The disease burden includes physical limitations, psychosocial discomfort, and a reduced quality of life (HRQoL). This study presents the results of a parent-reported survey on the psychosocial impact of AD on Italian pre-adolescent children (6– 11 years old), with a specific focus on bullying, self-isolation, absenteeism, and presenteeism.Methods: An online questionnaire was sent to 3067 random recipients and 160 matched the inclusion criteria for age, self-reported AD diagnosis, localizations (according to ISAAC), and disease severity (POEM ≥ 8). 100 children, with comparable ages, not matching the inclusion criteria for AD, were recruited as a control group.Results: Children with AD and their caregivers had a significantly lower quality of sleep (QoS) compared to the control group. The presence of AD was directly responsible for many restless nights, both in children and caregivers (58.9 and 55.4 respectively). Children with AD and their parents also experienced significantly more daytime drowsiness (43.6 and 54.6 days, respectively). Children with AD were more frequently victims of bullying at school (20.0% vs 9.0%; p≤ 0.05) or in other social environments (16.9% vs 3.0%; p≤ 0.05). AD caused 17.7 days of absenteeism and 20.1 days of presenteeism per student over the previous 12 months, accounting for 37.8 days of study impairment overall. Severe/very severe AD had a significantly greater impact on presenteeism than moderate AD (25.1 vs 17.5 days; p≤ 0.05). Presenteeism, which was more pronounced among bullied students, was positively correlated with absenteeism only in the AD cohort.Conclusion: AD has a detrimental impact on the HRQoL of pediatric patients, causing stigmatization and social isolation. Functional distress was also reported by caregivers. Aims The European Association of Cardiovascular Imaging (EACVI) Scientific Initiatives Committee performed a global survey on radiation exposure in interventional echocardiography. The survey aimed to collect data on local practices for radioprotection in interventional echocardiography and to assess the awareness of echocardiography operators about radiation-related risks. Methods and results A total of 258 interventional echocardiographers from 52 different countries (48% European) responded to the survey. One hundred twenty-two (47%) participants were women. Two-thirds (76%) of interventional echocardiographers worked in tertiary care/university hospitals. Interventional echocardiography was the main clinical activity for 34% of the survey participants. The median time spent in the cath-lab for the echocardiographic monitoring of structural heart procedures was 10 (5–20) hours/month. Despite this, only 28% of interventional echocardiographers received periodic training and certification in radioprotection and 72% of them did not know their annual radiation dose. The main adopted personal protection devices were lead aprons and thyroid collars (95% and 92% of use, respectively). Dedicated architectural protective shielding was not available for 33% of interventional echocardiographers. Nearly two-thirds of responders thought that the radiation exposure of interventional echocardiographers was higher than that of interventional cardiologists and 72% claimed for an improvement in the radioprotection measures. Conclusion Radioprotection measures for interventional echocardiographers are widely variable across centres. Radioprotection devices are often underused by interventional echocardiographers, portending an increased radiation-related risk. International scientific societies working in the field should collaborate to endorse radioprotection training, promote reliable radiation dose assessment, and support the adoption of radioprotection shielding dedicated to interventional echocardiographers. Background: the best management of symptomatic patients with low gradient (LG) severe aortic stenosis (AS) and preserved left ventricular ejection fraction (LVEF) has not been established in randomized controlled trials. The ROTAS trial aimed to assess the superiority of aortic valve replacement (AVR) versus optimized medical therapy (OMT) in symptomatic patients with LG severe AS and preserved LVEF. Fifty-two patients (age 79 ± 7 years; males 46%, mean aortic gradient: 31 ± 5 mmHg; aortic surface: 0.8 ± 0.1 cm2) who were randomized 1:1 to AVR or OMT. The follow-up lasted 14 ± 7 months. The study was interrupted early because of the low inclusion rate during the COVID-2019 pandemic. The AS severity was confirmed by a multimodality approach including dobutamine stress echocardiography and/or aortic calcium score. The primary end-point (overall death and/or hospitalisation for heart failure) occurred in 13 (25%) patients. Patients undergoing AVR showed a similar recurrence of events (overall death, heart failure hospitalisation) compared to patients receiving OMT (5 vs. 8 events). Patients receiving OMT did not experience worse survival compared to patients receiving AVR (HR 1.57, 95% CI: 0.51–4.83, P = 0.4275). Kaplan-Meier curves showed similar event-free survival (logrank P = 0.4236) among the 2 groups (Fig. 1). All grades CRS was observed in 49/57 (86%) PMBCL and 118/135 (87%) LBCL patients, with 9 (16%) and 12 (9%) severe (grade 3-4) CRS, respectively; all grades ICANS were reported in 25 (44%) PMBCL patients and in 46 (34%) LBCL, with 12 (21%) and 14 (10%) severe (grade 3-4) ICANS, respectively.
