This research explores the effectiveness of a Ground Source Heat Pump (GSHP) system for cooling purposes in Indian conditions. Key parameters considered include the water’s flow rate, grout material, run time, return air temperature, and borehole configuration. A series of trials were conducted to review the system’s cooling effectiveness by employing two types of grout materials, construction, and demolition (C&D) waste, and the surrounding soil in four distinct borehole layouts. A linear regression model was used to assess how critical factors affected the system’s efficiency for each type of grout. An analysis of variance was also performed to determine which factors had the most significant impact. The study found that factors such as borehole length, system efficiency, and thermal characteristics significantly influenced the GSHP system's thermal performance. The findings revealed that the Coefficient of Performance (COP) of the heat pump and the overall system increased with the number of boreholes and volumetric flow rate. At the same time, it decreased with the rise in initial air temperature and extended operation run time. The mean COP for the GSHP arrangement utilized in thermal management with C&D waste as grout material was determined to be 8.1% lower than the value achieved when using the surrounding soil as the grout material. The results signify that utilizing a GSHP arrangement has the potential to be a favorable choice for cooling purposes. The system is effective, offering considerable energy savings compared to traditional air-conditioning systems. Nevertheless, the effectiveness of the system is determined by various elements, such as the length of the borehole, overall system efficiency, and thermal properties. Therefore, it is vital to carefully consider these factors when designing and installing a GSHP system.

This paper conducts innovative research on the stand-alone equipment and warehousing mode in the automated warehousing system to address the automatic warehousing of various specifications of products in small and medium-sized enterprises in China. The research proposes a new type of robotic storage device and storage method. Furthermore, the proposed storage scheme is simulated, and the results show that the scheme is feasible. In addition, the system layout was optimized based on the simulation results. This research proposes a new solution for the automatic storage problem of multispecification and low-traffic products and, at the same time, proposes new storage devices and models, which enrich the research in the field of automatic storage. Purpose Prospective, multicentric observational cohort study in Switzerland investigating measures to prevent mother-to-child transmission in pregnant women with HIV (WWH) and assessing health and development of their exposed children as well as of children with HIV (CWH) in general.Participants Between January 1986 and December 2022, a total of 1446 mother–child pairs were enrolled. During the same period, the study also registered 187 CWH and 521 HIV-exposed but uninfected children (HEU), for whom detailed maternal information was not available. Consequently, the cohort comprises a total of 2154 children.Findings to date During these 37 years, research by the Swiss Mother and Child HIV Cohort Study (MoCHiV) and its international collaborators has strongly influenced the prevention of vertical transmission of HIV (eg, introduction and discontinuation of elective caesarean section, neonatal postexposure prophylaxis and breastfeeding). Contributions have also been made to the management of diagnostics (eg, p24 antigen assay) and the effects of antiretroviral treatment (eg, prematurity, growth) in HEU and CWH.Future plans Most children present within the cohort are now HEU, highlighting the need to investigate other vertically transmitted pathogens such as hepatitis B and C viruses, cytomegalovirus or Treponema pallidum. In addition, analyses are planned on the longitudinal health status of CWH (eg, resistance and prolonged exposure to antiretroviral therapy), on social aspects including stigma in CWH and HEU, and on interventions to further optimise antenatal and postpartum care in WWH.

The land conversion of rice farming could only be prevented by increasing farming competitiveness. The first step is to map the resource availability supporting paddy farming and income achievement. Therefore, this study aimed to analyze resource availability and income achievement to increase paddy business competitiveness. It used a survey method and observed two populations of farmers producing paddy seed and rice in Tanjung Lago and Rambutan Districts, Banyuasin Regency. Simple random sampling was adopted to determine 130 respondents from two populations, each with 65 rice and paddy seed farmers. The findings showed that resource availability supports farming in tidal land, though it is significantly higher for paddy seed than rice farmers. Regarding income achievements, paddy seed farmers also have higher R/C and B/C values than rice farmers. Income for rice farmers mainly comes from corn, rubber, and oil palm, while paddy seed farmers earn from their farming. This implies that the chance of land conversion into plantations and corn farming is greater for rice than for paddy seed farmers. When this happens on a large scale and in the long term, it impacts national rice production. Therefore, improved resource availability and better integration in production could sustainably increase paddy farming production in tidal land.

BackgroundAccurate diagnosis of enteric fever is challenging, particularly in low- and middle-income countries, due to the overlap of clinical and laboratory features with other pathogens. To better understand the difficulties in enteric fever diagnosis, we evaluated the characteristics of patients clinically diagnosed with enteric fever and the real-world performance of TUBEX TF, one of the most used tests in Indonesia.Methodology/principal findingsPatients were recruited through the AFIRE (Etiology of Acute Febrile Illness Requiring Hospitalization) study at eight Indonesian hospitals. Blood culture was performed for all patients, and TUBEX TF was performed for suspected enteric cases. Salmonella PCR and ELISA tests were performed at a reference lab. Sensitivity and specificity of TUBEX TF and IgM and IgG anti-S. Typhi ELISA were determined. Of 301 patients clinically diagnosed with enteric fever, 50 (16.6%) were confirmed by blood culture and/or PCR. Confirmed cases were mostly school-aged children presenting with fever, anorexia, dizziness and/or abdominal pain with normal leukocyte count or leukopenia. TUBEX TF demonstrated a sensitivity of 97.6% to 70.7% and specificity of 38.3% to 67.2% at cutoffs of 4 and 6, respectively. Acute IgG demonstrated the best sensitivity and specificity, at 90.7% and 82.7%, respectively, and the best ROC characteristics.Conclusions/significanceA substantial proportion of enteric fever was misdiagnosed at all study hospitals, likely due to the overlap of clinical characteristics and lab parameters with those of other common pathogens. The TUBEX TF rapid serological assay demonstrated suboptimal performance in our setting and tended to over-diagnose enteric fever. The role of IgG from acute specimens for identification of enteric fever cases merits additional consideration.

7. Geographic redistribution of farmed salmonids reduces salmon lice infestations and treatment frequency in a simulation study

Host density is a key driver in parasite population dynamics, and often the number of parasites increases rapidly with host density. In the context of Norwegian salmonid farming, this mechanism has led to a disparity between the desire to increase cultured salmonid production, and to reduce the negative effects of parasite infestations. Salmon lice infestations are detrimental to animal welfare due to salmon lice treatments and spillover from farms to wild salmonids. Here, we examine how a redistribution of the salmonid farm biomass may hamper exchanges of lice larvae between farms, and consequently reduce the salmon lice burdens and treatment frequencies. More specifically, we use a stochastic simulation model, fitted to empirical data from Norwegian aquaculture, to examine how lice abundances and treatments responded when the biomass in the system was distributed onto fewer, larger farms situated farther apart. To maintain realistic fish growth, seasonality and cohort development, lice population dynamics were simulated on top of historic production data from Norway. We simulated several scenarios, where an increasing number of farms were closed, and their biomass was redistributed to other farms with matching cohorts. The results indicate that fewer and larger farms reduce lice numbers and treatment frequency, and that a strategic removal of farms, based on their importance for connectivity in an oceanographic lice dispersal network, improves this effect. Some core mechanisms are highlighted that should be considered in regional production planning, and in the allocation of production concessions in salmonid farming.

Background Pre-existing cardiovascular disease (CVD) or cardiovascular risk factors have been associated with an increased risk of complications following hospitalisation with COVID-19, but their impact on the rate of recovery following discharge is not known.Objectives To determine whether the rate of patient-perceived recovery following hospitalisation with COVID-19 was affected by the presence of CVD or cardiovascular risk factors.Methods In a multicentre prospective cohort study, patients were recruited following discharge from the hospital with COVID-19 undertaking two comprehensive assessments at 5 months and 12 months. Patients were stratified by the presence of either CVD or cardiovascular risk factors prior to hospitalisation with COVID-19 and compared with controls with neither. Full recovery was determined by the response to a patient-perceived evaluation of full recovery from COVID-19 in the context of physical, physiological and cognitive determinants of health.Results From a total population of 2545 patients (38.8% women), 472 (18.5%) and 1355 (53.2%) had CVD or cardiovascular risk factors, respectively. Compared with controls (n=718), patients with CVD and cardiovascular risk factors were older and more likely to have had severe COVID-19. Full recovery was significantly lower at 12 months in patients with CVD (adjusted OR (aOR) 0.62, 95% CI 0.43 to 0.89) and cardiovascular risk factors (aOR 0.66, 95% CI 0.50 to 0.86).Conclusion Patients with CVD or cardiovascular risk factors had a delayed recovery at 12 months following hospitalisation with COVID-19. Targeted interventions to reduce the impact of COVID-19 in patients with cardiovascular disease remain an unmet need.Trail registration number ISRCTN10980107.

9. Habitat use and movement patterns of adult male and juvenile scalloped hammerhead sharks Sphyrna lewini throughout the Hawaiian archipelago

Scalloped hammerhead sharks Sphyrna lewini are a circumglobal species found in tropical and subtropical waters. Globally, populations of S. lewini have undergone dramatic declines in recent decades, and 4 of 6 distinct population segments are at risk of extinction and listed under the United States Endangered Species Act. Despite this, limited data exist on movement behavior or habitat use requirements of S. lewini, especially in the Central Pacific region. In this long-term (2009-2020) telemetry study, 27 S. lewini (24 males [22 adults, 2 juveniles], and 3 juvenile females) ranging in size from 106 to 310 cm (total length) were tagged with a combination of acoustic and/or satellite tags in a known nursery area, Kāne‘ohe Bay, Hawai‘i. Acoustic data revealed repeated movements of adult male S. lewini to Kāne‘ohe Bay between May and September across multiple years. Horizontal movements away from the Bay indicate these individuals are highly associated with the Hawaiian Archipelago (i.e. Northwestern and main Hawaiian Islands), while vertical movements were dynamic, with repeated, nocturnal deep dives to depths beyond 800 m and temperatures as low as 5.0°C. We conclude that adult male and juvenile S. lewini tagged in Kāne‘ohe Bay exhibit fairly restricted movements throughout the Hawaiian Archipelago, and mature males specifically exhibit strong seasonal site fidelity to Kāne‘ohe Bay. These data add crucial baseline information on habitat preferences of S. lewini around the Hawaiian Islands, and can be used to help structure conservation strategies for a portion of the Central Pacific population.

Mitra dalam pengabdian kepada masyarakat ini adalah Poltekkes Kemenkes Aceh Prodi Keperawatan Aceh Selatan. Solusi permasalahan yang ditawarkan dalam upaya pencegahan terjadinya obesitas pada remaja adalah melalui penyuluhan tentang “isi piringku” yang sedang digalakkan oleh pemerintah pusat melalui Kementerian Kesehatan. "Isi Piringku" menggambarkan porsi makan yang dikonsumsi dalam satu piring yang terdiri dari 50 persen buah dan sayur, dan 50 persen sisanya terdiri dari karbohidrat dan protein. "Isi Piringku" juga menekankan untuk membatasi gula, garam, dan lemak dalam konsumsi sehari-hari. Tujuan Kegiatan pengabdian kepada masyarakat ini untuk meningkatkan pengetahuan dan perubahan perilaku mahasiswa dalam mengkonsumsi asupan gizi seimbang. Manfaat kegiatan pengabdian kepada masyarakat ini agar mahasiswa mengetahui pentingnya mengkonsumsi menu seimbang melalui “isi Piringku”.

Objectives: Standard of care for patients with chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) include fludarabine plus cyclophosphamide and rituximab (FCR) or bendamustine plus rituximab (BR). Targeting Bruton's tyrosine kinase (BTK) has revolutionized the first-line treatment options for CLL/SLL. Nemtabrutinib is a noncovalent, reversible, competitive inhibitor of BTK that targets both wild-type and C481S-mutant BTK. Manageable safety and preliminary antitumor activity of nemtabrutinib has been demonstrated in patients with CLL/SLL in an ongoing, open-label, phase 1/2 dose-escalation study (BELLWAVE-001). BELLWAVE-008 is an open-label, randomized, phase 3 study (NCT05624554) designed to evaluate efficacy and safety of nemtabrutinib compared with investigator's choice of chemoimmunotherapy (FCR or BR) in patients with treatment-naive CLL/SLL without TP53 aberrations. Materials and methods: Eligible patients are ≥18 years old with treatment-naive, active CLL/SLL and without TP53 aberrations (17p deletion and/or TP53 mutation) and an ECOG PS of 0-2. Patients with Richter transformation, active central nervous system involvement, or a history of severe bleeding disorder are excluded. Approximately 300 patients will be enrolled and randomly assigned 1:1 to receive nemtabrutinib (65 mg orally once daily until progressive disease, unacceptable toxicity, or discontinuation) or intravenous infusion of the investigator's choice of chemoimmunotherapy for 6 cycles lasting 28 days each (fludarabine 25 mg/m2 and cyclophosphamide 250 mg/m2 on days 1, 2, and 3 of each cycle, plus rituximab 375 mg/m2 initially, followed by 500 mg/m2 on day 1 of each cycle; or bendamustine 70-90 mg/m2 on days 1 and 2 of each cycle plus rituximab 375 mg/m2 initially, followed by 500 mg/m2 on day 1 of each cycle). We propose a novel approach to enhance the performance of perovskite solar cells (PSCs) by incorporating grated Cadmium Sulfide (CdS) and plasmonic gold nanoparticles (Au NPs) into the absorber layer. The CdS grating acts as the electron transport layer and penetrates into the perovskite absorber layer, increasing the absorption of the active layer and reducing the electron–hole recombination rate. The plasmonic Au NPs enhance the absorption in the infrared region by scattering and trapping the incident light. We perform a coupled optical and electrical study that shows a significant improvement in the short circuit current density ( $\mathrm{J}_{\mathrm{SC}}$ ) and power conversion efficiency (PCE) of the PSC after introducing the CdS grating and plasmonic Au NPs. Specifically, we observe a 48% increase in average optical absorption from 800 nm to 1400 nm and a 7.42 mA cm ^−2 increase in J $_\mathrm{SC}$ . We also find that the PCE of the PSC is increased by 7.91% when comparing the planar reference structure (without the CdS grating and the plasmonic Au NP). However, metal nanoparticles introduce ohmic losses and temperature rise in the solar cell. We analyze the non-radiative heat profile, electric field distribution, and temperature distribution across the PSC. We observe a temperature increase of approximately 14 K above the ambient temperature for the grated CdS layer with incorporated Au NPs, which is comparable to the temperature increase observed in the planar reference structure. Our results have the potential to pave the way for the development of highly efficient and stable PSCs in the future.

ABSTRACT The objective of the current study was to examine the effect of replacing maize with new or stored wheat on the nutrient intake, digestibility, digesta viscosity, and growth performance of growing broilers from 1-21 days of age. For this purpose, 560 one-day-old male broiler chicks were randomly divided into seven experimental treatments, each with 8 replicates of 10 birds. The control diet was a corn soybean-based diet, while six other experimental treatments replaced the corn of the control diet with either 50% or 100% of new, 1.5 year, and 2.5-year-old wheat. Introduction Type 2 diabetes mellitus (T2DM) is a powerful risk factor for cardiovascular disease (CVD), conferring a greater relative risk in women than men. We sought to examine sex differences in cardiometabolic risk factors and management in the contemporary cohort represented by the Glycemia Reduction Approaches in Diabetes: A Comparative Effectiveness Study (GRADE).Research design and methods GRADE enrolled 5047 participants (1837 women, 3210 men) with T2DM on metformin monotherapy at baseline. The current report is a cross-sectional analysis of baseline data collected July 2013 to August 2017.Results Compared with men, women had a higher mean body mass index (BMI), greater prevalence of severe obesity (BMI≥40 kg/m2), higher mean LDL cholesterol, greater prevalence of low HDL cholesterol, and were less likely to receive statin treatment and achieve target LDL, with a generally greater prevalence of these risk factors in younger women. Women with hypertension were equally likely to achieve blood pressure targets as men; however, women were less likely to receive ACE inhibitors or angiotensin receptor blockers. Women were more likely to be divorced, separated or widowed, and had fewer years of education and lower incomes.Conclusions This contemporary cohort demonstrates that women with T2DM continue to have a greater burden of cardiometabolic and socioeconomic risk factors than men, particularly younger women. Attention to these persisting disparities is needed to reduce the burden of CVD in women.Trial registration number ClinicalTrials.gov (NCT01794143)

Over the past 4 decades there has been a growing concern for the conservation status of elasmobranchs (sharks and rays). In 2002, the first elasmobranch species were added to Appendix II of the Convention on International Trade in Endangered Species of Wild Fauna and Flora (CITES). Less than 20 yr later, there were 39 species on Appendix II and 5 on Appendix I. Despite growing concern, effective conservation and management remain challenged by a lack of data on population status for many species, human−wildlife interactions, threats to population viability, and the efficacy of conservation approaches. We surveyed 100 of the most frequently published and cited experts on elasmobranchs and, based on ranked responses, prioritized 20 research questions on elasmobranch conservation. To address these questions, we then convened a group of 47 experts from 35 institutions and 12 countries. The 20 questions were organized into the following broad categories: (1) status and threats, (2) population and ecology, and (3) conservation and management. For each section, we sought to synthesize existing knowledge, describe consensus or diverging views, identify gaps, and suggest promising future directions and research priorities. The resulting synthesis aggregates an array of perspectives on emergent research and priority directions for elasmobranch conservation.

There has been a longstanding historic interest in a possible link between amyotrophic lateral sclerosis (ALS) and levels of physical activity, fuelled by high profile athletes intermittently acquiring the disease and raising its profile. This is the first prospective cohort study on ALS and physical activity, in which the authors conclude that there is a slightly decreased risk of dying from ALS in those with high levels of physical activity at enrolment. There are also inherent difficulties with the accuracy of utilising large-scale registers as the primary source of ALS diagnosis, with potentially inaccurate coding or missing ALS cases. The current study was designed to reveal the impact of ultraviolet (UV) light on polysaccharide intercellular adhesion of the icaD gene expression and its relation to biofilm as virulence markers in Staphylococcus aureus. In total, 37 isolates of S. aureus were gathered from various bacterial infection sources, and 26 S. aureus isolates were definite to icaD gene using the Polymerase Chain Reaction (PCR) technique. Prior to and after exposure to UV light, RNAs from the isolates were extracted which had strong biofilm formation by crystal violet staining assay, and they were then exposed to quantitative real-time PCR (qPCR). In vitro, the survival of isolates was evaluated after the exposition to several periods of the UV light power via counting the number of colonies. The results showed that the exposure to the UV light at 320 nm for 2, 4, 6, 8, and 10 min had inhibitory effects on the bacterial growth. The sub-lethal exposure time was 8 min. The rapid reduction of bacterial survival rates was associated with the increment irradiation time of UV power from 5 to 1.24 log10 CFU/ml. In general, down regulation of the icaD gene expression was decreased upon exposure to the UV light which was used as a physical agent for controlling the bacterial infection and biofilm formation.

Consumerism is a prevalent phenomenon that is already symptomatic in today's society. This study aims to discuss an ethic that supports the existence of a consumer society that was born and developed in a very long historical process. Colin Campbell was the first to speak of the romantic ethic that underlies the consumer society. This study uses the method of literature review. The main reference used is a book written by Campbell entitled "The Romantic Ethic and the Spirit of Modern Consumerism". The author also explores the essence of several other supporting books that provide interpretations and perspectives on Campbell's thoughts. The study results show several important concepts from Campbell's thought, including qualitative individualism, romantic ethics, and self-cult. These three conceptions are deeply attached to the spirit of humanism which became the catalyst for the Age of Enlightenment. Romantic ethical doctrine emphasizes the uniqueness or peculiarity of a person. Humans are encouraged to determine their destiny in developing themselves that are "genius, unique, and creative". As for consumption, it becomes a medium in the process of discovery and self-expression. Abstrak Konsumerisme telah menjadi fenomena yang menggejala di tengah kehidupan masyarakat dewasa ini. Studi ini membahas tentang sebuah etika yang mendukung eksistensi masyarakat konsumen yang lahir dan berkembang dalam proses sejarah yang sangat panjang. Colin Campbell pertama kali mengemukakan tentang etika romantis yang melandasi masyarakat konsumen. Studi ini menggunakan metode kajian pustaka. Referensi utama yang digunakan ialah buku yang ditulis oleh Campbell berjudul “The Romantic Ethic and the Spirit of Modern Consumerism”. Penulis juga menggali intisari dari beberapa buku pendukung lain yang memberikan penafsiran dan sudut pandang terhadap pemikiran Campbell. Hasil studi menampilkan beberapa konsep penting dari pemikiran Campbell, diantaranya individualisme kualitatif, etika romantis, dan kultus diri. Ketiga konsepsi tersebut sangat melekat dengan semangat humanisme yang menjadi katalis Abad Pencerahan. Doktrin etika romantis menekankan keunikan atau kekhasan seseorang. This study uses a qualitative research method with a descriptive case study approach. Data collection techniques in this study are through observation, documentation, and interviews. The instruments in this study were: a) the researcher himself, b) interview guidelines, c) supporting tools in the form of books, pens, and d) related documents. The results showed that actor interaction can occur if there is communication between implementing actors. Communication in the implementation of online learning can be seen in the accuracy or accuracy of communication and consistency of communication regarding online learning information from the principal to the teacher and finally to students and parents. Interaction between actors, namely principals, teachers, students, and parents in the implementation of online learning has not been carried out effectively. This is because the interaction between implementing actors is unresponsive to one party, namely students and parents. Apart from that, giving complicated and difficult assignments by the teacher, which makes it difficult for students and parents to understand, is also a factor in the existence of negligence or delay

Segara Anakan areas can be divided into three different regions according to their salinity. Salinity differences suggested that Commerson’s anchovy population in that area can be divided into three subpopulations due to genetic differences. Genetic differences among subpopulation can be assessed through a population genetic study using random amplified polymorphic DNA. This study aims to evaluate the genetic variation and differences of Commerson's anchovy (Stolephorus commersonnii) collected at three different water salinities in Segara Anakan estuary Cilacap Indonesia. Total genomic DNA was isolated using the Chelex method. Genetic diversity and differences were assessed using RAPD markers and were analyzed statistically using an analysis of molecular variance, as implemented in Arlequin software. The results showed that high genetic diversity was observed within the subpopulations. However, no significant genetic differences were observed among subpopulations which indicate genetic similarity. A high number of offspring are likely to cause high genetic variation within subpopulations. Adult and larvae migration is the cause of genetics similarity across Segara Anakan. Another impressive result is that water salinity did not affect the genetic characteristic of Commerson,s anchovy. Genetic similarity of Commerson’s anchovy indicates that Segara Anakan forms a single genetic conservation unit.

Bacillus cereus is a gram-positive, anaerobic, spore-forming bacterium related to food poisoning in humans. Vomit and diarrhea are the symptoms of foodborne B. cereus infection caused by emetic toxins and three enterotoxins, respectively. This bacterium is broadly present in soil and foods such as vegetables, spices, milk, and meat. The antibiotics impenem, vancomycin, chloramphenicol, gentamicin, and ciprofloxacin are used for all susceptible strains of B. cereus. But these antibiotics cause side effects in the host due to the drug–host interaction; because the targeted proteins by the drugs are not pathogen specific proteins, they are similar to human proteins also. To overcome this problem, this study focused on identifying putative drug targets in the pathogen B. cereus and finding new drugs to inhibit the function of the pathogen. The identification of drug targets is a pipeline process, starting with the identification of targets non-homologous to human and gutmicrobiota proteins, finding essential proteins, finding other proteins that highly interact with these essential proteins that are also highly important for protein network stability, finding cytoplasmic proteins with a clear pathway and known molecular function, and finding non-druggable proteins. Through this process, two novel drug targets were identified in B. cereus. Among the various antibiotics, Gentamicin had showed good binding affinity with the identified novel targets through molecular modeling and docking studies using Prime and GLIDE module of Schrödinger. Hence, this study suggest that the identified novel drug targets may very useful in drug therapeutic field for finding inhibitors which are similar to Gentamicin and designing new formulation of drug molecules to control the function of the foodborne illness causing pathogen B. cereus.

Presidential Regulation Number 87 of 2017 Article 2 states that character education must be the leading spirit in Indonesia's delivery of education. This study aims to describe the religious character education model implemented at Al-Hilal Islamic Primary School Bekasi, which is supported by students' parents' participation to build a learning process that not only produces an output of students who can graduate from school but also produce students who have religious characters. This research uses a qualitative approach with the type of case study method. The data collection techniques used were interviews, observation, and documentation. The results are the synergistic relationship and active role of all parties; in this case, the school and parents have successfully shaped the Islamic religious character. Students' character is built on a strong foundation from the family and strengthened through the Islamic education system in schools.

31. Effects of ocean acidification on toxicity of two trace metals in two marine molluscs in their early life stages

Ocean acidification (OA) is usually thought to change the speciation of trace metals and increase the concentration of free metal ions, hence elevating metal bioavailability. In this study, embryos of the oyster Crassostrea angulata and abalone Haliotis discus hannai were cultured under 4 pCO2 conditions (400, 800, 1500 and 2000 µatm) with Cu and Zn added. Fertilization rate was measured 2 h post-fertilization (hpf), while larval deformation and larval shell length were measured 24 hpf. Our results show that OA can alleviate Cu and Zn inhibition of C. angulata fertilization by 86.1 and 26.4% respectively, and Zn inhibition of H. discus hannai fertilization by 43.7%. However, OA enhanced the inhibitory effect of Cu on fertilization of H. discus hannai by 34.7%. OA enhanced the toxic effect of Cu on larval normality of C. angulata by 22.0% and the effect of Cu and Zn on larval normality of H. discus hannai by 71.4 and 37.2%, respectively. OA also enhanced the inhibitory effects of Cu and Zn on larval calcification in H. discus hannai by 8.8 and 8.6%, respectively. However, OA did not change the effect of Cu on the calcification of C. angulata larvae. OA decreased Zn inhibition of oyster larval calcification from 3.1 to 1.5%. Based on our results, the toxic effects of metal on early development of molluscs are not always increased by rising pCO2 and differ across developmental stages, egg structure and species. This complexity suggests that caution should be taken when carrying out multiple environmental stressor tests on molluscan embryos.

Purpose: Augmented survival of childhood nephroblastoma and neuroblastoma has increased long-term side effects such as metabolic syndrome (Met S). Risk stratification is difficult after abdominal radiation because waist circumferenc e underestimates adiposity. We aimed to develop a strategy for determining MetS in irradiated survivors using an integrated biomarker profile and vascular ultrasonograp hy. Methods: The NCEP-ATPIII MetS-components, 14 additional serum biomarker s and 9 vascular measurements were assessed in a single-centre cohort of childhood nephroblastoma (n = 67) and neuroblastoma (n = 36) survivors and controls (n = 61). Multivariable regression models were used to study treatment eff ects. Principal component analysis (PCA) was used to study all biomarkers in a combined analysis, to identify patterns and correlations. Results: After 27.5 years of follow-up, MetS occurred more often in sur vivors (14%) than controls (3%). Abdominal radiotherapy and nephrectomy, to a lesser extent, were associated with MetS and separate components and with seve ral biomarker abnormalities. PCA of biomarkers revealed a pattern on PC1 from favourable lipid markers (HDL-cholesterol, adiponectin) towards unfavourable markers (triglycerides, LDL-cholesterol, apoB, uric acid). Abdominal radiotherapy was a ssociated with the unfavourable biomarker profile (β = 1.45, P = 0.001). Vascular measurements were not of added diagnostic value. Conclusions: Long-term childhood nephro- and neuroblastoma survivors freque ntly develop MetS. Additional assessment of biomarkers identified in PCA – adiponectin, LDL, apoB, and uric acid – may be used especially in abdominally irr adiated survivors, to classify MetS as alternative for waist circumference. Vascular ultrasono graphy was not of added value.

33. Relationship between fiber cotton grade and some related characteristics of long and extra-long staple Egyptian cotton varieties (Gossypium barbadense. L)

The materials used in this study were four commercial varieties of Egyptian cotton; two (Giza 86 and Giza 90) belonging to the long staple class and the others (Giza 88 and Giza 92) belonging to the extra-long staple category. Within each variety, nine lint cotton grades namely: Fully Good (FG), Good/Fully Good (G/FG), Good (G), Fully Good Fair/Good (FGF/G), Fully Good Fair (FGF), Good Fair/ Fully Good Fair (GF/FGF), Good Fair (GF), Fully Fair/Good Fair (FF/GF) and Fully Fair (FF) were used. Thus Fully Good (FG) is the top quality grade and the others are progressively lower; i.e. Fully Fair (FF) is the lowest grade. Fiber properties were measured by using the Cotton Classifying System Version-5 instrument (CCS-V5). Data collected for the following characteristics were: reflectance degree (Rd %), yellowness degree (+b), trash%, dust%, fiber fragments%, total trash%, No. of neps and Micronaire value. Mean squares due to all nine grades of long and extra-long staple cotton varieties in combined analysis were highly significant over seasons for all studied characters except yellowness (+b) and fiber fragments% in Giza 92. Highly significant mean performance values of four cotton varieties and nine lint grades for each variety for all studied characters, it became clear that there were significant genetically differences between studied varieties and fiber cotton lint grades within each variety. Gradually increased for yellowness degree, trash%, dust%, fiber fragments%, total trash% (trash, dust, fiber fragments) and number of neps, while gradually decreased for reflectance degree (Rd%) and micronaire value with significant level as transfer for all tested varieties from (FG) grade down to the (FF). All studied varieties showed highly significant negative correlation between fiber cotton grade with yellowness, trash, dust, total trash% and No. of neps, while highly significant positive correlation between fiber cotton grade with reflectance degree and micronaire value. Total trash% and micronaire value as well as reflectance degree and their interactions were the most contributing and influencing of fiber cotton grades. Also these properties and their joint effects are prevailing factors which affect the personal judgment of the grader at evaluating the cotton grade.

Boron (B), as a low-Z material, is widely employed for wall conditioning to enhance plasma performance in fusion devices. In the Experimental Advanced Superconducting Tokamak, a series of experiments involving real-time B powder injection has been conducted to investigate fuel particle behavior. It was observed that fuel particle recycling decreased with an increase in the amount of B powder injected, resulting in an increase in short-term fuel retention. The fuel recycling decreased by up to 80%, as indicated by divertor neutral pressure and D α line emission. Furthermore, each B atom exhibited a trapping capacity of 0.3 D particles during B powder injection at a typical flow rate. The real-time B injection had no wall hysteresis effect on D retention, implying that cumulative B injection and deposited film did not affect long-term D retention. The possible mechanism for D retention is the formation of B-C-O-D compounds and co-deposition between B and D particles during discharges. This investigation would be valuable for evaluating T retention when B is used as wall conditioning material in future fusion reactor devices.

Objectives Studies have demonstrated high rates of mortality in people with proximal femoral fracture and SARS-CoV-2, but there is limited published data on the factors that influence mortality for clinicians to make informed treatment decisions. This study aims to report the 30-day mortality associated with perioperative infection of patients undergoing surgery for proximal femoral fractures and to examine the factors that influence mortality in a multivariate analysis.Setting Prospective, international, multicentre, observational cohort study.Participants Patients undergoing any operation for a proximal femoral fracture from 1 February to 30 April 2020 and with perioperative SARS-CoV-2 infection (either 7 days prior or 30-day postoperative).Primary outcome 30-day mortality. Multivariate modelling was performed to identify factors associated with 30-day mortality.Results This study reports included 1063 patients from 174 hospitals in 19 countries. Overall 30-day mortality was 29.4% (313/1063). In an adjusted model, 30-day mortality was associated with male gender (OR 2.29, 95% CI 1.68 to 3.13, p80 years (OR 1.60, 95% CI 1.1 to 2.31, p=0.013), preoperative diagnosis of dementia (OR 1.57, 95% CI 1.15 to 2.16, p=0.005), kidney disease (OR 1.73, 95% CI 1.18 to 2.55, p=0.005) and congestive heart failure (OR 1.62, 95% CI 1.06 to 2.48, p=0.025). Mortality at 30 days was lower in patients with a preoperative diagnosis of SARS-CoV-2 (OR 0.6, 95% CI 0.6 (0.42 to 0.85), p=0.004). There was no difference in mortality in patients with an increase to delay in surgery (p=0.220) or type of anaesthetic given (p=0.787).Conclusions Patients undergoing surgery for a proximal femoral fracture with a perioperative infection of SARS-CoV-2 have a high rate of mortality. This study would support the need for providing these patients with individualised medical and anaesthetic care, including medical optimisation before theatre. Careful preoperative counselling is needed for those with a proximal femoral fracture and SARS-CoV-2, especially those in the highest risk groups.Trial registration number NCT04323644

Abstract Background The development and assessment of clinical judgment ability are essential in nursing education. The Lasater Clinical Judgment Rubric (LCJR) was shown to be valid in evaluating nursing students’ learning outcomes and skills in western cultures but has not been validated in mainland China. This study aimed to compare a simulation-teaching model with a traditional teaching method in enhancing the clinical judgment ability of nursing undergraduate students and to validate the Chinese version of the Lasater Clinical Judgment Rubric (C-LCJR). Methods Four classes of nursing students (n = 157) at Hubei University of Chinese Medicine, China, were randomly assigned to two control and two experimental classes. The experimental classes were taught using simulation teaching with standardized patients, while the control classes were taught using traditional teaching methods. At the end of the experiment, students in both kinds of classes evaluated their clinical judgment using the C-LCJR. Teachers also rated the students but without knowing who had received the simulation teaching. Confirmatory factor analysis and a Multiple Indicators Multiple Causes (MIMIC) model with Bayesian estimation was fit to the rating data to investigate measurement properties and experimental effects. Results Compared to the control classes, students in the experimental classes performed better in all subdomains of C-LCJR (noticing, interpreting, responding, and reflecting). The measurement properties of the C-LCJR were found to be satisfactory with high factor loadings and reliabilities and no bias from age, gender, and raters. Conclusions The simulation teaching model is more effective than the traditional (non-simulation-based) teaching method in improving clinical judgment of Chinese nursing students. The C-LCJR is a valid and reliable instrument for measuring clinical judgment in nursing students in China.

ABSTRACT The objective of this experiment was to investigate the effect of supplementation of an exogenous emulsifier (lyso-phospholipid) in the diet of growing broilers on growth performance and digestibility. A total of 1224 Ross-308 ten day old broiler chicks were distributed into two experimental treatments in such a way that each treatment had twelve replicates with fifty-one birds per replicate. Two experimental diets were formulated with and without emulsifier supplementation according to the nutrition standards of Ross 308. Feed intake and body weight gain of the broilers were measured on a daily basis and feed conversion ratio was also calculated. Nutrient digestibility was determined on the 25th day of age. Analysis of variance under completely randomized design technique was used to analyze the data. Objectives Informing research participants of the results of studies in which they took part is viewed as an ethical imperative. However, there is little guidance in the literature about how to do this. The Fluoxetine Or Control Under Supervision trial randomised 3127 patients with a recent acute stroke to 6 months of fluoxetine or placebo and was published in the Lancet on 5 December 2018. The trial team decided to inform the participants of the results at exactly the same time as the Lancet publication, and also whether they had been allocated fluoxetine or placebo. In this report, we describe how we informed participants of the results.Design In the 6-month and 12-month follow-up questionnaires, we invited participants to provide an email address if they wished to be informed of the results of the trial. We re-opened our trial telephone helpline between 5 December 2018 and 31 March 2019.Setting UK stroke services.Participants 3127 participants were randomised. 2847 returned 6-month follow-up forms and 2703 returned 12-month follow-up forms; the remaining participants had died (380), withdrawn consent or did not respond.Results Of those returning follow-up questionnaires, a total of 1845 email addresses were provided and a further 50 people requested results to be sent by post. Results were sent to all email and postal addresses provided; 309 emails were returned unrecognised. Seventeen people replied, of whom three called the helpline and the rest responded by email.Conclusion It is feasible to disseminate results of large trials to research participants, though only around 60% of those randomised wanted to receive the results. The system we developed was efficient and required very little resource, and could be replicated by trialists in the future.Trial registration number ISRCTN83290762; Post-results.

42. Clinical effectiveness of multimodality treatment on advanced pediatric hepatoblastoma.

OBJECTIVES: To investigate the effect of multimodality treatment of advanced paediatric hepatoblastoma and the factors affecting the prognosis. PATIENTS AND METHODS: 35 childhood patients were treated with multimodality treatments consisting of chemotherapy, surgery, interventional therapy, and autologous peripheral blood stem cell transplantation. Patients were followed up every month. RESULTS: 33 patients completed the follow-up, of which 17 were in complete remission, 5 were in partial remission, 1 case got worse, and 10 died. The remission rate was 66.7% (22/33), and the overall survival rate was 69.7% (23/33). 1 patient with advanced hepatoblastoma got high-dose chemotherapy combined with autologous peripheral blood stem cell transplantation (APBSCT) treatment, and a primary lesion by 18 x 15 x 9 cm reduced to 10 x 8 x 4 cm. Remote metastases significantly al leviated, and partial remission reached six months. The overall survival was 9 months after transplantation. Patients with the mixed phenotype of hepatoblastoma had a worse prognosis than with the epithelial phenotype (p < 0.001), and patients in stage IV had a lower survival rate than in stage III (p < 0.001). CONCLUSIONS: Multimodality treatment can effectively improve remission rate and prolong the survival of children with the advanced hepatoblastoma. In addition, alpha-fetoprotein (AFP), hepatoblastoma pathological classification and staging are of great use in prediction of prognosis. [ABSTRACT FROM AUTHOR]

In the detection of locked modes using saddle loops, the problem of how to remove the axisymmetric equilibrium flux picked up by the loops has still to be solved. The problem becomes more difficult when there are conductive structures located near the saddle loops. In this paper, we present an analytical model based on lumped eddy current circuits and use it to interpret the measured equilibrium flux and the corresponding eddy current fluxes. Using this model, precise compensation for fluxes induced by the horizontal field coils and the toroidal field coils, with relative errors of less than 1%, has been realized for the saddle loops in the Joint Texas Experimental Tokamak. Nitric oxide (NO) is an important signalling molecule involved in a diverse range of physiological processes. In this study, the effects of sodium nitroprusside (SNP), a donor of NO, on the growth and anti-oxidant capacity of leaves of tall rescue [Festuca arundinacea (Schreb.) cv. Arid3] were investigated under low shade (LS), moderate shade (MS), and heavy shade (HS) applied for 7 d and 14 d in all cases. LS plants exhibited slight injury, while MS and HS caused a significant increase in cell membrane damage throughout the stress period, which was measured by enhanced electrolyte leakage (EL) and increased malondialdehyde (MDA) concentrations. Exogenous application of 0.1 mM SNP alleviated this oxidative stress, increased leaf widths and chlorophyll (Chl) concentrations, and decreased leaf lengths in plants grown under shade stress. Spray applications of of 0.1 mM SNP also increased the activities of the anti-oxidant enzymes, superoxide dismutase (SOD), catalase (CAT), and peroxidase (POX), and the accumulation of proline in stressed leaves. Significant increases in endogenous NO concentration were found in tall rescue leaves after 7 d and 14 d of shade. SNP-treated plants grown under shade stress produced higher concentrations of NO than untreated, shaded plants. The application of SNP was more effective at reducing the detrimental effects of shade stress on tall rescue leaves under MS than under LS or HS stress. These results suggest that one or more of the breakdown product(s) of SNP might be signalling molecule(s) which enhance the activities of anti-oxidant enzymes and increase proline concentrations to alleviate oxidative damage in the leaves of tall rescue plants grown under shade stress. A technique for rapidly detecting microwave magnitude and phase has been developed using a spintronic device as a microwave sensor, which allows a lock-in amplifier to perform real-time microwave measurement. To demonstrate the feasibility and reliability of the proposed approach, the resonance including the amplitude and phase in a complementary electric inductive-capacitive resonator has been characterized. The results are in agreement with measurement preformed by a vector network. This sensor approach is not limited for use only with spintronic devices, but can also be used with semiconductor devices and hence offers a useful alternative to existing microwave imaging and characterization technologies. The bias drift generated by temperature characteristic of ring laser gyros (RLG) can deteriorate dramatically the performance of gyroscope. This thesis studies convenient bias drift compensation of RLG against thermal effects. The compensation effect in different compositions of the three ternperature sensors is compared, and the result shows a sensor redundancy around the anode; thus the model is simplified, and four different experiments have been devised for validation. ABSTRACT The study was conducted to check the effect of emulsifier supplementation on growth performance of broiler birds on starter phase, from days 1 to 10. In the current study, a total of twelve hundred twenty-four Ross 308 broiler one day old chicks were used. Ross 308 broiler chicks were divided into two groups in such a way that each group had12 replicates, and each replicate had 51 birds. The duration of the current trial was of 10 days. Chicks were fed experimental diets with or without emulsifier supplementation. Daily feed intake, body weight, feed conversion ratio and nutrient digestibility were recorded. Mortality was recorded on a daily basis, and feed intake was also corrected for mortality. Introduction With the widespread use of electronic health records and handheld electronic devices in hospitals, informatics-based antimicrobial stewardship interventions hold great promise as tools to promote appropriate antimicrobial drug prescribing. However, more research is needed to evaluate their optimal design and impact on quantity and quality of antimicrobial prescribing.Methods and analysis Use of smartphone-based digital stewardship applications (apps) with local guideline directed empirical antimicrobial use by physicians will be compared with antimicrobial prescription as per usual as primary outcome in three hospitals in the Netherlands, Sweden and Switzerland. Secondary outcomes will include antimicrobial use metrics, clinical and process outcomes. A multicentre stepped-wedge cluster randomised trial will randomise entities defined as wards or specialty regarding time of introduction of the intervention. We will include 36 hospital entities with seven measurement periods in which the primary outcome will be measured in 15 participating patients per time period per cluster. At participating wards, patients of at least 18 years of age using antimicrobials will be included. After a baseline period of 2-week measurements, six periods of 4 weeks will follow in which the intervention is introduced in 6 wards (in three hospitals) until all 36 wards have implemented the intervention. Thereafter, we allow use of the app by everyone, and evaluate the sustainability of the app use 6 months later.Ethics and dissemination This protocol has been approved by the institutional review board of each participating centre. Abstract Background Currently, no suitable clinical marker for detection of septic immunosuppression is available. We aimed at identifying microRNAs that could serve as biomarkers of T-cell mediated immunoparalysis in sepsis. Methods RNA was isolated from purified T-cells or from whole blood cells obtained from septic patients and healthy volunteers. Differentially regulated miRNAs were identified by miRNA Microarray (n = 7). Validation was performed via qPCR (n = 31). Results T-cells of septic patients revealed characteristics of immunosuppression: Pro-inflammatory miR-150 and miR-342 were downregulated, whereas anti-inflammatory miR-15a, miR-16, miR-93, miR-143, miR-223 and miR-424 were upregulated. Assessment of T-cell effector status showed significantly reduced mRNA-levels of IL2, IL7R and ICOS, and increased levels of IL4, IL10 and TGF-β. The individual extent of immunosuppression differed markedly. MicroRNA-143, − 150 and − 223 independently indicated T-cell immunoparalysis and significantly correlated with patient’s IL7R-/ICOS-expression and SOFA-scores. In whole blood, composed of innate and adaptive immune cells, both traits of immunosuppression and hyperinflammation were detected. Importantly, miR-143 and miR-150 – both predominantly expressed in T-cells – retained strong power of discrimination also in whole blood samples. Conclusions These findings suggest miR-143 and miR-150 as promising markers for detection of T-cell immunosuppression in whole blood and may help to develop new approaches for miRNA-based diagnostic in sepsis.

50. Impact of 3D echocardiography on grading of mitral stenosis and prediction of clinical events

Background: The mitral valve orifice area (MVOA) is difficult to assess accurately by 2D echocardiography because of geometric assumptions; therefore, 3D planimetry may offer advantages. We studied the differences in MVOA measurements between the most frequently used methods, to determine if 3D planimetry would result in the re-grading of severity in any cases, and whether it was a more accurate predictor of clinical outcomes. Methods: This was a head-to-head comparison of the three most commonly used techniques to grade mitral stenosis (MS) by orifice area and to assess their impact on clinical outcomes. 2D measurements (pressure half-time (PHT), planimetry) and 3D planimetry were performed retrospectively on patients with at least mild MS. The clinical primary endpoint was defined as a composite of MV balloon valvotomy, mitral valve repair or replacement (MVR) and/or acute heart failure (HF) admissions. Results: Forty-one consecutive patients were included; the majority were female (35; 85.4%), average age 55 (17) years. Mean and peak MV gradients were 9.4 (4) mmHg and 19 (6) mmHg, respectively. 2D and 3D measures of MVOA differed significantly; mean 2D planimetry MVOA was 1.28 (0.40) cm2, mean 3D planimetry MVOA 1.15 (0.29) cm2 (P = 0.003). Mean PHT MVOA was 1.43 (0.44) cm2 (P = 0.046 and P < 0.001 in comparison to 2D and 3D planimetry methods, respectively). 3D planimetry reclassified 7 (17%) patients from mild-to-moderate MS, and 1 (2.4%) from moderate to severe. Overall, differences between the two methods were significant (X2, P < 0.001). Only cases graded as severe by 3D predicted the primary outcome measure compared with mild or moderate cases (odds ratio 5.7). Conclusion: 3D planimetry in MS returns significantly smaller measurements, which in some cases results in the reclassification of severity. Routine use of 3D may significantly influence the management of MS, with a degree of prediction of clinical outcomes.
