Abstract

No robust evidence is provided by literature regarding the management of intracranial hypertension following severe traumatic brain injury (TBI). This is mostly due to the lack of prospective randomized controlled trials (RCTs), the presence of studies containing extreme heterogeneously collected populations and controversial considerations about chosen outcome. A scientific society should provide guidelines for care management and scientific support for those areas for which evidence-based medicine has not been identified. However, RCTs in severe TBI have failed to establish intervention effectiveness, arising the need to make greater use of tools such as Consensus Conferences between experts, which have the advantage of providing recommendations based on experience, on the analysis of updated literature data and on the direct comparison of different logistic realities. The Italian scientific societies should provide guidelines following the national laws ruling the best medical practice. However, many limitations do not allow the collection of data supporting high levels of evidence for intracranial pressure (ICP) monitoring and decompressive craniectomy (DC) in patients with severe TBI. This intersociety document proposes best practice guidelines for this subsetting of patients to be adopted on a national Italian level, along with joint statements from "TBI Section" of the Italian Society of Neurosurgery (SINch) endorsed by the Neuroanesthesia and Neurocritical Care Study Group of the Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI). Presented here is a recap of recommendations on management of ICP and DC supported a high level of available evidence and rate of agreement expressed by the assemblies during the more recent consensus conferences, where members of both groups have had a role of active participants and supporters. The listed recommendations have been sent to a panel of experts consisting of the 107 members of the "TBI Section" of the SINch and the 111 members of the Neuroanesthesia and Neurocritical Care Study Group of the SIAARTI. The aim of the survey was to test a preliminary evaluation of the grade of predictable future adherence of the recommendations following this intersociety proposal. The following recommendations are suggested as representing best clinical practice, nevertheless, adoption of local multidisciplinary protocols regarding thresholds of ICP values, drug therapies, hemostasis management and perioperative care of decompressed patients is strongly recommended to improve treatment efficiency, to increase the quality of data collection and to provide more powerful evidence with future studies. Thus, for this future perspective a rapid overview of the role of the multimodal neuromonitoring in the optimal severe TBI management is also provided in this document. It is reasonable to assume that the recommendations reported in this paper will in future be updated by new observations arising from future trials. They are not binding, and this document should be offered as a guidance for clinical practice through an intersociety agreement, taking in consideration the low level of evidence.

Abstract

Cholestasis in extremely premature infants (EPI) constitutes a nutritional challenge and maltodextrins have been reported as a possible strategy for hypoglycaemia. We aim to describe the nutritional management of an EPI with non-syndromic bile duct paucity (NSBDP) and feeding intolerance.A patient, born at 27 weeks of gestational age, presented cholestatic jaundice at 20 days of life with a clinical picture of NSBDP. Patient's growth was insufficient with formula rich in medium-chain triglyceride (MCT) and branched-chain amino acids (BCAA). Due to frequent fasting hypoglicemic episodes, maltodextrins supplements were provided. He subsequently presented severe abdominal distension and painful crises, which required hospital admission and withdrawal of maltodextrins. Hypercaloric extensively hydrolysed formula provided weight gain, glycemic control, and parallel improvement in cholestasis.Our case suggests caution with the use of maltodextrins in infants, especially if premature. Commercial preparations for hepatopatic patients contain higher concentrations of MCTs and BCAAs, but personalized strategies must be tailored to each patient.

Abstract

Hydrothermal carbonization (HTC) of sewage sludge has been investigated in this study to demonstrate its capability to segregate phosphorous element and produce a solid energy dense material, i.e. hydrochar, which could find application as solid bio-fuel and/or soil amendment. In this study, centrifuged anaerobically digested sewage sludge (total solid content: 21 wt.%) from Trento North (Italy) waste water treatment plant was hydrothermally carbonized at 190 and 210 °C at 1 and 3 h of residence time. Hydrochars, recovered via filtration, were leached using 4 M HCl solution at room temperature to transfer phosphorous (P) and inorganic elements into the aqueous solution. Recovery of P was achieved via phosphate salt precipitation by alkalinisation, up to pH 9, of the acidic leachate using a 5 M NaOH solution. Sewage sludge and the corresponding hydrochars, before and after acid leaching, were characterized in terms of energy properties (HHV) by calorimetric analysis, P and inorganic elements content via ICP-OES. Results showed that HTC promotes segregation of P element in the hydrochar (up to 90 wt.% of the initial content on a dry basis at 190 °C, 1 h). The study demonstrated that P can be effectively recovered via precipitation via acidic leaching and subsequent alkalization with total recovery yields higher than 70 wt.%. The hydrochar residues, after leaching, showed inorganic elements content and energy properties compatible with their possible use as soil amendment in agriculture and/or as solid biofuel.

Abstract

Because of the major international-level events that have recently been held in Brazil, concerns about the sensory and hygienic-sanitary conditions of food have increased. The objective of this study was to evaluate the implementation of good handling practices in food and beverage areas of hotels, with and without outsourced professional intervention. We evaluated 19 food and beverage areas in hotels in Porto Alegre, Rio do Sul, Brazil, using a checklist that was developed by a municipal surveillance team based on existing laws for good handling practices. The evaluation was done by a skilled professional in the food safety area on two occasions, at the beginning of the study (January to May 2013) and at the end (July to November 2014), and the establishments were classified as good, regular, or poor. After the baseline evaluation, an action plan listing the noncompliance found at each location was given to those responsible for the establishments, and a period of 1 year 6 months was stipulated for improvements to be made. In the repeat evaluation, those responsible for the establishments were asked whether they had hired an outsourced professional to assist them in the improvements. The hotels showed improvement during the repeat evaluation, but a significant increase in the percentage of overall adequacy was seen only in the food and beverages areas of the 12 hotels that used the intervention of an outsourced professional. The better percentage of adequacy in establishments with outsourced professional intervention underlines the importance of an external and impartial view of routine activities in the implementation of good handling practices. Abstract

Interacting with a continuously changing environment may necessitate cancelling a programmed movement, such as a step, after an environmental change to avoid errors and possible falls. This cancellation is successful if it occurs during the few milliseconds from the Anticipatory Postural Adjustments (APA) that precede the step onset, a period in which the center of mass (CoM) and the center of pressure (CoP) can still be aligned. Here we studied the inhibition of steps by the means of Stop Signal Task (SST), a behavioral task matched with a theoretical model developed to study the behavioral and physiological correlates of simple response inhibition [1], e.g., pressing a button. Aim of the present work was investigating if and how the inhibition of a complex multi-effectors movement as in step, accomplishes the assumptions of the model developed for simpler movements and if it is they are reflected in the time evolution of CoM and CoP. 12 healthy subjects (age: 33.3±7.1 y; weight: 69.3±17 kg; height: 169.7±8.2 cm) were required to initiate 300 walks from a stationary position (Go trials; GT) in response to the onset of an ahead pointing arrow (Go signal) on a pc monitor, and to interrupt the movement when in 30% of the trials (Stop trials; ST), a road stop signal (Stop signal) replaced the arrow after a variable time, determined using a behavioral tracking staircase algorithm [2]. A stereo-photogrammetric motion analysis system (SMART-DX 6000: BTS) and eight dynamometer platforms (Kistler 9286B; Kistler) were used to detect the kinematic and dynamic parameters of gait initiation. Ad-hoc analysis algorithms were developed by the MATLAB software (R2019b 9.7; MathWorks) to track the time evolution of CoM and CoP trajectories and capture the related reaction times (RT) for GT, error stop trials (eST), and correct stop trials (cST), and to conduct the statistical analysis. Fig. 1 displays the CoM (left upper panels) and CoP (left lower panels) trajectories in GT, eST, and cST, and the computed corresponding RT (rightmost panels). A repeated measures ANOVA and post-hoc tests detected (p<0.001), for both CoM and CoP, a significantly slowest RTs in cST (CoM=375±53ms; CoP=277±64ms), faster RT in GT (CoM=335±62ms; CoP=253±70ms), and the even faster in RT during the eST (CoM=296±61ms; CoP=213±64ms). By tracking the time evolution of CoM and CoP in a step version of the SST we obtained, during APA, a measure of RT, not only in GT and eST trials, but even in cST, a parameter hidden in the typical experimental setup. With these variables, we detected that the outcome of our version of SST complied with the main assumption of the race model: a movement is successfully cancelled when a stop process wins the race being faster than a go process. Accordingly, here we observed the eST were the trials approaching the race with significantly faster RT than cST. These results provide evidence that our version of SST is a valid tool for studying inhibition of complex movements and the associated kinematic variables. [ABSTRACT FROM AUTHOR]

Abstract: Lipid profiles of fish oil extracted from trout heads, spines and viscera using supercritical carbon dioxide and Randall extraction with hexane were measured. The amount of unsaturated fatty acids (as a percentage of total fatty acids) was within the range of 72.6–75.3% in all the substrates. A significant presence of the most important omega-3 fatty acids was detected. Eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) content in oil from spines, heads and viscera resulted to be 8.7% and 7.3%, 7.9% and 6.3%, and 6.4% and 6.0%, respectively. A low (≈3%), but worth noting, presence of lipids with omega-1 polyunsaturated fatty chains was observed in all the oils. Finally, significant differences were noticed in the relative amounts of triacylglycerides (TAG), diacylglycerides (DAG) and free fatty acids (FFA). Whereas oil from heads and spines was essentially composed of TAG (≈98%), in viscera oil the molar distribution ratio became TAG:DAG:FFA=87:8:5. [Copyright &y& Elsevier]

Abstract

The winery industry produces every year in the world about 270 millions of hectolitres of wine. A consequent amount of grape marc is then generated that has to be somehow treated and processed. For this reason a technique to treat grape marc (bio-drying) was studied and applied at the University of Trento. Grape marc, as is, is not suitable for direct combustion because of its high water content. By bio-drying the lower heating value has been increased up to the limit for a good combustion. Abstract

Large neutral amino acids (LNAA) have been used on a limited number of patients with phenylketonuria (PKU) with the purpose of decreasing the influx of phenylalanine (Phe) to the brain. In an open-label study using LNAA, a surprising decline of blood Phe concentration was found in patients with PKU in metabolic treatment centres in Russia, the Ukraine, and the United States. To validate the data obtained from this trial, a short-term double-blind placebo control study was done using LNAA in patients with PKU, with the participation of three additional metabolic centres – Milan, Padua and Rio de Janeiro. The results of the short trial showed significant lowering of blood Phe concentration by an average of 39% from baseline. The data from the double-blind placebo control are encouraging, establishing proof of principle of the role of orally administered LNAA in lowering blood Phe concentrations in patients with PKU. Long-term studies will be needed to validate the acceptability, efficacy and safety of such treatment. [ABSTRACT FROM AUTHOR]

Abstract

Optical fiber sensors are by now broadly accepted as an innovative and reliable device for structural health monitoring, to be used either embedded into or bonded on structures. The accuracy of the strain measurement achievable by optical fiber sensors is critically dependent on the characteristics of the bonding of the various interface layers involved in the sensor bonding/embedding (structure material and gluing agent, fiber coating and gluing agent, fiber coating and fiber core). In fact, the signal of the bonded/embedded optical fiber sensor must correspond to the strain experienced by the monitored structure, but the quality of each involved interface can affect the strain transfer. This paper faces the characterization, carried on by both mechanical tests and morphological analysis, of the strain transfer function resulting with epoxidic and vinylester gluing agent on polyimide and acrylate coated optical fibers.
