Abstract Background The COVID-19 pandemic highlights the importance of strong surveillance systems in detecting and responding to public health threats. We sought to evaluate attributes of Keur Massar district's existing COVID-19 surveillance system. Method A descriptive, cross-sectional study was conducted in June 2022; desk review covered data collected from March 03, 2020 to May 31, 2022 in 18 health posts. Data were collected using a standardized questionnaire completed during a face-to-face interview and a desk review of surveillance data gathered from different notification platforms (Excel, ODK, DHIS2 aggregated, and tracker). Study was conducted in Keur Massar department, in the Dakar region. We conducted face-to-face interviews with 18 nurses in June 2022. We utilized a standardized, semi-structured questionnaire adapted from CDC guidelines for surveillance evaluation. Results All 18 head nurses targeted, responded to the questionnaire, with an average age of 41.5 years and 63% aged between 30 and 44. The sex ratio (M/F) was 0.6, and respondents had an average of 15.1 years of experience. All nurses were involved in COVID-19 surveillance and had notified at least one suspected case. While 39% conducted COVID-19 data analysis, 55.6% received feedback from the national level. The usefulness score for the surveillance system was 77.7, with the lowest score (72.9) related to describing the pandemic’s magnitude. Simplicity scored 63.3, with low scores for the availability of guidelines (0) but high scores for training and equipment (94.4). Acceptability scored 76.6, with strong support for COVID-19 surveillance but weak community involvement (48.6). While no cases were reported through the DHIS2 aggregated platform, 1327 PCR-positive SARS-CoV-2 cases were reported through the national Excel sheet and 278 PCR-positive cases were reported through the COVID-19 DHIS2 tracker during the same period. Timeliness varied, averaging 3 days using ODK and 7 days with the national Excel sheet, with a combined average of 5 days across both systems. Conclusion The study highlights challenges in COVID-19 surveillance due to limited human resources, multiple data systems, and delays in notification. While most nurses were trained and equipped, gaps in data quality, timeliness, and community support emphasize the need for streamlined processes and increased workforce capacity.

We report an imported Crimean-Congo hemorrhagic fever case in Senegal. The patient received PCR confirmation of virus infection 10 days after symptom onset. We identified 46 patient contacts in Senegal; 87.7% were healthcare professionals. Strengthening border crossing and community surveillance systems can help reduce the risks of infectious disease transmission.

The COVID-19 pandemic necessitated the rapid development and implementation of effective surveillance systems to detect and respond to the outbreak in Senegal. In this documentation, we describe the design and implementation of the Community Event-Based Surveillance (CEBS) system in Senegal to strengthen the existing Integrated Disease Surveillance and Response system. The CEBS system used a hotline and toll-free number to collect and triage COVID-19-related calls from the community. Data from the CEBS system were integrated with the national system for further investigation and laboratory testing. From February to September 2020, a total of 10 760 calls were received by the CEBS system, with 10 751 calls related to COVID-19. The majority of calls came from the Dakar region, which was the epicentre of the outbreak in Senegal. Of the COVID-19 calls, 50.2% were validated and referred to health districts for further investigation, and 25% of validated calls were laboratory-confirmed cases of SARS-CoV-2. The implementation of the CEBS system allowed for timely detection and response to potential COVID-19 cases, contributing to the overall surveillance efforts in the country. Lessons learned from this experience include the importance of decentralised CEBS, population sensitisation on hotlines and toll-free usage, and the potential role of Community Health Workers in triaging alerts that needs further analysis. This experience highlights the contribution of a CEBS system in Senegal and provides insights into the design and operation of such a system. The findings can inform other countries in strengthening their surveillance systems and response strategies.

In Senegal, cervical cancer is the most common cancer among women and the leading cause of morbidity and mortality from all cancers. In 2018, Senegal launched a national human papillomavirus (HPV) vaccination program with Gavi, the Vaccine Alliance (Gavi), support. HPV vaccination was incorporated into the national immunization program as a two-dose schedule, with a 6-12-month interval, to nine-year-old girls via routine immunization (RI) services at health facilities, schools and community outreach services throughout the year. During February to March 2020, we conducted interviews to assess the awareness, feasibility, and acceptability of the HPV vaccination program with a cross-sectional convenience sample of healthcare workers (HCWs), school personnel, community healthcare workers (cHCWs), parents, and community leaders from 77 rural and urban health facility catchment areas. Participants were asked questions on HPV vaccine knowledge, delivery, training, and community acceptability of the program. We conducted a descriptive analysis stratified by respondent type. Data were collected from 465 individuals: 77 HCW, 78 school personnel, 78 cHCWs, 152 parents, and community leaders. The majority of HCWs (83.1%) and cHCWs (74.4%) and school personnel (57.7%) attended a training on HPV vaccine before program launch. Of all respondents, most (52.5–87.2%) were able to correctly identify the target population. The majority of respondents (60.2–77.5%) felt that the vaccine was very accepted or accepted in the community. Senegal’s HPV vaccine introduction program, among the first national programs in the African region, was accepted by community stakeholders. Training rates were high, and most respondents identified the target population correctly. However, continued technical support is needed for the integration of HPV vaccination as a RI activity for this non-traditional age group. The Senegal experience can be a useful resource for countries planning to introduce the HPV vaccine.

BACKGROUND: Outcomes of sexual violence care programmes may vary according to the profile of survivors, type of violence suffered, and local context. Analysis of existing sexual violence care services could lead to their better adaptation to the local contexts. We therefore set out to compare the Médecins Sans Frontières sexual violence programmes in the Democratic Republic of Congo (DRC) in a zone of conflict (Masisi, North Kivu) and post-conflict (Niangara, Haut-Uélé). METHODS: A retrospective descriptive cohort study, using routine programmatic data from the MSF sexual violence programmes in Masisi and Niangara, DRC, for 2012. RESULTS: In Masisi, 491 survivors of sexual violence presented for care, compared to 180 in Niangara. Niangara saw predominantly sexual violence perpetrated by civilians who were known to the victim (48%) and directed against children and adolescents (median age 15 (IQR 13-17)), while sexual violence in Masisi was more directed towards adults (median age 26 (IQR 20-35)), and was characterised by marked brutality, with higher levels of gang rape, weapon use, and associated violence; perpetrated by the military (51%). Only 60% of the patients in Masisi and 32% of those in Niangara arrived for a consultation within the critical timeframe of 72 hours, when prophylaxis for HIV and sexually transmitted infections is most effective. Survivors were predominantly referred through community programmes. Treatment at first contact was typically efficient, with high (>95%) coverage rates of prophylaxes. However, follow-up was poor, with only 49% of all patients in Masisi and 61% in Niangara returning for follow-up, and consequently low rates of treatment and/or vaccination completion. CONCLUSION: This study has identified a number of weak and strong points in the sexual violence programmes of differing contexts, indicating gaps which need to be addressed, and strengths of both programmes that may contribute to future models of context-specific sexual violence programmes.
