Pseudomonas aeruginosa PLC is a virulence factor mainly involved in inflammatory and cytotoxic responses. While ammonium sulphate purification requires large amounts of bacterial suspensions and leads to low yields, production of recombinant protein in Escherichia coli is no more successful because of frequent inclusion bodies and accumulation of inactive PLC in the periplasmic space. Using an inducible system based on the glucose-repressed inv1 promoter in the yeast Schizosaccharomyces pombe, we were able to produce up to 10 IU ml−1 of pure toxin within 24 h. This work describes the first method to easily get recombinant haemolytic PLC. This method provides a powerful tool to study the mechanisms leading to its cellular toxicity. [ABSTRACT FROM AUTHOR]

Objective: Although amikacin is primarily eliminated via glomerular filtration, drug concentrations are not consistently predicted in all patients. To better describe the relationship between amikacin clearance and both age and renal function, we used a new heuristic approach involving statistical analysis of dependence.Design and Setting: Retrospective pharmacokinetic study using data from seven centres in France.Participants: 634 patients with sepsis aged between 18 and 98 years of age who received intravenous amikacin.Methods: Clearance of amikacin was modelled using the NonParametric EM algorithm for a two-compartment model (NPEM2) with intravenous infusion.Results: A total of 2499 serum amikacin determinations was available for analysis. The relationship between the clearance of amikacin and age was weak. Interestingly, the Z method, which filters data based on dependence criteria, selected data that were best fitted by a polynomial function (r = 0.90; p < 0.001). This representation of the polynomial function was similar to a previously proposed theoretical model describing covariations between the clearance of amikacin and age. However, the polynomial function applied to only 33% of the patients that were selected by the Z method. The correlation between the clearance of amikacin and renal function was also relatively low (r = 0.39). The Z method exhibited a continuous and strong dependence pattern between the clearance of amikacin and age for 49% of the patients.Conclusions: The Z methodology, which filters data using dependence criteria, confirms that age, renal function and amikacin clearance are strongly related, but only in less than half of a large sample of patients with sepsis without renal pathology. These results suggest that other variables should be taken into account in order to improve the description of the behaviour of amikacin. Aims Identification of patients with congestive heart failure at risk of sudden death remains problematic and few data are available on the prognostic implications of QT dispersion. We sought to assess the predictive value of QT dispersion for arrhythmic events in heart failure secondary to dilated cardiomyopathy or ischaemic heart disease. Methods and Results Twelve-lead ECGs calculated for QT dispersion, 24h Holter ECGs and signal-averaged ECGs were prospectively recorded in 205 heart failure patients in sinus rhythm. The 86 patients with ischaemic heart disease and the 119 with dilated cardiomyopathy were not significantly different as regards NYHA grades (51 vs 49% in grades III–IV), cardiothoracic ratio (57±7 vs 57±6%) and ejection fraction (28±8 vs 29±9%). The mean QT dispersion (66±29 vs 65±27ms), the frequency of non-sustained ventricular tachycardia (37 vs 38%) and ventricular late potentials (41 vs 40%) were not significantly different in patients with ischaemic or dilated cardiomyop-athy. QT dispersion was not significantly related to other arrhythmogenic markers. During follow-up (24±16 months), 66 patients died, 22 of them died suddenly and seven presented a spontaneous sustained ventricular tachycardia. In patients with dilated cardiomyopathy, in multivariate analysis, only a QT dispersion >80ms was an independent predictor of sudden death (RR: 4·9, 95% CI 1·4–16·8,P<0·02) and arrhythmic events (RR: 4·5, 95% CI 1·5–13·5,P<0·01). In patients with ischaemic heart disease, no studied parameter was found significantly related to sudden death or arrhythmic events. Objective To evaluate the prognostic value of arrhythmogenic markers in hypertensive patients. Design Two hundred and fourteen hypertensive patients without symptomatic coronary disease, systolic dysfunction, electrolyte disturbances or anti-arrhythmic therapy were included. Recordings were made of 12-lead standard ECGs with calculations of QT interval dispersion, 24 h Holter ECGs (204 patients), echocardiography (187 patients) and signal-averaged ECGs (125 patients). Results Baseline data: echocardiographic left ventricular hypertrophy was found in 63 patients (33·7%), non-sustained ventricular tachycardia (Lown class IV b) in 33 patients (16·2%), ventricular late potentials in 27 patients (21·6%). Mortality: after a mean follow-up of 42·4±26·8 months, global mortality was 11·2% (24 patients), cardiac mortality 7·9% (17 patients), sudden death 4·2% (nine patients). Univariate analysis: predictors of global, cardiac and sudden death were age ≥65 years, ECG strain pattern, Lown class IV b and QT interval dispersion >80 ms (P≤0·01). Left ventricular mass index was closely related to cardiac mortality (P=0·002). Multivariate analysis: only Lown class IV b was an independent predictor of global (RR 2·6, 95% CI 1·2–6·0) and cardiac mortality (RR 3·5, 95% CI 1·2–9·7). To investigate whether detection of ventricular late potentials could provide prognostic information in patients with congestive heart failure with or without bundle branch block, we prospectively obtained a signal-averaged ECG from 151 patients with congestive heart failure, using specific criteria in 57 patients with bundle branch block. Late potentials were detected in 49 patients (32·5%); their incidence was not significantly different in patients without (3 1%; 29 patients) or with bundle branch block (35%; 20 patients). Late potentials were present in 25 of 73 patients (34%) with idiopathic dilated cardiomyopathy, in 20 of 57 patients (35%) with ischaemic cardiomyopathy and in four of 21 patients (19%) with hypertensive heart disease (ns). Age, NYHA class, ejection fraction and use of amiodarone were not statistically different among patients with or without late potentials. In contrast, patients with late potentials had more past episodes of sustained ventricular tachycardia (8·2% four patients) than those without late potentials (·9% two patients).Twenty four hour ambulatory ECGs were obtained in 135 patients (89%) Non-sustained ventricular tachycardia was not correlated with the presence of late potentials found in 45 of 88 patients (51%) without late potentials and in 29 of 47 patients (62%) with late potentials (ns). The mean follow-up was 27 ± 12 months; 51 patients died, 31 from progressive congestive heart failure and 13 suddenly; seven prospectively had sustained ventricular tachycardia. The total mortality rate, the cardiac mortality rate and sudden death risk were not significantly related to the presence of late potentials; their incidence were respectively 35% (36 patients), 32% (33 patients) and 10% (10 patients) in patients without late potentials and 31% (15 patients), 23% (11 patients) and 6% (three patients) in those without late potentials. The incidence of sustained ventricular tachycardia during follow-up was 2% (two patients) in patients without late potentials and l0% (five patients) in those with late potentials.The incidence of sustained ventricular tachycardia experienced by the patients before the study or seen during follow-up was significantly increased in the presence of late potentials: 18% (nine patients) vs 2% (two patients) in the absence of late potentials (P<0·001 Removal from the study of data from patients with bundle branch block, patients with severe congestive heart failure (NYHA 3 or 4) or patients taking amiodarone did not alter these results. Congestive heart failure is frequently associated with an increase in peripheral resistance induced by stimulation of the renin angiotensin system and of the sympathetic nervous system. The clinical, biological and hemodynamic effects of Captopril (SQ 14225), an oral inhibitor of angiotensin converting enzyme, were measured in 20 patients with heart failure poorly controlled by digitalis,diuretics and vasodilator therapy. After two months of Captopril administration from 150 to 250 mg per day, NYHA functional class improved from 34 to 206 (P<0.01). Heart rate decreased from 84.20 ± 17.46 to 76.26 ± 15.86 beats/min (mean ± standard error of the mean), (P<0.05). Mean arterial pressure was not significantly reduced from 89.25 ± 17.21 to 82.70 ± 10.86 mm Hg. Systemic vascular resistance decreased from 2234 ± 560 to 1680 ± 381 dynes scm (P<0.001), and pulmonary wedge pressure fell from 20.25 ± 7.10 to 14.60 ± 5.96 mm Hg fP<0.001). Cardiac index rose from 1.91 ± 0.41 to 2.37 ± 0.55 l/min/m (P<0.001) and stroke work index also increased from 23.62 ± 9.78 to 33.35 ± 14.48 g/syst/m. Plasma renin activity increased from 2.08 ± 2.99 to 6.73 ± 3.98 ng/mlh (P<0.001), and plasma aldosterone level fell from 21 ± 135 to 10.65 ± 4.16 ng/100 ml(P<0.01). Clinical improvement remained stable in seven patients for more than six months. In all cases theoptimal dose did not need to be altered because of lachyphylaxis. Objectifs Une relation en U entre la kaliémie et la mortalité chez les insuffisants rénaux a été décrite. Chez les hypertendus, cette relation n'est pas connue. L'objectif de cette analyse est d'évaluer l'influence conjointe de la kaliémie et de la fonction rénale sur la mort subite d'origine cardiaque (MSC) chez les sujets hypertendus âgés. Méthodes À partir de la base de données INDANA (qui regroupe les données de sept essais cliniques d'intervention randomisés dans l'HTA). Trois de ces essais (STOP, SHEP et EWEPH) ont recueilli les kaliémies et les créatininémies d'inclusion des patients. Les données individuelles de 3 620 hypertendus (PAS/PAD = 176 ± 16/81 ± 14 mmHg) âgés de plus de 70 ans (76,0 ± 4,9 ans) ont été extraites. En plus de la kaliémie et de la créatininémie, 15 variables (antécédent de maladies CV (angor, infarctus du myocarde, AVC, AOMI), âge, tabagisme, uricémie, glycémie, cholestérolémie totale, sexe, BMI, PAS/PAD, FC, diabète) enregistrées au début de l'étude ont été analysées en utilisant un réseau bayésien (Netica ® 2.05). La variable à décrire était la mort subite cardiologique. La fonction rénale a été considérée altérée (IRC) si la créatinémie des hommes était > 135; et celle des femmes > 103 μmol/l. La kaliémie a été classée en 4 stades. Hypokaliémie K < 3.5, normokaliémie (K entre 3.5 et 4.5) hyperkaliémie modérée (K entre 4.5 et 5) et hyperkaliémie sévère si K > 5.0 mmol/l Rèsultats Au cours d'un suivi moyen de 3,6 annèes [2.1 à 4.4], 81 patients (2.24 %) sont dècèdès de MSC. La distance de Kullback-Leibler qui èvalue la force de la dèpendance entre les variables a rèvèlè que l'AOMI, l'âge, l'angor, les antècèdents d'infarctus du myocarde, le tabagisme et la crèatininèmie ètaient par ordre dècroissant les variables plus lièes à la MSC. L'aire sous la courbe ROC du modèle bayèsien pour prèdire la MSC ètait de 0.90 Une hypokalièmie (< 3,5 mmol/l) augmente le risque de MSC plus que l'hyperkalièmie. Abstract CO-16 – Figure Une insuffisance rènale augmente le risque de MSC quelque-soit le niveau du potassium. Une hypokalièmie associèe à une IRC augmente de 370 % le risque de MSC. Une hyperkalièmie associèe à une IRC augmente d'environ 250 % le risque de MSC. Objectifs Prédire en ligne la survie avec ou sans évènements cardiovasculaires à 2 ans des diabétiques de type 2 protéinuriques Méthodes Les données proviennent de l'étude multicentrique observation-nelle prospective ALICE – Protect : 153 néphrologues exerçant en France ont recruté, entre janvier 2010 et février 2011, 986 adultes diabétiques de type 2 ayant une protéinurie clinique et un DFG > 15 ml/mn/1,73 m 2 . 27 variables ayant trait aux caractéristiques du sujet, à son traitement, à ses antécédents, à ses caractéristiques biologiques ont été recueillies. Le modèle de prédiction a été réalisé à l'aide d'une méthode non paramétrique s'appuyant sur la théorie des réseaux bayésiens. L'algorithme de découverte d'association utilisé est de type naïf augmenté. Résultats La population à l'inclusion était constituée de 74 % d'hommes (70 ± 10 ans), ayant un eDFG (MDRD) de 40,0 ± 20,3 ml/min/1,73 m 2 , aux stades de Maladie Rénale Chronique 2 (13 %), 3a (18 %), 3b (32 %), et 4 (37 %). Les taux de mortalité et morbidité observés parmi la population évaluable (n = 729) durant le suivi de 23 mois ont été de 8,2 % pour les décès (60 patients, dont 26 décès de cause cardiovasculaire) et 24,1 % pour les évènements cardiovasculaires. Le modèle prédit à 83.9 % le devenir à 2 ans des diabétiques inclus dans l'étude. L'ajustement du modèle aux données est très statistiquement significatif, l'AUC sous la courbe ROC est à 0.78. La valeur prédictive négative est de 94.6 % et la valeur prédictive positive pour la survenue d'un événement CV est de 34 %. Introduction Les patients DT2 protéinuriques sont exposés à deux risques « compétitifs »: l’évolution vers l’insuffisance rénale terminale et le décès de cause cardiovasculaire. Nous rapportons ici l’incidence des décès et des événements cardiovasculaires et rénaux au sein d’une vaste population de DT2 protéinuriques. Patients et méthodes Étude multicentrique observationnelle prospective ; 153 néphrologues français ont recruté en 2010, 986 adultes DT2 avec protéinurie clinique (≥ 0,5 g/j) et DFGe > 15 ml/mn/1,73 m². 729 patients sont évaluables après un suivi moyen de 23 mois. Résultats À l’inclusion, 74 % d’hommes (70 ± 10 ans), DFGe moyen 40,0 ± 20,3 ml/min/1,73 m², aux stades de maladie rénale chronique 2 (13 %), 3a (18 %), 3b (32 %), et 4 (37 %). Les taux de mortalité et morbidité observés parmi la population évaluable ( n = 729) à 2 ans ont été: – 8,2 % de décès (60 patients, dont 26 décès de cause cardiovasculaire). – 16,2 % d’événements rénaux (doublement de créatininémie 12 %, début de dialyse 10 %). – 24,1 % d’événements cardiovasculaires (6,9 % ont également un événement rénal). En analyse multivariée, les facteurs prédictifs indépendants des événements cardiovasculaires et/ou rénaux sont: antécédents d’insuffisance cardiaque et d’événements vasculaires périphériques à l’inclusion, SAS appareillé, protéinurie abondante à 2 ans, score élevé de traitement antihypertenseur à 2 ans et l’absence de blocage du système rénine angiotensine à 2 ans. Discussion Les résultats de la cohorte UKPDS (britannique, ancienne, comprenant peu de patients avec néphropathie) montraient que le risque de décès dépassait systématiquement celui de progresser vers un stade plus avancé de la néphropathie diabétique. La cohorte ALICE – Protect (française, contemporaine, ne comprenant que des patients avec néphropathie) montre l’inverse, avec un risque de progression de la néphropathie largement supérieur au risque de décès. L’incidence des complications cardiovasculaires et rénales et le taux de décès sont élevés dans cette étude, mais comparables à ceux observés au cours d’essais interventionnels récents. Introduction La vitesse du déclin du DFGe (MDRD) à 2 ans chez des patients diabétiques de type 2 protéinuriques est mal connue en dehors des essais thérapeutiques. Patients et méthodes Dans cette étude multicentrique observationnelle prospective, 986 adultes DT2 avec protéinurie (≥ 0,5 g/j) et DFGe > 15 ml/mn/1,73 m² ont été inclus par 153 néphrologues français. Résultats Sept cent vingt-neuf patients (74 % des inclus) ont été suivis en moyenne 23 mois. À l’inclusion, ils étaient âgés de 70 ± 10 ans et hypertendus à 99,9 %. Le DFGe a baissé significativement de 40,6 ± 21,0 à 36,5 ± 22,0 à 1 an puis à 33,9 ± 22,0 ml/min/1,73 m² à l’issue du suivi de 23 mois. La perte relative annuelle de fonction rénale était de 4,3 % au stade 1 de la maladie rénale chronique, de 8,0 % au stade 2, de 7,2 % au stade 3a, de 8,5 % au stade 3b et a atteint 11,3 % au stade 4. La perte relative annuelle de DFGe augmentait avec la protéinurie: de 5,3 % pour une protéinurie entre 0,5 et 1 g/L (ou g/24 heures ou g/g de créat), à 7,9 % pour une protéinurie entre 1 et 3 g/L et atteint 13 % pour une protéinurie > 3 g/L. Si la PA d’inclusion était < 140/90 mmHg, la perte de DFGe était inférieure à celle observée si la PA était > 140/90 mmHg (5,5 vs 9,5 %). Discussion La perte annuelle de filtration glomérulaire (– 3,5 ml/min/an) est légèrement inférieure à celle rapportée dans RENAAL et IDNT pour des patients diabétiques protéinuriques dans le bras traité par ARA2 (déclin voisin de – 5 ml/min/an). Ce résultat est rassurant quant à la qualité du suivi des patients diabétiques en France. Cette étude permet aussi de quantifier le rôle relatif des principaux facteurs influençant la vitesse de déclin du DFG: le contrôle tensionnel, la protéinurie et le stade initial de DFG. Objectif Nous rapportons les résultats de l’étude française prospective observationnelle ALICE-PROTECT sur la fréquence d’obtention, après 2 ans de suivi néphrologique, des cibles thérapeutiques usuelles de pression artérielle (PA) et de protéinurie (Pu). Patients et méthodes En 2010, 153 néphrologues ont inclus en médiane 6 patients DT2 consécutifs avec une Pu clinique (≥ 0,5g/j) et un DFGe > 15 ml/min/1,73 m². Nous rapportons les données, après 23 mois de suivi moyen, de 729 patients (74 % des 986 inclus). Résultats À l’inclusion, les patients avaient en moyenne 70 ans, un DFGe de 40 ml/min/1,73 m². 99 % étaient hypertendus traités (75 % sous trithérapie ou plus), 33 % avaient une PA < 140/90 mmHg, 34 % avaient une Pu entre 0,5 et 1 g/j, 47 % entre 1 et 3 g/j et 19 % > 3 g/j. À 2 ans, le double objectif de PA < 140/90 et Pu < 0,5 g/j était atteint dans 11 % des cas. La Pu était < 0,5 g/j chez 21 % des patients, mais parmi eux 57 % avaient une Pu initiale faible (< 1 g/j) à l’inclusion. L’augmentation franche de la protéinurie (passant de < 1 g à > 3 g/j) était rare (2,6 % des cas). La PA était < 140/90 mmHg dans 39 % des cas, avec un score thérapeutique antihypertenseur stable (3,5). 34 % des patients ayant une PA non contrôlée ont vu leur score de traitement antihypertenseur diminuer et 22 % des patients ne recevaient aucun bloqueur du système rénine angiotensine (SRA) ( vs 9 % à l’inclusion). La fréquence du double blocage du SRA a diminué (16 % vs 22 % à l’inclusion). Conclusion Les cibles thérapeutiques de contrôle de PA et de Pu restent difficiles à atteindre malgré un traitement intensif. Les résultats de l’étude ALTITUDE expliquent certainement une diminution de l’utilisation d’un double blocage du SRA. Les récentes recommandations des agences américaines et européennes de santé devraient accentuer cette tendance, y compris pour l’association des ARA2 et des IEC.
