Abstract

BackgroundPatient portal modules, including electronic personal health records, health education, and prescription refill ordering, may be leveraged to address the sexually transmitted infection (STI) burden, including HIV, among gay, bisexual, and other sexual minority men (SMM). Theoretical frameworks in the implementation sciences highlight examining constructs of innovation attributes and performance expectations as key determinants of behavioral intentions and the use of new web-based health technologies. However, behavioral intentions to use patient portals for HIV and other STI prevention and care among SMM is understudied. ObjectiveThe aim of this study is to develop a brief instrument for measuring attitudes focused on using patient portals for STI prevention and care among a nationwide sample of SMM. MethodsA total of 12 items of the American Men’s Internet Survey-Patient Portal Sexual Health Instrument (AMIS-PPSHI) were adapted from a previous study. Psychometric analyses of the AMIS-PPSHI items were conducted among a randomized subset of 2018 AMIS participants reporting web-based access to their health records (N=1375). Parallel analysis and inspection of eigenvalues in a principal component analysis (PCA) informed factor retention in exploratory factor analysis (EFA). After EFA, Cronbach α was used to examine the internal consistency of the scale and its subscales. Confirmatory factor analysis (CFA) was used to assess the goodness of fit of the final factor structure. We calculated the total AMIS-PPSHI scale scores for comparisons within group categories, including age, STI diagnosis history, recency of testing, serious mental illness, and anticipated health care stigma. ResultsThe AMIS-PPSHI scale resulting from EFA consisted of 12 items and had good internal consistency (α=.84). The EFA suggested 3 subscales: sexual health engagement and awareness (α=.87), enhancing dyadic communication (α=.87), and managing sexual health care (α=.79). CFA demonstrated good fit in the 3-factor PPSHI structure: root mean square error of approximation=0.061, comparative fit index=0.964, Tucker-Lewis index=0.953, and standardized root mean square residual=0.041. The most notable differences were lower scores on the enhanced dyadic communication subscale among people living with HIV. ConclusionsPPSHI is a brief instrument with strong psychometric properties that may be adapted for use in large surveys and patient questionnaires in other settings. Scores demonstrate that patient portals are favorable web-based solutions to deliver health services focused on STI prevention and care among SMM in the United States. More attention is needed to address the privacy implications of interpersonal use of patient portals outside of traditional health settings among persons with HIV.

Abstract

Little is known about the unique contribution of schools vs neighborhoods in driving adolescent marijuana use. This study examined the relative contribution of each setting and the influence of school and neighborhood socioeconomic status on use. We performed a series of cross-classified multilevel logistic models predicting past 30-day adolescent (N = 18 329) and young adult (N = 13 908) marijuana use using data from Add Health. Marijuana use differed by age, sex, race/ethnicity, and public assistance in adjusted models. Variance parameters indicated a high degree of clustering by school (σ2 = 0.30) and less pronounced clustering by neighborhood (σ2 = 0.06) in adolescence when accounting for both levels simultaneously in a cross-classified multilevel model. Clustering by school persisted into young adulthood (σ2 = 0.08). Parental receipt of public assistance increased the likelihood of use during adolescence (odds ratio = 1.39; 95% confidence interval: 1.19–1.59), and higher parental education was associated with increased likelihood of use in young adulthood. These findings indicate that both contexts may be promising locations for intervention.

Abstract

Among the characteristics acquired by many tumour cells is a shift from using oxidative phosphorylation to using glycolysis for ATP production. Although the nuclear factor (NF)-κB family of transcriptional regulators have important roles in tumorigenesis, their ability to function as regulators of metabolism has only been recently investigated. This has revealed the importance of crosstalk between NF-κB, the p53 tumour suppressor and other crucial cell signalling pathways. This review discusses the mechanisms through which NF-κB regulates tumour cell metabolism and the important role of p53 in determining the consequences of NF-κB activity. It also proposes a model in which NF-κB contributes to the shift to glycolytic ATP production through regulation of both nuclear and mitochondrial gene expression. [ABSTRACT FROM AUTHOR]

Abstract

With an ever-expanding stream of solid waste, and limited possibilities for disposal, communities increasingly have to make difficult decisions about where to store these by-products of daily living. The authors use survey data from seven counties in Florida that have recently dealt with solid waste location decisions to assess the extent to which citizen attitudes reflect a NIMBY perspective to these facilities. Contrary to what the NIMBY phenomena predicts, respondents who lived closer to existing or proposed facilities were not more likely to perceive them as dangerous as those living further away. In addition, the respondents were able to list problems and benefits to support their perceptions of danger posed by these facilities. These findings suggest that citizens do not always respond to environmental risks by gut reaction or without information as is sometimes suggested in the NIMBY literature. [ABSTRACT FROM PUBLISHER]
