Abstract The purpose of this work is to provide the methodological and instrumental framework for the establishment of a new absolute gravity and height reference network in Sicily. The aim of the network is to contribute to the new reference systems in the Italian area, useful for the scientific and technological activities related to the gravity field and to the proper definition of a modern height system in this region. The network is composed of 5 stations, evenly distributed to form a large mesh, which roughly covers the entire Sicily. Since four of the five selected stations were measured also in the 1990s, it was also possible to evaluate whether long-term gravity changes occurred at these sites (basic requirement for a reference network) and check the long-term ground deformation patterns, using data from the closest GPS/GNSS stations. The observed gravity changes over a time interval of about 30 years at the absolute stations and in the surrounding areas, confirm the long-term stability of the selected areas/sites.

We report a case of successful implantation of a subcutaneous implantable cardioverter-defibrillator in a young patient with severe pectus excavatum presenting with out-of-hospital ventricular fibrillation arrest who was recently surgically repaired with a MIRPE–Nuss procedure. No complications in lead positioning were observed, and the device was tested to determine that it functioned properly.

Heart failure with preserved ejection fraction (HFpEF) results from a complex interplay of age, genetic, cardiac remodeling, and concomitant comorbidities including hypertension, obesity, diabetes, and chronic kidney disease (CKD). Renal failure is an important comorbidity of HFpEF, as well as a major pathophysiological mechanism for those patients at risk of developing HFpEF. Heart failure (HF) and CKD are intertwined conditions sharing common disease pathways; the so-called “kidney tamponade”, explained by an increase in intracapsular pressure caused by fluid retention, is only the latest model to explain renal injury in HF. Recognizing the different phenotypes of HFpEF remains a real challenge; the pathophysiological mechanisms of renal dysfunction may differ across the HF spectrum, as well as the prognostic role. A better understanding of the role of cardiorenal interactions in patients with HF in terms of symptom status, disease progression, and prognosis remains essential in HF management. Historically, patients with HF and CKD have been scarcely represented in clinical trial populations. Current concerns affect the practical approach to HF treatment, and, in this context, physicians are frequently hesitant to prescribe and titrate both new and old treatments. Therefore, the extensive application of HF drugs in diverse HF subtypes with numerous comorbidities and different renal dysfunction etiologies remains a controversial matter of discussion. Numerous recently introduced drugs, such as sodium–glucose-linked transporter 2 inhibitors (SGLT2i), constitute a new therapeutic option for patients with HF and CKD. Because of their protective vascular and hormonal actions, the use of these agents may be safely extended to patients with renal dysfunction in the long term. The present review delves into the phenotype of patients with HFpEF and CKD from a pathophysiological perspective, proposing a treatment approach that suggests a practical stepwise algorithm for the proper application of life-saving therapies in clinical practice.

Construction materials contribute to about 75% of the CO2 emission of all the construction processes. Concrete is one of the most widely used construction materials and is thus primarily responsible for CO2 emission. In particular, 8 − 9% of global greenhouse gas (GHG) emission are produced by concrete. CO2 emissions can be considerably reduced in the construction phase through a careful selection of materials with low environmental impact or through specific admixtures. In this study, different concretes are taken into consideration, including foamed concrete, lightweight aggregate concrete, recycled concrete and ordinary concrete. A series of mix designs of these four classes of concrete, characterized by a comparable mechanical strength or a comparable density, are taken from the relevant literature and compared to one another in terms of CO2 emission. Some guidelines or possible research lines aimed at reducing CO2 emission are finally outlined in this contribution.

Abstract Background Body mass index (BMI), age, left atrium (LA) dimension and left ventricular ejection fraction (LVEF) have been linked to post-operative atrial fibrillation (POAF) after cardiac surgery. The aim of this study was to better define the role of these risk factors. Methods This retrospective cohort study evaluated 249 patients (without prior atrial dysrhythmia) undergoing cardiac or aortic surgery. Prior to surgery, the following data were collected: age, BMI, LA diameter, LA area, LVEF, thyroid stimulating hormone (TSH), creatinine and the presence of arterial hypertension (AH) and diabetes. Intraoperative data such as operation time, total clamp time, cardiopulmonary bypass time, and presence of pericardial/pleural effusion were also collected. Only patients without pre- and post-surgery prophylactic anti-arrhythmic therapy were included. Results Patients with (N = 127, 51%) and without POAF (N = 122, 49%) were compared. No difference was observed for sex, LA diameter, LA area, LVEF, TSH, diabetes and use of ACE inhibitors or statins prior to intervention. Moreover, no difference was observed in terms of operation time, total clamp time, cardiopulmonary bypass time, and presence of pericardial/pleural effusion. However, patients with POAF were older (70.6 ± 10.7 vs. 60.4 ± 16.4 years, p = 0.001), had higher BMI (26.8 ± 4.5 vs. 24.9 ± 3.6 kg/m2, p = 0.001), higher baseline creatinine (1.06 ± 0.91 vs. 0.88 ± 0.32 mg/dL, p = 0.038) and a higher frequency of arterial hypertension (73.2% vs. 50%, p = 0.001) and Bentall procedure (24.4% vs. 9.8%, p = 0.023). Multivariate analysis showed that the only independent predictors of POAF were age (OR = 1.05, 95%CI 1.02–1.07, p = 0.001) and BMI (OR = 1.11 95%CI 1.03–1.2,p = 0.006). Conclusions These findings suggest that advanced age and a higher BMI are strong risk factors for POAF in patients without previous AF even in the presence of comparable LA dimensions and LVEF.

The present study concerns the experimental investigation of foamed concretes with 1600 kg/m3 density that incorporate biochar additions in the mix. A series of small notched beams are prepared to determine the fracture energy in CMOD (crack mouth opening displacement) mode and the mechanical properties in terms of flexural and compressive strength. Besides the evaluation of such properties for classical foamed concrete, the influence of the addition of biochar in the lightweight cementitious paste is comparatively investigated. Two different concentrations of biochar are analyzed, namely 2% and 4% of the cement weight, and two different curing conditions are studied, namely in air and in water at controlled temperature for 28 days. The results demonstrate that better fracture behavior are obtained with 2% biochar and air curing conditions. The biochar additions in moderate concentrations (e.g. 2%) seems to make the fracture surface more tortuous, thus justifying the numerical outcomes, and does not impair the flexural strength. Further microstructural investigation is underway to confirm the experimental observations. This research paves the way for a promising construction material that is more environmentally friendly and sustainable than traditional materials used in the building industry.

The tensile properties of fabric-reinforced cementitious matrix (FRCM) composites are experimentally investigated through clevis-grip tensile tests (according to AC434 provisions) on FRCM coupons realized with customized (ad hoc developed in this paper) cement-based matrices. The tested FRCM coupons are reinforced with basalt, carbon, or steel fabrics, and are prepared with three different matrices: one-component mortar incorporating dispersible copolymer powders of vinyl acetate and ethylene (matrices A and B) and two-component mortar with carboxylated styrene–butadiene copolymer liquid resin (matrix C). This has made it possible to investigate the mechanical compatibility between different mortar matrices and fabrics and the resulting tensile properties of FRCM composites in the uncracked, cracking, and fully cracked phases. Experimental results are critically analyzed in terms of stress–strain curves and failure mechanisms comparatively for the analyzed FRCM systems. It has been shown that the matrix B exhibits a good compatibility with the basalt pre-impregnated fabric, while the matrix C appears to be the most suitable candidate to optimize the interfacial stress transfer at the fiber–matrix interface for all fabrics, thus exalting the mechanical performances in terms of tensile strength and ultimate strain. The results of this experimental program can be useful for designing optimized mortar mixes aimed at realizing novel FRCM composites or at improving existing FRCM systems by suitably accounting for compatibility behavior and slippage at the fabric–matrix interface.

Purpose. Myocardial scar is directly related to the response to CRT after implantation. The extent of myocardial scar can be detected not only by cardiac magnetic resonance but also by two electrocardiographic scores: fragmented QRS (fQRS) and Selvester score (SSc). The aim of our study is to compare the role of baseline SSc and fQRS in predicting response to CRT in a cohort of heart failure patients with true left bundle branch block (LBBB). As a secondary endpoint, we assessed the association of both scores with overall and cardiac mortality, heart failure hospitalizations, ventricular arrhythmias requiring ICD intervention, and major adverse cardiovascular event (MACE). Methods. We evaluated fQRS and SSc of 178 consecutive HF patients with severe systolic dysfunction (LVEF ≤ 35%), NYHA class II-III despite optimal medical treatment, and true-LBBB. Response to CRT was defined as the improvement of LVEF of at least 10% or as the reduction of LVESV of at least 15% at a 6-month follow-up. Each endpoint was related to fQRS and SSc. Results. SSc ≥7 was significantly associated with the absence of echocardiographic response to CRT (OR: 0.327; 95% C.I. 0.155–0.689; p=0.003), while the presence of fQRS at baseline ECG was not (OR: 1.133; 95% C.I. 0.539–2.381; p=0.742). No correlation was found between SSc and overall mortality, cardiac death, ventricular arrhythmias, hospitalizations due to heart failure, or for MACE. Similar results were observed between fQRS and all secondary endpoints. Conclusion. In HF patients with true-LBBB and LVEF ≤35% eligible for CRT, myocardial scar assessed by calculating the SSc on preimplant ECG is an independent predictor of nonresponse after multiple adjustments. Neither SSc nor fQRS is associated with overall and cardiac death, ventricular arrhythmias, or hospitalization for heart failure at a 24-month follow-up.

Water is a fundamental resource for human life and nature; flood management, water supply systems and water protection policies are a few examples of equally important disciplines across the whole hydrological cycle. The present work focuses on the creation and sharing of hydrological knowledge within public activities, with regard to materials and methods adopted for developing and supplying hydrological information, suitable to different stakeholders needs, throughout different disciplines and sectors of environment, economy, society, as well as research and analysis. The aim of this work is to better understand the market in order to increase the value of hydrological data, products and services, and to reduce potential gaps and overlapping areas. The method we developed is based on the example of the Hydrological Service of Emilia-Romagna Region, Italy. Institutional, legal and territorial frameworks as well as agency organization, materials, methods, instruments, activities, products and results are briefly described, focusing on those supporting civil and environmental protection, water management, infrastructure design, climate change adaptation and mitigation measures. We discuss the role of a public Administration in interdisciplinary activities, the links between the general background (e.g., territory, society, rules), organizations, actors, resources, tools, processes and results, by highlighting, where possible, a potential starting point for future research studies. Finally, this paper adopts a novel linguistic style, based on an informal format, in order to explore the set-up and follow-up of the Hydrological Service’s initiatives, with the final aim of sparking curiosity and building awareness, from different sectors and disciplines, which, ultimately, may benefit from the presented approaches.

The hysteretic behavior of friction isolators is affected by the variability of the friction coefficient caused by heating phenomena at the sliding interface. The aim of this paper is to investigate such heating phenomena through a series of full-scale experimental tests on a double curved surface slider. The prototype isolator is equipped with eight thermocouples placed in different points of the isolator, which are embedded in the sliding plate. The probes of the thermocouples are in contact with the stainless steel sheet covering the sliding plate, in such a manner that their measurements are representative of the temperature rise occurring at the sliding interface. By investigating different axial loads and sliding velocities, we discuss the measured temperature rise and its implications on the hysteretic behavior of the prototype isolator. Friction variation is observed in the cyclic response of the isolator, which reduces the energy dissipated per cycle and, consequently, may lead to some underestimations of the displacements occurring during real seismic events if a constant friction coefficient is assumed. The proposed data can be helpful to calibrate sophisticated thermo-mechanical finite element models, which is the object of ongoing research.

Digital Image Correlation (DIC) provides measurements without disturbing the specimen, which is a major advantage over contact methods. Additionally, DIC techniques provide full-field maps of response quantities like strains and displacements, unlike traditional methods that are limited to a local investigation. In this work, an experimental application of DIC is presented to investigate a problem of relevant interest in the civil engineering field, namely the interface behavior between externally bonded fabric reinforced cementitious mortar (FRCM) sheets and concrete substrate. This represents a widespread strengthening technique of existing reinforced concrete structures, but its effectiveness is strongly related to the bond behavior between composite fabric and underlying concrete. To investigate this phenomenon, a set of notched concrete beams are realized, reinforced with FRCM sheets on the bottom face, subsequently cured in different environmental conditions (humidity and temperature) and finally tested up to failure under three-point bending. Mechanical tests are carried out vis-à-vis DIC measurements using two distinct cameras simultaneously, one focused on the concrete front face and another focused on the FRCM-concrete interface. This experimental setup makes it possible to interpret the mechanical behavior and failure mode of the specimens not only from a traditional macroscopic viewpoint but also under a local perspective concerning the evolution of the strain distribution at the FRCM-concrete interface obtained by DIC in the pre- and postcracking phase.

Civil engineering structures and infrastructures are inherently vulnerable to exceptional loads related to natural disasters, primarily earthquakes, tsunamis, strong winds, and floods [...]

Existing civil engineering structures having strategic importance, such as hospitals, fire stations, and power plants, often do not comply with seismic standards in force today, as they were designed and built based on past structural guidelines. On the other hand, due to their special importance, structural integrity of such buildings is of vital importance during and after earthquakes, which puts demands on strategies for their seismic protection. In this regard, seismic base isolation has been widely employed; however, the existing limited seismic joint between adjacent buildings may hamper this application because of the large displacements concentrated at the isolation floor. In this paper, we compare two possible remedies: the former is to provide supplemental damping in conventional base isolation systems and the latter consists in a combination of base isolation with supplemental rotational inertia. For the second strategy, a mechanical device, called inerter, is arranged in series with spring and dashpot elements to form the so-called tuned-mass-damper-inerter (TMDI) directly connected to an isolation floor. Several advantages of this second system as compared to the first one are outlined, especially with regard to the limitation of floor accelerations and interstory drifts, which may be an issue for nonstructural elements and equipment, in addition to disturbing occupants. Once the optimal design of the TMDI is established, possible implementation of this system into existing structures is discussed.

Since 1982, high precision gravity measurements have been routinely carried out on Mt. Vesuvius. The gravity network consists of selected sites most of them coinciding with, or very close to, leveling benchmarks to remove the effect of the elevation changes from gravity variations. The reference station is located in Napoli, outside the volcanic area. Since 1986, absolute gravity measurements have been periodically made on a station on Mt. Vesuvius, close to a permanent gravity station established in 1987, and at the reference in Napoli. The results of the gravity measurements since 1982 are presented and discussed. Moderate gravity changes on short-time were generally observed. On long-term significant gravity changes occurred and the overall fields displayed well defined patterns. Several periods of evolution may be recognized. Gravity changes revealed by the relative surveys have been confirmed by repeated absolute measurements, which also confirmed the long-term stability of the reference site. The gravity changes over the recognized periods appear correlated with the seismic crises and with changes of the tidal parameters obtained by continuous measurements. The absence of significant ground deformation implies masses redistribution, essentially density changes without significant volume changes, such as fluids migration at the depth of the seismic foci, i.e. at a few kilometers. The fluid migration may occur through pre-existing geological structures, as also suggested by hydrological studies, and/or through new fractures generated by seismic activity. This interpretation is supported by the analyses of the spatial gravity changes overlapping the most significant and recent seismic crises.
