ObjectiveThis study aims to clinically and genetically assess 30 unrelated consanguineous Pakistani families from various ethnic backgrounds, all exhibiting features of neurodevelopmental disorders (NDDs).MethodsWe conducted clinical, genetic, biochemical, and molecular analyses on 30 consanguineous families with NDDs enrolled from various regions of Pakistan. The likely molecular causes of primary microcephaly and NDDs were identified. Detailed clinical investigations and molecular diagnoses were performed using whole exome sequencing (WES) of the proband, followed by Sanger sequencing for validation and segregation in the available family members of the affected families.ResultsWES identified likely disease-causing homozygous variants in 30 unrelated consanguineous families. Six families presented newly described variants in known NDD-related genes: ABAT (c.1439 T > G; p.Phe480Cys) [OMIM613163], SLC12A6 (c.2865_2865insT; p.Glu955Asnfs*5) [OMIM 218000], SHANK3 (c.1305-3_1,305-2delTT; p.Gln29-_Gly305del) [OMIM 606232], BCKDK (c.356_356insC; p.Gly119Alafs*24) [OMIM 614923], DDHD2 (c.2065G > T; p.Asp689Tyr) [OMIM 615033], ERCC2 (c.1255G > A; p.Glu419Lys) [OMIM 610756]. Additionally, 12 families had previously reported disease-causing variants associated with different types of NDDs: ATRX (c.109C > T; p.Arg37*) [OMIM 309580], GPR56 [ADGRG1] (c.1423C > T; p.Arg475*) [OMIM 606854], NAGLU (c.1694G > A; p.Arg565Gln) [OMIM 252920], DOLK (c.3G > A; p.Met1Ile) [OMIM 610768], GPT2 (c.815C > T; p.Ser272Leu) [OMIM 616281], DYNC1I2 (c.607 + 1G > A; p.?) [OMIM 618492], FBXL3 (c.885delT; p.Leu295Phefs25*) [OMIM 606220], LINGO1 (c.869G > A; p.Arg290His) [OMIM 618103], and ASPM (c.3978G > A; Trp1326*, c.9557C > G; p.Ser3186*, c.6994C > T; p.Arg2332*) [OMIM 608716]. All the identified variants showed segregation compatible with autosomal recessive inheritance.ConclusionIn the present study, we observed a high frequency of ASPM variants in the genetic analysis of 30 consanguineous families exhibiting features of NDDs, particularly those associated with autosomal recessive primary microcephaly. These findings contribute to studies on genotype–phenotype correlation, genetic counseling for families, and a deeper understanding of human brain function and development.

Bearings are critical components in modern manufacturing, yet they are prone to failures in induction machines. Detecting these faults early can reduce repair costs. To achieve efficient and accurate fault detection, we explore vibration-based analysis. Traditional methods rely on manual feature extraction, which is time-consuming. In contrast, our work leverages deep learning, particularly convolutional neural networks, to automatically extract fault features from raw data. We investigate various image sizes (16 × 16, 32 × 32, 64 × 64, 128 × 128, 256 × 256) and their performance in bearing fault diagnosis. Our convolutional neural networks-based approach is compared to traditional methods such as support vector machine, nearest neighbors, and artificial neural networks. Results demonstrate the superior performance of our data-driven fault diagnosis using convolutional neural networks.

Small intestine bacterial overgrowth (SIBO) has been associated with enteric inflammation, linear growth stunting, and neurodevelopmental delays in children from low-income countries. Little is known about the histologic changes or epithelial adherent microbiota associated with SIBO. We sought to describe these relationships in a cohort of impoverished Bangladeshi children. Undernourished 12-18-month-old children underwent both glucose hydrogen breath testing for SIBO and duodenoscopy with biopsy. Biopsy samples were subject to both histological scoring and 16s rRNA sequencing. 118 children were enrolled with 16s sequencing data available on 53. Of 11 histological features, we found that SIBO was associated with one, enterocyte injury in the second part of the duodenum (R = 0.21, p = 0.02). SIBO was also associated with a significant increase in Campylobacter by 16s rRNA analysis (Log 2-fold change of 4.43; adjusted p = 1.9 x 10−6). These findings support the growing body of literature showing an association between SIBO and enteric inflammation and enterocyte injury and further delineate the subgroup of children with environmental enteric dysfunction who have SIBO. Further, they show a novel association between SIBO and Campylobacter. Mechanistic work is needed to understand the relationship between SIBO, enterocyte injury, and Campylobacter. SIBO has been documented as early as 18 weeks of age using the glucose-hydrogen breath test. Other studies have linked SIBO to enteric inflammation but no work has directly examined histologic associations with the hydrogen breath test. Nutritional impairment during adolescence may result in adverse physical and reproductive health outcomes. We investigated the prevalence and determined the factors associated with underweight and overweight/obesity among ever-married adolescent girls in Bangladesh. We used Bangladesh Demographic and Health Surveys data conducted in 2004, 2007, 2011, 2014, and 2017. A total of 7040 ever-married adolescent girls aged 15–19 years were included in this analysis. Prevalence of underweight (body mass index [BMI]<18.5 kg/m2) significantly decreased from 39.53% (95% CI = 36.71, 42.43) to 23.62% (95% CI = 21.35, 26.05) during 2004–2017 (p < 0.001). However, prevalence of overweight/obesity (BMI ≥ 23 kg/m2) significantly increased from 5.9% (95% CI = 4.67, 7.43) to 22.71% (95% CI = 20.39, 25.20) during the same period (p < 0.001). The girls with higher age (OR = 0.94, 95% CI = 0.90, 0.99, p = 0.023), higher level of education (OR = 0.60, 95% CI = 0.43, 0.83, p = 0.002), and richest wealth quintile (OR = 0.78, 95% CI = 0.62, 0.98, p = 0.035) had significantly lower risk of being underweight. Adolescent girls having more than one child (OR = 1.41, 95% CI = 1.15, 1.73, p = 0.001) were more likely to be underweight. Elderly adolescents with better economic status were more at risk of being overweight/obese (OR = 2.57, 95% CI = 1.86, 3.55, p < 0.001). Girls married to skilled/unskilled workers (OR = 0.58, 95% CI = 0.44, 0.77, p < 0.001) and persons involved in small businesses (OR = 0.66, 95% CI = 0.49, 0.89, p = 0.007) had lower risk of having a high BMI. Using contraceptive (OR = 0.8, 95% CI = 0.69, 0.94, p = 0.006) was negatively associated with overweight/obese. Although prevalence of undernutrition among ever-married adolescent girls is declining, the proportion of being overweight/obese is increasing in Bangladesh warranting effective strategies to improve adolescent nutrition. There are no data on the nutritional status and dietary diversity of the pregnant and nonpregnant reproductive‐age Rohingya women who have recently shifted to the Bhasan Char Relocation Camp located on an island in the Bay of Bengal. A cross‐sectional survey was conducted in November–December, 2021 to assess the nutritional status and evaluate the dietary diversity of two vulnerable groups of the forcibly displaced Rohingya population: nonpregnant reproductive‐age women and pregnant mothers. Multivariable logistic regression was applied to identify the factors associated with nutritional impairments. Overall, 7.6% of the nonpregnant reproductive‐age women were underweight (Body Mass Index [BMI] < 18.5 kg/m2), and nearly one‐third of them had a BMI ≥ 25 kg/m2. However, 26.7% of the pregnant mothers were undernourished (BMI < 20.0 kg/m2) and almost one‐fourth of them were either overweight or obese (BMI ≥ 25.0 kg/m2). The prevalence of thinness (Mid Upper Arm Circumference [MUAC] < 23 cm) was 34.5% among pregnant mothers, and 10.1% of them were severely thin (MUAC < 21 cm). The mean (±SD) of the Women's Dietary Diversity Score (WDDS) was 3.3 (±1.1) for nonpregnant reproductive‐age women and 3.7 (±1.3) for pregnant mothers enrolled in this study. Overall, 63.8% of the nonpregnant women of childbearing age and 46% of the pregnant mothers had a low WDDS (WDDS < 4). The WDDS was found to be protective against thinness among nonpregnant reproductive‐age women (AOR = 0.61; 95% CI = 0.37, 0.93; p‐value =.03) and low BMI in pregnant mothers (AOR = 0.71; 95% CI = 0.55, 0.91; p‐value =.01). The results of this survey will assist in early recognition of the nutritional demands, and act as a guide to planning nutrition‐based programs among Rohingya reproductive‐age women relocated to the Bhasan Char Island. Background and Aim: Non‐alcoholic fatty liver disease (NAFLD) is a growing concern, affecting about 45 million of the Bangladeshi population. There is a paucity of research on the economic burden of NAFLD. The study aims to estimate the cost of illness of NAFLD in Bangladesh. Methods: In this prospective, cross‐sectional study, a total of 250 patients of NAFLD, non‐alcoholic steatohepatitis (NASH), and NASH cirrhosis were included from public and private hospitals. Costs of hospitalization, physician fees, investigation costs, expenditures on medical procedures, drugs; and nonmedical costs such as transport expenses and other informal payments (tips) were estimated. Results: The overall cost per patient per evaluation was (16.90–46 942.00) USD. The cost in public and private hospitals was 384.76 and 1146.93 USD, respectively. The cost per patient of NAFLD was 157.91 (16.90–955.08) USD, and for NASH cirrhosis was 1783.80 (422.48–46 942) USD. The cost of illness increased to USD 281.18 for diabetics and 254.52 USD for hypertensive. If all the NAFLD patients are evaluated once in healthcare settings, the projected cost will be 7.11 billion USD. In NAFLD, cost for investigations, medicines, transportation, and consultation of physicians was 49.08%, 32.41%, 11.11%, and 6.67%, respectively. Conclusions: NAFLD is causing a huge economic burden to the healthcare system. The cost of illness is increased with NASH cirrhosis. Overall, this study provides valuable insights into the economic burden of NAFLD in Bangladesh and emphasizes the several ways of intervention to reduce the cost by preventive measures and accessible healthcare for affected individuals. Malnourished children are susceptible to an increased risk of mortality owing to impaired immune functions. However, the underlying mechanism of altered immune functions and its interaction with malnutrition is poorly understood. This study investigates the immune function and evaluates the effect of a particular nutritional intervention on the immune cells of undernourished children. Stunted (LAZ <−2) and at‐risk of being stunted (length‐for‐age Z‐scores, LAZ <−1 to −2) children aged between 12 and 18 months were enrolled and were provided with the daily nutritional intervention of one egg and 150 mL cow's milk for 90 days. Peripheral blood mononuclear cells (PBMCs) were isolated at enrolment and upon completion of the intervention. Phenotypic profiles for CD3+ cells, CD4+ cells, CD8+ cells, NKT cells, and B cells were similar in both cohorts, both before and after the intervention. However, activated B cells (CD25+) were increased after nutritional intervention in the at‐risk of being stunted cohort. Several pro‐inflammatory cytokines, IL‐6, IFN‐γ, and TNF‐α, were elevated in the stunted children following the nutritional intervention. The results of the study indicate that nutritional intervention may have a role on activated B cells (CD25+) s in children who are at‐risk of being stunted and may alter the capacity of PBMC to produce inflammatory cytokines in stunted children. Abstract This study investigates mechanical properties, durability performance, non-destructive testing (NDT) characteristics, environmental impact evaluation, and advanced machine learning (ML) modelling techniques employed in the analysis of high-strength self-compacting concrete (HSSCC) incorporating varying supplementary cementitious materials (SCMs) to develop sustainable building construction. The findings from the fresh characteristics test indicate that mixes’ optimal flowability and passing qualities can be achieved using different concentrations of marble powder (MP) alongside a consistent amount of silica fume (SF) and fly ash (FA). Moreover, the incorporation of 10% MP along with 10% FA and 20% SF in HSSCC significantly improved the compressive strength by 14.68%, while the splitting tensile strength increased by 15.59% compared to the reference mix at 56 days. While random forest (RF), gradient boosting (GB), and their ensemble models exhibit strong coefficient correlation (R2) values, the GB model demonstrates more precision, indicating reliable predicted outcomes of the mechanical properties. Following subsequent testing, it has been demonstrated that incorporating SCMs improves the NDT properties of HSSCC and enhances its durability. The finer MP, SF, and FA particles enhanced microstructural performance by minimizing voids and cracks while improving the C–H–S bond. As noticed by its lower CO2-eq per MPa for SCMs, the HSSCC mix with up to 15% MP inclusion increased mechanical strength while reducing the environmental footprint, making it an eco-friendly concrete alternative.

There is lack of information on the histological characteristics of the intestinal mucosa in Bangladeshi children. Collection of intestinal biopsy samples and assessment of the histomorphological features is considered to be the traditional gold standard for diagnosis of environmental enteric dysfunction (EED). The purpose of the study was to evaluate the intestinal histological characteristics of stunted children aged between 12–18 months with possible EED. 110 children with chronic malnutrition (52 stunted with length-for-age Z score, LAZ<-2 and 58 at risk of stunting with LAZ <-1 to -2) from the Bangladesh Environmental Enteric Dysfunction (BEED) study protocol who underwent upper gastrointestinal (GI) endoscopy were selected for this study. To explore the association of EED with childhood stunting, upper GI endoscopy was done and the biopsy specimens were studied for histopathology. Villous height and crypt depth were measured and the presence and intensity of inflammatory infiltrates in the lamina propria was investigated. Bivariate analysis was performed to examine the relationship between stunting and histologic morphology. More than 90% children irrespective of nutritional status were diagnosed to have chronic non-specific duodenitis on histopathology. Half of the children from both groups had villous atrophy as well as crypt hyperplasia and lymphocytic infiltration was present in more than 90% children, irrespective of groups. However, no statistically significant difference was observed when compared between the groups. The prevalence of chronic non-specific duodenitis in Bangladeshi children, irrespective of nutritional status, was high. A significant number of these children had abnormal findings in intestinal histomorphology. Background: Severe acute malnutrition (SAM) contributes to a substantial number of child deaths globally per year. The mortality rates can be lowered markedly if guideline-based management protocol is properly followed. However, case-fatality rates in resource-poor centers remain high even after introducing the guidelines. Perhaps, the lack of adequate knowledge leading to inappropriate management by the health care providers is responsible for such burden.Objective: We aimed to evaluate health care providers' knowledge, attitude, and practice regarding the facility-based management of children with SAM in Bangladesh.Methods: This was a qualitative study where data were collected cross-sectionally from 4 district and 2 tertiary care hospitals. Twenty-six semi-structured in-depth interviews were conducted among the doctors and nurses involved in inpatient care of SAM. Twenty-eight hours of observation were done in each facility to obtain information regarding the management practices.Results: The doctors had substantial knowledge in managing children with SAM in the facilities. However, knowledge of nurses was found suboptimal when evaluated based on the national guideline. Both doctors and nurses demonstrated favorable attitude toward management of childhood SAM. Identification of SAM at the facilities was poor due to lack of practice in relation to anthropometric measurements. In addition, improper practices related to blood glucose testing, dehydration monitoring, essential micronutrient administration, and follow-up of children with SAM were observed.Conclusion: The study results underscore the importance of taking appropriate measures to enhance knowledge and ensure proper practice in relation to inpatient care of children with SAM according to the national guideline in Bangladesh. Aflatoxin can cross the blood–brain barrier, damage brain tissues, and have the potential to harm the development of the human brain. Although dietary aflatoxin exposure is common in children, there is a paucity of data on aflatoxin exposure and child developmental outcomes. The child's cognitive, motor, and language functions were assessed using the Bayley Scales of Infant and Toddler Development-III or BSID-III at the same time points. Association between exposure to aflatoxin and subtests of BSID-III were examined using mixed-effect linear regression. Aflatoxin assays were performed on 194, 167, and 163 children at 15, 24, and 36 months of age, and chronic aflatoxin exposure was detected in 20.6%, 16.8%, and 60.7% of children, respectively. Multi-variable analyses showed that aflatoxin exposure was independently related to the children's cognitive score (β: −0.69; 95% CI: −1.36, −0.02), receptive language score (β: −0.90; 95% CI: −1.62, −0.17), and expressive language score (β: −1.01; 95% CI: −1.96, −0.05). We did not observe any association between exposure to aflatoxin and the motor function of children. Chronic exposure to aflatoxin exposure was linked to reduced cognitive, expressive, and receptive language scores of the study children. Objective: To assess facility readiness and identify barriers to the facility-based management of childhood severe acute malnutrition (SAM) in public healthcare settings.Design: Qualitative methods were applied to assess readiness and identify different perspectives on barriers to the facility-based management of children with SAM. Data collection was done using in-depth interviews, key informant interviews, exit interviews and pre-tested observation tools.Settings: Two tertiary care and four district hospitals in Rangpur and Sylhet Divisions of Bangladesh.Participants: Healthcare professionals and caregivers of children with SAM.Results: Anthropometric tools, glucometer, medicines, F-75, F-100 and national guidelines for facility-based management of childhood SAM were found unavailable in some of the hospitals. Sitting and sleeping arrangements for the caregivers were absent in all of the chosen facilities. We identified a combination of health system and contextual barriers that inhibited the facility-based management of SAM. The health system barriers include inadequate manpower, rapid turnover of staff, increased workload, lack of training and lack of adherence to management protocol. The major facility barriers were insufficient space and unavailability of required equipment, medicines and foods for hospitalised children with SAM. The reluctance of caregivers to complete the treatment regimen, their insufficient knowledge regarding proper feeding, increased number of attendants and poverty of parents were the principal contextual barriers.Conclusions: The study findings provide insights on barriers that are curbing the facility-based management of SAM and emphasise policy efforts to develop feasible interventions to reduce the barriers and ensure the preparedness of the facilities for effective service delivery. Since the world's population is rising continuously, more cultivable land is being utilized for their dwellings. As a result, the amount of food supply is decreasing day by day. In order to address the food shortage, a proper plan and technological breakthroughs is must. Tomato is a kind of vegetable which has the healthy ingredients and essential for our daily food list. The proposed system suggests an IoT based tomato cultivation and pest management system, with the help of deep learning methods. In the IoT implementation, camera module and moisture sensor are used to collect images of tomato plant, soil condition respectively. Based on the moisture content, the water pump will supply the water when it necessary. Besides, the real-time images of tomato leaf will be sent to the server to identify and classify natural enemies like various insect species. In the proposed system seven types of pests are identified with the help of ten deep learning models like InceptionV3, Xception, InceptionResNetV2, MobileNet, MobileNetV2, MobileNetV3Large, MobileNetV3Small, DenseNet121, DenseNet169, DenseNet201. This study has trained with leaves and insects separately to identify whether an image from a tomato plant is insectoid or not. 458 images of pests and 912 images of leaves are utilized in the proposed architecture. The accuracy of classifying insects or leaves using DenseNet201 is 100 ​%. The highest accuracy of 94 ​% is obtained to classify the different insects using the DenseNet201 model.

Background: Severe acute malnutrition (SAM) and environmental enteric dysfunction (EED) are highly prevalent among children residing in resource-limited countries like Bangladesh. L-carnitine may play a role in improving the growth and ameliorating the EED among nutritionally vulnerable children. Objective: To investigate the role of L-carnitine supplementation on the rate of weight gain, duration of hospital stays, and EED biomarkers among children with severe acute malnutrition. Methods: This study is a double-blinded, placebo-controlled, randomized clinical trial aiming to enroll diarrheal children with SAM between 9–24 months of both sexes attending the nutritional rehabilitation unit (NRU) of Dhaka Hospital of icddr,b. It is an ongoing trial including two arms where one arm receives L-carnitine supplementation, and the other arms receive a placebo for 15 days in addition to the existing standard treatment of SAM. The primary outcome is the rate of weight gain, and the secondary outcomes include duration of hospital stay and EED biomarkers. Outcomes are assessed at baseline and 15 days of post-intervention. We hypothesize that the L- carnitine supplementation for 15 days in children with SAM will improve the rate of weight gain and biomarkers of EED. Trial registration: ClinicalTrials.gov # NCT05083637. The innovative utilization of sugarcane bagasse ash (SCBA) in producing lightweight concrete (LWC) needs to be explored experimentally and quantification with machine learning approaches. Therefore, this study aims to investigate fresh, mechanical and microstructural characteristics and machine learning modeling of incorporating SCBA as a partial substitute of cement. In this investigation, OPC was replaced partially by the weight of SCBA in the proportion of 0 %, 5 %, 10 %, 15 %, and 20 %. The influences of SCBA on the fresh characteristics of concrete were investigated through tests of the slump, compacting factor, Kelly ball, k-slump, density, and air content. In addition, for hardened properties compressive, splitting tensile, flexural strength, and static modulus of elasticity were tested at 7 and 28 days. Furthermore, a microstructural characteristic was carried out for varying content of SCBA. Different machine learning (ML) predictive models were performed utilizing artificial neural network (ANN) and random forest (RF), to predict the fresh and mechanical attributes of the LWC. It was noticed that the workability value of fresh concrete was enhanced almost by 40 % to 50 % with the incorporation of SCBA content. The hardened test results revealed that a mixture having SCBA up to 10 % content SCBA resulted in nearly 10 % to 30 % better performance to satisfy the requirement of LWC. Regarding the sustainability assessment, the less embodied CO2 (eCO2) is produced in SCBA mixes, whereas it is also economically beneficial to produce SCBA mixes. Models' efficacy was evaluated including mean absolute error (MAE), root mean squared error (RMSE), and coefficient of determination (R2). The maximum R2 and minimum RMSE came across for RF (0.989 and 1.393, respectively) which provides the well estimation of the model.

Abstract Multi‐walled carbon nanotubes (MWCNTs) were successfully synthesized and functionalized by chemical vapour deposition and acid reflux methods, respectively. Chitosan (CTS) was prepared by a chemical extraction method from waste prawn shells. Various weight fractions of functionalized multi‐walled carbon nanotubes (f‐MWCNTs) have been used as reinforcing agent in CTS biopolymer matrix. Fourier transform infrared spectroscopy analysis was done, which confirms the presence of absorption bands of the various functional groups of chitin, CTS, and MWCNTs. Raman spectra revealed the quality of MWCNTs, the extent of their functionalization, and the quality of nanocomposites. The X‐ray diffraction analysis showed the distinctive peaks for f‐MWCNTs’ and also revealed the formation of CTS/f‐MWCNTs nanocomposites. Transmission Electron Microscopy (TEM) analysis also exhibited that the CTS/f‐MWCNTs nanoparticles have a well‐defined crystalline structure. The highest coercivity and magnetization (Ms) of the CTS/5%f‐MWCNTs nanocomposite are 602 Oe and 0.1202 emu/g, respectively that have been enhanced by 3.83 and 5.27 times compared to the pure CTS respectively. It showed that the conductivity is getting higher with the addition of f‐MWCNTs in the CTS matrix. CTS/5% f‐MWCNTs composites exhibit the highest conductivity than other composites and the conductivity of CTS/5% f‐MWCNTs composite is 4.0×10−4 S/m.

Escherichia coli (E. coli) pathotypes are the most common cause of diarrhea, especially in developing countries. Environmental Enteric Dysfunction (EED) is presumed to be the result of infection with one or more pathotypes and can affect intestinal health and childhood growth. We sought to investigate the association of E. coli pathotypes infection with biomarkers of EED and nutritional status among slum-dwelling malnourished children in Bangladesh. This study comprised a total of 1050 stunted and at risk of stunting children. TaqMan Array Card assays were used to determine the presence of E. coli pathotypes in feces. Prevalence of infection with EAEC was highest (68.8%) in this cohort of children, followed by EPEC (55.9%), ETEC (44%), Shigella/EIEC (19.4%) and STEC (3.2%). The levels of myeloperoxidase and calprotectin were significantly higher in EAEC (P=0.02 and P=0.04), EPEC (P=0.02 and P=0.03) and Shigella/EIEC (P=0.05 and P=0.02) positive participants while, only calprotectin was significantly higher in ETEC (P=0.01) positive participants. Reg1B was significantly higher in participants with EAEC (P=0.004) while, neopterin levels were significantly lower in ETEC (P=0.003) and Shigella/EIEC (P=0.003) positive cases. A significant positive relationship was observed between EAEC and fecal levels of Reg1B (b = 0.28; 95% CI = 0.12, 0.43; p-value<0.001). Besides, ETEC was found to be positively and significantly associated with the levels of calprotectin (b = 0.14; 95 percent CI = 0.01, 0.26; p-value=0.037) and negatively with neopterin (b = -0.16; 95% CI = -0.30, -0.02; pvalue= 0.021). On the other hand, infection with EPEC was found to be negatively associated with length-for-age (b = -0.12; 95% CI = -0.22, -0.03; p-value=0.011) and weight-for-age (b = -0.11; 95% CI = -0.22, -0.01; p-value=0.037). The study findings suggest that infection with certain E. coli pathotypes (EAEC and ETEC) influences gut health and EPEC is associated with linear growth and underweight in Bangladeshi children. Infections and persistent immunological activation are linked to increased kynurenine (KYN) and the KYN-to-Tryptophan (TRP) or KT ratio and may be critical factors in undernutrition. We sought to determine the association between the KT ratio and adult malnutrition, as well as investigate if nutritional supplementation had any influence on the decrease of the KT ratio. A total of 525 undernourished adults aged 18–45 years were recruited and provided a nutrition intervention for 60 feeding days. TRP and KYN concentrations were determined from plasma samples using LC-MS/MS. At baseline, the median (interquartile range (IQR)) TRP, KYN and KT ratios were 24.1 (17.6, 34.3) µmol/L, 0.76 (0.53, 1.18) µmol/L and 30.9 (24.5, 41.7), respectively. Following intervention, the median (IQR) KYN and KT ratios were significantly reduced to 0.713 (0.46, 1.12) µmol/L and 27.5 (21.3, 35.8). The KT ratio was found to be inversely linked with adult BMI (coefficient: −0.09; 95% CI: −0.18, 0.004; p-value = 0.06) but not statistically significant. Additionally, Plasma CRP was correlated positively, while LRP1 was inversely correlated with the KT ratio. Our data suggest that in Bangladeshi adults, the KT ratio is not related to the pathophysiology of malnutrition but correlated with inflammatory and anti-inflammatory biomarkers, and the ratio can be reduced by a nutrition intervention. Children living in resource-limited settings often suffer from multiple micronutrient deficiencies (MMD). However, there lacks evidence on the correlates of MMD in young children. We investigated the role of diets, water, sanitation and hygiene practice, enteric infections, and impaired gut health on MMD in children at 24 months of age using data from the multi-country MAL-ED birth cohort study. Co-existence of more than one micronutrient deficiency (e.g., anemia, iron, zinc, or retinol deficiency) was considered as MMD. We characterized intestinal inflammation by fecal concentrations of myeloperoxidase (MPO) and neopterin (NEO) measured in the non-diarrheal stool samples. Bayesian network analysis was applied to investigate the factors associated with MMD. A total of 1093 children were included in this analysis. Overall, 47.6% of the children had MMD, with the highest prevalence in Pakistan (90.1%) and lowest in Brazil (6.3%). MMD was inversely associated with the female sex [OR: 0.72, 95% CI: 0.54, 0.92]. A greater risk of MMD was associated with lower vitamin C intake [OR: 0.70, 95% CI: 0.48, 0.94] and increased fecal concentrations of MPO [OR: 1.31, 95% CI: 1.08, 1.51]. This research aims to assess the rheological and mechanical characteristics of Self-compacting concrete (SCC) incorporating waste tire rubber aggregates (WRTA) as an interim substitute for coarse aggregates. However, the standard experimental modeling approach has significant obstacles when it comes to overcoming the nonlinearity and environmental susceptibility of concrete parts. Therefore, linear regression (LR) and extreme gradient boosting (XGBoost) were used as two standard single machine learning (ML) models to predict the aforementioned rubberized SCC features. In this study, conventional coarse aggregates were supplanted with WRTA at 0%, 5%, 10%, and 20% to uncover the optimal proportion of coarse aggregates substituting rubber. To find the optimum amount of WRTA to use as a substitute, the study follows the impacts of rubber on the self-compacting rubberized concrete's (SCRC) rheological and mechanical characteristics. The consequences on fresh properties were investigated by the slump flow, J-ring, and V-funnel tests, while compressive and splitting tensile strengths tests were conducted to assess mechanical properties. Increasing WRTA test outputs indicated a deterioration in workability and hardened qualities. While a 10% swapping ratio is deemed feasible for producing SCRC, optimal results were achieved by reducing environmental impacts and efficiently managing a significant volume of rubber tire waste with a 5% substitution of rubber within the coarse aggregates. The research findings indicated a noticeable decrease in fresh properties as the WRTA content increased. Notably, after 28 days, a 10% WRTA substitution led to a 34% reduction in compressive strength and a 28% decrease in splitting tensile strength, satisfying ACI standards. Furthermore, XGBoost demonstrated superior predictive performance with the highest R2 values, outperforming the LR model and affirming its efficacy in delivering more accurate predictions.

Background: Bangladesh has experienced remarkable transformation in demographic, health, and nutritional status of the population. The changes have exposed the population to a number of challenges, the detrimental effect of which on health and nutrition is likely to be increased by the coronavirus disease 2019 (COVID-19) pandemic. We provide an overview of health and nutritional challenges in Bangladesh in relation to demographic transition and the COVID-19 pandemic.Methods: We identified and reviewed recent reports, published articles, and pertinent gray literature on nutrition and food security in Bangladesh to provide historical and contextual information.Results: The review identifies the progress as well as existing burden regarding nutrition and food security in Bangladesh and highlights the challenges in the coming days in regard to population growth and the COVID-19 pandemic. The country is on track to reduce all forms of childhood undernutrition, while the proportion of nutrition-related noncommunicable diseases is rising owing to changes in dietary intake, low physical activity, and sedentary lifestyle.Conclusions: Despite remarkable progress, health and nutritional status of the population in Bangladesh faces challenges, particularly in relation to demographic transition and compounded by the COVID-19 pandemic, which require concerted attention from policymakers as well as stakeholders. Studies revealed that silencing of low-density lipoprotein receptor-related protein-1 (LRP1) expression can cause inhibition of adipogenesis in animal model and contribute to reduced body size. But there is no study that has explored the association of LRP1 with body mass index (BMI) of human adults. Therefore, the aim of this study was to investigate the relationship of LRP1 with undernutrition. A total of 270 Bangladeshi slum-dwelling adults were enrolled as case control design. Their socio-economic, demographic, anthropometric and biomedical data were collected. Plasma LRP1, C-reactive protein (CRP), alpha-1 acid glycoprotein (AGP) and ferritin levels were measured by ELISA, haemoglobin by HemoCue and zinc by atomic absorption spectrometry. The median (IQR) values of plasma LRP1 were 1673.1 (1382.5–1886.2) ng/mL in healthy participants and 707.7 (588.6–839.9) ng/mL in undernourished participants, respectively. A strong positive correlation (r = 0.70, p < 0.05) between LRP1 and BMI was found. Multivariable logistic regression analysis revealed a positive association between low plasma LRP1 (Adj. OR = 0.98, CI = 0.98, 0.99 and p < 0.05) and undernutrition. The study found that increased level of LRP1 is associated with increased BMI, whereas lower level is associated with low BMI. Background: Blastocystis spp. (Blastocystis) is a widely distributed gastrointestinal protist frequently reported in countries with tropical and sub-tropical climate. We sought to determine the factors associated with Blastocystis infection and investigate its role on biomarkers of intestinal health among slum-dwelling malnourished adults in Bangladesh. Methodology: Total 524 malnourished adults with a body mass index ≤18.5 kg/m2 were included in this analysis. Presence of Blastocystis in feces was evaluated by TaqMan Array Card assays. Principal findings: Blastocystis was tested positive in 78.6% of the participants. Prevalence of infection with atypical strains of enteropathogenic Escherichia coli (aEPEC) (56% vs. 38%, p<0.001), and Trichuris trichiura (28% vs. 15%, p-value = 0.02) was significantly greater in adults with Blastocystis, while Giardia intestinalis was significantly lower (8% vs. 14%, p-value = 0.04) in Blastocystis positive adults. Malnourished adults who were living in households with high crowding index (aOR = 2.18; 95% CI = 1.11, 4.65; p-value = 0.03), and infected with aEPEC (aOR = 2.14; 95% CI = 1.35, 3.44; p-value = 0.001) and Trichuris trichiura (aOR = 1.97; 95% CI = 1.08, 3.77; p = 0.03) were more likely to be infected with Blastocystis. A significant negative relationship was observed between Blastocystis and fecal concentrations of alpha-1 antitrypsin (β = -0.1; 95% CI = -1.7, -0.1; p-value<0.001) and Reg1B (β = -3.6; 95% CI = -6.9, -3.0; p-value = 0.03). Conclusions: The study findings suggest that the presence of Blastocystis in human intestine influences gut health and may have potential pathogenic role in presence of other pathogens. Objective: Chronic aflatoxin exposure has been associated with childhood stunting (length-for-age/height-for-age < -2 sd), while data lacks for Bangladesh, a country with substantial burden of childhood stunting. This paper examined the association between aflatoxin exposure and childhood stunting in a slum setting of Dhaka city.Design: In this MAL-ED aflatoxin birth cohort study, plasma samples were assayed for aflatoxin B1-lysine adduct (AFB1-lys) by MS at 7, 15, 24 and 36 months of age for 208, 196, 173 and 167 children to assess chronic aflatoxin exposure. Relationship between aflatoxin exposure and anthropometric measures was examined by mixed-effects logistic regression models.Setting and Participants: The study was conducted in Mirpur, Dhaka, where children were followed from birth to 36 months.Results: Prevalence of stunting increased from 21 % at 7 months to 49 % at 36 months of age. Mean AFB1-lys concentrations at 7, 15, 24 and 36 months were 1·30 (range 0·09-5·79), 1·52 (range 0·06-6·35), 3·43 (range 0·15-65·60) and 3·70 (range 0·09-126·54) pg/mg albumin, respectively, and the percentage of children with detectable AFB1-lys was 10, 21, 18 and 62 %, respectively. No association was observed between aflatoxin exposure and stunting in multivariable analyses. Factors associated with childhood stunting were age, low birth weight, maternal height, stool myeloperoxidase and number of people sleeping in one room.Conclusions: A relatively lower exposure to aflatoxin may not influence the linear growth of children. This finding indicates a threshold level of exposure for linear growth deficit and further investigation in other areas where higher concentrations of aflatoxin exposure exist. Sustainable practices in the construction sector have become a major issue due to the overuse of natural raw materials. Industrial by-products like fly ash (FA), silica fume (SF), and marble powder (MP) incorporation in concrete production will be assessed in this paper in terms of fresh, mechanical, and microstructural states. Consequently, this study proposes an in-depth investigation of SCMs applied with varied substitute cement amounts, looking at components like MP, SF, and FA. The impacts on fresh characteristics of HSSCC were assessed using slump flow, V-funnel time, T50 time, L-box ratio, and J-ring slump flow. Compressive, flexural, and splitting tensile strength tests were performed to analyze the mechanical properties. Energy-dispersive X-ray spectroscopy (EDS) and scanning electron microscope (SEM) techniques were used for the microstructural examination of HSSCC. It has been found that SCMs, including MP, FA, and SF, may enhance the characteristics of concrete. The slump flow values, which indicate the flowability and filling ability of HSSCC, were improved by a maximum of 9.6 % for 20 % SF and 10 % for both FA and MP replacement. The most promising outcomes on the basis of mechanical operation were gained by using SF, MP, and FA in HSSCC at respective percentages of 20 %, 10 %, and 10 %. This mixture revealed an increase in compressive strength of a maximum of 15 % at 56 days compared to the control mixture, which contained 10 % MP. However, the excessive addition of MP negatively impacted the strength of the concrete. The EDS examination illustrated that the ‘Ca’ to ‘Si’ proportion in HSSCC significantly affects the emergence of strength. This study will enlighten users during the preparation of high-strength self-compacting concrete (HSSCC) by replacing ordinary Portland cement (OPC) with different available supplementary cementitious material (SCM) combinations.

Rising natural resource consumption leads to increased hazardous gas emissions, necessitating the concrete industry's focus on sustainable alternatives like palm oil fuel ash (POFA) to replace cement. Also, advanced machine learning (ML) techniques can uncover previously unreported insights about the effects of POFA that may be missing from the literature. Hence, this study investigates the influence of varying concentrations of POFA on fresh and mechanical characteristics with quantifying ML approaches and microstructural performance, as well as the environmental impact of structural concrete. For this, cement substitutions of 5 %, 15 %, 25 %, 35 %, and 45 % (by weight of cement) were utilized. POFA enhanced the overall concrete workability, with slump increments ranging from approximately 9 %–55 % and compacting factor increments of 4 %–12 %. Mechanical performance of POFA concrete improved up to 25 % replacement levels, with the highest enhancements observed in compressive (4.5 %), splitting tensile (36 %), and flexural (31 %) strength, for the mix containing 15 % POFA. The finer particle size of POFA improved microstructural performance by reducing porosity, aligning with the enhanced mechanical strength. The environmental impact of POFA was assessed by measuring eCO2 emissions, revealing a potential reduction of up to 44 %. Incorporating 5 %–15 % POFA yielded ideal mechanical performance results, significantly enhancing sustainability and cost-effectiveness. Regarding the ML approach, it can be observed that a low regression coefficient (R2) contrasts sharply with the higher R2 values for the random forest (RF) and the ensemble model, indicating satisfactory precision prediction with experimental results.

Objectives: To measure the role of water, sanitation and hygiene (WASH) practices on recovery from stunting and assess the role of timing of stunting on the reversal of this phenomenon.Design: Data from the MAL-ED multi-country birth cohort study was used for the current analysis. Generalised linear mixed-effects models were used to estimate the probability of reversal of stunting with WASH practice and timing of stunting as the exposures of interest.Setting: Seven different countries across three continents.Participants: A total of 612 children <2 years of age.Results: We found that not WASH practice but timing of stunting had statistically significant association with recovery from stunting. In comparison with the children who were stunted at 6 months, children who were stunted at 12 months had 1·9 times (β = 0·63, P = 0·03) more chance of recovery at 24 months of age. And, children who were stunted at 18 months of age even had higher odds (adjusted OR = 3·01, β = 1·10, P < 0·001) of recovery than children who were stunted at 6 months. Additionally, mother's height (β = 0·59, P = 0·04) and household income (β = 0·02, P < 0·05) showed statistically significant associations with the outcome.Conclusions: The study provided evidence for the role of timing of stunting on the recovery from the phenomenon. Background: The changes of plasma retinol binding protein 4 (RBP4) level after a nutrition intervention can indicate the metabolic changes associated with the delivered intervention. Objective: We investigated the changes in plasma RBP4 level among 12- to 18-month-old children after a nutrition intervention and measured its association with subcutaneous adiposity, maternal characteristics, and inflammation. Methods: Data of 520 undernourished children (250 of them had length-for-age Z score [LAZ] <−1 to −2 and 270 had LAZ score <−2) were collected from the Bangladesh Environmental Enteric Dysfunction study conducted in Dhaka, Bangladesh. Multivariable linear regression and generalized estimation equations (GEE) modeling techniques were used to measure the association. Results: At baseline, median RBP4 level was 19.9 mg/L (interquartile range [IQR]: 7.96), and at the end of the intervention, it was 20.6 mg/L (IQR: 9.06). Percentage changes in plasma RBP4 level were not significantly associated (P >.05) with the percentage changes in child's height, weight, and subcutaneous adiposity. But maternal height (regression coefficient, β = −1.62, P =.002) and milk intake (β = −0.05, P =.01) were negatively and maternal weight was positively associated (β = 0.56, P =.03) with the changes in RBP4 levels. The GEE models revealed negative association of RBP4 levels with C-reactive protein (CRP; β = −0.14, P <.05) and α-1-acid glycoprotein (AGP; β = −0.03, P <.05). Conclusion: Children whose mothers were taller experienced less increase in plasma RBP4 level, and children whose mothers had a higher weight experienced more increase in the RBP4 level from baseline. Aims: Appropriate rehydration has always been significant in treating diarrhoeal diseases in children. Irrational antibiotic use among diarrhoeal children has remained a major public health concern. Information regarding antibiotic use in young infants suffering from diarrhoea is very limited and a unique aspect of research. We aimed to investigate the prevalence of antibiotic use in the community among 2–6 months infants with diarrhoeal illnesses and having different nutritional status. Methods: We investigated a total of 5279 infants aged 2–6 months at Dhaka hospital, International Centre for Diarrhoeal Disease Research, Bangladesh, between September 2018 and June 2019. Among them, 257 infants were suffering from severe acute malnutrition (SAM). History of taking antibiotics was ascertained by direct observation of a prescription by a physician, the bottle of antibiotic or asking the caregiver about the name of antibiotic or its price that is very close to the usual market price of an antibiotic. Results: Overall, 52% of infants received antibiotics before hospital admission. Non‐SAM infants had higher odds of receiving antibiotics (adjusted odds ratio [aOR] = 1.52, 95% confidence interval: 1.18, 1.97, P value = 0.003) compared to infants with SAM and use of antibiotics increased with age (aOR = 1.11, 95% confidence interval: 1.06, 1.17, P value<0.001). Commonly used antibiotics were azithromycin (13.3%), ciprofloxacin (7.7%), erythromycin (7.7%) and metronidazole (2.6%). The proportion of receiving ciprofloxacin was significantly lower in infants with SAM compared to their non‐SAM counterparts (2.7% vs. 7.97%, P value = 0.004). Conclusions: The study underscores the excessive use of antibiotics among diarrhoeal infants, which is already a major public health concern in low‐ and middle‐income countries. Aim: There is insufficient knowledge on the * duodenal histology and Helicobacter pylori infection in malnourished Bangladeshi children. Therefore, we attempted to explore the prevalence of H. pylori infection and duodenal histopathology in 2-year-old chronic malnourished Bangladeshi slum-dwelling children and investigate their association with dyspeptic symptoms.Methods: This cross-sectional study was conducted using the data of the Bangladesh Environmental Enteric Dysfunction study in an urban slum of Dhaka, Bangladesh. With a view to address the association of environmental enteric dysfunction (EED) with stunting, upper gastrointestinal endoscopy was performed on 54 chronic malnourished children {31 stunted [length-for-age Z-scores (LAZ) <-2] and 23 at risk of stunting (LAZ <-1 to -2)} aged between 12-24 months and the mucosal biopsies were subjected to histopathological examination after obtaining proper clinical history. Stool antigen for H. pylori (HpSA) was assessed to determine H. pylori status.Results: In all, 83.3% (45/54) of the children had histopathological evidence of duodenitis. Chronic mild duodenitis was found to be the most prevalent form of duodenitis (53.7%) in the children. Only 8.9% (4/45) of the children with duodenitis had dyspepsia (p < 0.05). The 14.8% (8/54) of the children were found positive for H. pylori infection. Logistic regression analysis revealed children positive for HpSA had significant association with dyspepsia (OR 9.34; 95% CI 1.54-56.80).Conclusions: The number of chronic malnourished children suffering from duodenitis was found to be very high. OBJECTIVES/GOALS: Full Spectrum Town Halls (FSTH) were organized to bring together patients, scientists, clinicians, advocates and others interested in Hidradenitis Suppurative (HS) – stakeholders across the spectrum of translational research – to infuse patient experiences and voices into designing HS research METHODS/STUDY POPULATION: The Rockefeller University (RU), Clinical Directors Network (CDN), and the HS Foundation organized three hybrid in-person/online HS FSTHs (2018-2023). The 2018 event focused on patients, describing the current state of HS knowledge and new research into HS biology (T2, T3). Patients shared lived experiences in testimonials and a panel discussion. Advocates described community and mental health resources (T4). The 2021 event returned research results (T2, T3) to the community and updates about treatments in the pipeline (T4). The 2023 FSTH targeted clinicians (T4) addressing a critical translational gap in HS care and presenting best practices in diagnosis, surgical, and medical treatments. RESULTS/ANTICIPATED RESULTS: Impact on science: FSTH attendees joined a Community Engagement Studio providing feedback used to refine study variables/procedures. Attendees joined research studies that advanced understanding HS mechanisms of disease, clinical course and treatment.Impact on patient experience: Participants appreciated having a forum to hear/share experiences of shame, mistrust, misdiagnosis and hope. Feedback improved practices for privacy, consenting, and returning study results. Impact on clinical practice and care: FSTH raised awareness of stigma, need for respect and empathy, and advanced learning about in disease mechanisms, effective biologics, importance of epithelial tunnels and surgical care. DISCUSSION/SIGNIFICANCE: HS is a stigmatizing, poorly understood disease. FSTHs fostered bidirectional learning among scientists, clinicians, patients and other stakeholders. Patients engaged with researchers to improve research design and participate in mechanistic and therapeutic clinical research, advancing the field.

Campylobacter jejuni (C. jejuni) is considered to be one of the most frequent causes of bacterial gastroenteritis globally, especially in young children. The genome of C. jejuni contains many proteins with unknown functions termed as hypothetical proteins (HPs). These proteins might have essential biological role to show the full spectrum of this bacterium. Hence, our study aimed to determine the functions of HPs, pertaining to the genome of C. jejuni. An in-silico work flow integrating various tools were performed for functional assignment, three-dimensional structure determination, domain architecture predictors, subcellular localization, physicochemical characterization, and protein-protein interactions (PPIs). Sequences of 267 HPs of C. jejuni were analyzed and successfully attributed the function of 49 HPs with higher confidence. Here, we found proteins with enzymatic activity, transporters, binding and regulatory proteins as well as proteins with biotechnological interest. Assessment of the performance of various tools used in this analysis revealed an accuracy of 95% using receiver operating characteristic (ROC) curve analysis. Functional and structural predictions and the results from ROC analyses provided the validity of in-silico tools used in the present study. The approach used for this analysis leads us to assign the function of unknown proteins and relate them with the functions that have already been described in previous literature.
