Individual in-depth interviews were conducted with 20 men, ages 21-48, who have fathered at least one unintended pregnancy. The goal of the interviews was to explore the experiences of these men with unintended pregnancy, their communication with partners and others, contraceptive beliefs and practices, their relationships with their partners, and the outcomes and consequences of the unintended pregnancies. This essay describes results derived from their comments regarding their contraceptive practices and the pregnancy-outcome decisions, with thematic analysis used to identify prominent themes from participant comments. Two strong themes, 'Deference' and 'Denial,' and one lesser theme, 'Exclusion,' emerged from participant responses. Discussion of the role of communication, the power of denial, and opportunities for further involvement of men in decision-making relating to unintended pregnancy are presented. The intersection of drug use, violence, and depression with HIV-risk among African American women is an under explored area of research. The current analyses examine whether particular sexual risk behaviors are associated with exposure to violence, depression or both among 420 African American out-of-treatment female drug users. Women were stratified into four mutually exclusive groups: drug users with exposure to violence (n=64), drug users with clinical depression (n=62), drug users with both (n=41), and drug users only (n=253). Multinomial logistic regression analyses examined the association of demographics and sexual risk behaviors across the tripartite groups. Women with a history of sexually transmitted diseases were more likely to experience violence and depression both alone and jointly. Women who had two or more sexual partners in the last 30 days (OR=2.26) and women who had an early onset of alcohol use (OR=2.50) were at an increased risk for having the full tripartite of drug use, violence and depression. Never being married was a protective factor for the full tripartite. As expected, more risk factors were found among women who had the full tripartite than among women with one or two of the factors. The co-existence of the tripartite factors and sexual risk behaviors may indicate a need to ultimately provide more specialized prevention and intervention efforts to combat HIV infection. This paper describes scales measuring perceptions of neighborhood and school quality. The scales are short and easy to administer, and easily understood by respondents. The measure of neighborhood quality (the NegNeb) has high reliability (alpha = .81), correlates with census data, shows sensitivity to change in neighborhood quality, and discriminates among urban neighborhoods. The measure of school quality (the NegEd) correlates to an index of aggregate school problems. Theories posit that the impact of environment on mental health is determined by perception of that environment. A regression model controlling for family and other characteristics showed that perception of neighborhood (as measured by the NegNeb) contributed unique variance to adolescent mental health. Perception of school did not. However, a second regression model showed that perception of school environment contributed unique variance to peer misbehavior, which was the largest predictor of mental health problems in the first equation. In each regression model aggregate measures of neighborhood and school quality contributed less to the model than did perceived measures. Because adolescents' perceptions of their neighborhood and school environments are clearly linked to their mental health and peer environment, researchers interested in effects of neighborhood and school environment should use subjects' perceptions to conceptualize and measure these realities. The patient was a 21-year-old male who was referred to physical therapy with a 1-week history of right knee pain and stiffness following an injury of traumatic onset. While attempting to jump off of both legs to dunk a basketball during a game, the patient heard and felt a pop in his right knee that was associated with an immediate onset of pain and swelling. He was unable to bear weight following the injury and, therefore, immediately went to the emergency department, where radiographs were completed and interpreted as negative for a fracture. However, the patella for the right knee was superiorly displaced. The patient was issued crutches and referred to physical therapy. At the time of the initial physical therapy examination, the patient was still not able to bear full weight on the right lower extremity or actively fully extend his right knee. Due to concern over possible meniscal, medial collateral ligament, or patellar tendon involvement, the patient's physician was contacted and magnetic resonance imaging was ordered. Five days later, the patient presented with decreased knee effusion and the special tests for the medial collateral ligament and meniscus were negative. However, the patient was still not able to actively extend his knee, suggesting a possible rupture of the patellar tendon, which was later confirmed on magnetic resonance imaging.
