The aim of this study was to identify haemodynamic changes in adult patients transported by the Helicopter Emergency Medical Service. Methods: A systematic review of studies published between January 2013 to April 2023 was conducted following the PRISMA 2020 guidelines criteria in the Pubmed, Scopus and Web of Science electronic databases. Methodological quality was assessed using the critical appraisal tool for non-randomised studies of the Joanna Briggs Institute (JBI). The followed protocol has been registered in the International Prospective Register of Systematic Reviews (PROSPERO) with code CRD4202222355798. Two independent reviewers read and extracted the information of the studies. Results: Eight studies were included in the review, which showed significant haemodynamic changes during transport by HEMS. All studies recorded readings at three points of the mission: pre-flight, in-flight, and post-flight. The greatest change in physiological variables is visible in oxygen saturation, during the in-flight phase, with a decrease in this value. Blood pressure mainly increased in various phases of the mission, especially in the in-flight phase. Heart rate also changed across the mission phases, mainly in the pre-flight and post-flight phases, increasing and decreasing during the flight. Conclusion: Patients transported by helicopter undergo haemodynamic changes during the different stages of evacuation (pre-flight, in-flight, and post-flight). However, there is a need for further studies on helicopter transport of patients due to the paucity of publications on this topic. Keywords: Air rescue, haemodynamics, patient transport, heart rate, oxygen saturation, blood pressure, haemodynamic monitoring

This paper addresses the historical development of the dis-courses and symbols that shaped the republican and national ideology around the political project known as the "Chilean road to socialism" promoted by the Popular Unity (UP) gov-ernment. The hypothesis argues that this ideology played a fundamental role in the process of ideological entrenchment and democratic legitimacy that contributed to the triumph of the left-wing parties in 1970. On the one hand, this ideology allowed the UP to fit in with the atmosphere of cultural na-tionalism of the period. On the other, it served to justify the democratic and institutional possibilities of the Chilean revo-lution. The analysis follows the development of these ideas in the Communist and Socialist parties between 1936 and 1973, paying attention to the particularities, common elements and divergences registered in the debate of both organisations. This period began with the formation of the Popular Front, as a milestone that prompted the left to adopt a republican and patriotic discourse, which was maintained over time until it was fully integrated into the political project of the UP. The study concludes that despite the Marxism and international-ism of the Chilean left, the appeal to republican and national history became constituent elements of their revolutionary project, which they outlined as a continuation of the inde-pendence process initiated in Chile in 1810.

Multimode silicon photonics, leveraging mode-division multiplexing technologies, offers significant potential to increase capacity of large-scale multiprocessing systems for on-chip optical interconnects. These technologies have implications not only for telecom and datacom applications, but also for cutting-edge fields such as quantum and nonlinear photonics. Thus, the development of compact, low-loss and low-crosstalk multimode devices, in particular mode exchangers, is crucial for effective on-chip mode manipulation. This work introduces a novel mode exchanger that exploits the properties of subwavelength grating metamaterials and symmetric Y-junctions, achieving low losses and crosstalk over a broad bandwidth and a compact size of only 6.5 µm × 2.6 µm. The integration of SWG nanostructures in our design enables precise control of mode exchange through different propagation constants in the arms and metamaterial, and takes advantage of dispersion engineering to broaden the operating bandwidth. Experimental characterization demonstrates, to the best of our knowledge, the broadest operational bandwidth covering from 1,420 nm to 1,620 nm, with measured losses as low as 0.5 dB and extinction ratios higher than 10 dB. Enhanced performance is achieved within a 149 nm bandwidth (1,471–1,620 nm), showing measured losses below 0.4 dB and extinction ratios greater than 18 dB.

Due to its resistance to multiple drugs, it is challenging to treat, which can result in serious illness and death. Conventional treatments for infected wounds often involve the topical or systemic application of antibiotics, which can lead to systemic toxicity and the development of drug resistance. The combination of wound dressings that promote wound healing with nanoparticles (NPs) represents a revolutionary strategy for optimizing the safety and efficacy of antibiotics. This review assesses a systematic search to identify the latest approaches where the evaluation of wound dressings loaded with antibiotic NPs is conducted. The properties of NPs, the features of wound dressings, the antimicrobial activity and biocompatibility of the different strategies are analyzed. The results indicate that most research in this field is focused on dressings loaded with silver NPs (57.1%) or other inorganic materials (22.4%). Wound dressings loaded with polymeric NPs and carbon-based NPs represent 14.3% and 6.1% of the evaluated studies, respectively. Nevertheless, there are no clinical trials that have evaluated the efficacy of NPs-loaded wound dressings in patients. Further research is required to ensure the safety of these treatments and to translate the findings from the bench to the bedside. The majority of the samples were cryopreserved prior to cancer treatment (460/600, 77%, median 5 days before treatment). Semen quality had a predisposed reduction in those collected from adolescents with testicular and/or hematological malignancies. Analyses of the 140 (23%) samples cryopreserved after treatment initiation (median of 84 days) revealed decreased median concentration and motility following high gonadotoxic-risk treatment. Rate of oligoasthenozoospermia was comparable in samples collected prior to treatment with those provided during follow-up spermiograms within 1 year after treatment initiation (45.5% vs 45.5%). However, an increase was seen in samples collected 1– 2 (9.1% to 90.9%) and 2– 3 (50.0% to 100.0%) years after treatment initiation. Conclusion: Cancer diagnosis and treatment may impair spermatogenesis; therefore, patient counseling prior to cancer treatment by an oncologist and/or fertility specialist is crucial. Keywords: cancer, fertility preservation, adolescent, men

This article aims to analyse the asset-backing risk of stablecoins, focusing on international accounting standards, classification criteria, and auditing standards and using Tether as a case study. It examines Tether’s issuance, backing, controls, ratios, and regulations to assess risk transmission and mitigation. The results suggest a need for unified and strengthened accounting and auditing standards to enhance user confidence. Liquidity, solvency, and debt ratios were applied to Tether’s balance sheets; while Tether has made efforts to increase its transparency, and although it possesses highly liquid assets, challenges remain regarding its liquidity, solvency, and debt. An independent auditors’ valuation is crucial for investor confidence, demonstrating that more specific regulations are required for stablecoins. Future research should explore other stablecoins to comprehensively understand the accounting and auditing challenges in the field.

In this research, we compared thermochronological and traditional one-dimensional thermal models across the Middle Magdalena Valley (MMV), Eastern Cordillera, and Eastern Llanos Basins in Colombia. These models allow us to relate cooling/heating histories to exhumation events, subsidence, fault activities, and fluid flow. The Eastern Cordillera and Eastern Llanos Basins are discriminated as prospective zones for hydrocarbon (HC) generation. The De Armas Syncline at the MMV basin shows high thermal maturity (127°C), describing rocks completely depleted in terms of HC generation. The eastern zone of this syncline is buried until it reaches temperatures above 150°C. The contrast between thermal models in the Eastern Cordillera basin suggests a possible sediment supply scenario from the west to the east of the Soapaga Fault. In the Eastern Llanos basin, the thermal histories between both models explain the latest uplift and sediment supply processes from the hinterland zone of the Eastern Cordillera to the foothill zone of the Eastern Llanos basin. In terms of thermal evolution, the three basins are different, reaching different levels of burial and HC maturity. However, in terms of cooling events related to exhumation, the basins have similarities as they are controlled by the mountain building of the Eastern Cordillera of Colombia.

In this work, we define the notions of Wronskian and simplified Wronskian for Stieltjes derivatives and study some of their properties in a similar manner to the context of time scales or the usual derivative. Later, we use these tools to investigate second-order linear differential equations with Stieltjes derivatives to find linearly independent solutions, as well as to derive the variation of parameters method for problems with gg-continuous coefficients. This theory is later illustrated with some examples such as the study of the one-dimensional linear Helmholtz equation with piecewise-constant coefficients.

Background: Few population-based studies have evaluated the epidemiology of infective endocarditis (IE). Changes in population demographics and guidelines on IE may have affected both the incidence and outcomes of IE. Therefore, the aim of our study is to provide contemporary population-based epidemiological data of IE in Spain. Methods: Retrospective nationwide observational study using data from the Spanish National Health System Discharge Database. We included all patients hospitalized with IE from January 2000 to December 2019. Results: A total of 64,550 IE episodes were included. The incidence of IE rose from 5.25 cases/100,000 person-year in 2000 to 7.21 in 2019, with a 2% annual percentage change (95% CI 1.3–2.6). IE incidence was higher among those aged 85 or older (43.5 cases/100.000 person-years). Trends across the study period varied with sex and age.Patients with IE were progressively older (63.9 years in 2000–2004 to 70.0 in 2015–2019, p

Urbanization and agricultural activities are increasingly threatening karstic systems and the water resources they provide, which are crucial for the livelihood of rural communities. Over the past decade, the Río Grande de Comitán-Lagos de Montebello (RGC-LM) watershed in Chiapas, Mexico, has experienced significant deterioration in water quality. The objective of this study was to analyze the water quality dynamics within the RGC-LM watershed and their potential interaction with land use change. We conducted assessments of the physicochemical and microbiological parameters across the lotic and lentic systems within the watershed over eight monitoring campaigns (from 2013 to 2020). Cluster Analysis (CA) and Principal Component Analysis (PCA) statistical approaches were performed to identify the main drivers of surface water quality variability. This approach aims to enhance water quality assessments within environmental management strategies, with the goal of promoting sustainable land-use practices aimed at protecting and improving water resources within the watershed. Our findings indicate that there is a substantial difference between the upper and lower watershed water quality. The good water quality conditions observed in the higher altitude lakes (mountain lakes) can be attributed to favorable topographical features, specific land use patterns, and effective conservation measures. In contrast, the lotic system in the middle watershed and the lentic water bodies located at lower altitudes (plain lakes) exhibit signs of water quality degradation due to inefficient wastewater treatment and agricultural runoff. This study highlights the importance of implementing adequate environmental management strategies to address these water quality challenges effectively. Given the pressing issue of water quality degradation in karstic environments, we emphasize on the need for increased monitoring frequency of both physicochemical and microbiological parameters to capture seasonal variations and to further understand the vulnerability of water resources in karstic environments. Future assessment of nutrient concentrations, pesticide levels, and blue-green algae populations will also be crucial for evaluating the trophic state at a watershed scale.

In this paper, we consider, from a numerical point of view, a two-temperature poro-thermoelastic problem. The model is written as a coupled linear system of hyperbolic and elliptic partial differential equations. An existence result is proved and energy decay properties are recalled. Then we introduce a fully discrete approximation by using the finite element method and the implicit Euler scheme. Some a priori error estimates are obtained, from which the linear convergence of the approximation is deduced under an appropriate additional regularity. Finally, some numerical simulations are performed to demonstrate the accuracy of the approximation, the decay of the discrete energy and the behaviour of the solution depending on a constitutive parameter.

This paper presents a correlational design that explores associations among leisure attitude, self-rated health, and psychological well-being, and analyzes the potential moderating role of gender in these relationships.Patients and Methods: The sample was selected using simple random probability sampling (N= 409; 61.9% female; Mean age = 72.9; SD = 8.43¸ Range of age 53 to 93 years). Diverse sociodemographic information was collected, and leisure attitudes, self-perceived health, and psychological well-being (positive affect and emotional ties) were assessed. Results: The results revealed a positive effect of leisure attitude on self-rated health and psychological well-being, with self-rated health fully mediating the association between the behavioral component of leisure attitude and psychological well-being. Furthermore, the moderating effect of gender on the relationship between self-rated health and psychological well-being was stronger among men. Conclusion: Positive cognitive and emotional perceptions of leisure among older adults seem to benefit psychological well-being and improve self-rated health, thereby contributing to healthy ageing. Keywords: ageing, older adults, gender, attitudes to leisure, psychological well-being, emotional ties

Thanks to their exceptional spatial, spectral and temporal resolution, highly-coherent free-electron beams have emerged as powerful probes for material excitations, enabling their characterization even in the quantum regime. Here, we investigate strong light–matter coupling through monochromatic and modulated electron wavepackets. In particular, we consider an archetypal target, comprising a nanophotonic cavity next to a single two-level emitter. We propose a model Hamiltonian describing the coherent interaction between the passing electron beam and the hybrid photonic–excitonic target, which is constructed using macroscopic quantum electrodynamics and fully parameterized in terms of the electromagnetic dyadic Green’s function. Using this framework, we first describe electron-energy-loss and cathodoluminescence spectroscopies, and photon-induced near-field electron emission microscopy. Finally, we show the power of modulated electrons beams as quantum tools for the manipulation of polaritonic targets presenting a complex energy landscape of excitations.

In this regard, affectivity is of particular interest, as emotions play a fundamental role in overall mental health. The literature on COVID-19 in this population has often focused on the analysis of risk factors for mental health, while less attention has been paid to protective factors. In this paper, we propose an inclusive model of risk and protective factors for adolescent mental health through the indirect effect of negative and positive affect. Methods: This was a cross-sectional study with a Spanish community sample of 477 adolescents (Mage = 16.23, range:14 to 20 years old; 43.4% were boys; response rate: 100%). We measured the perceived hardness of confinement, worrying about contagion, quality of relationships at home, pleasant activities, affect (positive and negative), and mental health. Results: The results revealed a positive relationship between the perceived hardness of confinement and worrying about contagion with mental health, through the indirect effect of negative affect, and a positive relationship between home relationships and pleasant activities with mental health through an indirect effect of positive affect. Conclusion: Our results suggest that different types of emotions generated by the various contextual variables associated with COVID-19 may have a differential impact on mental health. These findings support the inclusive model of risk and protective factors for adolescent mental health. Practical implications, future studies, and limitations are discussed. Keywords: COVID-19, adolescents, mental health, affectivity, contextual variables

This study investigates the dose-PL-response relationship of monthly aripiprazole (AOM) and three-month paliperidone (PP3M).Methods: Observational, 52-week prospective study of patients with severe schizophrenia (CGI-S ≥ 5) treated with PP3M or AOM for at least one year before their inclusion in the study (N=68). Dose-PL relationship was determined. Subjects were included in standard-dose and high-dose (above labeled) and standard/therapeutic range-PLs and high-PLs (above range) groups. Treatment adherence, effectiveness (hospitalizations, severity), tolerability and safety were assessed. PLs and clinical response were evaluated. Results: No clear linear relationship was found between doses and PLs. In a considerable number of cases, standard doses achieved PLs above the therapeutic range. A significant clinical improvement was related to high PLs, without less safety, tolerability, or treatment compliance being involved. Clinical severity decreased more frequently in patients who received high doses and reached high PLs. Hospital admissions decreased significantly in those patients with high PLs.Conclusion: Taking into account the absence of a linear relationship between doses and PLs, the effectiveness in people with severe schizophrenia of AOM and PP3M depends on reaching high PLs, achieved with high doses, but also with standard doses in some cases, without leading to worse treatment tolerability, safety, or adherence. Keywords: schizophrenia, long-acting injectable antipsychotic, dose, plasmatic levels, effectiveness, tolerability

The probability integral transform of a continuous random variable XX with distribution function FX{F}_{X} is a uniformly distributed random variable U=FX(X)U={F}_{X}\left(X). We define the angular probability integral transform (APIT) as θU=2πU=2πFX(X){\theta }_{U}=2\pi U=2\pi {F}_{X}\left(X), which corresponds to a uniformly distributed angle on the unit circle. For circular (angular) random variables, the sum modulus 2π\pi of absolutely continuous independent circular uniform random variables is a circular uniform random variable, that is, the circular uniform distribution is closed under summation modulus 2π\pi , and it is a stable continuous distribution on the unit circle. If we consider the sum (difference) of the APITs of two random variables, X1{X}_{1} and X2{X}_{2}, and test for the circular uniformity of their sum (difference) modulus 2π\pi , this is equivalent to test of independence of the original variables. In this study, we used a flexible family of nonnegative trigonometric sums (NNTS) circular distributions, which include the uniform circular distribution as a member of the family, to evaluate the power of the proposed independence test by generating samples from NNTS alternative distributions that could be at a closer proximity with respect to the circular uniform null distribution.

The scientific committee, composed of 15 allergists, led the study and participated in the preparation of the questionnaire. Two-hundred panelists from different Spanish regions were invited to complete a 16-item questionnaire on a nine-point Likert scale covering six topic blocks. Consensus was achieved if ≥ 66.6% of panelists reached agreement or disagreement. Results: Of the 200 experts invited to participate in the Delphi process, a total of 195 (97.5%) answered the questionnaire. The panel experts reached a consensus on “agreement” on a total of 12 of the 16 (75.0%) items, covering a total of six categories: (a) patient knowledge (2 questions), (b) barriers to patient adherence (3 questions), (c) patient behavior (4 questions), (d) future actions (3 questions), (e) treatment costs (2 questions), and (f) final patient preferences (2 questions). Conclusion: This Delphi consensus study validated a set of twelve recommended questions for patients objectively assessing their preferences and suitability for the most common AIT options available. The questionnaire intends to assist allergists in making an objective, unconditioned decision regarding the best AIT option for each patient, after informing them about the different routes. Keywords: allergen immunotherapy, Delphi consensus, shared decision-making, questionnaire

In this article, we obtain a very sharp version of some singular perturbation results going back to Dancer and Hess [Behaviour of a semilinear periodic-parabolic problem when a parameter is small, Lecture Notes in Mathematics, Vol. 1450, Springer-Verlag, Berlin, 1990, pp. 12–19] and Daners and López-Gómez [The singular perturbation problem for the periodic-parabolic logistic equation with indefinite weight functions, J. Dynam. Differential Equations 6 (1994), 659–670] valid for a general class of semilinear periodic-parabolic problems of logistic type under general boundary conditions of mixed type. The results of Dancer and Hess [Behaviour of a semilinear periodic-parabolic problem when a parameter is small, Lecture Notes in Mathematics, Vol. 1450, Springer-Verlag, Berlin, 1990, pp. 12–19] and [The singular perturbation problem for the periodic-parabolic logistic equation with indefinite weight functions, J. Dynam. Differential Equations 6 (1994), 659–670] were found, respectively, for Neumann and Dirichlet boundary conditions with L=−Δ{\mathfrak{L}}=-\Delta . In this article, L{\mathfrak{L}} stands for a general second-order elliptic operator.

Our second aim was to provide a descriptive analysis of COVID-19-related outcomes among the general population.Patients and Methods: We mapped patient-level data from SIDIAP to the OMOP CDM and we performed more than 3,400 data quality checks to assess its readiness for research. We established a general population cohort as of the 1st March 2020 and identified outpatient COVID-19 diagnoses or tested positive for, hospitalised with, admitted to intensive care units (ICU) with, died with, or vaccinated against COVID-19 up to 30th June 2022.Results: After verifying the high quality of the transformed dataset, we included 5,870,274 individuals in the general population cohort. Of those, 604,472 had either an outpatient COVID-19 diagnosis or positive test result, 58,991 had a hospitalisation, 5,642 had an ICU admission, and 11,233 died with COVID-19. A total of 4,584,515 received a COVID-19 vaccine. People who were hospitalised or died were more commonly older, male, and with more comorbidities. Those admitted to ICU with COVID-19 were generally younger and more often male than those hospitalised and those who died. Conclusion: We successfully transformed SIDIAP to the OMOP CDM. From this dataset, a general population cohort of 5.9 million individuals was identified and their COVID-19-related outcomes over time were described. The transformed SIDIAP database is a valuable resource that can enable distributed network research in COVID-19 and beyond. Keywords: electronic health records, medical ontologies, secondary data use, common data model, OMOP

Conventional treatment that focuses on its etiological factors continues to be the gold standard; however, 30% of ulcers do not heal with this treatment; thus, it has been seen that the use of growth factor can be used as an adjuvant for this pathology. A literature review was carried out to evaluate the evidence from systematic reviews, meta-analyses, case studies, and quantitative studies that respond to the objective of this analysis review in the different databases with specific inclusion criteria with publications between 2002 and 2022, initially finding the topical application of the factor and later, more recently, the intralesional and perilesional application, the latter being an alternative treatment for this type of pathology and generating some recommendations for using the Factor. Keywords: chronic venous fibroblast dysfunctions, recombinant human epidermal growth factor, rhEGF, intralesional, perilesional, fibroblast dysfunctions
