BackgroundStroke is a significant global cause of mortality and long-term disability, potentially influenced by infections that heighten systemic inflammation and thrombotic events. The full impact of influenza vaccination on stroke remains uncertain. This systematic review and meta-analysis aimed to investigate the association between influenza immunization and stroke incidence.MethodsWe searched for randomized controlled trials (RCTs), case–control, and cohort studies published in PubMed/Medline, Cochrane-Central-Register-of-Controlled-Trials (CENTRAL), and Embase until 5 December 2022, and identified articles investigating the effect of influenza vaccine on stroke occurrence. All articles were screened by two independent reviewers. We performed a meta-analysis to investigate the risk of stroke occurrence in vaccinated vs. unvaccinated individuals. The random-effects model was used in all statistical analyses.ResultsAmong the 26 articles meeting our criteria, 10 were retrospective cohort studies, 9 were case–control studies, 3 were prospective cohort studies, 3 were RCTs and 1 case-series. Overall, the studies showed a significant decrease in the risk of stroke incidence/hospitalization among vaccinated patients (OR = 0.81, 95% CI [0.77–0.86], p = 0.00001). Furthermore, studies showed flu vaccine decreases the occurrence of mortality among stroke patients (OR = 0.50, 95% CI [0.37–0.68], p = 0.00001). Sub-group analysis revealed significant protective effect for patients with specific comorbidities including atrial fibrillation (OR = 0.68, 95% CI [0.57–0.81], p = 0.0001), diabetes (OR = 0.76, 95% CI [0.66–0.87], p = 0.0001), Chronic obstructive pulmonary disease (OR = 0.70, 95% CI [0.61–0.81], p = 0.00001), and hypertension (OR = 0.76, 95% CI [0.70–83], p = 0.00001).ConclusionThe current meta-analysis further supports prior findings that influenza vaccination reduces stroke risk, particularly in patients with comorbidities. Guidelines should promote vaccination for at-risk individuals.

Background: Dental calculus is mineralized dental plaque formed on teeth and dental prosthesis surfaces in the oral cavity. Urinary stone is a crystal aggregation formed in urinary system due to minerals saturation present in urine. The structure of dental calculus is similar to that of urinary stone. Objective: To assess oral hygiene and gingival status in patients with urinary stone. And compared with healthy subjects. Patients and Methods: Sixty participants, 25-40 years, were involved in this study who were divided into study and control group. The study group involved patients with urinary stone while the control group involved healthy subjects. Clinical parameters including plaque, calculus and gingival indices were recorded for all participants. The correlation between the recorded clinical parameters was estimated. Results: The study group mean (±SD) plaque (1.435±0.499), gingival (0.995±0.288) and calculus (1.28 ±0.66) indices were found to be significantly high compared with control group(0.868±0.265), (0.602±0.265) (0.501±0.457 ) respectively. The formed calculus was positively associated with accumulated plaque (r: 0.608, p < 0.05) and gingival inflammation(r: 0.612, p < 0.05). Conclusion: Urinary stone patients were associated with more plaque and calculus accumulation, and had worse gingival inflammation compared to healthy participants. Thus, they need to be more aware of their gingival health status and improved preventive care of oral health.

The documentation and understanding of variations in the Earth's magnetic field through time is fundamental for several disciplines, but current geomagnetic models rely on datasets heavily biased toward the mid‐ and high northern latitudes. The African continent and surrounding islands and oceans are particularly underrepresented. Here, we present a new record of paleo‐secular variation (PSV) of the inclinations over the last 23 ka from Lake Chala, situated at 3°S near Mt Kilimanjaro in eastern equatorial Africa. This groundwater‐fed crater lake is characterized by a high sedimentation rate (ca. 1 cm/10 years) and a particularly well‐constrained age model based on 210Pb and 14C dating. The magnetic mineralogy of the sediments is tested with rock magnetic analyses. The Lake Chala inclination record shows four highs and lows over 20 ka and compares well with that of Lake Malawi (10°S) between 20 and 16.2 ka, and from 9.8 to 2.6 ka. This record is linked to PSV records at Lakes Victoria and Malawi using a sequence slotting technique to generate a composite PSV model for east Africa. Analyzed at best‐possible resolutions up to 200 years, the Lake Chala PSV record not only represents an important contribution to improve our understanding of local and global features of the Earth's magnetic field. It also expands the utility of paleomagnetism as a key tool for dating and correlation both for archeological sites throughout East Africa and the many volcanoes, active or dormant, of the East African Rift System. Plain Language Summary: The understanding of the behavior of the Earth's Magnetic field in the geological past is based on an even spatial and temporal distribution of data collections. Nonetheless, data from large areas, such as from the African continent, remain sparse. In this study, we present new data from Lake Chala in equatorial East Africa, near Mount Kilimanjaro. The new inclination data are measured in lacustrine sediments covering the last 23 ka. We combine the data from Lake Chala with other data from Lake Victoria and Lake Malawi to produce a composite record for east African Lakes, using sequence slotting which improves the coherence between the records, whilst still constrained by the sediment age models. The composite record, centered on Lake Chala, improves our understanding of how the Earth's magnetic field changed in the recent past and can serve as an independent tool for dating nearby archeological sites as well as volcanic deposits from the African Rift emplaced in the last 23 ka. Shewanella is an emerging human pathogen. It mostly causes skin and soft tissue infections. Osteosynthesis-associated infection involving Shewanella are rare and in most cases are secondary to direct contamination following open fractures in aquatic environments. Here, we present a rare case of hematogenous osteosynthesis-associated infection involving Shewanella algae affecting an 18-year-old patient who was operated on for 12th thoracic vertebrae and 4th lumbar vertebrae fractures occurring in an aquatic environment. We performed surgical debridement with subsequent double course parenteral antibiotherapy that was then adapted to bacteria sensitivities for three weeks. After a follow-up of six months, the patient had no signs of recurrent infection. The presence of infected dermabrasions and the concordance between germs isolated in operative samples and in blood cultures presumes that the contamination was hematogenous.

Abstract Cancer patients experience psychological symptoms such as depression during the cancer treatment period, which increases the burden of symptoms. Depression severity can be assessed using the beck depression inventory (BDI II). The purpose of the study was to use BDI-II scores to measure depression symptoms in cancer patients at a large tertiary hospital in Palestine. A convenience sample of 271 cancer patients was used for a cross-sectional survey. There are descriptions of demographic, clinical, and lifestyle aspects. In addition, the BDI-II is a tool for determining the severity of depression. Two hundred seventy-one patients participated in the survey, for a 95% response rate. Patients ranged in age from 18 to 84 years, with an average age of 47 years. The male-to-female ratio was approximately 1:1, and 59.4% of the patients were outpatients, 153 (56.5%) of whom had hematologic malignancies. Most cancer patients (n = 104, 38.4%) had minimal depression, while 22.5%, 22.1%, and 17.0% had mild, moderate, and severe depression, respectively. Education level, economic status, smoking status, and age were significantly associated with depression. The BDI-II is a useful instrument for monitoring depressive symptoms. The findings support the practice of routinely testing cancer patients for depressive symptoms as part of standard care and referring patients who are at a higher risk of developing psychological morbidity to specialists for treatment as needed.

Background: Invasive candidiasis (IC) has emerged worldwide as an important healthcare associated infection caused by Candida species. Nowadays, data on the epidemiology of IC and the antifungal susceptibility of Candida isolates in Tunisia are still limited. Hence, this study aimed to analyze the incidence, species distribution and antifungal susceptibility of Candida strains in the university hospital Sahloul (Sousse, Tunisia). Methods: A five-year (2016 to 2020) retrospective study was conducted including all Candida isolates recovered from blood, other sterile body fluids and deep-seated samples from hospitalized patients. Candida species were identified using the VITEK®2 automated system and the antifungal susceptibilities were determined by the Vitek®2 YST cards and/or the E-test method. Results: During the reported period, 138 nonrepetitive Candida isolated from separate patients were identified with a mean annual incidence of 1,04 case per 1000 admissions. Candida albicans (C. albicans) was the predominant species (42%), followed by C. parapsilosis (19,6%), C. tropicalis (11,6%) and C. glabrata (6,5%). The total resistance rates of fluconazole, voriconazole, amphotericin B and echinocandins were 6,7%, 2,9%, 2,9% and 3,4% respectively. The highest antifungal resistance rates were found with fluconazole (6,7%). This rate significantly increased from 5.3% in 2017 to 10,5% in 2020 (p=0,01). Echinocandins showed excellent in-vitro activities against the majority of Candida species. Conclusion: The present study provides valuable local surveillance data on the epidemiology and antifungal susceptibilities of invasive Candida species which can be useful to guide an appropriate empiric and targeted antifungal therapy.

This article presents improvements of large-signal RF power performance at Ka-band of gallium nitride high electron mobility transistors (HEMTs) utilizing a scandium aluminum nitride barrier. Three samples were fabricated and measured having different passivation schemes, including gallium nitride, silicon nitride, and aluminum gallium nitride. The pinning of the Fermi level against the valence band reduced the gain and output power density of the gallium nitride passivated device, resulting in 4.5 W/mm at a drain bias of 32 V. With silicon nitride alone as the passivation, dc/RF dispersion was reduced, and gain increased significantly, boosting the output power for this device to 8.0 W/mm at 28 V, though at the cost of a reduced breakdown voltage. Adding an epitaxial aluminum gallium nitride passivating layer increased the maximum bias voltage that the devices can operate at compared to devices with the silicon nitride passivation. When biased at 40 V, the output power of the devices reached 10.8 W/mm at 30 GHz. These devices show the importance of the passivation design to large-signal RF performance in scandium aluminum nitride-based HEMTs and will help guide further improvements. [ABSTRACT FROM AUTHOR]

Abstract Palliative care is critical to redundancy in cancer patients seeking to improve their quality of life. Evaluation should be incorporated into clinical practice routines at all stages of cancer. The Edmonton Symptom Assessment System (ESAS) was used to rate the intensity of ten symptom evaluations designed and validated for cancer patients in various languages and cultures. Therefore, the study aims to assess the symptoms reported using ESAS scores to identify patients who would benefit from palliative care that can improve the integration of palliative care into standard cancer care at An-Najah National University Hospital (NNUH). A cross-sectional study was selected for 271 cancer patients using a convenience sampling method at NNUH. Demographic, clinical, and lifestyle characteristics are described. Furthermore, patients' moderate to severe symptoms (score > 4) were obtained using ESAS-R. The survey consisted of 271 patients, with a response rate of 95%. The average age of the patients was 47 ± 17.7 years, ranging from 18 to 84 years. The male-to-female ratio was approximately 1:1, 59.4% of the patients were outpatients, and 153 (56.5%) had hematologic malignancies. Fatigue (62.7%) and drowsiness (61.6%) were the most common moderate to severe symptoms in ESAS. Furthermore, pain (54.6%), nausea (40.2%), lack of appetite (55.0%), shortness of breath (28.5%), depression (40.6%), anxiety (47.2%) and poor well-being (56.5%) were reported. In conclusion, fatigue and drowsiness were the most reported symptoms according to the ESAS scale among cancer patients, while moderate to severe symptoms were reported in cancer patients using the ESAS. The ESAS is a functional tool for assessing cancer patients' symptoms and establishing palliative care services.

We report the characterization of anthropogenic magnetic particulate matter (MPM) collected on leaves from roadside Callistemon (bottlebrush) trees from Lahore, Pakistan, and on known sources of traffic‐related particulates to assess the potential of first‐order reversal curve (FORC) diagrams to discriminate between different sources of anthropogenic magnetic particles. Magnetic measurements on leaves indicate the presence of surface‐oxidized magnetite spanning the superparamagnetic (<30 nm) to single domain (∼30–70 nm) to vortex size range (∼70–700 nm). Fe‐bearing particles are present both as discrete particles on the surface of larger mineral dust or carbonaceous particles and embedded within them, such that their aerodynamic sizes may be decoupled from their magnetic grain sizes. FORC diagrams of brake‐pad residue specimens show a distinct combination of narrow central ridge, extending from 0 to 200 mT, and a low‐coercivity, vertically spread signal, attributed to vortex and multi‐vortex behavior of metallic Fe. This is in agreement with scanning electron microscopy results that show the presence of metallic as well as oxidized Fe. Exhaust‐pipe residue samples display a more conventional "magnetite‐like" signal comprising a lower coercivity central ridge (0–80 mT) and a tri‐lobate signal attributed to vortex state and/or magnetostatic interactions. The FORC signatures of leaf samples combine aspects of both exhaust residue and brake‐pad endmembers, suggesting that FORC fingerprints have the potential to identify and quantify the relative contributions from exhaust and non‐exhaust (brake‐wear) emissions. Such measurements may provide a cost‐effective way to monitor the changing contribution; of future particulate emissions as the vehicle fleet is electrified over the coming years. This paper reviews the interpretation of one of the figures of modern interpretation, in which the interpretation is expected to be a solution to the problems of human life as the Qur'an was revealed as a mercy to the universe. Among the problems is leadership. Among the mufassir figures in Indonesia who have sat in government is Buya Hamka, who not interprets verses indices by getting involved in government. This study aims to find out how Buya Hamka interprets leadership in the Qur'an. The method used is a descriptive analysis where the primary source used is the book of al-Azhar interpretation. The author also explains asbab nuzul verses related to leaders in the Qur'an. Buya Hamka explained the meaning of a leader by using society's social context and conditions at that time and in a global context. Buya Hamka explained that fair leadership is omniscient and submissive to what God commands by providing benefit to the community without any elements to lead them astray.

Magnetic signals in deep‐sea sediments have the potential to unravel past continental environmental changes, via changes in primary terrigenous magnetic supply, but also record past marine environmental conditions, via in situ formation of secondary magnetic minerals, particularly when complemented by independent proxies. By combining environmagnetic, geochemical, and siliciclastic grain size data, we investigated marine sediment core GL‐1090 (24.92°S, 42.51°W, 2,225 m water depth) aiming to unravel changes in terrigenous sediment input and bottom water conditions during the last ∼184 ka at the western South Atlantic middepth. The Al/Si, Fe/κ and siliciclastic grain size data show that terrigenous sediments at this core location derived from the Plata River (southeastern South America). This material was transported northwards by the Brazilian Coastal Current and their delivery to our core site was modulated by sea‐level oscillations. Periods of low sea‐level were characterized by the input of coarser and more abundant terrigenous sediments. Environmagnetic parameters indicate significant downcore variations in the magnetic domain state, which we interpret as changes in the content of biogenic magnetite following glacial‐interglacial cycles. Coeval negative excursions in magnetic grain size and benthic δ13C suggests that concentrations of single domain magnetite (possibly magnetotactic bacterial magnetite) vary in response to middepth water ventilation. We suggest that reduced ventilation in the middepth western South Atlantic bottom waters during peak glaciations triggered a decrease in the production of biogenic magnetite. Peak glaciations were, in turn, linked with increases in the residence time of North Atlantic Deep Water (or its glacial counterpart). Background: Patients undergoing orthopedic surgery are at risk of post-operative complications and needing Medical Emergency Team (MET) review. We assessed the frequency of, and associations with MET calls in orthopedic patients, and whether this was associated with increased in-hospital morbidity and mortality.Methods: Retrospective cohort study of patients admitted over four years to a University teaching hospital using hospital administrative and MET call databases.Results: Amongst 6344 orthopedic patients, 55.8% were female, the median (IQR) age and Charlson comorbidity index were 66 years (47-79) and 3 (1-5), respectively. Overall, 54.5% of admissions were emergency admissions, 1130 (17.8%) were non-operative, and 605 (9.5%) patients received a MET call. The strongest independent associations with receiving a MET call was the operative procedure, especially hip and knee arthroplasty. Common MET triggers were hypotension (37.5%), tachycardia (25.0%) and tachypnoea (9.1%). Patients receiving a MET call were at increased risk of anemia, delirium, pressure injury, renal failure and wound infection. The mortality of patients who received a MET call was 9.8% compared with 0.8% for those who did not. After adjusting for pre-defined co-variates, requirement for a MET call was associated with an adjusted odd-ratio of 9.57 (95%CI 3.1-29.7) for risk of in-hospital death.Conclusions: Approximately 10% of orthopedic patients received a MET call, which was most strongly associated with major hip and knee arthroplasty. Such patients are at increased risk of morbidity and in-hospital mortality. Further strategies are needed to more pro-actively manage at-risk orthopedic patients. [ABSTRACT FROM AUTHOR]

Introduction Tuberculosis of the soft tissues is a rare form of extra pulmonary tuberculosis, and isolated localization in the popliteal fossa is particularly exceptional. Atypical clinical presentation can lead to delayed diagnosis and serious complications. Case report We report the case of a 17-year-old patient who was diagnosed with tuberculosis of the popliteal fossa. He presented with a painful inflammatory swelling of the right popliteal fossa associated with a homolateral inguinal lymph node, without knee joint effusion. Standard chest and knee X-rays were normal, while MRI showed an 8 cm well vascularized, partly liquefied mass in the popliteal fossa, developed in contact with the semimembranosus and medial gastrocnemius muscles, associated with a popliteal lymph node without synovial effusion or thickening. Microbiological tests did not isolate any germs. The diagnosis was made on histological examination after biopsy, which revealed a caseous granuloma surrounded by epithelioid cells. The patient was treated with anti-tuberculosis therapy for 9 months. The clinical and radiological regression of the swelling was observed without recurrence at 2 years of follow-up. Conclusion Any soft tissue abscess should raise suspicion of tuberculosis, especially in endemic countries. The importance of histopathological examination should be emphasized to establish the diagnosis in the absence of signs in favor of a primary localization.

Abstract Background Autologous hematopoietic stem-cell transplantation (HSCT) is the standard of care in many relapsed and refractory lymphoid malignancy, neuroblastoma, and multiple myeloma (MM). This study was conducted to describe the epidemiology of early infections that occurred within the first 100 days among patients who received HSCT for MM, Hodgkin (HL), and non-Hodgkin lymphoma (NHL) in Palestine. Methods This study was conducted in a retrospective cohort design in the only autologous HSCT in Palestine in the period between 2014 and 2021. The medical records of the patients were reviewed to identify and collect demographic, clinical, and microbiological data on bacterial, viral, fungal, and parasitic infections as diagnosed by cultures, polymerase chain reaction, and fluorescent antibody testing. Results A total of 145 patients were included in this study (median age = 44.0 [28.0, 53.5] years). Of those, 8 (5.5%) were younger than 18 years, 69 (47.6%) had MM, 53 (36.6%) had HL, and 23 (15.9%) had NHL. The source of fever had no focus in the majority of the cases 82 (56.6%), 12 (8.3%) had bloodstream infections, 8 (5.5%) had colitis, and 7.6 (5.0%) had pneumonia. Patients from whom gram-negative bacteria were isolated stayed in the hospital for longer duration compared to the other patients (median = 21.0 [19.0, 25.0] vs. 18.0 [15.0, 22.0] days, p-value = 0.043, respectively). The cumulative incidence of death in the first 100 days after infusion of stem cells was 3.4%. The cumulative incidence of death in the first 100 days post-transplantation was higher for patients with NHL compared to those with HL and MM (p-value = 0.017). Gram-negative and fungal infections were strong predictors of mortality. Conclusion Bacterial gram-positive and gram-negative infections were the most common early infections among patients who underwent autologous HSCT for hematological malignancies (HM) in the only center in Palestine. The findings of this study are informative to healthcare providers and planners of care for patients who are scheduled to receive autologous HSCT for HM.

Abstract The clinical use of circulating biomarkers for primary graft dysfunction (PGD) after lung transplantation has been limited. In a prospective single-center cohort, we examined the use of plasma protein biomarkers as indicators of PGD severity and duration after lung transplantation. The study comprised 40 consecutive lung transplant patients who consented to blood sample collection immediately pretransplant and at 6, 24, 48, and 72 h after lung transplant. An expert grader determined the severity and duration of PGD and scored PGD at T0 (6 h after reperfusion), T24, T48, and T72 h post-reperfusion using the 2016 ISHLT consensus guidelines. A bead-based multiplex assay was used to measure 27 plasma proteins including cytokines, growth factors, and chemokines. Enzyme-linked immunoassay was used to measure cell injury markers including M30, M65, soluble receptor of advanced glycation end-products (sRAGE), and plasminogen activator inhibitor-1 (PAI-1). A pairwise comparisons analysis was used to assess differences in protein levels between PGD severity scores (1, 2, and 3) at T0, T24, T48, and T72 h. Sensitivity and temporal analyses were used to explore the association of protein expression patterns and PGD3 at T48–72 h (the most severe, persistent form of PGD). We used the Benjamini–Hochberg method to adjust for multiple testing. Of the 40 patients, 22 (55%) had PGD3 at some point post-transplant from T0 to T72 h; 12 (30%) had PGD3 at T48–72 h. In the pairwise comparison, we identified a robust plasma protein expression signature for PGD severity. In the sensitivity analysis, using a linear model for microarray data, we found that differential perioperative expression of IP-10, MIP1B, RANTES, IL-8, IL-1Ra, G-CSF, and PDGF-BB correlated with PGD3 development at T48–72 h (FDR

In order for the researcher to get acquainted with the method of Ibn Kathir in this matter, the study applies inductive method to examine the verses and collect related scientific studies and analytical method to analyze these verses and reveal the Tarjih methodology using the Sunnah. Thereby, it is apparent that the Tarjih method using Sunnah of Ibn Kathir counts on what was reported from the Prophet peace be upon him, the authentic Sunnah and the Hadith even if there is a weak point in the requirements that needs to be cleared or strengthened such as Shawahid (Witnesses) and Mutabaat (Follow-ups) or the meaning of Quranic context.

This study was performed to examine the effect of ozonation treatment at different ozone concentrations (0.5, 2 and 5) ppm for different periods of time (5, 10 and 15) minutes on reduction percentage of spiked chlorfenapyr of (2, 6 and 10) ppm on lettuce leaves using gas chromatography equipped with micro electron capture detector. The results showed that ozonation treatment at 0.5, 2 and 10 ppm significantly reduced the spiked chlorfenapyr concentration on lettuce, after 15 minutes of ozone treatment the concentration of chlorfenapyr did not reach the allowed maximum residues limits in European regulations (0.01) ppm. The maximum reduction percentages after 15 min of ozone treatment at 0.5 ppm were 85.81, 88.47 and 91.21% for spiked concentrations of chlorfenapyr at 2, 6 and 10 ppm, respectively. On the other hand, the maximum reduction percentages after ozone treatment for 15 min at 2 ppm were 88.94, 93.25 and 94.77 % for spiked concentration of chlorfenapyrat 2, 6 and 10 ppm, respectively. At last the maximum reduction percentages for 15 min at 5 ppm ozone concentration were 92.25, 96.33 and 97.15 % for spiked concentrations of chlorfenapyrat 2, 6 and 10 ppm, respectively. Meanwhile, the reduction percentages after soaking with tap water for 15 min were 7.20,10.137 and 2.02 % for2, 6 and 10 ppm spiked chlorfenapyr concentrations on lettuce fruits. It was concluded from this research that ozone treatment has a significant effect on chlorfenapyr spiked concentrations and it could be used as a method for reduction of this pesticide residues in fruits and vegetables, but it need more than 15 min of ozone treatment to reduce its concentration below the European maximum residue limit.
