Abstract

Abstract Observing System Simulation Experiments (OSSEs) provide an effective way to evaluate the impact of assimilating data from a specific observing system on hindcasting, nowcasting, and forecasting of environmental systems. The NSF NCAR's Data Assimilation (DA) Research Testbed/Thermosphere‐Ionosphere‐Electrodynamics General Circulation Model (DART/TIEGCM) tool, to be hosted at the NASA Community Coordinated Modeling Center, serves as a valuable and accessible community resource for quantitatively evaluating the impact of observations from both current and future ionosphere‐thermosphere (IT) observing systems. This study demonstrates the utility of DART/TIEGCM as an IT OSSE tool, using synthetic observations simulated using a currently planned NASA Geospace Dynamics Constellation (GDC) observing system design. Five sets of OSSEs are carried out to compare the effects of assimilating various combinations of prospective GDC observations (e.g., neutral temperature, neutral wind, neutral composition, atomic oxygen ion density, and ion and electron temperature) during a major geomagnetic storm period of the St Patrick's Day Storm on 17 March 2013. The maximum error reduction in neutral temperature and atomic ion oxygen density is 24.6% and 43.3% compared to the control experiment. These OSSEs indicate the benefits of coupled IT DA approaches implemented in DART/TIEGCM to maximize the impact of multi‐parameter IT observations, such as those expected from the GDC mission. Although more work is required to draw any definitive conclusion on the GDC data impact, the study provides an illustrative example of how the DART/TIEGCM community tool can be used to evaluate observational impacts of planned or existing missions for geospace research and applications.

Abstract

AbstractThe field of environmental DNA (eDNA) has advanced over the past decade, with multiple approaches available for a variety of sampling media and species. While using eDNA for the purpose of simply detecting species is becoming a routine process, the utility of eDNA to estimate species abundance is not well understood. Here, we quantify salmon environmental DNA upstream of a fish counting fence along with river velocity, and together, use these values to determine the correlation between the number of salmon passing by the fish fence daily with daily eDNA rates in water before, during, and after the salmon spawning season for four Pacific salmonids (Oncorhynchus gorbuscha, O. kisutch, O. tshawytscha, and O. nerka; pink, coho, chinook, and sockeye, respectively). Throughout the spawning season, approximately 182,000 salmon were counted passing through the fence, of which >98% were pink salmon. Pink salmon exhibited strong correlation between human counts (effect size = 0.65, SE = 0.040) to eDNA rates in the present study and exhibited day‐to‐day variation and a unimodal profile rising and falling with human fish counts. However, the salmon species observed in much lower numbers exhibited a much weaker correlation with eDNA levels higher during the pre‐migratory period than during the migratory period for sockeye, coho, and chinook. Thus, for salmon spawning runs with less than ~1000 adults and daily counts of less than ~100, the juvenile and/or prior seasons eDNA signal appears to be indistinguishable from the adult spawning eDNA signal in our river system. However, for the large pink salmon run, eDNA rates appeared to reflect a local signal of salmon in space and time, essentially tracking these fish within days of passing through the eDNA sampling site.

6. Association of dementia and patient outcomes among COVID-19 patients: A multi-center retrospective case-control study

Abstract

BackgroundWe conducted a retrospective cohort study on COVID-19 patients with and without dementia by extracting data from the HCA Healthcare Enterprise Data Warehouse between January-September 2020.AimsTo describe the role of patients' baseline characteristics specifically dementia in determining overall health outcomes in COVID-19 patients.MethodsWe grouped in-patients who had ICD-10 codes for dementia (DM) with age and gender-matched (1:2) patients without dementia (ND). Our primary outcome variables were in-hospital mortality, length of stay, Intensive Care Unit (ICU) admission, ICU-free days, mechanical ventilation (MV) use, MV-free days and 90-day re-admission.ResultsMatching provided similar age and sex in DM and ND groups. BMI (median, 25.8 vs. 27.6) and proportion of patients who had smoked (23.3 vs. 31.3%) were lower in DM than in ND patients. The median (IQR) Elixhauser Comorbidity Index was higher in dementia patients 7 (5–10) vs. 5 (3–7, p < 0.01). Higher mortality was observed in DM group (30.8%) vs. ND group (26.4%, p < 0.01) as an unadjusted univariate analysis. The 90-day readmission was not different (32.1 vs. 31.8%, p = 0.8). In logistic regression analysis, the odds of dying were not different between patients in DM and ND groups (OR = 1.0; 95% CI 0.86–1.17), but the odds of ICU admissions were significantly lower for dementia patients (OR = 0.58, 95% CI 0.51–0.66).ConclusionsOur data showed that COVID-19 patients with dementia did not fare substantially worse, but in fact, fared better when certain metrics were considered.

Abstract

Understanding the timing and reproductive behavior of commercial fish species is a key part of well-informed stock assessments and fishery management, but this information is often limited, particularly for species that spawn in deep water. Petrale sole (Eopsetta jordani) is one of the most commercially important flatfish species in the US and is known to spawn off the West Coast during winter months. A number of spawning grounds have been identified using catch data and tagging studies, but to our knowledge there have been no direct visual observations of aggregating petrale sole. In 2018, we observed unusually high densities of petrale sole in autonomous underwater vehicle (AUV) imagery collected at a feature inshore of Santa Lucia Bank off the California coast. In addition to aggregations of fish, we observed fish in positions and configurations that we believe to be related to spawning behavior including physical contact between individuals and evidence of consistent size differentiation within clusters. We present images of the aggregating petrale sole and information on the physical and environmental conditions at this site. We also compare these observations to the results of AUV surveys carried out in 2005 at the same site. Analysis of commercial catch data from this area supports the hypothesis that this is a seasonal petrale sole aggregation as fishing at this location occurs mainly during winter months with catches dominated by petrale sole. In addition to the potential identification of a spawning aggregation, these observations show how advanced technologies can provide insight into the reproductive behavior of a commercially important species in-situ.

8. Functional Hyperconnectivity and Task-Based Activity Changes Associated With Neuropathic Pain After Spinal Cord Injury: A Pilot Study

Abstract

Neuropathic pain (NP) is a devastating chronic pain condition affecting roughly 80% of the spinal cord injury (SCI) patient population. Current treatment options are largely ineffective and neurophysiological mechanisms of NP are not well-understood. Recent studies in neuroimaging have suggested that NP patients have differential patterns of functional activity that are dependent upon the neurological condition causing NP. We conducted an exploratory pilot study to examine functional activation and connectivity in SCI patients with chronic NP compared to SCI patients without NP. We developed a novel somatosensory attention task to identify short term fluctuations in neural activity related to NP vs. non-painful somatosensation using functional magnetic resonance imaging (fMRI). We also collected high-resolution resting state fMRI to identify connectivity-based correlations over time between the two groups. We observed increased activation during focus on NP in brain regions associated with somatosensory integration and representational knowledge in pain subjects when compared with controls. Similarly, NP subjects showed increased connectivity at rest in many of the same areas of the brain, with positive correlations between somatomotor networks, the dorsal attention network, and regions associated with pain and specific areas of painful and non-painful sensation within our cohort. Although this pilot analysis did not identify statistically significant differences between groups after correction for multiple comparisons, the observed correlations between NP and functional activation and connectivity align with a priori hypotheses regarding pain, and provide a well-controlled preliminary basis for future research in this severely understudied patient population. Altogether, this study presents a novel task, identifies regions of increased task-based activation associated with NP after SCI in the insula, prefrontal, and medial inferior parietal cortices, and identifies similar regions of increased functional connectivity associated with NP after SCI in sensorimotor, cingulate, prefrontal, and inferior medial parietal cortices. This, along with our complementary results from a structurally based analysis, provide multi-modal evidence for regions of the brain specific to the SCI cohort as novel areas for further study and potential therapeutic targeting to improve outcomes for NP patients.

10. Ensemble Kalman filter assimilation of near-surface observations over complex terrain: comparison with 3DVAR for short-range forecasts

Abstract

Surface observations are the main conventional observations for weather forecasts. However, in modern numerical weather prediction, the use of surface observations, especially those data over complex terrain, remains a unique challenge. There are fundamental difficulties in assimilating surface observations with three-dimensional variational data assimilation (3DVAR). In this study, a series of observing system simulation experiments is performed with the ensemble Kalman filter (EnKF), an advanced data assimilation method to compare its ability to assimilate surface observations with 3DVAR. Using the advanced research version of the Weather Research and Forecasting (WRF) model, results from the assimilation of observations at a single observation station demonstrate that the EnKF can overcome some fundamental limitations that 3DVAR has in assimilating surface observations over complex terrain. Specifically, through its flow-dependent background error term, the EnKF produces more realistic analysis increments over complex terrain in general. More comprehensive comparisons are conducted in a short-range weather forecast using a synoptic case with two severe weather systems: a frontal system over complex terrain in the western US and a low-level jet system over the Great Plains. The EnKF is better than 3DVAR for the analysis and forecast of the low-level jet system over flat terrain. However, over complex terrain, the EnKF clearly performs better than 3DVAR, because it is more capable of handling surface data in the presence of terrain misrepresentation. In addition, results also suggest that caution is needed when dealing with errors due to model terrain representation. Data rejection may cause degraded forecasts because data are sparse over complex terrain. Owing to the use of limited ensemble sizes, the EnKF analysis is sensitive to the choice of horizontal and vertical localisation scales.

Abstract

Expression of the programmed death 1 (PD-1) receptor and its ligands are implicated in the T cell exhaustion phenotype which contributes to the persistence of several chronic viral infections, including human hepatitis C virus (HCV). The antiviral potential of BMS-936558 (MDX-1106) - a fully human anti-PD-1 monoclonal immunoglobulin-G4 that blocks ligand binding - was explored in a proof-of-concept, placebo-controlled single-ascending-dose study in patients (N = 54) with chronic HCV infection. Interferon-alfa treatment-experienced patients (n = 42) were randomized 5∶1 to receive a single infusion of BMS-936558 (0.03, 0.1, 0.3, 1.0, 3.0 mg/kg [n = 5 each] or 10 mg/kg [n = 10]) or of placebo (n = 7). An additional 12 HCV treatment-naïve patients were randomized to receive 10 mg/kg BMS-936558 (n = 10) or placebo (n = 2). Patients were followed for 85 days post-dose. Five patients who received BMS-936558 (0.1 [n = 1] or 10 mg/kg) and one placebo patient achieved the primary study endpoint of a reduction in HCV RNA ≥0.5 log10 IU/mL on at least 2 consecutive visits; 3 (10 mg/kg) achieved a >4 log10 reduction. Two patients (10 mg/kg) achieved HCV RNA below the lower limit of quantitation (25 IU/mL), one of whom (a prior null-responder) remained RNA-undetectable 1 year post-study. Transient reductions in CD4(+), CD8(+) and CD19(+) cells, including both naïve and memory CD4(+) and CD8(+) subsets, were observed at Day 2 without evidence of immune deficit. No clinically relevant changes in immunoglobulin subsets or treatment-related trends in circulating cytokines were noted. BMS-936558 exhibited dose-related exposure increases, with a half-life of 20-24 days. BMS-936558 was mostly well tolerated. One patient (10 mg/kg) experienced an asymptomatic grade 4 ALT elevation coincident with the onset of a 4-log viral load reduction. Six patients exhibited immune-related adverse events of mild-to-moderate intensity, including two cases of hyperthyroidism consistent with autoimmune thyroiditis. We then focus on service-learning in teacher education and how it has enjoyed a strong resurgence in the last 15 years. Finally, we offer a rationale for and a model of service-learning in the northwest region of the US, supported and endorsed by the Northwest Center for Service Learning in Teacher Education.

Abstract

BACKGROUND: Among patients with stable coronary disease and moderate or severe ischemia, whether clinical outcomes are better in those who receive an invasive intervention plus medical therapy than in those who receive medical therapy alone is uncertain. METHODS: We randomly assigned 5179 patients with moderate or severe ischemia to an initial invasive strategy (angiography and revascularization when feasible) and medical therapy or to an initial conservative strategy of medical therapy alone and angiography if medical therapy failed. The primary outcome was a composite of death from cardiovascular causes, myocardial infarction, or hospitalization for unstable angina, heart failure, or resuscitated cardiac arrest. A key secondary outcome was death from cardiovascular causes or myocardial infarction. RESULTS: Over a median of 3.2 years, 318 primary outcome events occurred in the invasive-strategy group and 352 occurred in the conservative-strategy group. At 6 months, the cumulative event rate was 5.3% in the invasive-strategy group and 3.4% in the conservative-strategy group (difference, 1.9 percentage points; 95% confidence interval [CI], 0.8 to 3.0); at 5 years, the cumulative event rate was 16.4% and 18.2%, respectively (difference, -1.8 percentage points; 95% CI, -4.7 to 1.0). Results were similar with respect to the key secondary outcome. The incidence of the primary outcome was sensitive to the definition of myocardial infarction; a secondary analysis yielded more procedural myocardial infarctions of uncertain clinical importance. There were 145 deaths in the invasive-strategy group and 144 deaths in the conservative-strategy group (hazard ratio, 1.05; 95% CI, 0.83 to 1.32). CONCLUSIONS: Among patients with stable coronary disease and moderate or severe ischemia, we did not find evidence that an initial invasive strategy, as compared with an initial conservative strategy, reduced the risk of ischemic cardiovascular events or death from any cause over a median of 3.2 years. The trial findings were sensitive to the definition of myocardial infarction that was used. 15. 513 Lovenox Titration in Burn Patients: Does a One Size Fits All Model Work?

Abstract

Introduction Venous thromboembolism (VTE) presents a significant morbidity to burn injured patients. The Western Trauma Association (WTA) and American Association of Trauma Surgeons (AAST) have recently developed guidelines for VTE prophylaxis dosing in trauma patients in which doses are adjusted based on anti-Xa levels. We sought to determine whether adopting these guidelines would lead to improved VTE prophylaxis in patients admitted to our burn service. Methods We initiated a protocol at our American Burn Association verified burn center of titrating VTE prophylaxis for our patients based on anti-Xa levels consistent with the WTA and AAST guidelines. We performed a retrospective chart review of all patients in which anti-Xa levels were measured between May and October 2022. Chi square, Fisher's exact, and ANOVA were used to analyze the data. Results A total of thirty six patients met our inclusion criteria. Age (49 [35.0, 61.0]), BMI (26.7 [6.8]), gender (67% male), and percent total body surface area (4.5 [2.0, 10.5]) were not statistically significant. Patients with a final lovenox dose of 30mg twice daily required a significantly lower dose adjustment than patients with a final lovenox dose of greater than 30mg (5.1 vs 7.9, p=0.0003). While not statistically significant, there was a trend for patients with a final lovenox dose of greater than 30mg twice daily to have larger percent TBSA affected (6.0 vs 13.6, p = 0.079). When comparing patients whose final dose of lovenox was less than, equal to, or greater than their initial dose, there was a significant difference in dose adjustment between patients whose lovenox had to be decreased as opposed to those who had to be increased (10.0 vs 15.8, p< 0.0001). None of our patients suffered a VTE event over the study period. Conclusions A “one size fits all” approach to VTE prophylaxis in burn patients may lead to inappropriate prophylaxis. Our study demonstrates that titration of VTE prophylaxis with lovenox using anti-Xa levels in the burn population is feasible and that patients with burns may require individualized dosing for appropriate prophylaxis. Applicability of Research to Practice Our research demonstrates that titration of VTE prophylaxis in a burn population may be more appropriate than the traditional “one size fits all” approach.

Abstract

Introduction Poverty is a known risk factor for burn injury and is associated with residency in food deserts and food swamps. Food deserts are areas with low or no access to nutritious, healthy foods while food swamps are areas with much higher access to high-calorie, unhealthy foods than to foods with good nutritious value. The purpose of our investigation was to determine the prevalence of residency in food deserts and food swamps in burn patients evaluated at an urban academic center and to determine the relationship between residency in these areas and the presence of patient comorbidities. Methods We performed a retrospective chart review of patients with burn injuries seen at an ABA verified urban academic center between January 2016 and January 2022. All patients over age 18 seen in the emergency department or admitted to the burn service were included in the study. Patient residential locations were used in conjunction with the Modified Retail Food Environment Index (mREFI) data to classify residency in food deserts and food swamps. Patient demographics and comorbidities were also recorded. Chi square and ANOVA analyses were completed to determine differences between the food desert, food swamp, and good access groups. Results A total of 3064 subjects were included in the study, of these 1370 (44.7%) were female and 1694 (55.3%) were male. In terms of access, 96 (3.1%) individuals lived in food deserts, 477 (15.6%) lived in good access areas, and 2490 (81.3%) resided in food swamps. This difference was found to be statistically significant (p < 0.0001). Age, sex, and BMI did not differ significantly between the three groups. A significantly higher percentage of food swamp residents had a diagnosis of diabetes (15.0%) as compared to subjects living in good food access areas (11.7%) and food deserts (8.3%), (p < 0.043). Hypertension was also found to be more prevalent in food swamps (24.8%) than food deserts (20.8%) and good access areas (19.3%), (p < 0.028). Finally, smoking was more common in food swamps (45.3%) than good access areas (40.4%) and food deserts (27.9%), (p < 0.0028). Conclusions A majority of burn patients reside in food swamps and residency in these areas is associated with a higher prevalence of hypertension, diabetes, and tobacco smoking. This is the first study to investigate the epidemiology of food access in an urban burn population. Applicability of Research to Practice Nutrition is known to be important to overall health and to wound healing in the setting of an acute injury. Our data contributes to the current literature on the role of nutrition in burns.

Abstract

Introduction Many victims of burn injury live in food deserts or food swamps. Food deserts are low-income areas with minimal or no access to nutritious foods while food swamps are areas with much greater access to unhealthy, high-calorie foods. We examined the relationship between residency in a food swamp or food desert by census tract and wound healing. Methods We performed a retrospective review of burn patients who underwent split-thickness skin grafting at an ABA verified urban academic burn center between September 2018 and August 2022. Inclusion criteria were burn area less than 20% total body surface area (TBSA), age ≥ 18, and single operation for split-thickness skin grafting. Patient residential locations were used in conjunction with Modified Retail Food Environment Index (mREFI) data to classify residency in food deserts and food swamps. The primary outcome was time to donor site healing. Time-to-event Cox proportional regression analysis was performed to evaluate risk factors for poor wound healing. Results A total of 238 patients were included in the study, of whom 110 (46.2%) were female and 128 (53.8%) were male. Mean age was 48, BMI was 28.5, TBSA 3.7, and average days to donor site healing was 26. In categorizing residency by mREFI data, 45 (18.9%) individuals had good access, 11 (4.6%) resided in food deserts, and 182 (76.5%) resided in food swamps. Although time to healing was longer on average in the food desert (25.8 days) and food swamp (26.4 days) groups compared to the group with good access to food within ½ mile (22.9 days), this difference was not statistically significant. Not having diabetes was found to be associated with shorter time to wound healing (HR=1.68, 95% CI [1.15, 2.47], p=0.007). Living in a good access area was associated with faster wound healing (compared to swamp: HR=1.23, 95% CI [0.86, 1.75], compare to desert: HR=1.06, 95% CI [0.51, 2.20]), however this association was not statistically significant. Conclusions Residency in a food swamp or food desert does not significantly influence time to healing in burn injured patients. However, not having diabetes was associated with a shorter time to wound healing. This is the first study to compare residency in food deserts or food swamps by census tract level data with time to wound healing in an urban burn population. Applicability of Research to Practice Wound healing is a complex and important process known to be affected by malnutrition. Our data contributes to the current body of literature on nutrition and wound healing.

Abstract

This paper describes the second phase of an Observing System Simulation Experiment (OSSE) that utilizes the synthetic measurements from a constellation of satellites measuring atmospheric composition from geostationary (GEO) Earth orbit presented in part I of the study. Our OSSE is focused on carbon monoxide observations over North America, East Asia and Europe where most of the anthropogenic sources are located. Here we assess the impact of a potential GEO constellation on constraining northern hemisphere (NH) carbon monoxide (CO) using data assimilation. We show how cloud cover affects the GEO constellation data density with the largest cloud cover (i.e., lowest data density) occurring during Asian summer. We compare the modeled state of the atmosphere (Control Run), before CO data assimilation, with the known 'true' state of the atmosphere (Nature Run) and show that our setup provides realistic atmospheric CO fields and emission budgets. Overall, the Control Run underestimates CO concentrations in the northern hemisphere, especially in areas close to CO sources. Assimilation experiments show that constraining CO close to the main anthropogenic sources significantly reduces errors in NH CO compared to the Control Run. We assess the changes in error reduction when only single satellite instruments are available as compared to the full constellation. We find large differences in how measurements for each continental scale observation system affect the hemispherical improvement in long-range transport patterns, especially due to seasonal cloud cover. A GEO constellation will provide the most efficient constraint on NH CO during winter when CO lifetime is longer and increments from data assimilation associated with source regions are advected further around the globe.

Abstract

Background & Aims Nivolumab, an immune checkpoint inhibitor, is approved in several countries to treat sorafenib-experienced patients with HCC, based on results from the CheckMate 040 study (NCT01658878). Marked differences exist in HCC clinical presentation, aetiology, treatment patterns and outcomes across regions. This analysis assessed the safety and efficacy of nivolumab in the Asian cohort of CheckMate 040. Methods CheckMate 040 is an international, multicentre, open-label, phase I/II study of nivolumab in adults with advanced HCC, regardless of aetiology, not amenable to curative resection or local treatment and with/without previous sorafenib treatment. This analysis included all sorafenib-experienced patients in the intent-to-treat (ITT) overall population and Asian cohort. The analysis cut-off date was March 2018. Results There were 182 and 85 patients in the ITT population and Asian cohort, respectively. In both populations, most patients were older than 60 years, had BCLC (Barcelona Clinic Liver Cancer) Stage C disease, and had received previous systemic therapy. A higher percentage of Asian patients had HBV infections, extrahepatic metastases and prior therapies. Median follow-up was 31.6 and 31.3 months for the ITT and Asian patients, respectively. Objective response rates were 14% and 15% in the ITT population and Asian cohort, respectively. In the Asian cohort, patients with HBV, HCV or those who were uninfected had objective response rates of 13%, 14% and 21%, respectively. The median duration of response was longer in the ITT (19.4 months) vs. Asian patients (9.7 months). Median overall survival was similar between the ITT (15.1 months) and Asian patients (14.9 months), and unaffected by aetiology in Asian patients. The nivolumab safety profile was similar and manageable across both populations. Conclusion Nivolumab safety and efficacy are comparable between sorafenib-experienced ITT and Asian patients. Lay summary The CheckMate 040 study evaluated the safety and efficacy of nivolumab in patients with advanced hepatocellular carcinoma who were refractory to previous sorafenib treatment or chemotherapy. This subanalysis of the data showed that treatment responses and safety in patients in Asia were similar to those of the overall treatment population, providing support for nivolumab as a treatment option for these patients. Abstract

Carbon capture and storage (CCS) is a prominent mitigation technology in many of the pathways to achieve global net-zero carbon-dioxide (CO2) emissions. Despite this proposed importance, only one operational power facility in the U.S. is currently equipped with CCS. Further CCS capacity may be promoted with recently-enhanced tax credits for carbon sequestration in Section 45Q of the U.S. Internal Revenue Code. In this paper, we expand the unit-specific techno-economic model ESTEAM to include coal-fired and natural gas combined cycle (NGCC) retrofitted CCS capacity and new NGCC CCS capacity to evaluate CCS for sequestration in the projected 2030 U.S. fossil-fuel fleet. Using this model in a parametric study, we conclude that unique credit levels for each CCS option are required for each option to separately achieve the same percent generation level of the projected 2030 net generation. We further find that increasing the credit duration can lower the CO2 avoided cost for the fleets and increase CCS-capacity bridging from 2030 to 2050. Overall, we determine that a lower avoided cost for immediate sequestration is achieved through promoting new and existing NGCC CCS.

Abstract

There is growing concern that progress toward reaching the carbon dioxide (CO2) targets set forth in the Paris Agreement is falling short of the mark, and efforts to decarbonize the global economy must be hastened and the reductions deepened. Doing so quickly through a buildup of natural gas generating capacity as a replacement for coal-fired capacity can greatly aid in lowering near-term emissions towards the Agreement target to limit future climate impact to well below 2 °C but would incur stranded costs as natural gas assets are retired before their end-of-life age. The stranding of such capital investment may inhibit efforts to further decarbonize the world economy because of technology lock-in. One possible solution to reduce such stranded costs is to mitigate CO2 emissions from the existing coal-fleet. For the evaluation of the costs and emissions, we develop a novel method that uses unique coal-fired electric generating unit (EGU) characteristics to evaluate multiple mitigation-technology options under local fuel prices; the result of which is a least-cost mitigation frontier for nine EGU-specific mitigation solutions created within a common assessment framework. With this EGU-specific method, we find the mitigation options that achieve the lowest capital cost and levelized cost of electricity to meet CO2 emission-intensity reduction-targets for a representative EGU in the U.S. coal-fired fleet and use this method to analyze the uncertainty for these deterministic solutions. For this EGU, we find that the portfolio of mitigations defining the frontier is sensitive to fuel price variation, as well as other EGU-specific factors such as efficiency and retirement age. A probabilistic analysis of projected fuel prices indicates that mitigation decision-related regret can be high but may be limited to specific intensity targets. A further insight is that for deep CO2 mitigation, high capital costs and predominantly low natural gas prices may limit the viability of coal-fired EGUs retrofitted with carbon capture and storage with an auxiliary power system, even with a tax credit (e.g., Section 45Q of the Bipartisan Budget Act of 2018).

Abstract

The purpose of this study was to evaluate the outcomes of the novel Fantom coronary bioresorbable scaffold at 6 months.The Fantom sirolimus-eluting bioresorbable scaffold incorporates a unique proprietary iodinated, polycarbonate copolymer of tyrosine analogs that is radiopaque, with thin struts (125 μm) that facilitate device delivery and precise target lesion treatment.The 6-month outcomes and performance of the Fantom scaffold were evaluated in 117 patients with single de novo native coronary artery lesions of length ≤20 mm and reference vessel diameter 2.5 to 3.5 mm. The primary angiographic endpoint was mean late lumen loss at 6 months measured by quantitative coronary angiography. Procedural outcomes were categorized as short-term technical success, short-term procedural success, and clinical procedural success. The primary clinical endpoint was major adverse cardiac events at 6 months, the composite of cardiac death, myocardial infarction (MI), or clinically driven target lesion revascularization (TLR).Short-term technical success, short-term procedural success, and clinical procedural success were achieved in 96.6%, 99.1%, and 99.1% of patients, respectively. Mean 6-month in-stent late lumen loss was 0.25 ± 0.40 mm (n = 100). Binary restenosis was present in 2 patients (2.0%). Major adverse cardiac events within 6 months occurred in 3 patients (2.6%), including no deaths, 2 MIs, and 2 TLRs (1 patient had both an MI and TLR). Scaffold thrombosis occurred in 1 patient (0.9%).The clinical results from 117 patients enrolled in cohort A of the multicenter FANTOM II (SafetyPerformance Study of the FANTOM Sirolimus-Eluting Bioresorbable Coronary Scaffold) study demonstrate favorable 6-month outcomes of this novel device in the treatment of noncomplex coronary artery disease.

Abstract

Background Nivolumab (NIVO), a programmed death-1 (PD-1) inhibitor, is approved in several countries for sorafenib-treated patients (pts) with advanced hepatocellular carcinoma (aHCC) based on CheckMate040. We report findings on exploratory biomarker analyses. Methods In CheckMate040, pts with aHCC received NIVO regardless of programmed death ligand 1 (PD-L1) status and HCC etiology. Baseline tumor samples were analyzed by IHC for expression of PD-L1, PD-1, T-cell markers (CD3, CD4, CD8, FOXP3), and macrophages (CD68, CD163), and (in a subset of pts) by RNA sequencing for inflammatory signatures (ISs). Results were correlated with clinical outcomes: response (complete response [CR]/partial response [PR]/stable disease [SD]/progressive disease [PD]) and overall survival (OS). Associations with etiology and geographical region (non-Asia vs Asia) were assessed. Data cutoff: June 2018. Results In 184 pts with evaluable data, increased tumor cell PD-L1 expression was associated with response (CR+PR vs SD [P = 0.00009]; CR+PR vs PD [P = 0.0007]) and OS (P = 0.03), as was increased PD-1 expression (CR+PR vs SD [P = 0.05]; CR+PR vs PD [P = 0.009]; OS [P = 0.05]). Of the T-cell markers assessed, CD3 was associated with response (n = 182, CR+PR vs SD; P = 0.05). In pts positive for CD3 or CD8, there was a trend toward improved OS (P = 0.08). There was no association between CD68 and CD163 expression and clinical outcomes. For 37 pts with RNA sequencing data available, median Bristol-Myers Squibb (BMS) IS score was higher in pts with PR vs SD (P = 0.05) and was correlated with improved OS (P = 0.01). For all inflammation markers assessed, there was no association with etiology or geographical region. Conclusions In pts with aHCC, improved OS and response to NIVO may be associated with higher PD-L1, PD-1, and CD3 expression, and higher BMS IS scores, supporting the role of PD-1 inhibition in HCC treatment. Further investigation of these biomarkers is required. Originally presented at the AACR 2019.

Abstract

The term, hip pointer, is applied in the setting of a blunt trauma injury to the iliac crest. It typically occurs in contact and collision sports and can cause significant pain and loss of practice or game time. A direct blow results in subperiosteal edema with hematoma formation within surrounding muscle or soft tissue and bone contusion of the iliac crest. Conservative management with compression, ice, antiinflammatories, and rehabilitation exercises are successful in treating hip pointers. Injection therapy with the use of local anesthetic can be helpful in minimizing pain and increasing function to allow more rapid return to play.

Abstract

Mycobacterium ulcerans is the causative agent of the debilitating skin disease Buruli ulcer, which is most prevalent in Western and Central Africa. M. ulcerans shares >98% DNA sequence identity with Mycobacterium marinum, however, M. marinum produces granulomatous, but not ulcerative, lesions in humans and animals. Here we report the differential expression of a small heat shock protein (Hsp18) between strains of M. ulcerans (Hsp18(+) ) and M. marinum (Hsp18(-) ) and describe the molecular basis for this difference. We show by gene deletion and GFP reporter assays in M. marinum that a divergently transcribed gene called hspR_2, immediately upstream of hsp18, encodes a MerR-like regulatory protein that represses hsp18 transcription while promoting its own expression. Naturally occurring mutations within a 70 bp segment of the 144 bp hspR_2-hsp18 intergenic region among M. ulcerans strains inhibit hspR_2 transcription and explain the Hsp18(+) phenotype. We also propose a biological role for Hsp18, as we show that this protein significantly enhances bacterial attachment or aggregation during biofilm formation. This study has uncovered a new member of the MerR family of transcriptional regulators and suggests that upregulation of hsp18 expression was an important pathoadaptive response in the evolution of M. ulcerans from a M. marinum-like ancestor.

Abstract

The Dawn Project is a system of care responsible for creating and implementing an interagency collaborative approach to service provision. Systems of care are spectrums of services organized to meet the needs facing youth with serious emotional and behavioral difficulties. This study examined the relationship between school involvement in the Dawn Project and outcomes for participating youth. This was done by determining whether school contact with the service coordinator and school involvement with the child and family team had an impact on educational placement restrictiveness. Findings indicate that age, ethnicity, and gender are significant predictors of educational placement restrictiveness. There is also evidence to suggest that school involvement may be associated with more restrictive educational placements.

Abstract

To improve rank constrained reconstructions for undersampled multi-image MRI acquisitions.Motivated by the recent developments in low-rank matrix completion theory and its applicability to rapid dynamic MRI, a new reordering-based rank constrained reconstruction of undersampled multi-image data that uses prior image information is proposed. Instead of directly minimizing the nuclear norm of a matrix of estimated images, the nuclear norm of reordered matrix values is minimized. The reordering is based on the prior image estimates. The method is tested on brain diffusion imaging data and dynamic contrast enhanced myocardial perfusion data.Good quality images from data undersampled by a factor of three for diffusion imaging and by a factor of 3.5 for dynamic cardiac perfusion imaging with respiratory motion were obtained. Reordering gave visually improved image quality over standard nuclear norm minimization reconstructions. Root mean squared errors with respect to ground truth images were improved by ∼18% and ∼16% with reordering for diffusion and perfusion applications, respectively.The reordered low-rank constraint is a way to inject prior image information that offers improvements over a standard low-rank constraint for undersampled multi-image MRI reconstructions.

Abstract

Previous studies have shown that the metabotropic glutamate receptor (mGluR)2/3 agonist LY354740 attenuated glutamate release in medial prefrontal cortex (mPFC) induced by the non-competitive N -methyl- d -aspartate (NMDA) receptor antagonist phencyclidine. In the present study we examined the effects of the more potent mGluR2/3 selective agonist LY379268 on ketamine-evoked glutamate and dopamine (DA) release in mPFC of male rats. Subjects were implanted with a unilateral microdialysis probe in the mPFC and were tested 12–24 h after implantation. Ketamine (18 mg/kg, s.c.) evoked a significant release of glutamate and DA, although the glutamate response was slower in onset compared with DA. Pretreatment with either systemic (3 mg/kg s.c.) or local (1 μM, in the probe) LY379268 blocked ketamine-evoked glutamate, but not DA, release. When applied directly to the mPFC via the dialysis probe, ketamine (1 mM in the probe) had no effect on glutamate release but did significantly enhance the release of DA. Application of NMDA (500 μM in the probe), on the other hand, decreased DA while increasing glutamate release. The effect of NMDA on evoking glutamate release was blocked by systemic but not local administration of LY379268. These findings indicate that systemic ketamine increases both glutamate and DA release in mPFC and that the effect on glutamate can be blocked by stimulating mPFC group II mGluR receptors. Local ketamine, on the other hand, does not increase glutamate but does increase DA release. This suggests that ketamine acts outside of the mPFC to enhance glutamate, but within the mPFC to enhance DA release. The origin of the ketamine effect on mPFC glutamate is currently not known.

Objective:To determine the effects of Vicoprofen® and ibuprofen on aerobic performance, agility, and pain after exercise-induced muscle damage.Design:Double-blind randomized, placebo-controlled, repeated-dose clinical trial.Setting:Human-performance and sports-medicine laboratory.Participants:36 healthy men.Methods and Measures:Baseline testing was performed, 72 hours after which subjects performed eccentric exercise to induce muscle damage. They were evaluated for pain 24 hours postdamage and placed randomly into 3 groups: Vicoprofen (VIC), ibuprofen, or placebo (P). Postdamage testing was performed every day for 5 days. Subjects performed an economy run and a t-agility test to determine exercise performance.Results:The drugs had no significant effect on performance throughout the 5-day evaluation period. Pain was lower at days 4 and 5 in the VIC group than in P.Conclusions:It appears that Vicoprofen reduced pain after muscle damage, but the drug interventions did not enhance performance in aerobic and agility tasks.

The article offers examples of how professionalism is taught in medicine, law, and business, and what each profession might learn from the others in developing their professionalism education and pedagogy.

Abstract

Alterations in expression of adhesion molecules are important in the trafficking of hematopoietic progenitors and probably in the mobilization process. Relatively little and conflicting data are currently available on the differences in expression between good and poor mobilizing patients.In this study, the expression of eight adhesion molecules on the collected CD34+ cells from 36 patients undergoing mobilization was determined.Good mobilizing patients, defined as those who collected their target in one apheresis procedure, had significantly fewer cells that expressed CD11a (LFA-1) and CD54 (ICAM-1) and borderline fewer that expressed CD11c, CD49d (VLA-4), and CD49d (VLA-5). No differences were detected in CD11b (Mac-1), CD15s (sLe(x)), or CD62L (L-selectin). Linear regression analysis identified number of prior chemotherapy courses and expression of CD11a (LFA-1) as independent predictive factors for mobilization efficiency. Good and poor mobilizing patients had approximately the same number of total CD34+ cells collected and little difference in times to engraftment.CD11a (LFA-1) expression inversely correlates with mobilization efficiency. Elucidation of the mechanism(s) underlying these observations will require further study.

38. A Prospective Study of Overuse Knee Injuries Among Female Athletes With Muscle Imbalances and Structural Abnormalities

OBJECTIVE: To prospectively examine the influence of hamstring-to-quadriceps (H:Q) ratio and structural abnormalities on the prevalence of overuse knee injuries among female collegiate athletes. DESIGN AND SETTING: We used chi-square 2 x 2 contingency tables and the Fischer exact test to examine associations among H:Q ratios, structural abnormalities, and overuse knee injuries. SUBJECTS: Fifty-three apparently healthy women (age = 19.4 +/- 1.3 years, height = 167.6 +/- 10.1 cm, mass = 65.0 +/- 10.0 kg) from National Collegiate Athletic Association Division I women's field hockey (n = 23), soccer (n = 20), and basketball teams (n = 10) volunteered. MEASUREMENTS: The H:Q ratio was determined from a preseason isokinetic test on a Biodex system at 60 degrees /s and 300 degrees /s. We measured athletes for genu recurvatum and Q-angles with a 14-in (35.56-cm) goniometer. Iliotibial band flexibility was assessed via the Ober test. RESULTS: Ten overuse knee injuries (iliotibial band friction syndromes = 5, patellar tendinitis = 3, patellofemoral syndrome = 1, pes anserine tendinitis = 1) occurred in 9 athletes. The H:Q ratio below the normal range at 300 degrees /s (P = 0.047) was associated with overuse knee injuries, as was the presence of genu recurvatum (P = 0.004). In addition, athletes possessing lower H:Q ratios at 300 degrees /s and genu recurvatum incurred more overuse knee injuries than athletes without these abnormalities (P = 0.001). CONCLUSIONS: The presence of genu recurvatum and an H: Q ratio below normal range was associated with an increased prevalence of overuse knee injuries among female collegiate athletes. Further investigation is needed to clarify which preseason screening procedures may identify collegiate athletes who are susceptible to overuse knee injuries.

This article examines the role of gambling as an addictive disorder experienced by athletes, both college and professional. Gambling may often be seen as a comorbid factor with other addictions and with depression among athletes. The focus on addictions among athletes has gained considerable attention among sports medicine clinicians. Diagnostic indicators, risk and protective factors, and a stage model of addiction among athletes are addressed. An algorithm and pathway of care for athletes with an addictive disorder is offered as are recommendations that sports physicians, sports medicine specialists, coaches and counsellors need to address athletes who have an addictive disorder.

Abstract

To describe growth patterns in infants with single ventricle physiology and determine factors influencing growth.Data from 230 subjects enrolled in the Pediatric Heart Network Infant Single Ventricle Enalapril Trial were used to assess factors influencing change in weight-for-age z-score (z) from study enrollment (0.7 ± 0.4 months) to pre-superior cavopulmonary connection (SCPC; 5.1 ± 1.8 months, period 1) and pre-SCPC to final study visit (14.1 ± 0.9 months, period 2). Predictor variables included patient characteristics, feeding regimen, clinical center, and medical factors during neonatal (period 1) and SCPC hospitalizations (period 2). Univariate regression analysis was performed, followed by backward stepwise regression and bootstrapping reliability to inform a final multivariable model.Weights were available for 197 of 230 subjects for period 1 and 173 of 197 subjects for period 2. For period 1, greater gestational age, younger age at study enrollment, tube feeding at neonatal hospitalization discharge, and clinical center were associated with a greater negative z (poorer growth) in multivariable modeling (adjusted R(2) = 0.39, P.001). For period 2, younger age at SCPC and greater daily caloric intake were associated with greater positive z (better growth; R(2) = 0.10, P = .002).Aggressive nutritional support and earlier SCPC are modifiable factors associated with a favorable change in weight-for-age z-score.
