores a cultural narrative that she suggests rests on the concepts of the Feminine and Masculine as such, employing both as though they contain an agreed set of universal givens. These givens are extrapolated from an androcentric perspective on female and male bodies, in particular their biological functions regarding reproduction. The metaphors of the baby‐in‐womb, mother's preoccupation with child and heteronormative sexual relations are the primary cyphers for the narrative. She suggests that remaining unconscious of this narrative, such that it is taken as a universal given, can hamper a person's relation to themselves, the world and others. The author names two concepts, Home and Identity: Home being an hospitable and accommodating space with Identity denoting the one who inhabits the space. In the narrative these two are unhelpfully categorized as belonging to the Feminine and the Masculine respectively. For ease of understanding the author uses a capital letter to designate an abstract idea, and lower case when referring to the concrete or particular. Clinical examples are given throughout the paper to illustrate how acknowledgement and awareness of this narrative might free the analyst or therapist to think more broadly around issues pertaining to space and identity. This essay proposes that the conceptualization of the "sexual" in psychoanalysis must be studied in its polysemy and complexity. It aims to identify some problematic knots in Freud's and Lacan's work when claiming the relationship between sexuality and the unconscious. It examines Oedipus and the castration complex in Freud's, and the formulae of sexuation and the logic of not-all in Lacan's. Finally, it establishes some links between psychoanalysis and the thinking of authors such as Butler and Preciado, among others from the Ljubljana philosophical school, in order to argue that sexuality is an epistemic-political field of action that confronts the biopolitical and historicist approaches with the negative ontology. 7. Inner-outer couple: anima and animus revisited. New perspectives for a clinical approach in transition.

The conflicts of the outer couple (crises, separations, divorces) with a high incidence in the rates of depression, reveal the archetypal dysfunction of the relationship between the inner-couple (in-shadow) and the outer-couple (in-crisis). To understand this in depth, a new animated and integrated model of the masculine (animus) and the feminine (anima) is explored. It is a transformational model, in which Jung's contributions are integrated and understood as knowledge in transition. Masculine and feminine cosmogonic principles are personified in the human psyche as anima-animus archetypes-complexes. Their relationship, mediated by the binding eros (cosmogonic-eros), configures the inner-couple-complex. Masculine and feminine are differentiations of the Self energy, present in both men and women. In the conscious dimension, they structure the ego-anima-animus identity; in the dimension of the personal unconscious, the anima-animus complexes; and at the archetypal level, the deep identity of the primordial feminine/masculine. In this initial restructured model, the anima-animus relationship and the description of the dual quaternity as the dynamics of the inner-outer couple interrelation are especially significant. The breadth and comprehensive richness of the new model is illustrated in the clinical/dream material of a patient under analysis, presenting the dynamics: inner couple in shadow - outer couple in crisis. Réhabiliter la tendresse comme vertu, la soustraire à la hiérarchie des styles ou des âges qui, d’ordinaire, en limite l’extension, c’est une manière de promouvoir une certaine idée du masculin et un certain regard sur les rapports entre hommes et femmes.

Cet article s'intéresse aux répercussions psychiques de la « transidentité » d'un adolescent sur son père. Son objectif est d'explorer le vécu psychique de ce dernier, notamment au niveau de son identité masculine et de sa parentalité. Face au peu de recherches nationales et internationales consacrées à ce thème, contrastant avec le nombre d'études s'intéressant à la « transidentité », nous avons construit une méthodologie composée d'un entretien clinique semi-directif et d'une épreuve projective, le TAT (Thematic Apperception Test). Suite aux rencontres avec notre participant, surnommé Michel, les résultats cliniques mettent en évidence une relation père/fils dégradée. La sphère de la dimension affective semble être la plus perturbée puisque nous repérons, chez notre participant, un mouvement dépressif important. Ces données cliniques nous laissent alors penser que l'exposition d'un père à la transidentité de son fils peut provoquer des perturbations à des niveaux conscients et inconscients, tant au niveau de sa parentalité que de son éprouvé personnel. This research focuses on the psychological repercussions of a teenager's "trans-identity" with his father. Its objective is to explore the experiences of this father, particularly in terms of parenting and male identity. Faced with the limited research devoted to this theme, in contrast to studies focusing on transidentity, we have developed a methodology composed of a semi-structured interview and a projective test, the TAT. Following the meeting with Michel, the clinical results highlight a degraded father/son relationship, the sphere of the affective dimension seems to be the most disturbed since a significant depressive movement is detected in our participant. These results suggest that a father's exposure to his son's "trans-identity" can cause disturbances at both conscious and unconscious levels, affecting both his parenting and his personal experience. This study contributes to the updating of knowledge in the transgender field. In particular, it paves the way for future research. The results have shown the usefulness of continuing research in this area to improve support for parents of transgender adolescents or young adults and thus help transgender people themselves in their transition. Indeed, the studies have stressed that family involvement and support in the transition of a transgender subject is essential for his or her well-being and facilitates his or her transition. It therefore seems essential to continue studies in this field so that they contribute to improving the care and psychological support of transgender people. [ABSTRACT FROM AUTHOR]

Rapporter les aspects cliniques, thérapeutiques et évolutifs de la tumeur de Buschke-Löwenstein à travers une série d´observations, colligées au service d´urologie du centre hospitalier universitaire Aristide Le Dantec (HALD) de Dakar. Il s´agissait d´une étude rétrospective et descriptive colligeant les dossiers des patients suivis pour tumeurs de Buschke-Löwenstein (TBL). L´étude a été réalisée entre Janvier 2009 à Décembre 2019. Pour chaque patient nous avons précisé le mode de vie, fait la sérologie rétrovirale et syphilitique et dosé la glycémie. Le traitement était chirurgical chez tous nos patients. Après la chirurgie, les patients ont été revus en consultations à 3 mois, 6 mois, 12 mois puis 24 mois afin de détecter une éventuelle récidive. Huit dossiers ont été colligés au service d´urologie de HALD sur une période de 10 ans. L´âge de nos patients variait entre 41 ans et 71 ans avec une moyenne de 57 ± 9,8 ans. Le tabagisme a été noté chez 2 patients, l´alcoolisme a été signalé chez un patient. Six patients avaient des partenaires sexuels multiples, il n´y avait pas de rapport homosexuel avoué. Les lésions étaient constantes dans tous les cas, produisaient un aspect végétant, bourgeonnant en chou-fleur. La sérologie VIH était positive chez un patient. La sérologie syphilitique et chlamydia, étaient négatives dans notre série. La glycémie à jeun était normale chez tous nos malades. La durée d´évolution des lésions variait entre 5 ans et 30 ans avec une moyenne de 13 ans.Le diamètre moyen des lésions était de 10 cm. La localisation scrotale était constante. La durée moyenne du suivi était de 2 ans. Aucune complication en rapport avec le geste chirurgical n´a été notée. Un patient a eu une récidive sous forme d´un carcinome épidermoïde. Les TBL sont des tumeurs rares qui affectent le plus souvent le sujet jeune. Le traitement chirurgical donne de bons résultats.

Introduction: Male infertility has a heterogeneous etiology, most commonly caused by spermatogenesis disorders clinically manifested by oligo-azoospermia. In about 30% of cases of male infertility associated with oligo-azoospermia a genetic cause can be detected. The Purpose: To determine chromosomal variations, Y-chromosome microdeletion of the AZF and cystic fibrosis transmembrane conductance regulator (CFTR) gene mutations in infertile men, before assisted reproductive techniques (ART). Materials and methods: A group of 45 infertile men were investigated during genetic counseling among infertile couples referred for ART treatment. Criteria for including patients were fulfilled if they presented with azoospermia, raised or normal levels of FSH, LH and testosterone. G-banding of metaphase chromosomes karyotype analysis were performed in all azoospermic patients. Genomic DNA was isolated and used to analyze AZF microdeletions and CFTR gene by PCR. The regions and sequence-tagged sites of AZFa (SY86, SY84), AZFb (SY127, SY134), and AZFc (SY254, SY255) were sequenced by multiplex PCR. Two common mutations of the CFTR gene were tested ΔF508 and G542X. Results: Of 45 cases of infertile men with azoospermia, 34 had normal karyotype (46,XY). In 11 cases chromosomal abnormalities have been found, including 7 patients with Klinefelter syndromes (47,XXY), 1 patient with karyotype 46,XX, 1 patients with balanced autosomal rearrangement 46,XY,der(5),t(9;5) and 2 persons with chromosomal polymorphism 46,XYqh+, 46,XY,14ps+. Deletions of Y chromosome were seen in the AZFc regions of 2 patients with features of normal karyotype, deleted markers were sY254 and sY255. Microdeletions were detected in each region of AZFa-sY84, sY86; AZFb-sY124, sY134; AZFc-sY254, sY255, and presence of gene SRY and ZFY in the patient with XX male syndrome. Two men have been found as carriers of a CFTR gene mutation ΔF508; for calculating the risk of recurrence in off spring were also investigated their spouses, who were homozygous. Conclusions: This study highlights for all couples with the diagnosis of male infertility with azoospermia the need of genetic testing and counseling prior to employment of assisted reproduction techniques. This is important for providing a firm diagnosis and fertility treatment to couples with infertility. [ABSTRACT FROM AUTHOR]

While transgenderism as a cultural phenomenon seems to be based on a collective taste for the sensational, its emergence represents a collective shift towards a new or more differentiated way of experiencing and expressing sex and gender, a movement of world soul. This paper attempts to explore that emergence from a Jungian perspective. The paper utilizes clinical examples which illustrate how dissociated aspects of the personality are seeking assimilation and expression in order to move the personality towards greater wholeness. In that sense, it attempts to understand the teleology of transgenderism on an individual and collective level. The paper is intended as a starting-off point for discussion and explores gender as fantasy, anima/animus dynamics, the psyche/soma relationship, the role of hormones/biochemistry in our experience of ourselves and what transgender people carry and suffer for our culture. From the viewpoint of the Bible and its Talmudic, Midrashic and Kabbalistic commentaries, a complex set of questions, with multiple ramifications, may be asked concerning the relation between God (unicity) and the question of duality as well as that of differentiation/separation with respect to the concept of sanctity/distinction with the idea of transcendence as the overall paradigm. The biblical text signals a process that begins with an androgynous model (Adam) in which God (unicity) and the duality (masculine and feminine) encompasses a radical alterity between Man and Woman inferring a combined specific sexuality in terms of rights, pleasure and procreation that thereby excludes the homosexual act. Le cancer du sein chez l'homme est une affection rare qui représente 0,4 % à 1,2 % de tous les cancers masculins. Il est souvent découvert à un stade tardif malgré son siège superficiel. Les manifestations cutanées semblent être plus fréquentes et plus précoces chez l'homme que chez la femme. Il s'agit le plus souvent d'un nodule cutané fixe et indolore de siège essentiellement rétroaréolaire. Les autres signes cliniques révélateurs sont à type d'écoulement mamelonnaire ou de modifications cutanées du mamelon à type de rétraction, ulcération ou d'eczématisation. Celles ci peuvent poser un problème de diagnostic différentiel avec la maladie de Paget, la maladie de Bowen, le carcinome basocellulaire ou le mélanome. Tous les types histologiques rencontrés chez la femme sont décrits chez l'homme : le carcinome canalaire infiltrant reste le plus fréquent. Les récepteurs hormonaux à la progestérone et aux éstrogènes sont fréquemment positifs que chez la femme avec des taux souvent élevés. La chirurgie reste la base de traitement. Les inhibiteurs de l'aromatase, hormonothérapie anti-éstrogénique et/ou la polychimiothérapie sont utilisés comme des traitements adjuvants. Le pronostic reste encore réservé étant donné son évolution imprévisible et son potentiel métastatique élevé. Nous rapportons le cas d'un carcinome mammaire gauche chez un homme de 72 ans révélé par une ulcération mamelonnaire gauche. L'écho-mammographie et l'étude histologique étaient en faveur d'un carcinome canalaire infiltrant avec des métastases cutanées. Un bilal l'extension révélait des métastases pulmonaires et osseuses. Le patient est décédé avant de commencer la première cure de polychimoithérapie.

19. A study of the behavior of Romanian and foreign basketball players in the U-Mobitelco team in FIBA Eurochallenge Cup games.

Background. The principle of the research started with the notion that in order to objectively evaluate the team members' behavior during play, which in turn would lead to better knowing the players and to a better use of them in competitions based on objective decisions, it is necessary to gather information by the means of recording and building a ready-to-use database. Aims. The purpose for analyzing the main behavior indicators during games was to obtain data on which the coach could better assess the evolution of his players and could objectively plan the training process in order to increase the players' performing efficiency during competitions. The aim is to determine to what extent the foreign players from U-Mobitelco represent a plus and add value to the basketball game. Methods. The six players' contribution to the results of the team in this competition was monitored, by comparing the results obtained by the foreign players and Romanian ones, analyzing specific parameters: number of games played, number of minutes played, number of shots, offensive rebounds, defensive rebounds, steals, blocks - blocked basketball throws, decisive passes - assists, stolen balls, scored points. Results. A model of the Romanian and foreign players' game behavior was subsequently developed. Comparing the main game parameters, the foreign players have better results than the Romanian ones, except for the 2-point throws. The foreigners' game efficiency index is 49.6, whereas the Romanian players' index is only 14.2. Conclusions. Processing the data regarding game specific parameters gives the coach objective information on which to better know his players, to assess the overall evolution of the team and to take better decisions. On average, the foreigners get to play 10.41 minutes longer than their Romanian teammates. Regarding playing time, the foreigners outperform the Romanians by far, with a mean value of 30.84 vs. 20.43 minutes played. In the case of 9 out of 10 parameters, the foreigners have better results and thus, they contribute to a greater extent to the achievements of the team and to raising the performance index. Abstract: Inguinal hernias are due to the patent vaginoperitoneal canal; surgical treatment lead to a definitive cure. The aim is to describe the inguinal hernias of male children and report our experience in their management. We conducted a retrospective study of the hernias on male children from 1993 to 2003. We analyzed its prevalence, its clinical characteristics, the associated malformations and the results of the treatment. Inguinal hernias represented 18% of the consultations over the period of study. Diagnosis was delivered within 3.6 months on average with extremes of 1 day and 15 years. It was associated to prematurity in 8% of the cases. They were inguino-scrotal in 54% of the cases and predominantly in the right side in 59% of the cases. They were associated to other genito-urinary abnormalities in 9%. Incarceration was the clinical pattern in 16% of the cases. The cure had been made by the vaginoperitoneal canal ligature, and a prefunicular link of the conjoint tendon and the inguinal ligament in 100% of the cases. The postoperative complications were 2.4% with one recurrence; the mortality was 0.7% due to incarcerations. The inguinal hernia of the male is known as a benign pathology but it can be fatal when not recognized in its incarceration form with late treatment. [Copyright &y& Elsevier]
