Cataracts are ocular conditions characterized by the opacification of the natural lens within the eye, which develops gradually over time and can affect one or both eyes. This condition commonly results from age-related changes in the lens, but can also arise from various factors. Cataract surgeries are expensive, particularly in states such as Spain, where they receive full support from the Spanish social welfare system. Despite a significant body of research on cataracts, few studies address the social and environmental factors triggering their development or consider the spatiotemporal evolution of their impacts. We analysed the incidence of cataracts in a southern region of Spain, differentiating between senile cataracts (those over 60 years old) and early cataracts (those between 15 and 59 years old). Twenty-one socio-economic, climate, and air pollution variables were statistically analysed using bivariate correlation, cluster analysis, and Geographic Information Systems. Eleven years of observation show a decadal increase in annually averaged maximum temperature and a decrease in annual precipitation, partially explaining the rising incidence of operable cataracts in the following year (r = 0.77 and −0.84, respectively; p < 0.05). Furthermore, early cataracts responded spatially to % agricultural employment (r = 0.85; p < 0.05) and moderately to maximum temperatures, insolation, and various constituents.

Abstract Nucleic acid‐based therapeutics that regulate gene expression have been developed towards clinical use at a steady pace for several decades, but in recent years the field has been accelerating. To date, there are 11 marketed products based on antisense oligonucleotides, aptamers and small interfering RNAs, and many others are in the pipeline for both academia and industry. A major technology trigger for this development has been progress in oligonucleotide chemistry to improve the drug properties and reduce cost of goods, but the main hurdle for the application to a wider range of disorders is delivery to target tissues. The adoption of delivery technologies, such as conjugates or nanoparticles, has been a game changer for many therapeutic indications, but many others are still awaiting their eureka moment. Here, we cover the variety of methods developed to deliver nucleic acid‐based therapeutics across biological barriers and the model systems used to test them. We discuss important safety considerations and regulatory requirements for synthetic oligonucleotide chemistries and the hurdles for translating laboratory breakthroughs to the clinic. Recent advances in the delivery of nucleic acid‐based therapeutics and in the development of model systems, as well as safety considerations and regulatory requirements for synthetic oligonucleotide chemistries are discussed in this review on oligonucleotide‐based therapeutics.

Objetivo: Describir que el ojo, ante procesos infecciosos sistémicos diversos que afectan a sus estructuras, tales como tuberculosis, infección por virus Ébola o sida (con sus infecciones oportunistas), una vez curado de su fase activa infecciosa reacciona de manera similar. Caso clínico: Presentamos el caso de una paciente que, tras ser tratada con éxito de tuberculosis ocular, desarrolló una coroiditis multifocal por hipersensibilidad retardada, de efectos potencialmente más devastadores que la propia infección activa, con inflamación coroidea y de vasos retinianos, papilitis y edema macular que, sin tratamiento, conduciría a una gran pérdida visual. Para su control, precisó corticoides sistémicos a corto plazo e inmunosupresores sistémicos de mantenimiento a largo plazo. Conclusiones: Las reacciones inflamatorias oculares por hipersensibilidad son más destructivas que la infección directa de diversos patógenos, ya que pueden generar una reacción inflamatoria basada en el reconocimiento del sistema inmunitario de restos antigénicos infecciosos en las estructuras oculares. Comprobar el control de la infección activa y posteriormente introducir corticoides sistémicos o inmunosupresores ahorradores de corticoides tiene efectos muy beneficiosos.

Antisense oligonucleotides (AONs) hold promise for therapeutic splice-switching correction in many genetic diseases. However, despite advances in AON chemistry and design, systemic use of AONs is limited due to poor tissue uptake and sufficient therapeutic efficacy is still difficult to achieve. A novel class of AONs made of tricyclo-DNA (tcDNA) is considered very promising for the treatment of Duchenne muscular dystrophy (DMD), a neuromuscular disease typically caused by frameshifting deletions or nonsense mutations in the gene-encoding dystrophin and characterized by progressive muscle weakness, cardiomyopathy, and respiratory failure in addition to cognitive impairment. Herein, we report the efficacy and toxicology profile of a 13-mer tcDNA in mdx mice. We show that systemic delivery of 13-mer tcDNA allows restoration of dystrophin in skeletal muscles and to a lower extent in the brain, leading to muscle function improvement and correction of behavioral features linked to the emotional/cognitive deficiency. More importantly, tcDNA treatment was generally limited to minimal glomerular changes and few cell necroses in proximal tubules, with only slight variation in serum and urinary kidney toxicity biomarker levels. These results demonstrate an encouraging safety profile for tcDNA, albeit typical of phosphorothiate AONs, and confirm its therapeutic potential for the systemic treatment of DMD patients. Presentamos el caso de un ex toxicómano de 50 años que acudió a consulta de oftalmología con visión borrosa bilateral. En la anamnesis refería su desintoxicación 20 años atrás, con abstinencia total desde entonces y buena reinserción social. Su historia clínica reflejaba serología de hepatitis B+ y C+, pero VIH− y recientemente comenzó con esclerosis múltiple, controlada con interferón beta. Señalaba haber padecido últimamente un resfriado invernal prolongado. En la exploración oftalmológica se apreciaba una uveítis bilateral con precipitados queráticos. En funduscopia del ojo derecho destacaban múltiples granulomas coroideos y zonas de vasculitis periférica, activas y cicatriciales. La funduscopia del ojo izquierdo era normal. Tras diversas pruebas diagnósticas, como radiografía torácica y TAC pulmonar, se detectaron varios nódulos pulmonares. Asimismo, se realizó una prueba de Mantoux que resultó fuertemente positiva. La analítica mostraba neutropenia y linfopenia importantes. Por todo ello se hizo un diagnóstico de presunción de panuveítis secundaria a tuberculosis sistémica. Se administraron fármacos antituberculosos y corticoides sistémicos, con buena respuesta clínica sistémica y ocular. El interferón beta 1b es un inmunomodulador apropiado para la esclerosis múltiple, pero sus principales efectos secundarios son alteraciones analíticas como leucopenia, linfocitopenia y trombocitopenia. Los linfocitos CD4+ T, leucocitos, macrófagos y granulocitos, con la producción de sus mediadores interferón gamma, IL-12 o TNF-α son fundamentales para controlar al Mycobacterium tuberculosis. Por ello, antes de introducir Interferón beta 1b, convendría realizar técnicas de screening, como la prueba de Mantoux o el interferon gamma release assay–(quantiferon-TB) para detectar posibles tuberculosis latentes potencialmente activables.

La retinosis pigmentaria unilateral es una patología relativamente infrecuente, no habiendo sido recogidos más de un centenar de casos en la bibliografía científica desde 1865. Por tanto, la asociación con foseta papilar es todavía más infrecuente, no quedando descartado un mecanismo intrínseco común. Presentamos el caso de una paciente de 66 años de edad, seguida desde el año 2000 en nuestro servicio, con una coincidencia de foseta papilar y retinosis pigmentaria unilateral en el mismo ojo. Como consecuencia de la foseta papilar desarrolló un proceso de desprendimiento seroso macular que no se reabsorbió espontáneamente y que acabó produciendo un agujero lamelar macular. Este tipo de complicaciones actualmente son en algunos casos tratadas con vitrectomía; sólo que la coincidencia en el mismo ojo de retinosis pigmentaria y foseta papilar hizo que se modificara la indicación terapéutica por la enorme degeneración y atrofia de la retina periférica de dicho ojo. Así, se optó finalmente por una fotocoagulación con láser de argón en el borde de la foseta papilar, consiguiendo una importante ganancia visual y evitando las variaciones visuales que a lo largo del día experimentaba la paciente, por haberse acumulado líquido en la mácula a lo largo de la noche, frenando el proceso de deterioro macular.

Lo que no dice la antropología, de Sophie Caratini, fue publicado por primera vez en Francia en 2004. Más tarde, en 2012, vio la luz una edición actualizada y aumentada, que ha sido traducida al castellano por Inmaculada Jiménez Moll, y publicada en España en 2013.

Antisense Oligonucleotides (ASOs) represent very attractive therapeutic compounds for the treatment of numerous diseases. The antisense field has remarkably progressed over the last few years with the approval of the first antisense drugs and with promising developments of more potent and nuclease resistant chemistries. Despite these recent clinical successes and advances in chemistry and design, effective delivery of ASOs to their target tissues remains a major issue. This review will describe the latest advances obtained with the tricyclo-DNA (tcDNA) chemistry which displays unique pharmacological properties and unprecedented uptake in many tissues after systemic administration. We will examine the variety of therapeutic approaches using both fully modified tcDNA-ASOs and gapmers, including splice switching applications, correction of aberrant splicing, steric blocking strategies and targeted gene knock-down mediated by RNase H recruitment. We will then discuss the merits and potential liabilities of the tcDNA chemistry in the context of ASO drug development.
