The purpose of this descriptive study was to examine school string teachers' beliefs about and practices of teaching nontraditional music courses. We surveyed middle and high school string teachers (N = 42) about what nontraditional music courses and ensembles (NMCEs) they currently offer and would like to offer to their students. In addition, we examined the educational rationales that string teachers have for offering NMCEs. Our findings indicated that the most frequently offered NMCEs were guitar class, pit orchestra, and music technology. Participants expressed strongest interest in adding fiddle club, popular music, and mariachi ensemble. The participants explained that NMCEs provide opportunities to teach content that closely may more reflect students' cultures and possibly attract a wider range of students. The participants used NMCEs to cultivate musical skills that are often overlooked in large-group setting, including improvisation, playing by ear, and collaborative arranging and composition.

Abstract Obesity adversely affects bone and fat metabolism in mice and humans. Omega-3 polyunsaturated fatty acids (omega-3 PUFAs) have been shown to improve glucose metabolism and bone homeostasis in obesity. However, the impact of omega-3 PUFAs on bone marrow adipose tissue (BMAT) and bone marrow stromal cell (BMSC) metabolism has not been intensively studied yet. In the present study we demonstrated that omega-3 PUFA supplementation in high fat diet (HFD + F) improved bone parameters, mechanical properties along with decreased BMAT in obese mice when compared to the HFD group. Primary BMSCs isolated from HFD + F mice showed decreased adipocyte and higher osteoblast differentiation with lower senescent phenotype along with decreased osteoclast formation suggesting improved bone marrow microenvironment promoting bone formation in mice. Thus, our study highlights the beneficial effects of omega-3 PUFA-enriched diet on bone and cellular metabolism and its potential use in the treatment of metabolic bone diseases.

Recent studies highlighted the crucial contribution of subchondral bone to OA development. Yet, only limited data have been reported on the relation between alteration to cartilage morphology, structural properties of the subchondral bone plate (SBP) and underlying subchondral trabecular bone (STB). Furthermore, the relationship between the morphometry of the cartilage and bone in the tibial plateau and the OA-induced changes in the joint’s mechanical axis remains unexplored. Therefore, a visualisation and quantification of cartilage and subchondral bone microstructure in the medial tibial plateau was performed. End stage knee-OA patients with varus alignment and scheduled for total knee arthroplasty (TKA) underwent preoperative fulllength radiography to measure the hip-knee-ankle angle (HKA) and the mechanical-axis deviation (MAD). 18 tibial plateaux were μ-CT scanned (20.1 μm/voxel). Cartilage thickness, SBP, and STB microarchitecture were quantified in 10 volumes of interest (VOIs) in each medial tibial plateau. Significant differences (p < 0.001) were found for cartilage thickness, SBP, and STB microarchitecture parameters among the VOIs. Closer to the mechanical axis, cartilage thickness was consistently smaller, while SBP thickness and STB bone volume fraction (BV/TV) were higher. Moreover, trabeculae were also more superior-inferiorly oriented, i.e. perpendicular to the transverse plane of the tibial plateau. As cartilage and subchondral bone changes reflect responses to local mechanical loading patterns in the joint, the results suggested that region-specific subchondral bone adaptations were related to the degree of varus deformity. More specifically, subchondral sclerosis appeared to be most pronounced closer to the mechanical axis of the knee.

Abstract Spearing mantis shrimps are aggressive crustaceans using specialized appendages with sharp spikes to capture fishes with fast movement. Each spike is a biological tool that has to combine high toughness, as required by the initial impact with the victim, with high stiffness and strength, to ensure sufficient penetration while avoid breaking. We performed a multimodal analysis to uncover the design strategies of this harpoon based on chitin. We found that the spike is a slightly hooked hollow beam with the outer surface decorated by serrations and grooves to enhance cutting and interlocking. The cuticle of the spike resembles a multilayer composite: An outer heavily mineralized, stiff, and hard region (with average indentation modulus and hardness of 68 and 3 GPa), providing high resistance to contact stresses, is combined with a less mineralized region, which occupies a large fraction of the cuticle (up to 50%) and features parallel fibers oriented longitudinally, enhancing stiffness and strength. A central finding of our work is the presence of a tiny interphase (less than 10 μm in width) based on helical fibers and showing a spatial modulation in mechanical properties, which has the critical task to integrate the stiff but brittle outer layer with the more compliant highly anisotropic parallel‐fiber region. We highlighted the remarkable ability of this helicoidal region to stop nanoindentation‐induced cracks. Using three‐dimensional multimaterial printing to prototype spike‐inspired composites, we showed how the observed construction principles can not only hamper damage propagation between highly dissimilar layers (resulting in composites with the helical interphase absorbing 50% more energy than without it) but can also enhance resistance to puncture (25% increase in the force required to penetrate the composites with a blunt tool). Such findings may provide guidelines to design lightweight harpoons relying on environmentally friendly and recyclable building blocks. ABSTRACT: Segmental defects affecting the continuity of the mandible have a profound impact on a patient's quality of life. Adequate reconstruction of such continuity defects is essential to restore aesthetics and function. While reconstruction using an autologous bone transplant supported by a stock reconstruction plate is the gold standard treatment, it has various drawbacks that can be attested to the plates’ off-the-shelf nature.To mitigate these drawbacks, this study develops a patient-specific implant for the reconstruction of Brown class II defects with a high ramal osteotomy. The implant is intended to be additively manufactured in Ti6Al4V grade 23 ELI and features porous scaffold zones at the symphyseal and condylar sides which can induce bone ingrowth.Finite element (FE) analyses were used to assess the implants’ performance in terms of failure, stability and stress shielding by simulating four clenching tasks. In addition, the implant was topologically optimized and re-evaluated.The results showed that the implant experienced stress below its yield strength and fatigue limit. Relative micromotions between the implant and the bone indicated adequate stability to allow bone ingrowth to occur. Strains in the bone indicated limited stress shielding should occur between screw connections and around the osteotomy planes.Finally, topological optimization reduced implant volume by 49% compared to the initial design, while FE analyses showed similar performance to the original design. The resulting implant is a promising first prototype that is numerically evaluated and can be optimized further in terms of fixation, surgical approach and dental restoration by in situ testing.

Abstract Femoroplasty is a procedure where bone cement is injected percutaneously into a weakened proximal femur. Uncertainty exists whether femoroplasty provides sufficient mechanical strengthening to prevent fractures in patients with femoral bone metastases. Finite element models are promising tools to evaluate the mechanical effectiveness of femoroplasty, but a thorough validation is required. This study validated a voxel-based finite element model against experimental data from eight pairs of human cadaver femurs with artificial metastatic lesions. One femur from each pair was left untreated, while the contralateral femur was augmented with bone cement. Finite element models accurately predicted the femoral strength in the defect (R2 = 0.96) and augmented (R2 = 0.93) femurs. The modelled surface strain distributions showed a good qualitative match with results from digital image correlation; yet, quantitatively, only moderate correlation coefficients were found for the defect (mean R2 = 0.78) and augmented (mean R2 = 0.76) femurs. This was attributed to the presence of vessel holes in the femurs and the jagged surface representation of our voxel-based models. Despite some inaccuracies in the surface measurements, the FE models accurately predicted the global bone strength and qualitative deformation behavior, both before and after femoroplasty. Hence, they can offer a useful biomechanical tool to assist clinicians in assessing the need for prophylactic augmentation in patients with metastatic bone disease, as well as in identifying suitable patients for femoroplasty.

HR-pQCT has become standard practice when quantifying volumetric BMD (vBMD) in vivo. Yet, it is only accessible to peripheral sites, with small fields of view and lengthy scanning times. This limits general applicability in clinical workflows. The goal of this study was to assess the potential of photon counting CT (PCCT) in quantitative bone imaging. Using the European Forearm Phantom, PCCT was calibrated to hydroxyapatite (HA) density. Eight cadaveric forearms were scanned twice with PCCT and once with HR-pQCT. The dominant forearm of two volunteers was scanned twice with PCCT. In each scan, the carpals were delineated. At bone level, accuracy was assessed with a paired measurement of total vBMD (Tt.vBMD) calculated with PCCT and HR-pQCT. At voxel-level, repeatability was assessed by image registration and voxel-wise subtraction of the ex vivo PCCT scans. In an ideal scenario, this difference would be zero; any deviation was interpreted as falsely detected remodeling. For clinical usage, the least detectable remodeling was determined by finding a threshold in the PCCT difference image that resulted in a classification of bone formation and resorption below acceptable noise levels (<0.5%). The paired measurement of Tt.vBMD had a Pearson correlation of 0.986. Compared to HR-pQCT, PCCT showed a bias of 7.46 mgHA/cm3. At voxel-level, the repeated PCCT scans showed a bias of 17.66 mgHA/cm3 and a standard error of 96.23 mgHA/cm3. Least detectable remodeling was found to be 250 mgHA/cm3, for which 0.37% of the voxels was incorrectly classified as newly added or resorbed bone. In vivo , this volume increased to 0.97%. Based on the cadaver data, we conclude that PCCT can be used to quantify vBMD and bone turnover. We provided proof of principle that this technique is also accurate in vivo , hence, that it has high potential for clinical applications. [ABSTRACT FROM AUTHOR]

Objective: The use of thiazolidinediones (TZDs) as insulin sensitizers has been shown to have side effects including increased accumulation of bone marrow adipocytes (BMAds) associated with a higher fracture risk and bone loss. A novel TZD analog MSDC-0602K with low affinity to PPARγ has been developed to reduce adverse effects of TZD therapy. However, the effect of MSDC-0602K on bone phenotype and bone marrow mesenchymal stem cells (BM-MSCs) in relation to obesity has not been intensively studied yet. Methods: Here, we investigated whether 8-week treatment with MSDC-0602K has a less detrimental effect on bone loss and BM-MSC properties in obese mice in comparison to first generation of TZDs, pioglitazone. Bone parameters (bone microstructure, bone marrow adiposity, bone strength) were examined by μCT and 3-point bending test. Primary BM-MSCs were isolated and measured for osteoblast and adipocyte differentiation. Cellular senescence, bioenergetic profiling, nutrient consumption and insulin signaling were also determined. Results: The findings demonstrate that MSDC-0602K improved bone parameters along with increased proportion of smaller BMAds in tibia of obese mice when compared to pioglitazone. Further, primary BM-MSCs isolated from treated mice and human BM-MSCs revealed decreased adipocyte and higher osteoblast differentiation accompanied with less inflammatory and senescent phenotype induced by MSDC-0602K vs. pioglitazone. These changes were further reflected by increased glycolytic activity differently affecting glutamine and glucose cellular metabolism in MSDC-0602K-treated cells compared to pioglitazone, associated with higher osteogenesis. Conclusion: Our study provides novel insights into the action of MSDC-0602K in obese mice, characterized by the absence of detrimental effects on bone quality and BM-MSC metabolism when compared to classical TZDs and thus suggesting a potential therapeutical use of MSDC-0602K in both metabolic and bone diseases.

Abstract The enthesis allows the insertion of tendon into bone thanks to several remarkable strategies. This complex and clinically relevant location often features a thin layer of fibrocartilage sandwiched between tendon and bone to cope with a highly heterogeneous mechanical environment. The main purpose of this study was to investigate whether mineralized fibrocartilage and bone close to the enthesis show distinctive three-dimensional microstructural features, possibly to enable load transfer from tendon to bone. As a model, the Achilles tendon-calcaneus bone system of adult rats was investigated with histology, backscattered electron imaging and micro-computed tomography. The microstructural porosity of bone and mineralized fibrocartilage in different locations including enthesis fibrocartilage, periosteal fibrocartilage and bone away from the enthesis was characterized. We showed that calcaneus bone presents a dedicated protrusion of low porosity where the tendon inserts. A spatially resolved analysis of the trabecular network suggests that such protrusion may promote force flow from the tendon to the plantar ligament, while partially relieving the trabecular bone from such a task. Focusing on the tuberosity, highly specific microstructural aspects were highlighted. Firstly, the interface between mineralized and unmineralized fibrocartilage showed the highest roughness at the tuberosity, possibly to increase failure resistance of a region carrying large stresses. Secondly, fibrochondrocyte lacunae inside mineralized fibrocartilage, in analogy with osteocyte lacunae in bone, had a predominant alignment at the enthesis and a rather random organization away from it. Finally, the network of subchondral channels inside the tuberosity was highly anisotropic when compared to contiguous regions. This dual anisotropy of subchondral channels and cell lacunae at the insertion may reflect the alignment of the underlying collagen network. Our findings suggest that the microstructure of fibrocartilage may be linked with the loading environment. Future studies should characterize those microstructural aspects in aged and or diseased conditions to elucidate the poorly understood role of bone and fibrocartilage in enthesis-related pathologies.

Purpose of Review: Recently, photon-counting computed tomography (PCCT) has been introduced in clinical research and diagnostics. This review describes the technological advances and provides an overview of recent applications with a focus on imaging of bone. Recent Findings: PCCT is a full-body scanner with short scanning times that provides better spatial and spectral resolution than conventional energy-integrating-detector CT (EID-CT), along with an up to 50% reduced radiation dose. It can be used to quantify bone mineral density, to perform bone microstructural analyses and to assess cartilage quality with adequate precision and accuracy. Using a virtual monoenergetic image reconstruction, metal artefacts can be greatly reduced when imaging bone-implant interfaces. Current PCCT systems do not allow spectral imaging in ultra-high-resolution (UHR) mode. Summary: Given its improved resolution, reduced noise and spectral imaging capabilities PCCT has diagnostic capacities in both qualitative and quantitative imaging that outperform those of conventional CT. Clinical use in monitoring bone health has already been demonstrated. The full potential of PCCT systems will be unlocked when UHR spectral imaging becomes available. [ABSTRACT FROM AUTHOR]

The primate thumb plays a central role in grasping and the basal trapeziometacarpal (TMC) joint is critical to its function. The TMC joint morphology varies across primates, yet little is known about form-function interaction within in the TMC joint. The purpose of this study was to investigate how stress distributions within the joint differ between five grasping types commonly employed by bonobos (Pan paniscus). Five cadaveric bonobo forearms were CT scanned in five standardized positions of the hand as a basis for the generation of parametric finite element models to compare grasps. We have developed a finite element analysis (FEA) approach to investigate stress distribution patterns in the TMC joint associated with each grasp type. We hypothesized that the simulated stress distributions for each position would correspond with the patterns expected from a saddle-shaped joint. However, we also expected differences in stress patterns arising from instraspecific variations in morphology. The models showed a high agreement between simulated and expected stress patterns for each of the five grasps (86% of successful simulations), while partially (52%) and fully (14%) diverging patterns were also encountered. We identified individual variations of key morphological features in the bonobo TMC joint that account for the diverging stress patterns and emphasized the effect of interindividual morphological variation on joint functioning. This study gives unprecedented insight in the form-function interactions in the TMC joint of the bonobo and provides an innovative FEA approach to modelling intra-articular stress distributions, a valuable tool for the study of the primate thumb biomechanics.

Patients with bone metastases have an increased risk to sustain a pathological fracture as lytic metastatic lesions damage and weaken the bone. In order to prevent fractures, prophylactic treatment is advised for patients with a high fracture risk. Mechanical stabilization of the femur can be provided through femoroplasty, a minimally invasive procedure where bone cement is injected into the lesion, or through internal fixation with intra- or extramedullary implants. Clinicians face the task of determining whether or not prophylactic treatment is required and which treatment would be the most optimal. Finite element (FE) models are promising tools that could support this decision process. The aim of this paper is to provide an overview of the state-of-the-art in FE modeling for the treatment decision of metastatic bone lesions in the femur. First, we will summarize the clinical and mechanical results of femoroplasty as a prophylactic treatment method. Secondly, current FE models for fracture risk assessment of metastatic femurs will be reviewed and the remaining challenges for clinical implementation will be discussed. Thirdly, we will elaborate on the simulation of femoroplasty in FE models and discuss future opportunities. Femoroplasty has already proven to effectively relieve pain and improve functionality, but there remains uncertainty whether it provides sufficient mechanical strengthening to prevent pathological fractures. FE models could help to select appropriate candidates for whom femoroplasty provides sufficient increase in strength and to further improve the mechanical benefit by optimizing the locations for cement augmentation.

Nonlinear finite element (FE) models can accurately quantify bone strength in healthy and metastatic femurs. However, their use in clinical practice is limited since state-of-the-art implementations using tetrahedral meshes involve a lot of manual work for which specific modelling software and engineering knowledge are required. Voxel-based meshes could enable the transition since they are robust and can be highly automated. Therefore, the aim of this work was to bridge the modelling gap between the tetrahedral and voxel-based approach. Specifically, we validated a nonlinear voxel-based FE method relative to experimental data from 20 femurs with and without artificial metastases that had been mechanically loaded until failure. CT scans of the femurs were segmented and automatically converted into a voxel-based mesh with hexahedral elements. Nonlinear material properties were implemented in an open-source linear voxel-based FE solver by adding an additional loop to the routine such that the material properties could be adapted after each increment. Bone strength, quantified as the maximum force in the force-displacement curve, was evaluated. The results were compared to a previously established nonlinear tetrahedral FE approach as well as to the experimentally measured bone strength. The voxel-based FE model predicted the experimental bone strength very well both for healthy (R2 = 0.90, RMSE = 0.88 kN) and metastatic femurs (R2 = 0.93, RMSE = 0.64 kN). The model precision and accuracy were very similar to the ones obtained with the tetrahedral model (R2 = 0.90/0.93, RMSE = 0.90/0.64 kN for intact/metastatic respectively). The more intuitive voxel-based meshes thus quantified macroscale femoral strength equally well as state-of-the-art tetrahedral models. The robustness, high level of automation and time-efficiency (< 30 min) of the implemented workflow offer great potential for developing FE models to improve fracture risk prediction in clinical practice.

Knee osteoarthritis is a whole joint disease highlighting the coupling of cartilage and bone adaptations. However, the structural properties of the subchondral bone plate (SBP) and underlying subchondral trabecular bone (STB) in the femoral compartment have received less attention compared to the tibial side. Furthermore, how the properties in the femoral compartment relate to those in the corresponding tibial site is unknown. Therefore, this study aimed to quantify the structural bone and cartilage morphology in the femoral compartment and investigate its association with those of the tibial plateau. Specifically, tibial plateaus and femoral condyles were retrieved from 28 patients with end-stage knee-osteoarthritis (OA) and varus deformity. The medial condyle of tibial plateaus and the distal part of the medial femoral condyles were micro-CT scanned (20.1 μm/voxel). Cartilage thickness (Cart.Th), SBP, and STB microarchitecture were quantified. Significant (P  < <.001; 0.79 ≤  r  ≤ 0.97) correlations with a relative difference within 10% were found between the medial side of the femoral and tibial compartments. The highest correlations were found for SBP porosity (r  = 0.97, mean absolute difference of 0.50%, and mean relative difference of 9.41%) and Cart.Th (r  = 0.96, mean absolute difference of 0.18 mm, and relative difference of 7.08%). The lowest correlation was found for trabecular thickness (r  = 0.79, mean absolute difference of 21.07 μm, and mean relative difference of 5.17%) and trabecular number (r  = 0.79, mean absolute difference of 0.18 mm−1, and relative difference of 5.02%). These findings suggest that the distal femur is affected by OA in a similar way as the proximal tibia. Given that bone adaptation is a response to local mechanical forces, our results suggest that varus deformity similarly affects the stress distribution of the medial tibial plateau and the medial distal femur.
