Background: Ménière's disease is a chronic condition of the inner ear that causes vertigo, tinnitus and hearing loss. Its diagnosis relies on clinical criteria that are subjective and pure-tone audiometry results that are not specific. Its pathological substrate is endolymphatic hydrops. Its imaging was recently made possible by the late-enhanced 3D FLAIR sequence. This technique was primarily tested on 3 T. Our objective was to prove its feasibility using a 1.5 T magnet. Methods: We conducted a prospective study including 30 patients who fulfilled the Bárány society criteria for Ménière's disease. We performed the late-enhanced 3D FLAIR sequence on all patients. We used it to look for and grade endolymphatic hydrops in the utricle, the saccule and the cochlear canal using the Kahn method. Results: We found endolymphatic hydrops in all of the 30 patients who fulfilled the diagnostic criteria for Ménière's disease. We had no false positives and only one false negative with a patient presenting with bilateral disease clinically but having endolymphatic hydrops only on one side. Thus, our correspondence rate between clinical and imaging findings was 97%. Conclusions: It is possible to diagnose endolymphatic hydrops with the late-enhanced 3D FLAIR sequence using a 1.5 T MRI machine. Since Ménière's disease diagnosis is sometimes tricky, imaging endolymphatic hydrops can aid in the diagnosis when the clinical picture is incomplete. It also helps guide invasive treatment plans. Feasibility at 1.5 T ensures broader access to the late-enhanced 3D FLAIR sequence. Introduction and objective: Within acute and episodic childhood vertigos, those secondary to trauma, otitis media, neuritis/labyrinthitis, perilymphatic fistula, Ménière's disease, vestibular paroxysmia, benign paroxysmal positional vertigo and bilateral vestibulopathy account for around 10% of all cases of infant vestibular disorders. Diagnosis must be early accomplished based on the defining criteria, although in children there are other accessory criteria that should be considered (behavioral changes, digestive, etc...). The treatment should achieve prompt symptomatic resolution of the crises, prevention or reduction of recurrences, minimization and palliation of sequelae and maximum normalization of daily life and activities. Objective: To carry out a brief and practical review on the symptoms, the diagnostic criteria and the treatment of infantile vertigo in the previously mentioned topics. Method: Narrative review using PubMed, Fisterra, ClinicalKey and Google Scholar. Results: Citations were obtained, of which non-significant ones were discarded due to their general nature, age, non-relevance according to the authors' criteria, or non-availability in English or Spanish. A total of 60 articles were considered for this review. Discussion: Narrative review on the topics described. Conclusions: The diagnostic criteria and treatment for the topics proposed in this chapter are defined for adults; however, in children, we must consider accessory criteria for a prompt diagnosis. The treatment is conditioned by age and is based on several pillars (pharmacological, lifestyle, rehabilitation and psychological). Objective: To evaluate otolith function by comparing the findings of the Subjective Visual Vertical, the cervical Vestibular Evoked Myogenic Potential and the ocular Vestibular Evoked Myogenic Potential tests in patients in the inter-crisis period of unilateral definite Meniere's disease. Methods: The sample consisted of an experimental group (n = 22; 10 men and 12 women, mean age 47.32 ± 12.82 years) with definite unilateral Meniere's disease and a control group (n = 14; 5 men and 9 women, with a mean age of 41.64 ± 13.45 years). They all underwent vestibular evaluation by means of Subjective Visual Vertical with the bucket method and, cervical and ocular Vestibular Evoked Myogenic Potential tests. The data were collected and subjected to statistical analysis. Results: The results of the comparison of the Subjective Visual Vertical, the cervical Vestibular Evoked Myogenic Potential, the ocular Vestibular Evoked Myogenic Potential and, the association of cervical Vestibular Evoked Myogenic Potential with the ocular Vestibular Evoked Myogenic Potential showed no significant difference, indicating concordance among the tests. Conclusion: The identified abnormalities and the concordance between the combined proportion of the Subjective Visual Vertical, the cervical Vestibular Evoked Myogenic Potential and the ocular Vestibular Evoked Myogenic Potential findings indicate that the association of these three tests contributes to the identification of sustained and transient otolith dysfunction in the inter-crisis of unilateral definite Meniere’s disease. This prospective pilot study aimed to evaluate whether cerebral inflow and outflow abnormalities assessed by ultrasonographic examination could be associated with recurrent benign paroxysmal positional vertigo (BPPV). Twenty-four patients with recurrent BPPV, affected by at least two episodes, and diagnosed according to American Academy of Otolaryngology–Head and Neck Surgery (AAO–HNS) criteria, evaluated at our University Hospital, between 1 February 2020 and 30 November 2021, have been included. At the ultrasonographic examination, 22 of 24 patients (92%) reported one or more alterations of the extracranial venous circulation, among those considered for the diagnosis of chronic cerebrospinal venous insufficiency (CCSVI), although none of the studied patients were found to have alterations in the arterial circulation. The present study confirms the presence of alterations of the extracranial venous circulation in recurrent BPPV; these anomalies (such as stenosis, blockages or regurgitation of flow, or abnormal valves, as per the CCSVI) could cause a disruption in the venous inner ear drainage, hampering the inner ear microcirculation and then possibly causing recurrent otolith detachment. Introduction. The most common clinical symptoms of coronavirus disease 2019 include cough, high body temperature, malaise, weakness, headache, and diarrhea. When the pandemic slowed down, more and more patients reported symptoms atypical for the infection, such as hearing loss, tinnitus, and vertigo. The aim of this study was to assess current knowledge and data on the existence and prevalence of otorhinolaryngology sings and symptoms of coronavirus disease 2019. Material and Methods. The literature was reviewed from May to December 2022 and included articles published in 2020 or later. The main criterion was confirmed severe acute respiratory syndrome coronavirus 2 infection by means of the polymerase chain reaction test. Results. Sensorineural hearing loss presents as acute, chronic and subclinical. Tinnitus is associated with direct viral invasion and social factors. Vertigo is also associated with direct viral invasion and prolonged bed rest. Olfactory and gustatory disorders are known symptoms of viral infections. Olfactory dysfunction occurs as a consequence of the existence of angiotensin-converting enzyme 2 receptors in the nasal mucosa, which is the primary site for viral binding, and which explains the absence of nasal congestion. It has been shown that the pediatric population presents with different clinical symptoms of the infection. In children, rhinorrhea and pharyngitis are the most common symptoms, while in adults they are generally absent. Conclusion. The available literature data showed that otorhinolaryngology symptomatology of coronavirus disease 2019 is present, but the data are still very limited. The literature showed vast discrepancies in the prevalence and risk factors associated with coronavirus disease 2019. Background: The endolymphatic sac is an organ devoid of sensory receptors. It is connected with the endolymphatic compartment and contains endolymph. Endolymphatic sac tumor is a rare neoplasm involving the middle and inner ear described in humans and dogs, does not show cellular characteristics of malignancy, but can be locally invasive and involve destruction of the temporal bone and adjacent structures. Case Description: An eight months old female cat was referred because of sudden onset of vestibular signs starting three days prior to referral. On clinical examination the patient showed depression, right head-tilt, left sided facial paralysis, horizontal nystagmus with fast phase to the left. The magnetic resonance images showed a voluminous extra axial lesion, with irregular morphology and well-defined margins, with intracranial extension in the region of the pons, rostral medulla oblongata, cerebellar vermis, floccule and left cerebellar hemisphere. Due to the progressive clinical deterioration, the cat was euthanized two weeks later. A necropsy was then performed and histological samples taken. The necropsy revealed the presence of a voluminous dark red irregular mass extending from the tympanic bulla to the posterior cranial fossa following the left glossopharyngeal nerve. The histopathologic exam of the extra-axial lesion featured a non-encapsulated, moderately cellular, rather loose, proliferation of cuboidal to columnar epithelium breaching through chunks of an otherwise normal appearing dura mater and invading some cranial nerves. Sections of the cerebellum and brainstem revealed moderate, focal, impingement of the parenchyma with very mild extension of the proliferating cells into the ventral left side of the medulla oblongata. Based on these histological characteristics, the lesion was defined as endolymphatic sac tumor, a rare neoplasm described in human beings and with two reports in dogs. Conclusion: To our knowledge, this is the first report describing an endolymphatic sac tumor in a cat.

A clinical diagnosis of Ménière’s disease (MD) is made based on medical history and audiometry findings. The 1995 American Academy of OtolaryngologyHead and Neck Surgery (AAO-HNS) guidelines requires histopathological confirmation of endolymphatic hydrops (EH) for a diagnosis of “certain” MD. Symptoms such as dizziness and ear fullness are important diagnostic features; however, the descriptions provided by patients are frequently vague and nonspecific. A recently developed magnetic resonance imaging (MRI) protocol to document EH is, therefore, useful for the evaluation of inner ear status in patients with MD. In this study, patients with MD were assessed using MRI and the HYDROPS (HYbriD of Reversed image Of Positive endolymph signal and native image of positive perilymph Signal) protocol to investigate the eectiveness of MRI for visualization of the endolymphatic space in the diagnosis of MD by correlating clinical laboratory parameters with the grade of EH. Of the 123 patients with MD recruited in this study, 80 had definite MD, 11 had probable MD, and 32 had possible MD based on the 1995 AAOHNS guidelines. The EH grade based on HYDROPS MRI was determined independently by two otorhinolaryngologists and compared with several clinical parameters, including the diagnostic scale of MD (1995 AAO-HNS guidelines), pure tone average (PTA), low tone average (LTA), canal paresis (CP) on the caloric test, and disease duration. Cochlear hydrops and vestibular hydrops were detected in 58 and 80% of 80 definite MD ears, in 33 and 58% of 12 probable MD ears, and in 5 and 27% of 37 possible MD ears, respectively. The proportion of higher hydrops grades increased significantly with grade according to the MD diagnostic scale (p < 0.0001). Both PTA and LTA were significantly higher in patients with hydrops grade 2 than hydrops grade 0 in both the cochlea and the vestibule. CP was significantly higher in patients with grade 2 than grade 0 vestibular hydrops. Disease duration was not associated with hydrops grade. Radiological evaluation of MD using the HYDROPS protocol is useful for evaluation of the extent and severity of EH in the diagnosis of MD based on its pathophysiological mechanism. Labyrinthine fistula (LF) is one of the most important complications of cholesteatoma and is defined as an abnormal communication between the inner and the middle ear. This study aims to describe our experience with the partial labyrinthectomy evaluating the post-operative hearing results. Twenty-one patients who presented labyrinthine fistula in the semicircular canals were included in the present study. Hearing impairment was present in 48% of patients (10/21). A pre-operative assessment using the Gardner–Robertson hearing classification showed the following: 52%, Class I; and 48%, Class II. A post-surgical Gardner–Robertson hearing classification evidenced the following: 43%, Class I; and 57%, Class II. The presence of LF is usually considered a negative prognostic factor for hearing preservation. The key point of partial labyrinthectomy surgery is the preservation of structures, keeping them wet with Ringer's solution throughout the procedures, and not performing suction that is close to the opened LF. The bony labyrinth is drilled underwater without suction, removing the entire cholesteatoma matrix and quickly plugging the site before and after the LF. This faster plugging of the labyrinth makes it possible to preserve the peri-lymph and the endo-lymph fluid and the hearing function. Objectives: (1) To determine how otologic/neurotologic surgeons counsel patients with superior semicircular canal dehiscence (SSCD). (2) To understand the plethora of presenting symptoms associated with SSCD and appropriate management. (3) To suggest appropriate management; oftentimes avoiding surgery. Methods: This was a survey study of both community and academic physicians. A 23-question survey was distributed to all members of the American Neurotological (ANS) and American Otologic Societies (AOS) via email in the Fall of 2018. A total of 54 responses were received from a possible pool of 279 for a response rate of 19.4%. Inferences were made about the population through sample proportions and confidence intervals. Results: All respondents use computed tomography (CT) in diagnosing SSCD and 11.1% use CT exclusively. Cervical vestibular evoked myogenic potential (VEMP; 77.8%) are used more often than ocular VEMPs (38.9%). Magnetic resonance imaging (7.4%) is used infrequently; 96.3% of surgeons surveyed have seen patients with SSCD on imaging that are asymptomatic. Following surgical treatment, respondents reported balance issues and mild-to-moderate high-frequency sensorineural hearing loss (88.4%); 32.6% reported that the majority (>50%) of their patients needed further intervention after surgery, typically aggressive vestibular rehabilitation. Conclusions: There is a discrepancy in the systematic approach to SSCD between both the surgeons and the published literature. Patients with SSCD on ultra-high-resolution CT may have myriad symptoms while others are asymptomatic, and surgery may lead to additional complications. We will present a methodical recommendation to assist in the management of patients with SSCD depending upon their symptoms. This may improve patient selection, counseling, and outcomes. Introduction: Labyrinthine fistulae (LF) are the common complications of chronic otitis media (COM) of squamosal variety. The final therapeutic outcome of this condition is to preserve the cochlear and vestibular functions. Herein, we present the data of the cases managed at our institute with respect to their presenting complaints, adopted therapeutic approaches and outcomes. Materials and Methods: A retrospective chart review was conducted on all cases with COM squamosal type in adult patients. A total of 275 patients were reviewed, out of whom 30 cases had LF. The results were mainly studied with respect to the postoperative improvement of hearing and vertigo. Results: The incidence rate of LF in the present study was obtained at 10.9%. Only 50% of the cases had the symptoms of vertigo. Furthermore, positive fistula test was elicited in 3.3% of the cases. All cases undergoing preoperative imaging were diagnosed successfully. In addition, 42.85% of the cases had profound hearing loss preoperatively, which sustained after the operation. However, 47.61% of the cases showed an improvement of at least ≥ 10 dB in the air-bone gap. Out of the 15 LF cases with vertigo as the main complaint, only 11 cases referred for follow-up. In this regard, 63.63% of the cases had no postoperative vertigo symptoms. Conclusion: Patients with LF may not have complaints of vertigo and a positive fistula sign upon admission. Pre-operative imaging facilitates the diagnosis of this condition. The removal of the matrix under constant irrigation, followed by repair with bone wax and/or autologous tissue, is sufficient to preserve the cochlear and vestibular symptoms postoperatively.

Patients can experience disabling symptoms and opt for surgical management. Limited data are available on the impact of SCDS on health-related quality of life (HRQoL) and disease-specific HRQoL more specifically.Objective: To perform a prospective analysis on generic HRQoL in SCDS patients compared to healthy age-matched controls.Methods: A prospective study was performed on patients diagnosed with SCDS and who did not undergo reconstructive surgery yet. Patients were recruited between November 2017 and January 2020 and asked to complete the Health Utility Index (HUI) Mark 2 (HUI2)/Mark 3 (HUI3) questionnaire. For the control group, age-matched participants without otovestibular pathology or other chronic pathology were recruited. The multi-attribute utility function (MAUF) score was calculated for the HUI2 and HUI3. Results of both groups were compared using the Mann-Whitney U test.Results: A total of 20 patients completed the questionnaire. Age ranged from 37 to 79 years with a mean age of 56 years (45% males and 55% females). The control group consisted of 20 participants with a mean age of 56.4 years and ranged from 37 to 82 years (35% males and 65% females). For the case group, median HUI2 MAUF score was 0.75 and median HUI3 MAUF score was 0.65. For the control group, the median scores were 0.88 and 0.86 respectively. There was a statistically significant difference for both HUI2 (p = 0.024) and HUI3 (p = 0.011). SCDS patients had a worse generic HRQoL than age-matched healthy controls. One patient with unilateral SCDS had a negative HUI3 MAUF score (−0.07), indicating a health-state worse than death.Conclusion: SCDS patients have significantly lower health utility values than an age-matched control group. This confirms the negative impact of SCDS on generic HRQoL, even when using an instrument that is not designed to be disease-specific but to assess health state in general. These data can be useful to compare impact on HRQoL among diseases.

ABSTRACT Purpose: to identify, in the literature, the factors associated with the development of vestibular dysfunctions in individuals with type 2 diabetes mellitus (DM2). Methods: an integrative review of the literature, whose survey was conducted in the databases ISI, SciELO, LILACS and PubMed, using the following descriptors: “type 2 diabetes mellitus”, “vertigo”, “dizziness”, and “vestibular diseases". Articles published in the last 10 years that answered the research question (“What factors are associated with the development of vestibular disorders in individuals with DM2?”) were included in the study. Results: the search returned 426 articles, 10 of which met the eligibility criteria. Most of the participants of the selected studies who had vestibular dysfunctions were women over 40 years old and had more than one comorbidity related to DM2, the main one being the systemic arterial hypertension (SAH). According to the literature, the physiology of the inner ear allows small glucose alterations to influence its normal functioning, which makes diabetic individuals more susceptible to developing vestibular dysfunctions. Conclusion: according to this study, DM2 can trigger or contribute to the manifestation of vestibular dysfunction, whose main associated factors are advanced age, female gender, and various comorbidities, as dyslipidemia, SAH and metabolic syndrome.

Background: Betahistine is used worldwide to treat patients with Menière's disease. However, despite it being used for decades, diverging opinions on the effect of betahistine on Menière's symptomatology still exist. Aims: The objective of this systematic review was to provide an overview and rate the certainty of the current evidence base regarding the use of betahistine to treat patients with Menière's disease. Materials and methods: A systematic literature search was conducted in October 2019. The search strategy was subdivided into searches for existing guidelines, systematic reviews and individual randomized controlled trials (RCT) investigating the usage of betahistine as compared to placebo, in patients with Ménière's disease. The primary outcome was the frequency of vertigo attack(s) and occurrence of serious adverse events. Results: We identified three relevant guidelines and three systematic reviews: however, neither included any relevant trials matching our inclusion criteria. An individual search for RCTs identified one trial. The results from this particular trial showed no difference in effects on symptoms following treatment with betahistine. Conclusions and Significance: There is a need for further well-conducted placebo RCTs. Currently, there is still a lack of substantial evidence supporting betahistine as a significant and adequate treatment for patients diagnosed with Menière's disease. Introduction: Labyrinthine fistulae (LF) are the common complications of chronic otitis media (COM) of squamosal variety. The final therapeutic outcome of this condition is to preserve the cochlear and vestibular functions. Herein, we present the data of the cases managed at our institute with respect to their presenting complaints, adopted therapeutic approaches and outcomes. Materials and Methods: A retrospective chart review was conducted on all cases with COM squamosal type in adult patients. A total of 275 patients were reviewed, out of whom 30 cases had LF. The results were mainly studied with respect to the postoperative improvement of hearing and vertigo. Results: The incidence rate of LF in the present study was obtained at 10.9%. Only 50% of the cases had the symptoms of vertigo. Furthermore, positive fistula test was elicited in 3.3% of the cases. All cases undergoing preoperative imaging were diagnosed successfully. In addition, 42.85% of the cases had profound hearing loss preoperatively, which sustained after the operation. However, 47.61% of the cases showed an improvement of at least ≥ 10 dB in the air-bone gap. Out of the 15 LF cases with vertigo as the main complaint, only 11 cases referred for follow-up. In this regard, 63.63% of the cases had no postoperative vertigo symptoms. Conclusion: Patients with LF may not have complaints of vertigo and a positive fistula sign upon admission. Preoperative imaging facilitates the diagnosis of this condition. The removal of the matrix under constant irrigation, followed by repair with bone wax and/or autologous tissue, is sufficient to preserve the cochlear and vestibular symptoms postoperatively. DFNA9 is an autosomal dominant disorder that causes a combination of both sensory deficits by the 3rd to 5th decade. We therefore hypothesize a combined detrimental effect on cognition. The aim of this systematic review was to identify studies related to DFNA9 in general and its relationship with cognitive impairment more specifically.Materials and Methods: Several databases including Medline, Cochrane Database of Systematic Reviews, Cochrane Central Register of Controlled Trials, ISI Web of Knowledge, and Web of Science were searched to accumulate information about DFNA9-mutations, including phenotype, genotype, pathophysiology, quality of life (QOL), and imaging in general and cognitive function more specifically. A qualitative analysis was performed on the 55 articles that qualified.Results: The clinical features of DFNA9 are different along the 24 COCH mutations, described up to now. Vestibular symptoms generally present themselves a few years after SNHL onset in mutations associated with the vWFA-domain although they can precede SNHL onset in other mutations associated with the LCCL-domain. QoL has not been studied extensively in DFNA9, although scarce work is available on the positive impact of cochlear implantation to rehabilitate hearing. No studies were found evaluating cognition in DFNA9 patients.Conclusion: Although cognitive impairment has been demonstrated in patients with hearing loss as well as in patients with BVL, no studies have been reported on the combination of both sensory deficits, such as in DFNA9. Objective: To verify the sensitivity of the Dizziness Handicap Inventory (DHI) and the Vestibular Disorders Activities of Daily Living Scale (VADL) questionnaires in order to apprehend the impact of vestibulocochlear symptoms on patients with unilateral and bilateral temporomandibular joint dysfunction (TMD). Materials and Methods: A descriptive, retrospective, transverse cohort study was conducted that evaluated 20 female patients, 26-72 years of age (mean age, 52.3 ± 12.2 years) from February 2015 to July 2016. The procedures were as follows: anamnesis, otorhinolaryngological and vestibular assessments, as well as the application of the DHI and VADL questionnaires. Results: (a) Prevalence of peripheral vestibular deficit disorder was observed, disregarding the affected side; (b) Dizziness symptom was more evident in the emotional aspect and tinnitus in the functional aspect in the DHI; (c) The VADL revealed greater difficulty in the functional, locomotion and instrumental aspects for activities (F5, L15 and I22); (d) In the correlation between the results of the vestibular testing and the VADL, dizziness had a significant effect (p < .05); (e) In the correlation between VADL and DHI, there was statistical significance for the functional and physical aspects (p < .05). Objectives/hypothesis: To aggregate symptoms reported by patients with superior canal dehiscence syndrome (SCDS) and to develop an evidence-based symptom set by performing a systematic review of the literature.Study Design: Database search and critical assessment of research studies.Methods: Medline and PubMed databases were searched for articles that reported the preoperative symptoms of adult and pediatric patients with unilateral and bilateral SCDS. Articles were excluded if they reported on associated diseases or did not report symptoms.Results: Of the 397 articles retrieved, 66 were retained for quantitative analysis. Among 431 patients with SCDS, 91 symptom terms were reported. After combining synonymous terms, 22 symptoms were derived by consensus. Of the raw total number of reported symptoms, 92.5% can be attributed to five common symptoms: spontaneous dizziness (51%), autophony (42.5%), pressure-induced vertigo (37.4%), hearing loss (39.9%), and sound-induced vertigo (42.7%).Conclusions: This systematic review of symptoms reported by patients with SCDS identified a 22-item common symptom set. Patients with a dehiscence in the bone overlying the superior semicircular canal experience symptoms of pressure or sound-induced vertigo, bone conduction hyperacusis, and pulsatile tinnitus. The initial series of patients were diagnosed based on common symptoms, a physical examination finding of eye movements in the plane of the superior semicircular canal when ear canal pressure or loud tones were applied to the ear, and high-resolution computed tomography imaging demonstrating a dehiscence in the bone over the superior semicircular canal. Research productivity directed at understanding better methods for diagnosing and treating this condition has substantially increased over the last two decades. We now have a sound understanding of the pathophysiology of third mobile window syndromes, higher resolution imaging protocols, and several sensitive and specific diagnostic tests. Furthermore, we have a treatment (surgical occlusion of the superior semicircular canal) that has demonstrated efficacy. This review will highlight some of the fundamental insights gained in SCDS, propose diagnostic criteria, and discuss future research directions.

Benign paroxysmal positional vertigo (BPPV) is the most frequent type of vertigo. The treatment of canalithiasis of the posterior semicircular canal consists in performing a particle-repositioning maneuver, such as the Epley maneuver (EM). However, the EM is not effective in all cases. The objective of this study is to identify risk factors, which predict the EM failure, among the clinical variables recorded in anamnesis and patient examination. This is an observational prospective multicentric study. All patients presenting with BPPV were recruited and applied the EM and appointed for a follow-up visit 7 days later. The following variables were recorded: sex, age, arterial hypertension, diabetes, hyperlipidemia, smoking habit, alcohol consumption, migraine, osteoporosis, diseases of the inner ear, previous ipsilateral BPPV, previous traumatic brain injury, previous sudden head deceleration, time of evolution, sulpiride or betahistine treatment, experienced symptoms, outcome of the Halmagyi maneuver, laterality, cephalic hyperextension of the neck, intensity of nystagmus, intensity of vertigo, duration of nystagmus, occurrence of orthotropic nystagmus, symptoms immediately after the EM, postural restrictions, and symptoms 7 days after the EM. Significant differences in the rate of loss of nystagmus were found for six variables: hyperlipidemia, previous ipsilateral BPPV, intensity of nystagmus, duration of nystagmus, post-maneuver sweating, and subjective status. The most useful significant variables in the clinical practice to predict the success of the EM are previous BPPV and intensity of nystagmus. Technical advances in magnetic resonance imaging have allowed to accurately detect and grade endolymphatic space distension in Ménière disease; this was only possible in post-mortem histological studies until a few years ago. Magnetic resonance imaging rules out other causes of vertigo and hearing loss, and is able to evaluate the cochlear and vestibular compartments of the endolymphatic space using a dedicated protocol. A avaliação do reflexo vestíbulo ocular é importante; porém, insuficiente. Sistemas de sensores eletromagnéticos tridimensionais são uma nova opção para avaliação posturográfica. OBJETIVO: Avaliar a oscilação corporal em voluntários hígidos, com Manobras de Epley e Dix-Hallpike positivas e com outras disfunções vestibulares usando um sistema tridimensional. Forma de estudo: Prospectivo. MATERIAL E MÉTODO: Participaram: 23 mulheres saudáveis, 15 com disfunção vestibular periférica detectada à prova calórica e 10 com manobras de Epley e Dix-Hallpike positivas. Todas realizaram exames nas posições: olhos abertos e fechados em superfície estável e instável. RESULTADOS:Na condição Olhos Abertos e Superfície Estável, houve p < 0,01 entre o grupo controle e o com disfunção vestibular periférica em todas variáveis, exceto no deslocamento máximo a-p. Velocidade total e velocidade na trajetória médio-lateral apresentaram p < 0,01 entre o grupo com disfunção vestibular e o controle em todas as posições. O grupo com manobras de Epley e Dix-Hallpike positivas obteve p < 0,01 nas velocidades total e em seus componentes x e y nas posições de olhos abertos e fechados em superfície instável. CONCLUSÃO: O sistema de sensores eletromagnéticos tridimensionais utilizado foi capaz de gerar informações confiáveis quanto à oscilação corporal nos voluntários do estudo.The vestibular-ocular reflex assessment is important, but not enough. Tridimensional electromagnetic sensor systems represent a new method to assess posturography. AIM: To assess body sway in healthy subjects who had positive Dix Hallpike and Epley maneuvers and with other vestibular dysfunctions by means of a three-dimensional system. Study design: Prospective. MATERIALS AND METHODS: We had 23 healthy women, 15 with peripheral vestibular dysfunction found upon caloric test and 10 with positive Epley and Dix Hallpike maneuvers. All tests performed in the following positions: open and closed eyes on stable and unstable surfaces. RESULTS: With the Eyes Open and on a stable surface, p < 0.01 between the control group and the one with peripheral vestibular dysfunction in all variables, except the a-p maximum, full speed and mediolateral trajectory velocity, which had a p < 0.01 between the group with vestibular dysfunction and controls in all positions. The group with positive Epley and Dix Hallpike maneuvers had p < 0.01 at full speed and in its components in the x and y in positions with open and eyes closed on an unstable surface. CONCLUSION: The tridimensional electromagnetic sensors system was able to generate reliable information about body sway in the study volunteers.

O alcoolismo é uma doença crônica que consiste em um estado de intoxicação causado pelo consumo de álcool. Os sinais e sintomas frequentemente encontrados são entre outros: instabilidade ao andar, tontura e descoordenação psicomotora. OBJETIVO: Verificar a influência do alcoolismo no equilíbrio postural. MATERIAL E MÉTODOS: Este é um estudo prospectivo; a amostra compreendeu 32 indivíduos para o grupo experimental, frequentadores do grupo de Alcoólicos Anônimos da cidade de Santa Maria-RS e 32 indivíduos não alcoólicos para o grupo controle. Os indivíduos foram submetidos à avaliação otorrinolaringológica, avaliação do equilíbrio estático e dinâmico e provas cerebelares, avaliação vectoeletronistagmográfica e posturografia dinâmica. RESULTADOS: Constatou-se que a vectoeletronistagmografia mostrou-se normal na maioria dos indivíduos do grupo experimental, indicando que o labirinto e as vias vestíbulo-oculomotoras estariam normais e que o distúrbio do equilíbrio por eles apresentado seria decorrente de disfunção em outras áreas do sistema nervoso central ou periférico. Na posturografia dinâmica constatou-se que alcoólicos abstinentes apresentam alterações significantes do equilíbrio postural quando comparados com indivíduos não alcoólicos. CONCLUSÃO: O álcool tem influência deletéria no equilíbrio corporal.Alcoholism is a chronic condition, consisting on a state of intoxication caused by the consumption of alcohol beverages. Frequently found signs and symptoms are: gait instability, dizziness and lack of psychomotor coordination, among others. AIM: to study the influence of alcoholism on postural balance. MATERIALS AND METHODS: this is a prospective study. The sample comprehended 32 individuals in the experimental group, members of the Alcoholic Anonymous Group of the city of Santa Maria-RS and 32 non-alcoholic individuals making up the control group. The individuals were submitted to an otorhinolaryngological evaluation, static and dynamic balance study and cerebellar tests, vecto-electro-nystagmographic evaluation and dynamic posturography. RESULTS: we noticed that the vectoelectronystagmography was normal for most of the individuals in the experimental group, indicating that the labyrinth and the oculomotor-vestibular pathways were normal and that the balance disorder they presented would stem from the dysfunction in other areas of the central or peripheral nervous system. In the dynamic posturography we noticed that alcoholics who were not drinking presented significant alterations in their postural balance when compared to non-alcoholic individuals. CONCLUSION: alcoholic beverages have a deleterious influence on body balance.

The saccadic pathway involves numerous regions of the brain cortex, the cerebellum and the brainstem. Saccadic movement latency, velocity and precision parameters assess the efficacy of central nervous system (CNS) control over rapid eye movements. Very few disorders which alter the CNS are missed when these parameters are carefully measured using a computer. Pendular tracking assesses the integrity of the oculomotor system in controlling slow eye movements - vulnerable to CNS and vestibular system dysfunctions. Optokinetic nystagmus represents a stereoceptive response which compensates environment movements by psycho-optical inputs. AIMS: to compare the oculomotricity values found in children with and without learning complaints. MATERIALS AND METHODS: prospective study. We included in the study 28 children of both genders, within the age range between 8 and 12 years, with learning disorders (study group) and 15 without (control group). We carried out the fixed and randomized saccadic movement tests, pendular tracking study and optokinetic nystagmus. RESULTS: There was a statistically significant difference between the groups concerning the randomized saccadic movement velocity parameters and in the pendular tracking test. CONCLUSION: The children with learning disorders presented alterations in some oculomotricity tests when compared to children without complaints.A via sacádica envolve várias regiões do córtex cerebral, cerebelo e tronco encefálico. Os parâmetros latência, velocidade e precisão dos movimentos sacádicos avaliam a eficiência do controle do sistema nervoso central (SNC) sobre os movimentos rápidos dos olhos. Poucas desordens que alteram o SNC deixam de ser detectadas quando esses parâmetros são medidos com rigor por meio de um computador. O rastreio pendular avalia a integridade do sistema oculomotor no controle dos movimentos oculares lentos, vulneráveis a disfunções do SNC e do sistema vestibular. O nistagmo optocinético representa uma resposta exteroceptiva que compensa os movimentos do meio ambiente por impulsos psico-ópticos. OBJETIVO: Comparar os valores da oculomotricidade encontrados em crianças com e sem queixas de aprendizagem. MATERIAL E MÉTODO: Estudo prospectivo. Foram incluídas no estudo 28 crianças, de ambos os gêneros, faixa etária de 8 a 12, anos com distúrbios de aprendizagem (grupo estudo) e 15 sem (grupo controle). Foram realizados os testes de movimentos sacádicos fixos e randomizados, pesquisa do rastreio pendular e nistagmo optocinético. RESULTADOS: Houve diferença estatisticamente significante entre os grupos nos parâmetros de velocidade dos movimentos sacádicos randomizados e na pesquisa do rastreio pendular. CONCLUSÃO: As crianças com distúrbios de aprendizagem apresentaram alterações em algumas provas de oculomotricidade quando comparadas com crianças sem queixas.

OBJETIVO: Estudar os sintomas vestibulares em crianças com queixas de dificuldades escolares. MÉTODOS: Foram estudadas 88 crianças entre sete e 12 anos, que freqüentavam escolas públicas da cidade de Piracicaba no período de 2004 a 2006. Os procedimentos utilizados foram: a anamnese; exame otorrinolaringológico; exame audiológico e questionário dirigido utilizado como instrumento de coleta de dados. RESULTADOS: Das crianças avaliadas 51% não relataram dificuldades escolares e 49% referiram ter dificuldades escolares. A queixa referida mais comum foi a de vertigem (22,7%), e os sintomas referidos mais comuns no ambiente escolar foram de ansiedade (95,5%) e cefaléia (53,4%), as dificuldades escolares mais citadas foram a de ler (56,8%) e a de copiar (43,2%). CONCLUSÕES: A queixa de tontura e as dificuldades em ler e copiar apresentaram relação estatisticamente significante nas crianças com queixas de dificuldades escolares.PURPOSE: To study vestibular symptoms in children with complaints of school difficulties. METHODS: Eighty-eight children between seven and 12 years old were studied. The subjects attended public schools from Piracicaba - SP (Brazil) during the period from 2004 to 2006. The procedures included: anamnesis, otorhinolaryngological examination, audiological examination, and a directed questionnaire used as an instrument for data gathering. RESULTS: From the evaluated children, 51% had no complaints of school difficulties, and 49% reported school difficulties. The most common complaint reported was vertigo (22.7%), and the most common symptoms reported in school environment were anxiety (95.5%) and chronic headache (53.4%). The most cited school difficulties were reading (56.8%) and copying (43.2%). CONCLUSIONS: Vertigo complaints and reading and copying difficulties presented a statistically significant relation in children with complaints of school difficulties.

OBJETIVO: avaliar o comportamento vestibular em pacientes com disfunção temporomandibular. MÉTODOS: avaliaram-se 27 pacientes do sexo feminino, na faixa etária de 30 a 53 anos, encaminhadas do Centro de Diagnóstico e Tratamento da Articulação Temporomandibular para o Laboratório de Otoneurologia da Universidade Tuiuti do Paraná. Realizaram-se os seguintes procedimentos: anamnese, inspeção otológica e avaliação vestibular por meio da vectoeletronistagmografia. RESULTADOS: as queixas mais freqüentes foram: dificuldade ou dor ao movimento do pescoço (77,7%), dor irradiada para ombro/braço (77,7%), zumbido e formigamento de extremidade superior (77,7%), tontura e dor de cabeça (66,6%), ansiedade (55,5%), sensação de cabeça oca (51,8%), agitação durante o sono (51,8%) e depressão (51,8%). O exame vestibular esteve alterado em 20 pacientes (74,0%) na prova calórica. Houve freqüência de alteração no sistema vestibular periférico. Houve predomínio das síndromes vestibulares periféricas deficitárias. CONCLUSÃO: ressalta-se a importância de se estudar a relação do sistema vestibular com a disfunção temporomandibular uma vez que observamos, na presente pesquisa, um número elevado de alteração no exame labiríntico.PURPOSE: to evaluate the vestibular functioning in patients with temporomandibular joint dysfunction. METHODS: 27 female patients were evaluated, with age varying from 30 to 53-year-old, referred from the Centre of Diagnosis and Treatment of Temporomandibular Joint Dysfunction to the Otoneurological Laboratory of Tuiuti University of Paraná. The following exams were carried out: anamnesis, otoscopy and vestibular evaluations through vectoelectronystagmography. RESULTS: the most frequent complaints were: difficulty or pain with movement of the neck (77.7%), pain irradiated to the shoulder/arm (77.7%), tinnitus and paresthesia of superior extremities (77.7%) in each one, dizziness and headaches (66,6%), anxiety (55.5%), empty head sensations, agitation during sleep and depression (51.8%) in each one. The vestibular exam showed alterations in 20 patients (74.0%) in the caloric test. There was a prevalence of alterations in the peripheral vestibular system. There was a prevalence of deficient peripheral vestibular syndrome. CONCLUSION: the importance of studying the relationship between the vestibular system and temporomandibular joint dysfunction is emphasized, since a high number of alterations in the labyrinthic exam have been noted in this research.

Labyrinthine fistulae represent a well-known complication of chronic otitis media with cholesteatoma. Its estimated frequency is close to 10%. They are an abnormal communication between inner ear membranous system and middle ear space. According to the erosion they can be classified by defect length or depth. They are associated with chronic infection, vestibular symptoms and sensorial hearing loss. The objective of this review is to provide an actualized description of proper classification and management, using three cases recently presented at our institution. The most important factor for opportune diagnosis still remains a high level of suspicious. Fine cut computed tomography is an invaluable tool for diagnosis. Treatment is based on four factors: patient's hearing level, accounting contralateral ear; skills and experience of the surgeon; size and location of fistulae and the mechanism of erosion caused by cholesteatoma. The lack of a standardized accepted classification system makes published data related to fistulae management hard to interpret. Classically they have been treated in a conservative way, by leaving cholesteatoma matrix over the defect site to seal it. However, recent studies show that the complete removal of cholesteatoma matrix and defect closure are more effective measures for disease control. The etiology of superior canal dehiscence (SCD) is controversial. An embryological perspective suggests that SCD may occur through the failure of postnatal bone formation over the superior semicircular canal (SC), whereas an acquired theory suggests that trauma or pressure from the overlying temporal lobe could break or gradually thin the SC. We infer the etiology of SCD by comparing the thickness of the bony otic capsule of the unaffected side of SCD patients with that of non-SCD participants. Twelve SCD patients (13 SCD ears and 11 normal ears) and 34 age-matched controls (68 ears) were included. The control group was subdivided into an aerated group (49 ears) and a nonaerated group (19 ears), as defined by the presence of air cells above the SC. A high-resolution temporal bone CT was performed in all participants. The thicknesses of the SC, horizontal canal (HC) and posterior canal (PC) of the unaffected ears of SCD patients were compared with those of the controls. The SC of the unaffected side in the SCD group (n = 11, 0.41 ± 0.23 mm) was significantly thinner than the one in the control group (n = 68, 0.64 ± 0.21 mm, p = 0.002). The HC and PC were also thinner in the SCD group (n = 24, 0.58 ± 0.11 and 1.39 ± 0.31 mm, respectively) than in the controls (0.70 ± 0.08 and 1.61 ± 0.32 mm; p < 0.0001 and p = 0.005, respectively). The SC, HC and PC thicknesses were also compared between the aerated and nonaerated ears within the control group. The SC was significantly thicker in the aerated group (0.73 ± 0.14 mm) than in the nonaerated group (0.60 ± 0.23 mm; p = 0.046); however, no significant difference was observed for the HC and PC thickness (aerated group, n = 49, 0.72 ± 0.07 and 1.67 ± 0.34 mm; nonaerated group, n = 19, 0.70 ± 0.09 and 1.59 ± 0.34 mm; p = 0.350 and p = 0.428, respectively). The bony otic capsule was significantly thinner in the SCD patients than in the controls. However, even within unaffected individuals, SCs lacking overlying air cells were also thinner than those with overlying air cells. Endolymphatic sac tumors (ELSTs) are rare neoplasms of the inner and middle ear described in humans. Diagnosis of such neoplasms is difficult and largely dependent on a combination of histologic, immunohistochemical, and clinical findings. Although the neoplastic cells lack cellular features of malignancy, these are clinically aggressive tumors that often invade the surrounding temporal bone. Here, we describe 2 dogs with middle ear masses that share morphologic, immunohistochemical, and clinical similarities with human ELSTs. Advanced imaging of the masses revealed evidence of aggressive behavior such as bony lysis of the temporal bone. Histologically, the neoplastic epithelial cells formed papillary structures, lacked mitotic figures, and had mild anisocytosis and anisokaryosis. The neoplastic cells were immunohistochemically positive for cytokeratin AE1/AE3 but were negative for chromogranin, synaptophysin, and thyroglobulin. Abstract: Objective: The purpose of this case series is to describe the management of benign paroxysmal positional vertigo in a chiropractic clinical setting. Clinical Features: Eight patients (4 women, 4 men) with symptoms of persistent benign paroxysmal positional vertigo presented for chiropractic care. The outcome measures included self-reported resolution of vertigo, a Short Form 12 Health Survey, Measure Yourself Medical Outcome Profile, and the Dix-Hallpike maneuver. Outcome measures were assessed at initial assessment, 6 days, 30 days, and 3 months postintervention. Intervention and Outcome: The patients underwent one or more canalith repositioning procedures (Epley maneuver). Scores in each of the categories decreased from the initial to 6-day assessment and then again at the 30-day assessment. The effects of the treatment on the Short Form 12 scores showed changes between the initial assessment and 30 days posttreatment. Conclusion: The patients in this case series demonstrated reduction in symptoms with chiropractic management. [Copyright &y& Elsevier]

Blunt head trauma without any temporal bone fracture or longitudinal temporal bone fracture, with an associated fracture of the labyrinth may cause labyrinthine injury with sensor neural hearing loss and vertigo because of a concussive injury to the membranous labyrinth. Sudden sensory neural hearing loss is relatively frequent. In most cases, the etiology is not discovered. One of the possible causes for sudden deafness is inner labyrinth bleeding or concussion, which were difficult to diagnose before the advent of magnetic resonance imaging. Vertigo without a demonstrable fracture may also be the result of labyrinthine concussion, cupololithiasis and perilymphatic fistula. We describe the clinical case of a patient with acute traumatic hearing loss and vertigo, without skull base fracture detected on computed tomography. Magnetic resonance study was also performed. Objective: This article describes and discusses the case of a patient with benign paroxysmal positional vertigo (BPPV) characterized by severe vertigo with dizziness, nausea, and nystagmus, treated without the use of spinal manipulation by a doctor of chiropractic. Clinical Features: A 46-year-old woman presented for care with complaints of acute vertigo and dizziness. Intervention and Outcome: The patient was examined and diagnosed with left posterior canalolithiasis by means of the Dix-Hallpike maneuver. She was treated successfully with the Epley maneuver once and subsequently discharged without further treatment. Conclusion: This case demonstrates the importance of correctly identifying patients with BPPV. This case also demonstrates the successful treatment of BPPV. [ABSTRACT FROM AUTHOR]

The objective of this report is to emphasize the clinical features of delayed endolymphatic hydrops in comparison with Ménière's disease. Four cases were selected from a series of 16 examined between the years 1993 and 2005, due to their unusual characteristics. A short clinical history and significant tests are presented for each of the four cases. The cases of delayed endolymphatic hydrops discussed in this article suggest that hydrops is the most important underlying pathology that causes the hearing loss and the vestibular symptoms both in the better ear and in the ear with severe hearing loss. They also suggest that this condition probably occurs in patients with congenitally sensitive ears that make them prone to the development of the late hydrops. [ABSTRACT FROM AUTHOR]

Background: Endolymphatic sac tumours (ELSTs) have been known as an individual tumour entity only since 1984. ELSTs may occur either solitarily and sporadically or as a hereditary manifestation associated with von Hippel-Lindau (VHL) disease. The latter association was first observed in 1992 and confirmed by molecular genetic analysis of the VHL gene. No consensual diagnostic and treatment strategy of ELST exists at present. Methods: Based on imaging criteria in computed tomography, magnetic resonance imaging (MRI), and magnetic resonance angiography, we developed a staging system to classify ELST in a series of seven consecutive patients in an attempt to custom-tailor the surgical approach. Type A referred to tumours that were locally confined without temporal bone erosion or infiltration of the dura (n = 2); type B tumours showed evidence of bone infiltration of the osseous labyrinth and sensorineural hearing loss (n = 2); and in type C, the tumour further invaded the sigmoid sinus and jugular bulb (n = 3). Two patients suffered from VHL disease. Results: In all patients, the tumour was completely removed. Stage-adapted surgical approaches included various transpetrosal procedures, from the translabyrinthine to the infratemporal approaches. The functional integrity of the facial nerve was maintained in all tumour stages, whereas the vestibulocochlear nerve could be preserved only in patients with type A tumours. Follow-up MRI demonstrated no local tumour recurrence during a postoperative observation period ranging from 4 to 38 months. Conclusion: Stage-based surgical strategy enables the complete removal of ELST with minor morbidity. Transmastoid approaches are most efficient for resection of the tumour matrix to prevent local recurrence. [ABSTRACT FROM AUTHOR]
