2. Morphometric analysis of the fascicular organisation of the optic nerve.

Background/Aim. The optic nerve is anatomically observed in four segments: intrabulbar, orbital, canalicular, and cranial. According to the literature, the surface of the transversal cut of the nerve is different through it. The aim of this study was to evaluate the fascicular organisation of the optic nerve, throughout its three segments from the eye. Methods. Five pairs of optic nerves, obtained from the autopsies were examined. Using Heidenhain's (azan) staining, the cuts were prepared for microscopy. Morphometric analysis was performed using the stereological methods for morphometric cytology -- the Weible's testing system M42. The following measures were established: the surface of the transverse cut of the nerve, the entire surface of fasciculi, the entire surface of connective tissue and blood vessels, the number of fasciculi, the surface of a single fasciculus. Results. The surface of the transverse cut of the nerve was found to grow from the orbital to the cranial segment, as well as the entire surface of fasciculi. While their number is significantly lower in the cranial segment, the number of fasciculi varied slightly between the orbital and the canalicular segment. The surface of a single fasciculus grows from the bulb to the chiasma. There is probable a cause to believe that this may be due to fusion of the "small" fasciculi in the orbitocranial direction. Abstract: To discover a gastric antisecretory agent more potent than existing proton pump inhibitors, novel pyrrole derivatives were synthesized, and their H+,K+-ATPase inhibitory activities and inhibitory action on histamine-stimulated gastric acid secretion in rats were evaluated. Among the compounds synthesized, compound 17a exhibited selective and potent H+,K+-ATPase inhibitory activity through reversible and K+-competitive ionic binding; furthermore, compound 17c exhibited potent inhibitory action on histamine-stimulated gastric acid secretion in rats and Heidenhain pouch dogs. Abstract: Helicobacter pylori is a bacterium associated with upper gastrointestinal diseases in humans. However, only a small proportion of infected people become sick. Although several studies have tried to establish an association between known virulence markers and clinical outcomes, in many cases the results have been conflicting. The aim of this study was to investigate the importance of virulence markers to predict clinical outcome in Brazil. Mixed infections by genetically unrelated strains detected by vacA genotyping were found in 18% of the patients. The cagA and cagE genes and the vacAs1 genotype were associated with the development of peptic ulcer disease (PUD). The cagAvacAs1m1 genotype was associated with PUD and duodenal ulcer (DU). Conversely, jhp947 was not associated with DU or PUD, indicating that this gene is not a universal virulence marker. These results also show that a high proportion of the patients were simultaneously infected by cag-positive and cag-negative H. pylori types. This finding suggests the existence of a dynamic equilibrium between the loss and gain of the cag pathogenicity island, probably depending on the physiologic conditions of the patient''s stomach. In three dogs with denervated Heidenhain pouches, ingestion of cooked beef liver caused increased acid output and gastrin secretion. After antrectomy and gastrojejunal anastomosis, ingestion of the liver did not increase serum gastrin levels throughout the experiment. Acid output, however, although markedly reduced, was elevated over basal levels during the last three hours of the four-hour experiment. Total abdominal vagotomy did not alter gastrin secretion but further inhibited acid output in response to the test meal. These results indicate that in the dog intestinal gastrin is not released in response to a beef liver meal. Acid output postantrectomy, although reduced, does respond to the meal and is further decreased after vagotomy. A hormonal mechanism other than the release of intestinal gastrin is responsible for the intestinal phase of gastric acid secretion. [ABSTRACT FROM AUTHOR]

The Heidenhain variant of Creutzfeldt-Jakob disease (CJD) is a rapidly progressive neurodegenerative disease with an occipito-parietal focus.It initially presents with visual disturbances such as visual agnosia or cortical blindness ( Kropp et al., 1999 ).After presentation of early visual symptoms, rapid progression of dementia and death follows.While the Heidenhain variant matches neuropathological criteria of CJD, differential diagnosis from other neurological disorders using biomarkers from electroencephalography (EEG), magnetic resonance imaging (MRI), and cerebrospinal fluid (CSF) reveal low sensitivity and specificity.As a consequence, these tools may lead to an erroneous diagnosis of the Heidenhain variant. We report about a 81-year old female who was transferred to the intensive care unit due to a generalized tonic-clonic seizure followed by prolonged somnolence, additionally exhibiting myoclonic jerks of the distal legs, left horizontal gaze palsy and horizontal nystagmus to the right.A history of cognitive decline including short-term memory and word-finding difficulties as well as visuospatial and visuoconstructive deficits over the last few weeks was reported. Initial EEG revealed 1.5 Hz left lateralized periodic sharp-wave and polyspike-wave complexes of high amplitude and triphasic morphology suspicious for CJD. Brain MRI showed a hyperintense lesion in the left occipito-parietal lobe on diffusion-weighted and FLAIR sequences, respectively.Protein 14-3-3 was positive in CSF (ELISA). However, RT-QuIC confirmatory test was negative.Other serum and CSF parameters including anti-neuronal antibodies were negative.Given this information, is it reasonable to consider the Heidenhain variant of CJD? The patient suffered from a cardioembolic stroke due to atrial fibrillation affecting the parietal branches of the middle cerebral artery.Edoxaban had been removed three weeks previously because of repeated falls.During ICU treatment, the patient developed a prolonged nonconvulsive status epilepticus (NCSE) characterized by deep coma without significant motor phenomena.EEG was refractory to an initial antiepileptic drug treatment, but finally returned to normal with valproate monotherapy.The patient, however, remained comatose until transfer to rehabilitation, but she regained consciousness two weeks later. This case points out that a parietal stroke with complicating NCSE may mimic the Heidenhain variant of CJD, particularly by under-recognition of so-called CJD red flags: First, the occurrence of 14-3-3 proteins is known to be positive after either acute stroke or seizures.Secondly, though increased intensity on DWI in CJD is discussed as a noninvasive biomarker, such signal abnormalities – particularly if sparing the basal ganglia – are not specific to CJD. Thirdly, NCSE patients with altered consciousness often present with localized or lateralized sharp waves of triphasic configuration.These atypical triphasic waves are considered to be of epileptic origin if they disappear following antiepileptic drug treatment as in our case ( Trinka and Leitinger, 2015 ). Abstract: Heidenhain variant of Creutzfeldt–Jakob disease (CJD) is a rare progressive neurodegenerative disorder, characterized by inaugural visual symptoms isolated for some weeks. We report a new case of Heidenhaim variant and present MRI evolution at different stages of disease. Despite the fact that cerebral MRI is helpful for diagnosis particularly with the sensitivity and specificity of fluid-attenuated inversion recovery (FLAIR), diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC) mapping, this imaging must be negative at an early stage. The effect of compound 48/80 on the mucosal barrier of the canine Heidenhain pouch and mast cell content of gastric mucosa has been studied. 48/80 was administered in increasing doses (0.5--2.0 mg/kg) over 5 days. 0.5 mg/kg produced signs of systemic histamine release and stimulation of acid secretion by the pouch; these effects being most marked on the 1st day and absent on the 5th. Subsequent exposure of the mucosa to sodium taurocholate produced damage to the barrier similar to that in untreated animals. Histological examination revealed no evidence of mucosal mast cell degranulation by 48/80. The results suggest that the effects of 48/80 on gastric mucosa are mediated by systemic release of histamine and that local mast cells are resistant to its action.

The effects of long term treatment with cortisone on the gastric secretion induced by histamine, pentagastrin, porcine gastrin and a meal have been investigated in four dogs with both gastric fistula and Heidenhain pouch. Cortisone increased the postcibal acid output and the observed maximal acid response from the pouch to all three exogenous stimuli. The ED 50′s remained unchanged. The same effects although less marked were observed in the innervated stomach. These data indicate that the increased acid secretion observed after long term treatment with cortisone is largely due to an increased secretory capacity of the gastric mucosa. This latter could result from an increase in the number of secretory units or to partial removal of a non competitive inhibitor of gastric secretion. The inhibition of pepsin secretion by metiamide and atropine has been studied in the gastric fistula rat and the Heidenhain pouch rat. A comparison of the effectiveness of metiamide and atropine in inhibiting pepsin secretion was made by using doses of the antagonists which produced a similar level of inhibition of acid secretion. In the gastric fistula rat both atropine and metiamide inhibited the basal pepsin output, but atropine was more effective than metiamide in this respect. In the Heidenhain pouch rat a large dose of metiamide which inhibited bethanechol-stimulated acid secretion had no significant effect on the corresponding output of pepsin. In this preparation atropine inhibited both acid and pepsin secretion. Possible reasons for these differences in the two preparations are discussed.

Rapid intravenous injections of secretin and cholecystokinin (Vitrum) inhibited the acid response of Heidenhain pouch dogs to continuous intravenous gastrin extract. Cholecystokinin caused greater inhibition than secretin. Responses to small doses of histamine were inhibited by cholecystokinin but not by secretin.

SummaryBy serial sampling at 10- to 20- minute intervals, electrolytes and water secretion per minute were measured in dog Heidenhain pouch juice after stimulation with histamine. Thirty to 60 minutes after the beginning of stimulation, a highly significant correlation of K+ to H+ was observed. The regression coefficient of K+ on H+ ranged from 0.006 to 0.082 in 11 experiments. The early high K+ output after histamine is still under investigation.

17. Sodium secretion as part of the formation of gastric juice

Experiments with Heidenhain pouch dogs showed the well known high sodium concentrations at low secretion rates and low sodium concentrations at high ones after stimulation with a continuous intravenous injection of histamine. It was previously thought that the primary secretion did not contain any sodium, but that all the sodium present in the gastric juice appeared as a result of diffusion. A more detailed analysis of the experimental data showed, however, that there is in fact sodium present also in the primary juice in the small concentration of 3-5 mEq/l. The origin of the secreting volume containing this concentration of sodium is unknown.

SummaryA method has been devised for making a Heidenhain pouch and 2 separate, marsupialized, antral pouches in the dog. Studies performed on 4 dogs prepared in this way indicate that reduction of pH within one antral pouch will inhibit production of HCl in the Heidenhain pouch when the latter is under stimulation by perfusion of alcohol in the second antral pouch. It is assumed that gastric inhibition produced in this manner demonstrates release of an inhibitory hormone by the antrum; however, this awaits further study for clarification.

It is concluded that in accordance with numerous previous reports acid secretion in response to histamine is mediated by H2-receptors. The powerful effect of histamine on GMBF is at least in part mediated by H1-receptors. Since NDMH is much less effective than histamine in increasing GMBF and is not inhibited by mepyramine, this compound is more selective in stimulating histamine H2-receptors than histamine.

Several analyses of the gastric juice of isolated stomach pouches are presented before and after oral administration of RD 11, a detergent complex. These analyses show a trend of inhibition of the secretion of total available pepsin, and lysozyme, but not much change in the acidity of the secretion. The viscosity increases in the post-medicated samples, as does the amount of mucin per sample. A humoral or/and nervous inhibition mechanism therefore seems to be concerned here.
