1. Prospective Study on Sexual Dysfunction in Male Chinese Liver Transplant Recipients

Abstract: In patients with end-stage liver disease, hypogonadism and erectile dysfunction are often seen. This study was to determine the incidence of erectile dysfunction before and after liver transplantation (LT) with correlation to change in sex hormone levels from a Chinese cohort. This prospective longitudinal study was registered with The University of Hong Kong Clinical Trials Centre (HKUCTR-1563). The Institutional Review Board approval number is UW-12-273. The study period was from January 2012 to December 2016. Adult male patients with end-stage liver disease enlisted for LT were recruited on informed written consent. All recruited patients were to complete a cross-sectional cohort questionnaire—International Index of Erectile Function, version 5 (IIEF5)—and to receive serum sex hormone checks before and after LT. Twenty-eight patients who underwent LT were included in the analysis. The included patients had significantly reduced prolactin ( p < .001) and 17-beta-estradiol ( p = .024) after LT. There was also a significant drop of IIEF5 score at 1 month after LT, but the score returned to pre-LT level at 6 months. This study demonstrated that there was improvement in sex hormone levels after LT, namely, normalization of estradiol level and lowering of prolactin and progesterone levels. However, improvement in sex hormone levels did not translate into improvement of erectile dysfunction.

2. Aortopathy in a Mouse Model of Marfan Syndrome Is Not Mediated by Altered Transforming Growth Factor β Signaling

Abstract: BackgroundMarfan syndrome (MFS) is caused by mutations in the gene encoding fibrillin‐1 (FBN1); however, the mechanisms through which fibrillin‐1 deficiency causes MFS‐associated aortopathy are uncertain. Recently, attention was focused on the hypothesis that MFS‐associated aortopathy is caused by increased transforming growth factor‐β (TGF‐β) signaling in aortic medial smooth muscle cells (SMC). However, there are many reasons to doubt that TGF‐β signaling drives MFS‐associated aortopathy. We used a mouse model to test whether SMC TGF‐β signaling is perturbed by a fibrillin‐1 variant that causes MFS and whether blockade of SMC TGF‐β signaling prevents MFS‐associated aortopathy. Methods and ResultsMFS mice (Fbn1C1039G/+ genotype) were genetically modified to allow postnatal SMC‐specific deletion of the type II TGF‐β receptor (TBRII; essential for physiologic TGF‐β signaling). In young MFS mice with and without superimposed deletion of SMC‐TBRII, we measured aortic dimensions, histopathology, activation of aortic SMC TGF‐β signaling pathways, and changes in aortic SMC gene expression. Young Fbn1C1039G/+ mice had ascending aortic dilation and significant disruption of aortic medial architecture. Both aortic dilation and disrupted medial architecture were exacerbated by superimposed deletion of TBRII. TGF‐β signaling was unaltered in aortic SMC of young MFS mice; however, SMC‐specific deletion of TBRII in Fbn1C1039G/+ mice significantly decreased activation of SMC TGF‐β signaling pathways. ConclusionsIn young Fbn1C1039G/+ mice, aortopathy develops in the absence of detectable alterations in SMC TGF‐β signaling. Loss of physiologic SMC TGF‐β signaling exacerbates MFS‐associated aortopathy. Our data support a protective role for SMC TGF‐β signaling during early development of MFS‐associated aortopathy.

3. Risk Factors and Post-Resection Independent Predictive Score for the Recurrence of Hepatitis B-Related Hepatocellular Carcinoma.

Abstract: Independent risk factors associated with hepatitis B (HBV)-related hepatocellular carcinoma (HCC) after resection remains unknown. An accurate risk score for HCC recurrence is lacking.We prospectively followed up 200 patients who underwent liver resection for HBV-related HCC for at least 2 years. Demographic, biochemical, tumor, virological and anti-viral treatment factors were analyzed to identify independent risk factors associated with recurrence after resection and a risk score for HCC recurrence formulated.Two hundred patients (80% male) who underwent liver resection for HBV-related HCC were recruited. The median time of recurrence was 184 weeks (IQR 52-207 weeks) for the entire cohort and 100 patients (50%) developed HCC recurrence. Stepwise Cox regression analysis identified that one-month post resection HBV DNA >20,000 IU/mL (p = 0.019; relative risk (RR) 1.67; 95% confidence interval (C.I.): 1.09-2.57), the presence of lymphovascular permeation (p100ng/mL before resection (p = 0.021; RR 1.63; 95% C.I.: 1.08-2.47) were independently associated with HCC recurrence. Antiviral treatment before resection (p = 0.024; RR 0.1; 95% C.I.: 0.01-0.74) was independently associated with reduced risk of HCC recurrence. A post-resection independent predictive score (PRIPS) was derived and validated with sensitivity of 75.3% and 60.6% and specificity of 55.7% and 79.2%, to predict the 1- and 3-year risks for the HCC recurrence respectively with the hazard ratio of 2.71 (95% C.I.: 2.12-3.48; p

8. Impact of Tumour Biology on Outcomes of Radical Therapy for Hepatocellular Carcinoma Oligo-Recurrence after Liver Transplantation.

Abstract: It is uncertain whether tumour biology affects radical treatment for post-transplant hepatocellular carcinoma (HCC) oligo-recurrence, i.e. recurrence limited in numbers and locations amendable to radical therapy. We conducted a retrospective study on 144 patients with post-transplant HCC recurrence. Early recurrence within one year after transplant (HR 2.53, 95% CI 1.65–3.88, p < 0.001), liver recurrence (HR 1.74, 95% CI 1.12–2.68, p = 0.01) and AFP > 200 ng/mL upon recurrence (HR 1.62, 95% CI 1.04–2.52, p = 0.03) predicted mortality following recurrence. In patients with early recurrence and liver recurrence, radical treatment was associated with improved post-recurrence survival (early recurrence: median 18.2 ± 1.5 vs. 9.2 ± 1.5 months, p < 0.001; liver recurrence: median 28.0 ± 4.5 vs. 11.6 ± 2.0, p < 0.001). In patients with AFP > 200 ng/mL, improvement in survival did not reach statistical significance (median 18.2 ± 6.5 vs. 8.8 ± 2.2 months, p = 0.13). Survival benefits associated with radical therapy were reduced in early recurrence (13.6 vs. 9.0 months) and recurrence with high AFP (15.4 vs. 9.3 months) but were similar among patients with and without liver recurrence (16.9 vs. 16.4 months). They were also diminished in patients with multiple biological risk factors (0 risk factor: 29.0 months; 1 risk factor: 19.7 months; 2–3 risk factors: 3.4 months): The survival benefit following radical therapy was superior in patients with favourable biological recurrence but was also observed in patients with poor tumour biology. 9. Weighted Vest Load Arrangement and Data Normalization Effects on Lower Limb Biomechanics During Countermovement Jump Landings

Abstract: This paper assessed the weighted vest load arrangement and data normalization method effects on ground reaction forces (GRF), joint kinematics, and joint kinetics during the landing portion of the countermovement jump. Vertical GRF and sagittal kinematic data were obtained from 12 males and 12 females during countermovement jump-landings in 4 different loading arrangements (unloaded, 10% body mass load placed anteriorly, posteriorly, and split anterior/posterior). Two methods (body mass vs. mass*landing height) were used to normalize joint torques to determine whether common mass-normalization (type A) yielded different results than a jump-landing specific mass*landing height normalization (type B) in statistical significance. Mixed-model analyses of variance (α=0.05) and effect sizes (ES) were used to assess differences between sexes and loading conditions for each normalization method. Results show that for normalization A, significant statistical differences were found between sexes for peak vertical GRF, hip moment, and knee moment. Pooled sex peak vertical GRF and hip moments showed significant differences when comparing the unloaded with the back and front-loaded conditions. For normalization B, the peak vertical GRF also showed significant differences between men and women but with smaller effect sizes. Only the hip moment showed significant differences for both normalization methods but changed the magnitude of its effect sizes. Results suggest that different normalization methods could be considered for joint moments or GRF depending on the nature of the statistical significance of jump height.

13. Two-phase flow modelling by an error-corrected population balance model

Abstract: High-velocity aerated flow is a common phenomenon in spillways. Its accurate modelling is challenging, mainly due to the lack of realistic physics in the conventional two-phase models. To this end, this study establishes a population balance model (PBM) approach to account for the evolutionary process of air bubbles. The air-water flow in a stepped chute is examined. The model performance is evaluated by statistical metrics: correlation coefficient (CC), root mean squared error (RMSE), and mean absolute error (MAE). Compared with conventional models, the PBM generates improved air-water predictions. However, the flow parameters are still underestimated, particularly in areas with intense air-water interactions. For further development, an error-corrected PBM (EPBM) is proposed by incorporating machine learning (ML) techniques into the PBM. Compared with the PBM, the EPBM leads to a mean augmentation in velocity prediction by 19.8% for the CC, 73.0% for the RMSE, and 77.1% for the MAE. The gains in air concentration estimation are 2.0%, 67.6% and 73.5%, respectively. The EPBM generates the most accurate results, with 99.6% and 89.6% of the velocity and air concentration predictions within a 20% relative error range. The main contributions are establishing a PBM for air-water flows and developing an error-corrected PBM using ML.

This comprehensive survey provides an up-to-date review of the different driver behavior models and modeling approaches. In heterogeneous streets where humans and autonomous vehicles operate simultaneously, predicting the intent and action of human drivers is crucial for AVs with the help of wireless communication and artificial intelligence (AI) technologies. Therefore, the review also summarizes the applications of driver behavior modeling (DBM) for effective behavior recognition and human-like AV driving. Moreover, the review also covers the application of DBM in capturing behaviors of complex dynamic driving tasks. In this review, we solely cover car-following (CF) and lane-changing (LC) maneuvers.

15. Modelling of suspended sediment load by Bayesian optimized machine learning methods with seasonal adjustment

Abstract: Suspended sediment load (SSL) is essential to river and dam engineering. Due to the complexity and stochastic nature of sedimentation, SSL prediction is a challenging task and conventional methods often fail to generate accurate results. Aiming to provide an improved estimation, this paper contributes to a new forecasting framework by integrating the seasonal adjustment (SA) and Bayesian optimization (BOP) into a machine learning (ML) model (denoted as BMS). The SA is used for de-seasonalisation and trend extraction; the BOP is to optimize the ML architecture. The BMS is evaluated using the daily SSL records from the Yangtze River. Its performance is appraised by statistical criteria of Nash-Sutcliffe efficiency (NSE), correlation coefficient (CC), root mean squared error (RMSE) and mean absolute error (MAE). With the de-noising and hyper-parameter tuning modules, the BMS effectively heightens the accuracy of the standard ML models. The most significant improvement occurs in the Boosting model, with its augments in NSE and CC by 4.3% and 1.6%, and reductions in RMSE and MAE by 24.9% and 24.2%. The BMS gains by comparison even under flood conditions, where it remarkably reduces the errors of the constituent models by up to 47.9% for RMSE and 48.3% for MAE.

Abstract: Background: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: High FiO2 may be cost-effective (cheaper and effective). In Nigeria, the average cost for high FiO2 was $216 compared with $222 for low FiO2 leading to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the average cost for high FiO2 was $184 compared with $195 for low FiO2 leading to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the average cost for high FiO2 was $1164 compared with $1257 for low FiO2 leading to a −$93 (95% CI: −$132 to −$65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this.

17. Tension pneumothorax in a patient with Covid-19 pneumonia initially misdiagnosed as volvulus

Abstract: Tension pneumothorax is a condition that can present with a myriad of symptoms, including chest pain, shortness of breath, rapid breathing, and tachycardia. If left untreated, these signs and symptoms can progress to shock causing haemodynamic collapse and even death. At times, it may be difficult to identify tension pneumothorax. We present the case of a 59-year-old male with a prolonged initial hospital course that eventually led to a diagnosis of tension pneumothorax with the use of CT scans rather than traditional X-rays. This case reinforces the idea that clinicians should have a wide differential diagnosis in mind when dealing with vague symptoms and should not hesitate to use different diagnostic modalities to help confirm a diagnosis. Abstract: A spillway discharging a high-speed flow is susceptible to cavitation damages. As a countermeasure, an aerator is often used to artificially entrain air into the flow. Its air demand is of relevance to cavitation reduction and requires accurate estimations. The main contribution of this study is to establish an embedded multi-gene genetic programming (EMGGP) model for improved prediction of air demand. It is an MGGP-based framework coupled with the gene expression programming acting as a pre-processing technique for input determination and the Pareto front serving as a post-processing measure for solution optimization. Experimental data from a spillway aerator are used to develop and validate the proposed technique. Its performance is statistically evaluated by the coefficient of determination (CD), Nash–Sutcliffe coefficient (NSC), root-mean-square error (RMSE) and mean absolute error (MAE). Satisfactory predictions are yielded with CD = 0.95, NSC = 0.94, RMSE = 0.17 m3/s and MAE = 0.12 m3/s. Compared with the best empirical formula, the EMGGP approach enhances the fitness (CD and NSC) by 23% and reduces the errors (RMSE and MAE) by 48%. It also exhibits higher prediction accuracy and a simpler expressional form than the genetic programming solution. This study provides a procedure for the establishment of parameter relationships for similar hydraulic issues. 20. Sediment and morphological changes along Yangtze River’s 500 km between Datong and Xuliujing before and after Three Gorges Dam commissioning

Abstract: Abstract The impoundment of the Three Gorges Dam on the Yangtze River begins in 2003 and a full pool level is first attained in 2010. This process leads to reciprocal adjustments in flow discharge, sediment transport and morphology downstream of the dam. Based on 26-year recorded hydrologic data 1990–2015 and surveyed bathymetries 1998, 2010 and 2015, this study elucidates, before and after the commissioning of the dam, the alterations along the 500-km reach of the river. Two-dimensional numerical simulations are performed to predict future morphological changes by 2025. The analyses demonstrate that the impoundment modulates the seasonal flow discharges and traps an appreciable amount of sediment, resulting in enhanced erosion potential and coarsening of sediment. On a multi-year basis, the maximum discharge varies by a factor of 1.3 and the corresponding suspended load concentration and transport rate differ by a factor of 3.0 and 3.8, respectively. Combinations of surveyed and simulated bathymetries reveal its morphological responses to the changes. A general pattern of erosion is observed along the reach. In its upper 120 km, the process slows down towards 2025. In the middle 200 km, the erosion shifts, following the gradual impounding, to slight deposition, which then shifts back to erosion around September 2018. In the final 180 km, erosion continues without any sign of de-escalation, which is presumedly ascribed to tidal actions. The reach has not yet achieved a hydro-morphological equilibrium; the riverbed down-cutting is supposed to continue for a while. The combination of the field and numerical investigations provides, with the elapse of time, insight into the morpho-dynamics in the 500 km river reach.

Abstract: A pooled fishway is an artificial structure to improve the river flow connectivity and facilitate fish migration. Its conventional layout is characterized by straight weirs and suffers from limited passage rate. To improve its flow behaviors, an alternative design, featuring a V-shaped weir arrangement, is devised. CFD simulations are performed to examine its effects on the flow conditions. The governing geometrical parameters include weir angling, height, and spacing. Depending on weir arm angle, the flow is directed towards either the channel’s central part or the sidewalls, leading to both across- and along-channel differences in flow velocity and turbulent kinetic energy. This offers conducive zones for multiple fish species with distinct swimming preferences. For a given location in a pool, the cross-sectional velocity exhibits a cosine-type variation; a relationship is established to predict the maximum longitudinal velocity. The pressure distribution exhibits a similar pattern, with high-pressure areas residing upstream of the weir. A power law describes the relationship between the energy dissipation rate and unit discharge with an exponent at approximately 2/3. With an increase in either weir spacing or height, the energy dissipation rate declines. A change in weir angle shows however a negligible influence on it.

Abstract: Flexible materials embedded with hard magnetic particles have recently gained widespread recognition as small-scale actuators due to their capacity to be a rapid and precise shape-shifting material. Strontium ferrite (SrFe12O19) particles have been shown as a great candidate for such applications, since it is an inert hard magnetic material that, in contrast to barium ferrite and neodymium, is also biocompatible. The preparation of such material is done by mixing the magnetic particles into the uncured elastomer (polydimethylsiloxane (PDMS)), in liquid form, and then pouring the mixture in a mold for curing. If the samples are subjected to a magnetic field during the curing process, chains of particles are formed in the direction of the applied field, thus creating an easy axis in this same direction. The magnetic properties of such composite cannot yet be found in literature. In this study, we analyzed three concentrations of strontium ferrite particles in PDMS under three field configurations, resulting in 9 different samples. The concentrations used were 1:1, 2:1, and 4:1 ratios of PDMS to strontium ferrite per weight. All three types of samples were cured either in a zero magnetic field, or over the north pole of a neodymium permanent magnet, or over the side of said magnet. A biaxial vibrating sample magnetometer (VSM) was used to measure hysteresis curves parallel and perpendicular to the curing field. The samples cured in a field show a squareness ratio of up to 0.94 while the samples cured in zero field, only close to 0.5. The samples cured in a field show a magnetic anisotropy with an easy axis parallel to the curing field. Harvesting these modified properties, a mobile robot manufacturing method is proposed that bypasses the need of applying a high intensity magnetic field.

23. Improved heat coefficients for joint-space metabolic energy expenditure model during level, uphill, and downhill walking

Abstract: A previously developed joint-space metabolic energy expenditure (MEE) model includes subject-specific parameters and was validated using level walking gait data. In this work, we determine how well this joint-space model performs during various walking grades (-8%, 0%, and 8%) at 0.8 m·s ⁻1 and 1.3 m·s ⁻1 using published gait data in the literature. In response to those results, we formulate an optimization problem and solve it through the particle swam method plus fmincon function in MATLAB to identify a new optimal weighting parameter set for each grade that produces more accurate predicted MEE and we compare our new findings with seven other MEE models in the literature. The current study matched the measured MEE the best with the lowest RMSE values for level (0.45 J·kg ⁻1·m ⁻1) and downhill (0.82 J·kg ⁻1·m ⁻1) walking and the third lowest RMSE value for uphill (1.56 J·kg ⁻1·m ⁻1) walking, where another MEE model, Looney et al., had the lowest RMSE for uphill (1.27 J·kg ⁻1·m ⁻1) walking. Bland-Altman plots and three independent-samples t-tests show that there was no statistical significant difference between experimentally measured MEE and estimated MEE during the three walking conditions, meaning that the three new optimal weighting parameter sets can be used with 6 degree of freedom (DOF) lower extremity motion data to better estimate whole body MEE in those scenarios. We believe that this work is a step towards identifying a single robust parameter set that allows for accurate estimation of MEE during any task, with the potential to mitigate a limitation of indirect calorimetry requiring lengthy steady state motion.

Abstract: The collection of expression quantitative trait loci (eQTLs) is an important resource to study complex traits through understanding where and how transcriptional regulations are controlled by genetic variations in the non-coding regions of the genome. Previous studies have focused on associating eQTLs with traits to identify the roles of trait-related eQTLs and their corresponding target genes involved in trait determination. Since most genes function as a part of pathways in a systematic manner, it is crucial to explore the pathways’ involvements in complex traits to test potentially novel hypotheses and to reveal underlying mechanisms of disease pathogenesis. In this study, we expanded and applied loci2path software to perform large-scale eQTLs enrichment [i.e., eQTLs’ target genes (eGenes) enrichment] analysis at pathway level to identify the tissue-specific enriched pathways within trait-related genomic intervals. By utilizing 13,791,909 eQTLs cataloged in the Genotype-Tissue Expression (GTEx) V8 data for 49 tissue types, 2,893 pathway sets reported from MSigDB, and query regions derived from the Phenotype-Genotype Integrator (PheGenI) catalog, we identified intriguing biological pathways that are likely to be involved in ten traits [Alzheimer’s disease (AD), body mass index, Parkinson’s disease (PD), schizophrenia, amyotrophic lateral sclerosis, non-small cell lung cancer (NSCLC), stroke, blood pressure, autism spectrum disorder, and myocardial infarction]. Furthermore, we extracted the most significant pathways for AD, such as BioCarta D4-GDI pathway and WikiPathways sulfation biotransformation reaction and viral acute myocarditis pathways, to study specific genes within pathways. Our data presented new hypotheses in AD pathogenesis supported by previous studies, like the increased level of caspase-3 in the amygdala that cleaves GDP dissociation inhibitor and binds to beta-amyloid, leading to increased apoptosis and neuronal loss. Our findings also revealed potential pathogenesis mechanisms for PD, schizophrenia, NSCLC, blood pressure, autism spectrum disorder, and myocardial infarction, which were consistent with past studies. Our results indicated that loci2path′s eQTLs enrichment test was valuable in unveiling novel biological mechanisms of complex traits. The discovered mechanisms of disease pathogenesis and traits require further in-depth analysis and experimental validation.

Abstract: Does delayed retirement crowd out the welfare of the workforce? To answer this question, a dynamic optimization framework is established to simulate the impact of delayed retirement on the welfare of the working population over time. Simulations are conducted based on practical and feasible parameters. Delayed retirement was found to improve the welfare of the working population rather than crowding it out. Furthermore, the results are robust against changes in parameters and modes of supporting elderly individuals. In terms of policymaking, it is suggested that such facts be shared with the public and that a delayed retirement plan be introduced as soon as possible to manage the pension and retirement wave caused by post-1960s baby boomers. However, to ensure that the delayed retirement plan does not lead to a reduction in the welfare of the working population, increases in fertility costs and the pension replacement rate should be appropriately controlled.

Abstract: Background: Independent risk factors associated with hepatitis B (HBV)-related hepatocellular carcinoma (HCC) after resection remains unknown. An accurate risk score for HCC recurrence is lacking. Methods: We prospectively followed up 200 patients who underwent liver resection for HBV-related HCC for at least 2 years. Demographic, biochemical, tumor, virological and anti-viral treatment factors were analyzed to identify independent risk factors associated with recurrence after resection and a risk score for HCC recurrence formulated. Results: Two hundred patients (80% male) who underwent liver resection for HBV-related HCC were recruited. The median time of recurrence was 184 weeks (IQR 52–207 weeks) for the entire cohort and 100 patients (50%) developed HCC recurrence. Stepwise Cox regression analysis identified that one-month post resection HBV DNA >20,000 IU/mL (p = 0.019; relative risk (RR) 1.67; 95% confidence interval (C.I.): 1.09–2.57), the presence of lymphovascular permeation (p<0.001; RR 2.69; 95% C.I.: 1.75–4.12), microsatellite lesions (p<0.001; RR 2.86; 95% C.I.: 1.82–4.51), and AFP >100ng/mL before resection (p = 0.021; RR 1.63; 95% C.I.: 1.08–2.47) were independently associated with HCC recurrence. Antiviral treatment before resection (p = 0.024; RR 0.1; 95% C.I.: 0.01–0.74) was independently associated with reduced risk of HCC recurrence. A post-resection independent predictive score (PRIPS) was derived and validated with sensitivity of 75.3% and 60.6% and specificity of 55.7% and 79.2%, to predict the 1- and 3-year risks for the HCC recurrence respectively with the hazard ratio of 2.71 (95% C.I.: 2.12–3.48; p<0.001). The AUC for the 1- and 3-year prediction were 0.675 (95% C.I.: 0.6–0.78) and 0.746 (95% C.I.: 0.69–0.82) respectively. Conclusion: Several tumor, virological and biochemical factors were associated with a higher cumulative risk of HCC recurrence after resection. PRIPS was derived for more accurate risk assessment. 28. Air-vent layouts and water-air flow behaviors of a wide spillway aerator

Abstract: A spillway aerator should guarantee favorable flow conditions in the coupled water-air system even if the aerator is unconventionally wide. Eight air-vent configurations are devised and incorporated into a 35-m wide chute aerator for a generalized study. Computational fluid dynamics (CFD) simulations are performed to explore their effects on water-jet and air-cavity features. The Re-normalisation group (RNG) k – ɛ turbulence model and the two-fluid model are combined to predict the two-phase flow field. The results demonstrate appreciable influences of the vent layouts on the water-air flow. The air vents stir the air motion and re-distribute the cavity air pressure. Once the vent layout is modified, reciprocal adjustments exist between the jet behavior and air-pressure field in the cavity, thus leading to considerable differences in air-flow rate, jet-trajectory length, vent air-flow distribution across the chute, etc. The large width plays a discernable role in affecting the aerated flow. Telling differences exist between the near-wall region and the central part of the chute. To improve the duct pressure propagation, a gradual augment of the vent area should be assigned towards the chute center. Relative to single-slot vents across the flow, the layouts with segregated vents gain by comparison. Abstract: Mathematical formulations of two-phase flows at an aerator remain a challenging issue for spillway design. Due to their complexities in terms of water–air interactions subjected to high flow velocities, experiments play an essential role in evaluations of numerical models. The paper focuses on the underlying influence of the air–water momentum exchange in the two-phase Two-Fluid Model. It is modified to better represent the drag force acting on a group of air bubbles and the wall lubrication force accounting for near-wall phase interactions. Based on data from a large aerator rig with an approach velocity of 14.3 m/s, the models are evaluated for calculations of entrained air characteristics of a flow mixture. The air bubble diameter used in the modeling ranges from 0.5 to 4 mm as suggested by the experiments. In terms of air cavity configurations and aerator air demand, smaller air bubbles lead to better agreement with the test results. As far as air concentrations are concerned, the modified model gains by comparison. In the air cavity zone, smaller bubble sizes also provide better matches with the experiments. However, the near-base air concentration remains overestimated downstream from the impact area. The fact that the program user must pre-define a single air bubble size in simulations presumably limits the correct reproduction of near-base air concentrations and of their decay.

Abstract: At the multigate sluice structure on a fluvial river, undesired sediment deposition affects the normal operation of the reservoir in question. Physical and numerical models are hybridized to help explore flow and sedimentation patterns. Field and laboratory investigations show that the deposition is attributable to the formation of large recirculation zones at low and medium discharges. As a potential countermeasure, an array of guide vanes is recommended to cope with the concern. Their attack angle with the flow is a dominant parameter that needs to be evaluated. Tests in the fixed-bed model demonstrate that the vanes bend the reservoir flow towards the sluice and suppress the circulation zones along both banks. The favorable range of attack angle is 15–20°. With the examination of sedimentation of both bed and suspended loads, the numerical modeling indicates that the sediment-removal efficiency increases with an increase in attack angle. By weighing the flushing efficiency and the risk of local scouring at the vanes, the 15° vane layout is recommended. This study is expected to provide a reference for guide-vane design in similar situations.

Abstract: In order to cope with population aging and emerging labor shortages, the Chinese government may soon introduce a policy to address this problem by raising the normal retirement age. However, the effects of planning for delayed retirement on the welfare of the elderly remain unknown. From an intergenerational support perspective, we develop a dynamic optimization model that can simulate changes in the welfare of the elderly over the years under different delayed retirement scenarios. Simulation results show that delaying retirement will produce a detrimental effect on old-age welfare. We further analyze strategies to mitigate this adverse effect and improve people’s welfare. First, the delayed retirement policy should raise the pension replacement rate, which could transfer part of the social welfare improvements from delayed retirement to the elderly through the transfer payment mechanism. Second, when adopting a defined-contribution pension system, the delayed retirement could increase the welfare of the elderly and the social well-being. Implications for future research and public policies concerning welfare effects of delayed retirement are discussed.

Abstract: Introduction Surgical site infection (SSI) is one of the most common healthcare-associated infections (HAIs). However, there is a lack of data available about SSI in children worldwide, especially from low-income and middle-income countries. This study aimed to estimate the incidence of SSI in children and associations between SSI and morbidity across human development settings.Methods A multicentre, international, prospective, validated cohort study of children aged under 16 years undergoing clean-contaminated, contaminated or dirty gastrointestinal surgery. Any hospital in the world providing paediatric surgery was eligible to contribute data between January and July 2016. The primary outcome was the incidence of SSI by 30 days. Relationships between explanatory variables and SSI were examined using multilevel logistic regression. Countries were stratified into high development, middle development and low development groups using the United Nations Human Development Index (HDI).Results Of 1159 children across 181 hospitals in 51 countries, 523 (45·1%) children were from high HDI, 397 (34·2%) from middle HDI and 239 (20·6%) from low HDI countries. The 30-day SSI rate was 6.3% (33/523) in high HDI, 12·8% (51/397) in middle HDI and 24·7% (59/239) in low HDI countries. SSI was associated with higher incidence of 30-day mortality, intervention, organ-space infection and other HAIs, with the highest rates seen in low HDI countries. Median length of stay in patients who had an SSI was longer (7.0 days), compared with 3.0 days in patients who did not have an SSI. Use of laparoscopy was associated with significantly lower SSI rates, even after accounting for HDI.Conclusion The odds of SSI in children is nearly four times greater in low HDI compared with high HDI countries. Policies to reduce SSI should be prioritised as part of the wider global agenda.

Abstract: This study presents experimental data on the hydrodynamic features of a nonlinear solitary wave of height H0 = 2.9 cm propagating over a horizontal bed in otherwise still water depth h0 = 8.0 cm. High-speed particle image velocimetry is used to quantify the local and convective particle accelerations and pressure gradients under the solitary wave. It is found that the magnitudes of the horizontal and vertical particle local accelerations are invariably larger than their convective counterparts, highlighting the important contribution of local acceleration to the pressure gradient in a solitary wave. The dimensionless pressure gradient in the horizontal direction is favorable, zero, and adverse for time before, at, and after the wave crest passes the section of interest. The dimensionless adverse pressure gradient exhibits a maximum for the dimensionless time T[=t(g/h0)1/2, where g is the gravitational acceleration and t is the time with respect to the crest passing the section] = 1.39, at which the dimensionless local acceleration has a negative maximum. Subsequently, flow reversal takes place above the bed surface. The relationship is elucidated between flow reversal at the bed surface and the evolution of the adverse pressure gradient in the near-bed zone, where uniform horizontal/free stream velocity exists.

Abstract: Recent collapses of SIP servers (e.g., Skype outage) indicate that the built-in SIP overload control mechanism cannot mitigate overload effectively. We introduce our analytical approach by investigating an overloaded tandem server scenario. Our analytical model: (1) considers a general case that both arrival rate and service rate for signaling messages are generic random processes; (2) makes a detailed analysis of departure processes; (3) allows us to run fluid-based simulations to observe and analyze SIP system performance under some specific scenarios. This approach is much faster than event-driven simulation which needs to track thousands of retransmission timers for outstanding messages and may crash a simulator due to limited computing resources. Our numerical results help us reach a counterintuitive conclusion: A SIP system with a large buffer size may continuously exhibit overload and long queuing delay after experiencing a short period of demand burst or a temporary server slowdown. Small buffer size, on the other hand, can mitigate overload quickly by rejecting a large portion of the requests from a demand burst, and then resume normal operation after a short period of time. Furthermore, numerical results demonstrate that overload at a downstream server may propagate or migrate to its upstream servers and therefore cause widespread server crashes in a real SIP network. Abstract: Box delivery is a complicated manual material handling task which needs to consider the box weight, delivering speed, stability, and location. This paper presents a subtask-based inverse dynamic optimization formulation for determining the two-dimensional (2D) symmetric optimal box delivery motion. For the subtask-based formulation, the delivery task is divided into five subtasks: lifting, the first transition step, carrying, the second transition step, and unloading. To render a complete delivering task, each subtask is formulated as a separate optimization problem with appropriate boundary conditions. For carrying and lifting subtasks, the cost function is the sum of joint torque squared. In contrast, for transition subtasks, the cost function is the combination of joint discomfort and joint torque squared. Joint angle profiles are validated through experimental results using Pearson’s correlation coefficient (r) and root-mean-square-error (RMSE). Results show that the subtask-based approach is computationally efficient for complex box delivery motion simulation. This research outcome provides a practical guidance to prevent injury risks in joint torque space for workers who deliver heavy objects in their daily jobs.

Abstract: Air entrainment at the intake of a bottom outlet often gives rise to air pockets in its conduit and formation of geysers. The outlet in question comprises a bulkhead gate, gate shaft, horizontal conduit, and exit. Operations show that it suffers from appreciable flow fluctuations and blowouts in the tailwater, which leads to gate operation restrictions. For the purpose of understanding the hydraulic phenomenon, both prototype discharge tests and three-dimensional computational fluid dynamics (CFD) modeling of two-phase flows are performed. The operational focus of the facility are small and large gate openings. The CFD results reveal that, with air entrained in the gate shaft, continual breakup and coalescence of air bubbles in the conduit typify the flow. At small openings below 1 meter, the air–water flow is characterized by either distinct blowouts of regular frequency or continuous air release. In terms of geyser behaviors inclusive of frequency, the agreement is good between field and numerical studies. At large openings, the gate becomes fully submerged, and the flow is discharged without air entrainment and blowouts. The paper showcases the air–water flow features in a typical bottom outlet layout in Sweden, which is intended to serve as an illustration of the study procedure for other similar outlets.

Abstract: Owing to its effective energy dissipation and aeration, a stepped spillway is commonly used for flood release in hydraulic projects. Its conventional design features horizontal step surfaces. Designed for certain flow rates, it does not function satisfactorily at larger discharges. To improve this, layouts with inclined step surfaces, both downward and upward, are proposed. Computational fluid dynamics (CFD) modelling in 3D is performed to examine and compare their flow properties in the skimming flow. The results suggest that a shift from a downward to an upward layout leads to a gradual decrease in the flow velocity at the chute end; the latter exhibit higher energy dissipation efficiency. Moreover, equations are developed to estimate the velocity and energy loss. The flow velocity in the developing zone, described by a power law, shows a decline with an increase in the angle of inclination. The downward layout is subjected to somewhat higher risk of cavitation if implemented in a prototype. The extreme pressure loads acting upon an upward layout are larger, and a correlation is proposed for its prediction. On an inclined surface, either upward or downward, the pressure demonstrates an S-shaped distribution. On a vertical surface, the flow pressure increases, after an initial decline over a short distance, towards the chute bottom.

42. Transient Air-Water Flow and Air Demand following an Opening Outlet Gate

Abstract: In Sweden, the dam-safety guidelines call for an overhaul of many existing bottom outlets. During the opening of an outlet gate, understanding the transient air-water flow is essential for its safe operation, especially under submerged tailwater conditions. Three-dimensional CFD simulations are undertaken to examine air-water flow behaviors at both free and submerged outflows. The gate, hoisted by wire ropes and powered by AC, opens at a constant speed. A mesh is adapted to follow the gate movement. At the free outflow, the CFD simulations and model tests agree well in terms of outlet discharge capacity. Larger air vents lead to more air supply; the increment becomes, however, limited if the vent area is larger than 10 m2. At the submerged outflow, a hydraulic jump builds up in the conduit when the gate reaches approximately 45% of its full opening. The discharge is affected by the tailwater and slightly by the flow with the hydraulic jump. The flow features strong turbulent mixing of air and water, with build-up and break-up of air pockets and collisions of defragmented water bodies. The air demand rate is several times as much as required by steady-state hydraulic jump with free surface.

Abstract: The empirical study presented in this chapter has implications for performance studies, philosophy of music, music aesthetics and music perception. The impetus for the experiment was pedagogical and originated in the piano studio. The working hypothesis was that in the teaching of classical technique there can be negative consequences for students (physical tension, self-consciousness) when instructors give too much detailed verbal information about how specific body parts should function when performing this phrase or that. Our research poses the questions: Are there distinctive and measurable changes in pianists’ body use and gestures when they are invited simply to “enjoy” playing a piece? Are such distinctive qualities perceived by others, and if so, in what way? The protocol for our study comprised four advanced-level classical pianists, all given the same two (contrasting) pieces to learn and memorize on their own. In the motion-capture laboratory, they performed them in two ways: (1) “as correctly as you can” and (2) “just enjoying yourself.” The resulting three-dimensional, point-light recordings of piano-playing “avatars” were subsequently viewed by two groups — trained musicians and non-musicians — from inside an fMRI scanner. The neurological activity of the subjects in the scanner was monitored while they viewed the contrasting videos and answered a series of questions about each. These questions dealt with their perceptions of the congruence of music and gesture, what the gestures themselves conveyed, and how the subjects responded to observing the distillation of expressive human movement that is presented by a performing “avatar.”

Abstract: Meandering is a common feature in natural alluvial streams. This study deals with alluvial behaviors of a meander reach subjected to both fresh-water flow and strong tides from the coast. Field measurements are carried out to obtain flow and sediment data. Approximately 95% of the sediment in the river is suspended load of silt and clay. The results indicate that, due to the tidal currents, the flow velocity and sediment concentration are always out of phase with each other. The cross-sectional asymmetry and bi-directional flow result in higher sediment concentration along inner banks than along outer banks of the main stream. For a given location, the near-bed concentration is 2−5 times the surface value. Based on Froude number, a sediment carrying capacity formula is derived for the flood and ebb tides. The tidal flow stirs the sediment and modifies its concentration and transport. A 3D hydrodynamic model of flow and suspended sediment transport is established to compute the flow patterns and morphology changes. Cross-sectional currents, bed shear stress and erosion-deposition patterns are discussed. The flow in cross-section exhibits significant stratification and even an opposite flow direction during the tidal rise and fall; the vertical velocity profile deviates from the logarithmic distribution. During the flow reversal between flood and ebb tides, sediment deposits, which is affected by slack-water durations. The bed deformation is dependent on the meander asymmetry and the interaction between the fresh water flow and tides. The flood tides are attributable to the deposition, while the ebb tides, together with run-offs, lead to slight erosion. The flood tides play a key role in the morphodynamic changes of the meander reach.

45. The Past and Present of Discharge Capacity Modeling for Spillways—A Swedish Perspective

Abstract: Most of the hydropower dams in Sweden were built before 1980. The present dam-safety guidelines have resulted in higher design floods than their spillway discharge capacity and the need for structural upgrades. This has led to renewed laboratory model tests. For some dams, even computational fluid dynamics (CFD) simulations are performed. This provides the possibility to compare the spillway discharge data between the model tests performed a few decades apart. The paper presents the hydropower development, the needs for the ongoing dam rehabilitations and the history of physical hydraulic modeling in Sweden. More than 20 spillways, both surface and bottom types, are analyzed to evaluate their discharge modeling accuracy. The past and present model tests are compared with each other and with the CFD results if available. Discrepancies do exist in the discharges between the model tests made a few decades apart. The differences fall within the range −8.3%–+11.2%. The reasons for the discrepancies are sought from several aspects. The primary source of the errors is seemingly the model construction quality and flow measurement method. The machine milling technique and 3D printing reduce the source of construction errors and improve the model quality. Results of the CFD simulations differ, at the maximum, by 3.8% from the physical tests. They are conducted without knowledge of the physical model results in advance. Following the best practice guidelines, CFD should generate results of decent accuracy for discharge prediction.

Abstract: The chute aerator of a spillway is a structure in such a sense that air is, in the intense emulsification, entrained into the high-velocity water flow. Correctly predicting the air entrainment and two-phase flow pattern at the aerator would contribute to reliable spillway operation. Based on experimental data, 2D numerical simulations are preformed to predict streamwise air concentrations in the aerated flow, in which a two-fluid model is used. Depending on the air bubble size, relatively good agreement is seen with the experiments in the air cavity zone. The simulations give rise to higher air concentration downstream of the cavity, which is presumably due to underestimation of the interfacial forces in the two-fluid model.

47. Prospective Study on Sexual Dysfunction in Male Chinese Liver Transplant Recipients.

Abstract: In patients with end-stage liver disease, hypogonadism and erectile dysfunction are often seen. This study was to determine the incidence of erectile dysfunction before and after liver transplantation (LT) with correlation to change in sex hormone levels from a Chinese cohort. This prospective longitudinal study was registered with The University of Hong Kong Clinical Trials Centre (HKUCTR-1563). The Institutional Review Board approval number is UW-12-273. The study period was from January 2012 to December 2016. Adult male patients with end-stage liver disease enlisted for LT were recruited on informed written consent. All recruited patients were to complete a cross-sectional cohort questionnaire—International Index of Erectile Function, version 5 (IIEF5)—and to receive serum sex hormone checks before and after LT. Twenty-eight patients who underwent LT were included in the analysis. The included patients had significantly reduced prolactin (p <.001) and 17-beta-estradiol (p =.024) after LT. There was also a significant drop of IIEF5 score at 1 month after LT, but the score returned to pre-LT level at 6 months. This study demonstrated that there was improvement in sex hormone levels after LT, namely, normalization of estradiol level and lowering of prolactin and progesterone levels. 48. Effects of Gender and Recurrent Low Back Pain on Lifting Style

Abstract: Objective: The purpose of this study was to examine the influence of gender and existing, recurrent low back pain (rLBP) on lower extremity and trunk mechanics, as well as neuromuscular control, during a lift task. Design: A multivariate design was used to examine the effects of gender and group on biomechanical and neuromuscular control variables in randomized symmetric and asymmetric lifting. Methods: 68 Males and females with rLBP and healthy performed symmetric and asymmetric weighted box lifting trials to a 1 meter height table. Results: Lifting style was different between gender and between the rLBP versus healthy groups during a 1m box lifting. A significant two-way interaction effect between gender and group was observed for multifidus muscle activity and knee rotation in asymmetric lifting. Several gender and group main effects were observed in pelvis obliquity, trunk flexion and side flexion, knee abduction angles in symmetric lifting; and in pelvis obliquity and rotation, trunk flexion and side flexion, hip abduction, knee abduction angles, external oblique and internal oblique muscles activity in asymmetric lifting. Conclusions: Females and individuals with rLBP appear to use different lifting styles that emphasize movement at the pelvis accompanied by poor kinematic control features at the hip, trunk and knee. Clinicians should be mindful of these changes when developing prevention and rehabilitation programs aimed at improving trunk control in preparation for lifting tasks during domestic and occupational activities.

Abstract: This report describes a novel way to generate a highly effective hydrophobic cement surface via a carbonation route using sodium stearate. Carbonation reaction was carried out at different temperatures to investigate the hydrophobicity and morphology of the calcium carbonate formed with this process. With increasing temperatures, the particles changed from irregular shapes to more uniform rod-like structures and then aggregated to form a plate-like formation. The contact angle against water was found to increase with increasing temperature; after 90 °C there was no further increase. The maximum contact angle of 129° was obtained at the temperature of 60 °C. It was also found that carbonation increased the micro hardness of the cement material. The micro hardness was found to be dependent on the morphology of the CaCO3 particles. The rod like structures which caused increased mineral filler produced a material with enhanced strength. The 13C cross polarization magic-angle spinning NMR spectra gave plausible explanation of the interaction of organic-inorganic moieties.
