Abstract

This report advances our global understanding of drug policy by focusing on two key components: a bibliometric analysis and the creation of preliminary country profiles. We aim to identify trends, challenges, and opportunities within drug policy globally, emphasizing how these aspects can be better aligned with humanitarian principles, Rome Consensus 2.0, and Sustainable Development. Importantly, as the world has recognized the inability of the War on Drugs as a strategy to reduce drug use or drug-related crime, which is to say, to make us more safe and healthier (the two aims of the War on Drugs), this Project provides a new vision for where nations can go next that achieves the desired public safety and public health aspects all peoples want to see in their own communities and countries. Drawing from the preliminary insights, the report advocates for a more holistic and evidence-driven approach to drug policy. This entails broadening support for harm reduction, improving treatment services and the professional workforce, encouraging international collaboration to address the challenges of the transnational illicit drug trade, and reassessing policies that perpetuate stigma. Serving as an overview for the upcoming Global Policy Review (GPR), this report emphasizes the importance of drug policy frameworks that respect human dignity and contribute to achieving Sustainable Development Goals, with intentions to expand the review to more UN member states and provide bi-annual updates.

Abstract

Se analizó el crecimiento relativo de langosta espinosa Panulirus gracilis, con un enfoque de selección multimodelo y la teoría de la información de Akaike. Se realizaron muestreos en capturas comerciales de langosta espinosa del sur de Sinaloa, México entre las temporadas de pesca de 2014 y 2015 y, 2017 y 2018. En 298 hembras de P. gracilis se registró la longitud cefalotórax (LC) mm, peso total (PT) g y fase de madurez gonádica. La estructura de tallas varió de 54.6 a 103.1 mm de LC. Se observaron hembras maduras a partir de 57.5 mm de LC, con un incremento de proporción de madurez en tallas mayores. El mejor modelo fue Línea Rota (Wi=65.09%) y Dos Segmentos (Wi%=34.87). El cambio en el crecimiento relativo a la talla en ambos modelos, coincidió con un incremento en la proporción de madurez en tallas mayores a 60 mm de LC en P. gracilis.

Abstract Over the past few decades, international research has documented that stigma and discrimination is one of the major barriers to care in the field of drug abuse worldwide. Objective: To show the findings of the international literature about specific populations that experience stigma or discrimination as one of the main barriers to care in substance abuse as well as to account for the development of public policies against stigma in mental health and addictions in consonance with goals 3, 10 and 16 of the Sustainable Development Goals. Resumen En las últimas décadas, se ha documentado en la investigación a nivel internacional, que el estigma y la discriminación constituyen una de las principales barreras para la atención en el ámbito del abuso de drogas en todo el mundo. Neurofibromatosis type 1 is a rare medical condition that raises the probability of having distinct types of malignant and benign neoplasms. Nevertheless, the association with breast cancer is rare, and metachronic neoplasia with a distinct histologic subtype is an association that has never been reported before. Here we describe a case of a primary breast tumor, with posterior development of basal cell carcinoma of the skin, and a low-grade peripheral nerve sheath sarcoma.

Abstract

Gastrointestinal bleeding in HIV patients secondary to coinfection by HHV8 and development of Kaposi’s sarcoma (KS) is a rare complication even if no skin lesions are detected on physical examination. This article indicates which patients might develop this type of clinical sign and also tries to recall that absence of skin lesions never rules out the presence of KS, especially if gastrointestinal involvement is documented. Gastrointestinal bleeding in terms of hematemesis has rarely been reported in the literature. We review some important clinical findings, diagnosis, and treatment approach. We present the case of an HIV patient who presented to the emergency department with hematemesis and gastrointestinal signs of KS on upper gastrointestinal endoscopy without any dermatological involvement.

Abstract

Primary pleomorphic sarcoma of the left atrium is a rare tumor. There is no actual evidence of the management of this pathological entity, so the main treatment is individualized, surgical management being the cornerstone of the treatment. We present a 78-year-old female who had a clinical picture of heart failure, documenting an atrial mass of the left atrium, with high-grade pleomorphic sarcoma revealed in histopathology. The tumor was surgical removed, with no clinical evidence of residual mass. The tumor recurred again within 3 years, to which the patient succumbed.

33. Dimensional changes in free epithelialized gingival/mucosal grafts at tooth and implant sites: A prospective cohort study.

Background: A study was made of the dimensional changes in free epithelialized gingival/mucosal grafts (FEGs) used to augment keratinized tissue (KT) at tooth and implant sites, and of the confounders influencing the dynamic changes over 6 months of follow-up.Methods: A prospective cohort interventional study was made of implant and tooth sites needing KT augmentation by means of an apically positioned flap and FEG. Six intraoperative variables were recorded at baseline (T0). In addition, graft width (GW), graft length (GL), and graft dimension (GD) were assessed at 3 weeks (T1), 3 months (T2), and 6 months of follow-up (T3). Univariate and multivariate analyses were performed to explore associations between the demographic and intraoperative variables and the outcomes over the study period.Results: Based upon an a priori power sample size calculation, a total of 56 consecutive patients were recruited, of which 52 were available for assessment. A total of 73 graft units were included in 122 sites. At T3, the mean change in GD in FEG was 40.21%. In particular, the mean changes in GL and GW were 12.13% and 33.06%, respectively. Statistically significant changes in GD were recorded from T0 to T1 (P < 0.0005) and from T1 to T2 (P < 0.0005), but not from T2 to T3 (P = 0.13). The change in GD at T3 was 33.26% at tooth and 43.11% at implant site level (P = 0.01). Age and GW assessed at T0 proved to be related to the changes in GD and GW in the univariate and multivariate analyses. The univariate analysis showed the avascular area (AA) to be related to the changes in GD and GW at the implant sites, whereas graft thickness (GT) was associated to changes in GD and GW at the tooth sites in the univariate and multivariate analyses.Conclusion: Free epithelialized grafts are exposed to dimensional changes that result in a reduction of approximately 40% of the original graft dimension-the changes being approximately 10% greater at the implant sites than at the tooth sites (NCT04410614). Abstract

Introduction: Alveolar ridge preservation (ARP) is a well‐defined treatment performed to reduce bone dimensional changes occurring during the healing of post‐extraction sockets to allow for adequate implant placement. Leukocyte and platelet‐rich fibrin (L‐PRF) has been showing to potentially promote bone and tissue regeneration during wound healing. Therefore, the aim of this study is to evaluate its efficacy for ARP when applied to fresh extraction sockets, in comparison with spontaneous healing. Materials and Methods: Twenty‐seven patients with hopeless non‐molar teeth were treated. After randomization, fresh extraction sockets were either filled with L‐PRF or allowed to heal spontaneously. CBCTs and intraoral scans were obtained immediately after extraction and at 4 months. Through superimposition of the obtained images, changes in the horizontal ridge width, height, buccal volume, and ridge contour changes were measured, as well as patient‐reported outcome measures (PROM's). Results: The ridge dimensions changed similarly in both groups. Although less reduction occurred in the test group at 1 mm from the bone crest, differences were not statistically significant (p > 0.05). Application of L‐PRF did not prevent reductions of ridge contours, neither in the linear vertical aspect nor in volumetric changes. There were no differences between groups in the need for bone regeneration when placing implants. Patients in both groups reported similar outcomes in terms of bleeding, pain, inflammation, and function at 1 and 4 weeks postoperatively. Conclusion: Alveolar preservation with L‐PRF neither minimized bone resorption occurring after tooth extraction in non‐molar sites nor reduced the need for bone regeneration when placing implants. Abstract

Society is ageing. In Europe, the ageing of the population is a recurrent and discussed theme. The impact of the ageing of the population is varied and transversal in different fields. The increase in the number of elderly people implies an increase in the levels of dependence and, consequently, more sanitary, physical, and human resources. Also, the number of professionals linked directly or indirectly to the elderly increases according to the increase in the ageing population. Opportunities for the development of professional careers progressively appear and the new demographic panorama demands innovative tasks and services, appealing to the imagination of the professionals to answer the emerging needs. Psychology students can potentially develop their professional careers close to the elderly. The history of psychology close to the elderly is still recent when compared to other disciplines such as social work or nursing. Initial proposals on human development, of which Freud and Piaget are undoubtedly the better known representatives, imposed a restrictive view of development stagnation after adolescence. Starting in the middle of the 20th century, however, new perspectives on the potential of human development emerged, keeping in mind different factors for the promotion of and adaptation to the ageing process, gathered under the common designation of psychology of the life cycle. At present, psychology of the life cycle is subscribed consensually as one of the preferred ways of understanding the ageing processes, opening a wide field of research and activity. The role to be played by researchers and psychology professionals is progressively increasing. Despite the existence of a vast field of work in this area, psychology students' interest in working with the elderly is generally low. In fact, they do not manifest a spontaneous interest either in the field of research or the performing of specific tasks. When analysing their lack of interest, international literature seems to indicate the existence of three significant factors: (1) the manifested attitudes facing the age variable and a hypothetical scenario of working with the elderly; (2) the prejudice and stereotypes associated with the ageing processes and the elderly, and also the performing of professional functions in this sphere; and (3) the gaps in factual knowledge. It is in this context that the authors first analyse some general concepts on attitudes, suggesting afterwards some studies that analysed this theme from the specific point of view of the development of a professional career. 42. PUVA therapy in persistent cutaneous histiocytosis: Case report and literature review.

Abstract

Background: Cutaneous and mucocutaneous histiocytosis (group C) comprise a wide variety of entities affecting skin and/or mucosae. Although they are considered as reactive proliferations, their exact pathophysiology remains unknown and, therefore, they lack a specific treatment. Aims: The aim of this study is to review the evidence on cases of histiocytosis treated with UVB and/or UVA and to report a new case of relapsing group C histiocytosis that has been successfully treated with PUVA therapy. Materials & Methods: We have conducted a review of the literature published over the last 40 years on the treatment of histiocytosis with phototherapy in the online PubMed database. We also describe a new case of successful treatment of histiocytosis with PUVA therapy. Results: Our patient was a 27‐year‐old man with persistent outbreaks of cutaneous histiocytosis over the previous 8 years. He responded successfully to PUVA therapy, and no relapse has been detected after one year of follow‐up. Discussion: Self‐involution is usual in group C histiocytosis, so conservative management is usually the first approach. Relapsing cases pose a therapeutic challenge. Reported treatment options for these patients include isotretinoin, cryotherapy, immunosuppressants, low‐dose chemotherapy, CO2 laser, radiotherapy, and surgery. Phototherapy and photochemotherapy have been used in a small number of patients with considerable success. The main limitation to provide firm recommendations on histiocytosis therapy is the absence of solid evidence, as the articles published are mainly case reports with a short follow‐up. In our patient, despite the short follow‐up we have considered photochemotherapy to be effective since no spontaneous remission had been achieved in the previous 8 years. Conclusion: PUVA therapy could be a safe and effective option to treat persistent cutaneous manifestations in patients with histiocytosis, although more evidence is required to support this statement. According to the survey conducted by Criminal Study, shows that there is a high perception of insecurity and there is a significantly relation among education as prevention. However, public opinion continue to give a high value to the harsh penalties as a solution to crime, unfortunately these only focus the consequences (offense) and not the causes.
