Speech and language pathologists (SLPs) often rely on judgements of speech fluency for diagnosing or monitoring patients with aphasia. However, such subjective methods have been criticised for their lack of reliability and their clinical cost in terms of time. This study aims at assessing the relevance of a signal-processing algorithm, initially developed in the field of language acquisition, for the automatic measurement of speech fluency in people with aphasia (PWA). Twenty-nine PWA and five control participants were recruited via non-profit organizations and SLP networks. All participants were recorded while reading out loud a set of sentences taken from the French version of the Boston Diagnostic Aphasia Examination. Three trained SLPs assessed the fluency of each sentence on a five-point qualitative scale. A forward-backward divergence segmentation and a clustering algorithm were used to compute, for each sentence, four automatic predictors of speech fluency: pseudo-syllable rate, speech ratio, rate of silent breaks, and standard deviation of pseudo-syllable length. The four predictors were finally combined into multivariate regression models (a multiple linear regression — MLR, and two non-linear models) to predict the average SLP ratings of speech fluency, using a leave-one-speaker-out validation scheme. All models achieved accurate predictions of speech fluency ratings, with average root-mean-square errors as low as 0.5. The MLR yielded a correlation coefficient of 0.87 with reference ratings at the sentence level, and of 0.93 when aggregating the data for each participant. The inclusion of an additional predictor sensitive to repetitions improved further the predictions with a correlation coefficient of 0.91 at the sentence level, and of 0.96 at the participant level. The algorithms used in this study can constitute a cost-effective and reliable tool for the assessment of the speech fluency of patients with aphasia in read-aloud tasks. Outcome measurement is recommended in stroke clinical practice guidelines, however there is considerable variability in how this activity is performed in clinical practice. Factors driving clinician behaviour have been explored in English-speaking countries, but little is known about the factors influencing clinical practice in non-English-speaking populations. (1) To explore barriers and facilitators to aphasia outcome measurement from the perspective of international aphasia clinicians. (2) To determine whether barriers and facilitators to outcome measurement differ in English- and non-English- speaking countries. The primary hypothesis was that clinicians working with people with aphasia in non-English-speaking countries would experience more barriers to outcome measurement than those in English-speaking countries. An international sample of aphasia clinicians completed an online survey informed by the Theoretical Domains Framework. Quantitative data were analysed using descriptive statistics and a Mann-Whitney U Test to compare English- and non-English- speaking groups. TDF domains identified as influencing clinician behaviour were mapped to the Behaviour Change Wheel, producing theory-informed strategies to improve practice. A total of 208 clinicians from 25 countries completed the survey. Almost all (93.7%) reported measuring outcomes to some extent, most commonly to measure client progress. Facilitators to outcome measurement included "social/professional role and identity" (understanding that measuring outcomes is part of the clinicians' role), "optimism" (feeling positive about measuring outcomes), and "emotion" (enjoying, and feeling comfortable measuring outcomes). Barriers were "environmental context and resources" (time and resource limitations, and competing caseload priorities), "behavioural regulation" (a lack of personal and workplace systems to measure outcomes) and "skills" (having insufficient training and experience in outcome measurement). There was no significant difference between the barriers and facilitators experienced by clinicians in English- and non-English- speaking countries. Implementation strategies, informed by Behaviour Change Techniques, were created to improve clinical practice. Internationally, clinicians working with people with aphasia measure outcomes and believe that this is part of their role, and a positive aspect of their work. Common barriers to outcome measurement included insufficient time and access to resources, inadequate personal and workplace systems, and insufficient skills necessary for performing outcome measurement. Preliminary, theory-informed strategies (e.g., improving access to culturally and linguistically appropriate measurement instruments; developing protocols, templates or checklists guided by recommended practice; and providing training in outcome measurement) would assist with uptake of clinical practice guidelines in this area. We examine the inflectional productions of seven Catalan, seven Galician, and seven Spanish speaking agrammatic subjects in an elicitation and a sentence repetition task and consider them in the light of the Tree Pruning Hypothesis (TPH). The results show relatively spared subject person/number agreement with the verb and impaired tense marking for all subjects in all the languages. Oxford: Oxford University Press] array of functional mood/tense/aspect projections and we show that subject agreement must occur between the subject DP and a low functional head for selective impairment to result. Feature underspecification, formerly considered, is rendered unnecessary.

Coordination of international aphasia research would minimise duplication of effort, support synergistic international activities across languages and multidisciplinary perspectives, and promote high-quality conduct and reporting of aphasia research, thereby increasing the relevance, transparency, and implementation of findings. The Collaboration of Aphasia Trialists (CATs) sought to develop an aphasia research agenda to direct future research activities, based on priorities shared by people with aphasia, family members, and healthcare professionals. Our established international research network spanning 33 countries contributed to this activity. Research literature reporting the priorities of stakeholders was reviewed and synthesized (phase 1). Representatives from Working Groups on Aphasia Assessment & Outcomes, Prognosis & Predictors of Recovery, Effectiveness of Interventions, and Societal Impact & Reintegration participated in a two-day research agenda-setting meeting. The CATs expert panel refined research objectives and identified constituent components of research and methodological developments required to address these research components. The objectives and research components were grouped into overarching themes (phase 2). The resultant list was then circulated to more than 180 CATs members for review, revision, and approval. Consensus on the final aphasia research agenda and roadmap was reached by CATs executive committee (phase 3). The expert panel identified five overarching research themes: (i) evidence-based interventions for people with aphasia, (ii) effective interventions to support those communicating with people with aphasia, (iii) cross-linguistic assessment and core outcomes for aphasia research, (iv) predictors of language recovery, and (v) clinical implementation of research findings. Within these broad themes, 30 research objectives and 91 individual aphasia research components were identified and sequentially ordered. This agenda builds on research priorities identified by people with aphasia and their families, and includes priorities of healthcare professionals and researchers, and will support the rehabilitation and recovery of people with aphasia. Our internationally relevant research agenda promotes rigor in methodology, considers international applicability, synergistic activities, and sharing of resources and expertise. Background: The distribution of pronouns varies cross-linguistically. This distribution has led to conflicting results in studies that investigated pronoun resolution in agrammatic indviduals. In the investigation of pronominal resolution, the linguistic phenomenon of "resumption" is understudied in agrammatism. The construction of pronominal resolution in Akan presents the opportunity to thoroughly examine resumption. Aims: To start, the present study examines the production of (pronominal) resumption in Akan focus constructions (who-questions and focused declaratives). Second, we explore the effect of grammatical tone on the processing of pronominal (resumption) since Akan is a tonal language. Methods & Procedures: First, we tested the ability to distinguish linguistic and non-linguistic tone in Akan agrammatic speakers. Then, we administered an elicitation task to five Akan agrammatic individuals, controlling for the structural variations in the realization of resumption: focused who-questions and declaratives with (i) only a resumptive pronoun, (ii) only a clause determiner, (iii) a resumptive pronoun and a clause determiner co-occurring, and (iv) neither a resumptive pronoun nor a clause determiner. Outcomes & Results: Tone discrimination.both for pitch and for lexical tone was unimpaired. The production task demonstrated that the production of resumptive pronouns and clause determiners was intact. However, the production of declarative sentences in derived word order was impaired; wh-object questions were relatively well-preserved. Conclusions: We argue that the problems with sentence production are highly selective: linguistic tones and resumption are intact but word order is impaired in non-canonical declarative sentences. Background: Comprehension of reversible sentences that have derived word order has often been reported as impaired in agrammatic aphasia. Most accounts of this phenomenon refer to the syntactic differences between derived and base word order of the arguments. However, it has been demonstrated that in agrammatic spontaneous speech in standard Indonesian (SI) passives are produced at a rate that is proportional to that of healthy speakers. The main difference between passives in SI and in other languages is the frequency with which passives are used: passives in SI are highly frequent. The high frequency can be explained by the fact that passives are used for politeness reasons, saliency of the passive morphology, earlier acquisition, and formal simplicity of the passive structure. Aims: The purpose of the current study is to investigate comprehension of the passive as a derived structure in SI and the influence of frequency. Methods & Procedures: A sentence-to-picture matching task was developed to test four reversible sentence types (active, passive, subject cleft and object cleft). There are three variables that are of interest, that is, word order, embedding and relative frequency of structures. Eleven agrammatic speakers classified as suffering from Broca’s aphasia were tested. Outcomes & Results: The passive sentences were comprehended equally well as the active sentences. Embedding had limited effects: subject clefts were understood as well as actives and passives. Object clefts, however, were understood poorly and significantly worse than the three other sentence types. Conclusions: The sentence comprehension deficit pattern shown in SI individuals with Broca’s aphasia introduces frequency of a syntactic structure as an additional factor to consider. Whether frequency or pragmatic constraints protects against erosion of the passive in Broca’s aphasia in SI remains an open question. [ABSTRACT FROM AUTHOR]

Despite their ample reading experience, higher education students with dyslexia still show deficits in reading and reading-related skills. Lateralized print tuning, the early sensitivity to print of the left parietal cortex signalled by the N1 event-related potential (ERP) component, differs between beginning dyslexic readers and controls. For adults, the findings are mixed. The present study aims to investigate whether print tuning, as indexed by the N1 component, differs between 24 students with dyslexia and 15 non-dyslexic controls. Because handedness has been linked to lateralization, first, a separate analysis was conducted including only right-handed participants (n = 12 in both groups), like in most previous studies. ERPs were measured during a judgement task, requiring visual, phonological, or semantic judgments. In both groups, the N1 was earlier and stronger in the left than in the right hemisphere. However, when only strongly right-handed participants were evaluated, the N1 was less left-lateralized for participants with dyslexia as compared with controls. Participants with dyslexia had longer reaction times during the ERP experiment and performed worse on many reading (-related) tasks. These findings suggest that abnormal print tuning can still be found among higher education students with dyslexia and that handedness should be regarded in the study of print tuning.
