The depth of convective clouds affects vertical transport of atmospheric constituents, influencing downstream weather and climate. Atmospheric controls on the maximum depth reached by moist convection are investigated with radar‐tracked convective cells tagged with sounding‐derived atmospheric parameters from a field campaign in central Argentina. Regression analyses show that narrow (<12‐km diameter) and wide (>16‐km diameter) cell depths respond to disparate factors, where cell areas are defined using composite reflectivity signatures. Undiluted lifted parcel indices including convective available potential energy (CAPE) and level of neutral buoyancy (LNB) are top predictors of wide cell maximum depth while mid‐tropospheric relative humidity is the top predictor of narrow cell maximum depth. Because narrow cells are more numerous than wide cells, the overall outcome of the full cell population does not strongly correlate with CAPE and LNB conditions. Tracked cells and atmospheric conditions in a simulation with 3‐km grid spacing covering the field campaign produce similar results to those observed. Narrow cells that are relatively deep have a cooler and moister mid‐troposphere with weaker free tropospheric subsidence, while relatively deep wide cells have much warmer and moister lower tropospheric conditions. These atmospheric differences are present 1 hr before cell initiation at both a fixed observing site and variable cell initiation locations. Simulated narrow cell maximum equivalent potential temperature decreases with height at a rate similar to the ambient vertical gradient, causing these cells to fall short of their LNB and supporting the view that entrainment‐driven dilution is a dominant control on their depth. Plain Language Summary: The depth that storms reach impacts where the heat, moisture, momentum, aerosols, and trace gases that they are transporting are deposited, but this depth is difficult to predict because most storms fall short of their theoretically maximum possible depth. The reason for this shortfall is that most storms are narrow, such that they are effectively diminished by mixing with surrounding drier air as the storm deepens. Thus, a dominant control on the depth of these clouds is the humidity of this surrounding air. However, wide storms are less vulnerable to destructive mixing with dry air. As a result, they more consistently reach depths that are well predicted by theory. These conclusions were reached via analysis of over a thousand storms over several months using field experiment measurements and a computer simulation. Nonadherence to medication treatment regimens is a major preventable risk behavior in both acute and chronic diseases. Community pharmacists are facilitators in community care for promoting medication adherence and they should implement interdisciplinary medication adherence programs. To do so, pharmacists should be educated in medication adherence, and new pharmaceutical care policies should be implemented. The healthcare system should evolve to better meet the specific needs of patients. Aims: this article describes what has been undertaken in the last decade in medication adherence in terms of education, research, practice and policy in Switzerland.Methods: Medline was searched, with the search limited to Switzerland. The three Swiss pharmacy schools were also contacted to collect information about the medication adherence content of both their courses and research programs. National policies related to medication adherence were also reviewed for relevant content.Results: Education: two pharmacy schools offer courses devoted specifically to medication adherence. The number of hours dedicated to the topic varies between 4 to13. Research: a total of 16 studies met the inclusion criteria. Chronic patients were the focus of 9 studies. Medication adherence was the primary outcome of all studies; 10 studies also measured clinical outcomes. Nine studies evaluated the prevalence of medication nonadherence; three studies evaluated the feasibility of new technologies for monitoring adherence; three studies evaluated medication adherence enhancing programs. Policies: three cognitive pharmaceutical services are reimbursed by healthcare insurers, which are directly related to medication adherence.Conclusions: Pharmacists in Switzerland have been actively involved in medication adherence research since the mid ’90s. Specific medication adherence courses have entered the curriculum of pharmacy schools, and policies in Switzerland are slowly beginning to meet needs of chronic patients by the introduction of pharmaceutical cognitive services and reimbursement fees.

The purpose of this study was to examine the effects of listening to preferred vs. nonpreferred music on resistance exercise performance. Twelve resistance-trained college-aged males (age = 20.5 ± 1.24 years, height = 183.9 ± 6.8 cm, and body mass = 97.0 ± 18.2 kg) were recruited for this study. In a within-groups counterbalanced study design, subjects either listened to preferred or nonpreferred music during a bench press exercise test. Subjects completed as many repetitions as possible at 75% of their 1 repetition maximum with maximum explosive intent. Power and velocity of the barbell movement was measured for the first 3 repetitions using a linear position transducer. Motivation was measured using a visual analog scale immediately after exercise. Each exercise trial was separated by a 48-hour washout period. Results indicate that listening to preferred music increased overall bench press repetitions completed (p = 0.005; effect size [ES] = 0.84). During the first 3 repetitions, mean velocity (p = 0.001; ES = 1.6), relative mean power (p = 0.012; ES = 0.55), peak velocity (p = 0.011; ES = 0.99), and peak power (p = 0.009; ES = 0.35) were higher while listening to preferred music vs. nonpreferred music. Finally, motivation during the lift (p < 0.001; ES = 5.9) was significantly higher while listening to preferred vs. nonpreferred music. Current findings suggest that listening to preferred music by the individual results in greater performance than nonpreferred during resistance exercise. Accurate measurement of substrate temperature is one of the most critical process control parameters for molecular beam epitaxy (MBE) growth. Band-edge thermometry instruments have proven to be a valuable tool for process control during MBE growth of semiconductor films, providing as high as ±1 °C temperature resolution. The increasing use of InAs, GaSb, and AlSb iii-v materials necessitates a method for accurately measuring the temperature of their closely lattice-matched GaSb substrates. Current-technology instruments typically rely on InGaAs detector materials which have a maximum wavelength λ detection of ∼1.7 μm, but GaSb substrates have a band gap energy corresponding to λ > 2 μm. A band-edge thermometry instrument capable of λ > 2 μm has been developed using an InAs/InGaSb strained-layer superlattice detector sensitive to 2-9.5 μm long-wave IR wavelengths. PEBELS is a method for estimating the expected loss cost for each loss layer of an individual property risk regardless of size. By providing maximum resolution in estimating layer loss costs, PEBELS facilitates increased accuracy and sophistication in many actuarial pricing applications such as ratemaking, predictive modeling, catastrophe modeling, and reinsurance pricing. The existing actuarial literature provides methods for estimating high layer loss cost for credible property portfolios in aggregate, but does not provide a method to produce similar provisions for smaller non-credible risks. This study investigated the mechanisms underlying tubular apoptosis in diabetes by identifying proapoptotic genes that are differentially upregulated by reactive oxygen species in renal proximal tubular cells (RPTCs) in models of diabetes. Total RNAs isolated from renal proximal tubules (RPTs) of 20-week-old heterozygous db/m+, db/db, and db/db catalase (CAT)-transgenic (Tg) mice were used for DNA chip microarray analysis. Real-time quantitative PCR assays, immunohistochemistry, and mice rendered diabetic with streptozotocin were used to validate the proapoptotic gene expression in RPTs. Cultured rat RPTCs were used to confirm the apoptotic activity and regulation of proapoptotic gene expression. Additionally, studies in kidney tissues from patients with and without diabetes were used to confirm enhanced proapoptotic gene expression in RPTs. Bcl-2-modifying factor (Bmf) was differentially upregulated (P<0.01) in RPTs of db/db mice compared with db/m+ and db/db CAT-Tg mice and in RPTs of streptozotocin-induced diabetic mice in which insulin reversed this finding. In vitro, Bmf cDNA overexpression in rat RPTCs coimmunoprecipated with Bcl-2, enhanced caspase-3 activity, and promoted apoptosis. High glucose (25 mmol/L) induced Bmf mRNA expression in RPTCs, whereas rotenone, catalase, diphenylene iodinium, and apocynin decreased it. Knockdown of Bmf with small interfering RNA reduced high glucose-induced apoptosis in RPTCs. More important, enhanced Bmf expression was detected in RPTs of kidneys from patients with diabetes. These data demonstrate differential upregulation of Bmf in diabetic RPTs and suggest a potential role for Bmf in regulating RPTC apoptosis and tubular atrophy in diabetes. [ABSTRACT FROM AUTHOR]

Forward genetics is an experimental approach in which gene mapping and positional cloning are used to elucidate the molecular mechanisms underlying phenotypic differences between two individuals for a given trait. This strategy has been highly successful for the study of inbred mouse strains that show differences in innate susceptibility to bacterial, parasitic, fungal, and viral infections. Over the past 20 years, these studies have led to the identification of a number of cell populations and critical biochemical pathways and proteins that are essential for the early detection of and response to invading pathogens. Strikingly, the macrophage is the point of convergence for many of these genetic studies. This has led to the identification of diverse pathways involved in extracellular and intracellular pathogen recognition, modification of the properties and content of phagosomes, transcriptional response, and signal transduction for activation of adaptive immune mechanisms. Profiles of long-term respite use are reported for a diverse sample of 4,369 families from seven states who participated in the Alzheimer's Disease Demonstration Grant to States program. Data from service records spanning a period of seven years were analyzed using hierarchical linear modeling techniques. Respite use was examined in both day care and in-home settings using three different measures of use. Client characteristics related to use include the caregiver's gender and relationship to the elder, and the ethnicity, income, functional level, and problem behaviors of the elder. Provider characteristics linked with different patterns of use include level of service, fee structure, and level of capitation. Implications for service delivery are discussed. Unlabelled: The effect of energy, protein, fat, and phenylalanine on serum phenylalanine concentrations during pregnancy for a set of identical twins with phenylketonuria (PKU) was examined. Blood samples were collected one to two times per week. The subjects completed a 3-d food record prior to each blood collection. The effect of the factors on serum phenylalanine levels was evaluated statistically using time-series analysis. Dietary intakes of the nutrients evaluated were similar for the subjects. For one subject, there were highly significant effects of energy, protein, and fat on serum phenylalanine levels. In contrast, these nutrients had no significant effect on serum phenylalanine for the other subject. Dietary phenylalanine had no significant effect on serum phenylalanine for either twin.Conclusions: There was no effect of phenylalanine intake and no consistent effect of energy, protein, or fat on serum phenylalanine. Other dietary or environmental factors or a combination of factors may impact serum phenylalanine levels of pregnant women with PKU. [ABSTRACT FROM AUTHOR]

Parents of infants and young children with disabilities or special health care needs are thrust, often without warning, into an unfamiliar and complex world that requires support for and information about a whole host of medical, educational, legal, financial, social, and emotional realities. Statewide early intervention efforts have a valuable ally in statewide parent-to-parent programs in meeting the needs of families of young children with special needs. This article reports on the descriptive results of a national survey of statewide parent-to-parent programs and suggests ways in which parent-to-parent programs and early intervention efforts can support and enhance their respective efforts on behalf of young children and families. Prolonged ethanol consumption produces neuropathology and neurologic symptoms in humans and experimental animals. As a means of understanding the central nervous system (CNS) consequences of ethanol abuse, we sought whether cholinergic neurochemical functions were altered by long-term ethanol consumption by rats. Eighteen weeks of ethanol consumption in a liquid diet reduced rat striatal and mammillary body choline acetylase (ChAT) by 53% and 58%, respectively. In these same regions, the density of muscarinic cholinergic receptors was elevated 117% and 12%. No such alterations were observed in cortex or in hippocampus. No alterations followed short-term ethanol consumption (2 wk), but they persisted after ethanol withdrawal (4 wk). After 13 mo of ethanol consumption, the number of neuroleptic binding sites in striatum was diminished by 29%. These results demonstrate a persistent, possibly permanent, alteration in brain cholinergic function as a consequence of long-term ethanol ingestion. Since brain acetylcholine is involved in numerous neurologic functions, including memory processes, these findings may be relevant to the lasting memory disorder and other permanent effects associated with chronic alcoholism.
