Background: Physicians suffering from burnout are more likely to develop depression, substance dependence, and cardiovascular diseases, which can affect their practices. Stigmatization is a barrier to seeking treatment. This study aimed to understand the complex links between burnout among medical doctors and the perceived stigma. Methods and findings: Online questionnaires were sent to medical doctors working in five different departments of the Geneva University Hospital. The Maslach Burnout Inventory (MBI) was used to assess burnout. The Stigma of Occupational Stress Scale in Doctors (SOSS-D) was used to measure the three stigma dimensions. Three hundred and eight physicians participated in the survey (response rate: 34%). Physicians with burnout (47%) were more likely to hold stigmatized views. Emotional exhaustion was moderately correlated with perceived structural stigma (r = 0.37, P <.001) and weakly correlated with perceived stigma (r = 0.25, P = 0.011). Depersonalization was weakly correlated with personal stigma (r = 0.23, P = 0.04) and perceived other stigma (r = 0.25, P = 0.018). Conclusion: These results suggest the need to adjust for existing burnout and stigma management. Further research needs to be conducted on how high burnout and stigmatization impact collective burnout, stigmatization, and treatment delay. [ABSTRACT FROM AUTHOR]

The dependence with temperature of elastic modulus of relaxor ferroelectric ceramics is modeled with a Landau-Devonshire-type cluster theory. The effective elastic modulus obtained from experimental data of ultrasonic longitudinal velocity in PCT and PLZT ferroelectric ceramics are compared with the proposed model. This comparison shows that the model is able to reproduce the dependence with temperature of elastic modulus c very well. We obtained that as impurity concentration increases in both families, the diffuseness of the transition shows important variations, but the strengths of the couplings between the polarization and the strain remain almost unchanged. In contrast, other models assigned a change in the strengths of the couplings between the polarization and the strain to explain the diffuse transition in these compounds. [ABSTRACT FROM AUTHOR]

We present a comparative study of the magnetic flux motion in ceramic pellets made of (PrxY1-x)Ba2Cu3O7-δ as a function of their composition and morphology. Samples produced in Ar or O2 atmosphere present noticeable differences in their magnetic response that we explain in terms of their structural parameters. The material's parameters that most influence the flux dynamics are morphology and oxygen content, that change dramatically with the sintering atmosphere. Moderate changes are also observed as a function of the Pr content. Magnetic pinning efficiency is discussed in terms of intergranular couplings and effective activation energies, estimated from AC-susceptibility and magnetoresistance measurements. Abstract: Three samples of calcium modified lead titanate (Pb1−x Ca x )TiO3 with x =0.475, 0.50 and 0.55 were studied in terms of their ultrasonic properties. The samples were prepared using the solid state reaction technique. Ultrasonic attenuation and velocity were measured as a function of the temperature at 5MHz and 10MHz with X-cut quartz transducers, using the ultrasonic pulse-echo technique and Papadakis method for accuracy determination of the transit time of ultrasonic RF pulses. An equation regarding elastic–electric coupling between strain and spontaneous polarization was used in order to fit the elastic modulus. The nature of the phase transition was characterized through the diffusivity exponent obtained from this fit. Our results show that, as calcium concentration increases from 0.475 to 0.55, the behaviour of ferroelectric phase transition changes from normal to relaxor. This process is accompanied by a systematic decrease of the critical temperature. [Copyright &y& Elsevier]

Introduction Face à une demande grandissante (file active en augmentation de 200 % en 3 ans), l'objet de ce travail est de réaliser un retour d'expérience sur l'optimisation de nos pratiques pour la prise en charge du diabète gestationnel (DG) au sein de notre établissement hospitalier. Patients et Méthodes Depuis 2011, la prise en charge du DG repose sur un programme d'éducation thérapeutique, autorisé par l'Agence Régionale de Santé (ARS) Rhône-Alpes, comprenant un diagnostic éducatif individuel et un atelier collectif hebdomadaire incluant 3 séances de 45 minutes à 1 heure chacune, animées successivement par un médecin, par une infirmière pour l'apprentissage de l'auto surveillance glycémique, et par une diététicienne. Une semaine après cet atelier collectif, une évaluation individuelle des compétences acquises et des résultats glycémiques est réalisée systématiquement en consultation. Le suivi ultérieur est alors adapté et personnalisé quant à sa fréquence et à ses modalités : proposition d'un suivi par télésurveillance (transmission des résultats glycémiques par email) ou suivi par consultation en face à face. Les consultations et la télésurveillance sont réalisées soit par les médecins, soit par les infirmières cliniciennes expertes dans le cadre d'un protocole de coopération entre professionnels de santé (soumis récemment à l'ARS). Résultats Sur les 10 premiers mois de 2015, 350 patientes ont été prises en charge pour un DG et 700 actes de télésurveillance ont été réalisés. La part de la télésurveillance, plébiscitée par les patientes, est actuellement de 88 %, contre 12 % de consultations en face à face, ces dernières concernant essentiellement les patientes présentant un déséquilibre glycémique non résolu notamment pour mise à l'insuline. Une évaluation de la satisfaction des patientes actuellement en cours montre des premiers résultats très favorables quant à ce mode de suivi. Les retours des professionnels de santé libéraux sont aussi très positifs nourrissant un adressage en augmentation visible. Conclusions Notre expérience montre que la mise en place de la télésurveillance et des consultations infirmières par délégation de tâches, a permis de faire face à une forte affluence des demandes de prise en charge du DG. The aim of this study was to evaluate internal as well as external validity of the two most frequently used mania scales, the Young Mania Rating Scale (YMRS) and the Bech–Rafaelsen Mania Scale (MAS), in patients with DSM-IV mania. Mokken analysis was used to evaluate internal validity in which a coefficient of homogeneity of 0.40 or higher indicates that the total score is a sufficient statistic. The external validity was evaluated by plasma-level relationship of olanzapine. In total, 20 patients with DSM-IV mania were analysed, and the coefficient of homogeneity was acceptable for the MAS, but not for the YMRS. In a subgroup of females who over 2&ThickSpace;weeks had received a fixed dose of 20&ThickSpace;mg olanzapine daily, a significant association was found between MAS scores and plasma levels, but this association was not obtained with the YMRS. Objectif: Évaluer le pronostic ainsi que l’impact psychologique d’une prise en charge du diabète gestationnel dans la région Rhône-Alpes. Patients et méthodes: De novembre 2010 à juin 2011, 260 femmes d’âge moyen 31,6±5,3 ans avaient un diabète gestationnel à 30 [28 ; 32] SA, dont 77 % (198) avec le test OMS avec 75 g de glucose et 23 % (59) suivant le test O'Sullivan avec 100 g. Ces femmes avaient un IMC pré-gestationnel de 25 [21,8 ; 30,1]. Parmi ces 260 femmes, 57,7 % (150) avaient un antécédent familial de diabète, 26,2 % (68) une obésité familiale, 26,1 % (55) un antécédent de fausse couche spontanée. Le niveau d’anxiété (basale et aigue) a été apprécié par les questionnaires Spielberger (A et B). Résultats: Seules 6,2 % (16) femmes ont nécessité de l’insuline, alors que toutes avaient bénéficié de conseils diététiques et de la pratique d’une auto-surveillance glycémique, avec 6 mesures quotidiennes pour 75,8 % (182) d’entre elles. Lors de l’accouchement, 30 % des femmes ont bénéficié d’une césarienne avec dans 10,5 % un problème néonatal mais bénin avec dans 98,2 % un score d’apgar à 10. Le suivi métabolique 6 mois après accouchement chez 170 (59 %) femmes a révélé 2,4 % (4) femmes avec un diabète 2, et 12,9 % (22) intolérances au glucose. Les facteurs prédictifs d’une anomalie glycémique à 6 mois étaient : les antécédents familiaux d’obésité (OR = 3 [1,26 ; 7,13], p=0,013), l’âge maternel (OR : 1,09 [1,01 ; 1,18], p = 0,032), les antécédents de diabète gestationnel (OR : 3,11 [1,21 ; 7,95], p=0,018). L’annonce du diabète gestationnel chez ces femmes légère s’accompagnait d’une augmentation du niveau d’anxiété moyen (classe 46-55) à 41 %, résolutif en post-partum à 29 % sur un fond d’anxiété général (questionnaires Etat) faible. Conclusion: Cette étude qui a précédé les recommandations IADSPG, a identifié un risque obstétrical et néonatal modéré et un risque ultérieur de diabète chez les femmes dépistées. Le dépistage n’a entraîné qu’une augmentation modérée et transitoire du niveau d’anxiété grâce à une bonne collaboration multidisciplinaire et multiprofessionnelle. [ABSTRACT FROM AUTHOR]

Thirteen major depressive patients not responding to a 4-week venlafaxine 300 mg treatment were eligible for a 4-week open trial of lithium addition.
