Current clinical diagnostics are based on biochemical, immunological, or microbiological methods. However, these methods are operator dependent, time-consuming, expensive, and require special skills, and are therefore, not suitable for point-of-care testing. Recent developments in gas-sensing technology and pattern recognition methods make electronic nose technology an interesting alternative for medical point-of-care devices. An electronic nose has been used to detect urinary tract infection from 45 suspected cases that were sent for analysis in a U.K. Public Health Registry. These samples were analyzed by incubation in a volatile generation test tube system for 4-5 h. Two issues are being addressed, including the implementation of an advanced neural network, based on a modified expectation maximization scheme that incorporates a dynamic structure methodology and the concept of a fusion of multiple classifiers dedicated to specific feature parameters. This study has shown the potential for early detection of microbial contaminants in urine samples using electronic nose technology.

The potential neuroprotective action of the corticotrophin-releasing factor-related peptide urocortin (UCN) was investigated in the rat 6-hydroxydopamine (6-OHDA) and lipopolysaccharide (LPS) paradigms of Parkinson's disease. UCN (20 fmol) was either given at the same time as (T = 0) or 7 days after (T = +7) intracerebral 6-OHDA or LPS injection. At 14 days after 6-OHDA or LPS injection, circling behaviour was measured following apomorphine challenge. Circling was significantly lower in rats given UCN at either T = 0 or T = +7 compared with animals given 6-OHDA or LPS and vehicle. Sham-treated rats showed no circling. Consistent with these observations, striatal dopamine concentrations were markedly higher in 6-OHDA/LPS + UCN rats vs. 6-OHDA/LPS + vehicle groups. Additionally, L-dihydroxyphenylalanine production by tyrosine hydroxylase was greatly reduced in the striata of 6-OHDA/LPS + vehicle rats, whereas this was not the case in rats coadministered UCN. Finally, the numbers of tyrosine hydroxylase-positive cells recorded in the substantia nigra of 6-OHDA/LPS + vehicle-treated animals were markedly lower than those of sham-operated or 6-OHDA/LPS + UCN rats. Critically, UCN was effective in reversing lesion-induced deficits when given either at the same time as or 7 days after the neurotoxic insult. To our knowledge, this is the first time that such an effect has been demonstrated in vivo. The apparent ability of UCN to arrest the progression of or even reverse nigral lesions once established suggests that pharmacological manipulation of this system could have substantial therapeutic utility.

We have examined the effects of adrenalectomy (ADX) on neuropeptide Y (NPY) content in the posterior pituitary of the rat. In rats given 2% saline to drink for 12 days, plasma osmolality increased to 343 +/- 18 compared to 291 +/- 5 mosm/kg in control animals on water (mean +/- SEM). Hypertonic saline treatment resulted in increased posterior pituitary NPY content measured by radioimmunoassay (2.42 +/- 0.36 ng) compared to the control group (1.18 +/- 0.16 ng). Total NPY immunoreactivity in tissue extracts coeluted with synthetic NPY(1-36) on reversed-phase high-performance liquid chromatography. In ADX rats given 2% saline to drink, plasma osmolality increased to 355 +/- 35 mosm/kg, but posterior pituitary NPY content was not different from the control group (1.41 +/- 0.11 ng). Immunohistochemical staining for NPY in the posterior pituitary also revealed a saline-induced increase in the number of NPY-immunoreactive nerve fibers, an increase which was not evident in the ADX group. No NPY immunoreactivity was observed in the intermediate zone. These results are the first evidence that the saline-induced increase in NPY in the posterior pituitary is under corticosteroid control.

In situ hybridisation histochemistry and immunohistochemistry was used to study the response of the noradrenergic A6 and A2 cell groups in the rat following oral administration of 2% saline for 12 days. Messenger ribonucleic acid (mRNA) encoding tyrosine hydroxylase and neuropeptide Y increased by 85 and 65%, respectively (p less than 0.05) in A6, while levels in A2 were unaltered after saline ingestion. The observed changes in mRNA were accompanied by increased expression of neuropeptide Y and tyrosine hydroxylase immunoreactivities in A6 neurons, whereas A2 neurons were unaffected. In contrast, mRNA encoding galanin, which is also colocalised with noradrenaline in A6 cells, was unaltered by osmotic stimulation. Our results suggest that noradrenaline and neuropeptide Y transmitter systems in the A6 group are activated whereas the galanin transmitter system remains unaffected during saline ingestion.

We have investigated the central effects of substance P (SP) on plasma concentrations of immunoreactive ACTH and on immunoreactive and bioactive arginine vasopressin (AVP) in the rat. The injection of SP (20 nmol) into the lateral ventricle intracerebroventricular, (i.c.v.) of ethanol-anaesthetised rats produced a prolonged antidiuresis lasting at least 30 min, associated with an increase in plasma AVP (from 7.8 +/- 0.6 to 12.5 +/- 1.9 fmol/ml, mean +/- SEM, n = 6). Concentrations of plasma ACTH were significantly decreased 30 min following SP (from 320 +/- 70 to 135 +/- 15 fmol/ml, n = 12). In rats anaesthetised with urethane, a significant decrease in plasma ACTH was observed 15 and 30 min following i.c.v. injection of SP (20 nmol); a downward trend was also observed in ACTH following a 40 nmol dose, but this was not significant. No effect of SP was observed on either basal or CRF-41-stimulated ACTH release from isolated rat anterior pituitary cells in vitro. These results demonstrate for the first time that SP exerts opposite effects upon the release of ACTH and AVP in the same animal, and suggest that these actions occur at the level of the hypothalamus.

Some strains of rat develop arthritis, and have profoundly elevated adrenocorticotropin (ACTH) and corticosterone, following intradermal injection of an adjuvant containing heat-killed mycobacteria. Transfer to syngeneic recipients of immune spleen (IS) cells taken from arthritic rats 14 days after injection of the adjuvant, but not of non-immune cells, causes increased circulating ACTH and increased pituitary proopiomelanocortin (POMC) mRNA. Transfer of immune cells does not transfer the disease, but does protect recipients from subsequent challenge with the adjuvant. In these immune-protected rats, the secondary immune response raises ACTH and POMC to levels similar to those seen in arthritic rats. These data show that endogenous inflammatory mediators have direct actions on the neuroendocrine system.

We have investigated the effects of glucocorticoid manipulation on corticotrophin-releasing factor (CRF-41) in the neurointermediate lobe (NIL) and median eminence (ME) of the rat using a radioimmunoassay specific for CRF-41. CRF-41 content in the NIL (88+/-7 fmol, mean +/- SEM) was not significantly altered by administration of dexamethasone in drinking water for 12 days (67+/-9 fmol) or adrenalectomy for 12 days (96+/-17 fmol). The saline-stimulated increase in NIL CRF-41 content (154+/-24 fmol) was not affected by dexamethasone (152+/-24 fmol) or adrenalectomy (179+/-21 fmol). In contrast, the content of CRF-41 in the ME declined following dexamethasone treatment (1247+/-92 fmol to 864+/-26 fmol), or adrenalectomy (854+/-78 fmol). Our results provide further evidence that CRF-41 in the NIL and ME can be differentially regulated.

This study evaluates whether cardiac ischemia induces release of urocortin, before and independently from myocyte cell death. Urocortin levels rose after 5-min ischemia and peaked after 10-min ischemia, when cell death was not detected. However, myocyte apoptosis and/or necrosis occurred following 20- and 30-min ischemia, which paralleled a fall in urocortin levels, suggesting that urocortin expression and release are mainly sustained by metabolically challenged, though still viable myocytes. Hence, since cardiac release of urocortin, unlike that of conventional biomarkers, occurs before and apart from cell death, urocortin levels may be clinically useful in the diagnosis of sublethal myocardial ischemia.
