Introduction: Cerebrovascular accidents (CVAs), commonly known as strokes, represent a significant public health concern worldwide due to their high morbidity and mortality rates. They result from the sudden interruption of blood flow to a part of the brain, leading to neurological deficits that can range from mild to severe. Ischemic strokes, caused by the blockage of blood vessels supplying the brain, account for the majority of cases, while hemorrhagic strokes, caused by the rupture of blood vessels, constitute a smaller but equally critical subset. Among the various biomarkers studied in the context of strokes, plasma fibrinogen levels have garnered considerable attention. Elevated levels of fibrinogen are associated with increased thrombotic potential, potentially contributing to the pathophysiology of ischemic strokes. Materials And Methods: This is a hospital based Prospective analytical study was conducted in Department of General Medicine at Sapthagiri Institute of Medical Sciences and Research Centre, Bengaluru, Karnataka over a period of 1 year. Total 50 adult patients who are getting admitted in medical wards and acute medical care with first episode of stroke and full filling the Inclusion and exclusion criteria. Plasma Fibrinogen level estimation will be carried between 24-72 hours after onset of Cerebro Vascular accidents. After completion of selection of patients, written consent is taken from the patients or their attender’s, detailed history was taken, Physical examination was done, subjects are investigated with routine blood tests like CBC, Blood Urea, Serum creatinine, Random blood sugar, CRP, urine Routine Microscopy, ECG, carotid doppler, Lipid profile, CT BRAIN, MRI BRAIN done in selected cases. Results: In the present study, Ischemic stroke is seen in 68% of patients and Hemorrhagic stroke in 32% of patients. It shows that ischemic stroke is more common than hemorrhagic stroke. Out of 34 patients of ischemic stroke,22(65%) showed their plasma fibrinogen levels ≥350 mg/dl4. Of them, 2 patients showed higher plasma fibrinogen levels between 450-549 mg/dl. Normal plasma fibrinogen levels are seen in 12 patients (35%). Higher fibrinogen levels are seen in 9 out of 16 patients (56%) in haemorrhagic stroke group. 7 patients showed normal fibrinogen level. Out of 50 patients, 7 patients expired and 43 patients survived. Among deceased 7 patients, 4 patients had ischemic stroke and 3 patients had hemorrhagic stroke. Conclusions: Stroke due to ischemia (68%) is more common than stroke due to haemorrhage (32%). Elevated fibrinogen level is one of the independent risk factors for stroke. Fibrinogen levels are slightly higher in ischemic stroke individuals compared to hemorrhagic stroke but not statistically significant. All non survivors have higher fibrinogen levels. Resumen: Objetivo: encontrar una relación entre los niveles de proteína C reactiva (PCR) y fibrinógeno, y la extensión de la aterosclerosis en el síndrome coronario agudo. Métodos: estudio observacional prospectivo, en el que se incluyeron 873 pacientes con síndrome coronario atendidos en un hospital entre 2016 y 2018. Se analizaron niveles de PCR y fibrinógeno, marcadores metabólicos y extensión de la aterosclerosis coronaria. Resultados: no se halló correlación positiva entre los niveles de PCR y fibrinógeno y los marcadores metabólicos, así como tampoco con enfermedad de uno, dos y tres vasos (p 0,829; p 0,810). Conclusiones: los niveles sanguíneos de PCR y fibrinógeno se relacionan con la tasa de eventos cardiovasculares, pero no se ha podido demostrar que exista relación entre estos y la severidad de la aterosclerosis coronaria. Abstract: Objective: To determine whether there is a relationship between C - reactive protein and fibrinogen levels and the extent of atherosclerosis in acute coronary syndrome. Methods: A prospective observational study was conducted that include 873 patients with coronary syndrome treated in a hospital between the years 2016 and 2018. An analysis was made that included C - reactive protein and fibrinogen levels, metabolic markers, extent of coronary atherosclerosis. Results: No positive correlation was found between the C - reactive protein and fibrinogen levels and the metabolic markers, nor with one, two, or three vessel disease (P = .829; P = .810). Conclusions: Although blood C-Reactive Protein and fibrinogen levels are associated with the rate of cardiovascular events, this study was unable to demonstrate whether there is a relationship between these and the severity of the coronary atherosclerosis.

Introduction. Cirrhotic patients have been considered for decades to have a pro-haemorrhagic pattern and were treated as such based on the results from standard coagulation tests. The aim of our study was to determine the effects of platelet count and fibrinogen levels on rotational thromboelastometry (ROTEM) parameters.

Introduction: Abnormally invasive placenta (AIP) is often associated with high maternal morbidity. In surgical treatment, caesarean hysterectomy or partial uterine resection may lead to high perioperative maternal blood loss. A conservative treatment by leaving the placenta in utero after caesarean delivery of the baby is an option to preserve fertility and to reduce peripartum hysterectomy-related morbidity. Nevertheless, due to increased placental coagulation activity as well as consumption of clotting factors, a disseminated intravascular coagulation (DIC)-like state with secondary late postpartum bleeding can occur. Purpose: Systematic review after the presentation of a case of conservative management of placenta percreta with secondary partial uterine wall resection due to vaginal bleeding, complicated by local hyperfibrinolysis and consecutive systemic decrease in fibrinogen levels. Methods: Systematic PubMed database search was done until August 2019 without any restriction of publication date or journal Results: Among 58 publications, a total of 11 reported on DIC-like symptoms in the conservative management of AIP, in the median on day 59 postpartum. In most cases, emergency hysterectomy was performed, which led to an almost immediate normalization of coagulation status but was accompanied with high maternal blood loss. In two cases, fertility-preserving conservative management could be continued after successful medical therapy. Conclusion: Based on these results, we suggest routinely monitoring of the coagulation parameters next to signs of infection in the postpartum check-ups during conservative management of AIP. One of the most common manifestations in HIV infected patients are hematological. Few studies on these manifestations have been conducted in Romania and worldwide. Anemia and infections occur frequently among people living with HIV and reflect the immune suppression of the host. Besides, the etiology of anemia in HIV infection often remains unclear. Therefore, C Reactive Protein (CRP) and fibrinogen can be used as markers of the degree of immune suppression. It is noteworthy that anti-anemic therapies may accelerate the progression of HIV disease. Primary infection or reactivation of known or unknown pathogens is still a major concern associated with blood transfusions and possibly lead to a reduced prognosis. Existing data suggest that severe anemia is one of the main factors. Chronic inflammation and increased hemostasis in HIV-infected persons are thought to be a consequence of viral replication or persistence and it is associated with poor prognostic in HIV-infected patients. Whether low levels of viremia predispose to increased inflammation is unclear. We investigated the relationship between anemia, inflammatory markers, and blood transfusions. Objective: Since IL-6 has been associated with activation of the coagulation cascade and upregulation of fibrinogen transcription, we retrospectively tested the hypothesis that patients with rheumatoid arthritis (RA) treated with tocilizumab (TCZ) may lose more blood when undergoing total knee arthroplasty (TKA). Methods: This study included 115 RA patients who underwent primary TKA and were preoperatively tested for fibrinogen levels. The blood volume of each patient was calculated using the Nadler formula, and estimated blood loss after TKA was calculated as the change between pre-operative and postoperative hematocrits. If salvaged blood was reinfused, the volume was measured and added to the volume of the estimated blood loss. Results: We observed that patients treated with TCZ had significantly lower pre-operative fibrinogen levels than those not treated with TCZ (190.0mg/dL versus 347.0 mg/dL, respectively; p=.00018). We also observed a statistically significant increase in mean total volume of estimated blood loss after TKA in RA patients who had been treated with TCZ compared with those not treated with TCZ (797.1mL versus 511.4 mL, respectively; p =.0039). Conclusion: TCZ treatment in patients with RA may increase the risk of blood loss after TKA because of decreased fibrinogen levels. [ABSTRACT FROM AUTHOR]

Objective: To investigate the correction of plasma D-dimer, fibrinogen levels with the traumatic degree of patients with orthopedic trauma. Methods: One hundred and eighty patients with orthopedic trauma in our hospital from January 2015 to January 2016 were selected as the observation group, and one hundred healthy people in the same period were chosen as the control group. The plasma D-dimer and fibrinogen levels from 1st to 10th day after admission were compared, and the correction of plasma D-dimer (DD), fibrinogen (FIB) levels with the traumatic degree were analyzed. Results: The DD and FIB levels in observation group were obviously lowered, which were significantly higher than those of the control group (P<0.05). The DD and FIB levels in serious injury group and moderated injury group were much higher than those of the slight injury group, and those in serious injury group was the highest (P<0.05). And DD and FIB levels in the three groups had a remarkable decrease from 1st to 10th day (P<0.05). Additionally, plasma D-dimer and fibrinogen levels were significantly and positively correlated with the orthopedic traumatic degree (r=0.64, P=0.003;r=0.71, P=0.002). Conclusion: The levels of plasma DD and FIB in patients with orthopedic trauma were markedly higher than those of the healthy people, which were significantly and positively correlated with the orthopedic traumatic degree and could be served as a biomarker on assessment of traumatic degree and prognosis. The study included 101 patients with acute Q-wave infarction. Standard examination was accompanied by assessment of myocardial repolarization dispersion assessment (Day 1 and Day 7) and Chlamydia pneumoniae and Cytomegalovirus infections. Increased fibrinogen level and Cytomegalovirus infection activity were independent risk factors of ventricular repolarization asynchrony.
