Abstract

Purpose: Perioperative pelvic floor muscle training can hasten recovery of bladder control and reduce severity of urinary incontinence following radical prostatectomy. Nevertheless, most men undergoing prostatectomy do not receive this training. The purpose of this trial was to test the effectiveness of interactive mobile telehealth (mHealth) to deliver an evidence-based perioperative behavioral training program for post-prostatectomy incontinence. Materials and Methods: This was a 3-site, 2-arm, randomized trial (2014e2019). Men with prostate cancer scheduled to undergo radical prostatectomy were randomized to a perioperative behavioral program (education, pelvic floor muscle training, progressive exercises, bladder control techniques) or a general prostate cancer education control condition, both delivered by mHealth for 1e4 weeks preoperatively and 8 weeks postoperatively. The primary outcome was time to continence following surgery measured by the ICIQ (International Consultation on Incontinence Questionnaire) Short-Form. Secondary outcomes measured at 6, 9 and 12 months included Urinary Incontinence Subscale of Expanded Prostate Cancer Index Composite; pad use; International Prostate Symptom Score QoL Question and Global Perception of Improvement. Results: A total of 245 men (ages 42e78 years; mean[61.7) were randomized. Survival analysis using the Kaplan-Meier estimate showed no statistically significant between-group differences in time to continence. Analyses at 6 months indicated no statistically significant between-group differences in ICIQ scores (mean[7.1 vs 7.0, p[0.7) or other secondary outcomes. Conclusions: mHealth delivery of a perioperative program to reduce post-prostatectomy incontinence was not more effective than an mHealth education program. More research is needed to assess whether perioperative mHealth programs can be a helpful addition to standard prostate cancer care. [ABSTRACT FROM AUTHOR]

Abstract

Background. The purpose of this study was to evaluate the effects of a multicomponent palliative care intervention on choice and use of opioid pain medications for symptom control for patients dying in an acute care inpatient setting. Methods. A preintervention/postintervention trial was conducted between 2001 and 2003. Participants were physician, nursing, and ancillary staff of inpatient services of an urban, tertiary care Veterans Affairs (VA) Medical Center. The intervention included staff education to better identify actively dying patients and a Comfort Care Order Set to guide care in the last hours of life. Data abstracted from computerized medical records of 191 veterans who died during a 6-month period before (N = 98) and after (N = 93) the intervention were used to examine changes in choice and amount of medication administered in the last 3 days of life. Results. Findings show a significant increase in orders specifically for morphine from 47.4% to 81.7% (p < .001). Orders for hydromorphone or oxycodone did not increase significantly, and no patients had orders for meperidine or codeine. There was an increase in the administration of opioids from 16.7% to 73.0% of patients (p < .001). The amount of opioid administered (in oral morphine equivalents) increased from 31.9 mg/72 hours preintervention to 52.9 mg/ 72 hours postintervention (p = .12). Conclusions. The results indicate that the availability of morphine as a preferred opioid and the number of patients who received opioid medication during the last 3 days of life increased after introduction of the inpatient palliative care program. Pelvic floor electrical stimulation may not be better than Kegel exercises for treating urinary incontinence in women, according to a study of 200 women. The electrical stimulation is delivered using a small probe that the woman places insider her vagina. Kegel exercises involve contracting the muscles that control urination, which many women can learn to do voluntarily.

Abstract

Teaching women how to control their pelvic muscles to prevent urinary incontinence is effective with or without biofeedback, according to a study of 222 women. One group received biofeedback involving the placement of electrodes, another received verbal feedback, and the third group was given a self-help booklet with no feedback. All three groups achieved significant improvements in their urinary control.

Abstract

Purpose: The purpose of this study was to assess the effects of age and patients' attribution of incontinence to aging on health-related behaviors associated with incontinence. Design and Methods: Participants in this study were 74 women who either sought treatment for urinary incontinence at a multidisciplinary continence program or volunteered for a randomized clinical trial of behavioral and drug therapy for incontinence. As part of their clinical evaluation, women were interviewed about how they managed their incontinence and their perceptions of what had caused the condition. Self-management of incontinence was defined as behaviors used to cope with incontinence, rather than treat or cure incontinence. Self-treatment was defined as self-implementation of Kegel exercises, and formal treatment was defined as interaction with a health care provider. Results: Over half of the respondents attributed their incontinence to aging. In multivariate analyses, age was associated with self-management of incontinence, but not self-treatment or formal treatment. In contrast, attribution of incontinence to aging was associated with self-management and self-treatment of incontinence. There was also a trend for attribution of incontinence to aging to be associated with formal treatment for incontinence. Women who attributed their incontinence to aging were less likely to have engaged in self-management strategies and to have received a previous evaluation or treatment; but, they were more likely to have engaged in self-treatment for incontinence. When other relevant variables were added to the regression models, perception that incontinence restricted one's activities became the most significant predictor of performing self-management strategies and performing Kegel exercises. Implications: Attribution to aging may be an impediment to seeking treatment. Education to promote understanding of the actual causes and treatment of urinary incontinence may encourage people to seek appropriate intervention. Additionally, whereas attribution to aging is an important factor contributing to health-related behaviors, other factors, such as perception that one's activities are restricted, may play an important role.
