Introdução: O SARS-CoV-2 infecta células humanas através da interação entre sua proteína spike e enzima Conversora de Angiotenciana-2 (ACE2), que é um elemento-chave do Sistema Renina-Angiotensina-Aldosterona (SRAA). Assim, o SARS-CoV-2 pode induzir um desequilíbrio do SRAA levando, entre outros sintomas, à pneumonia grave associada à COVID-19. Nossa hipótese é que a ativação do receptor MAS pela 20-hidroxiecdisona (20E) poderia modular o braço protetor do SRAA, levando a uma taxa reduzida de insuficiência respiratória e morte precoce entre pacientes hospitalizados com COVID-19. Objetivo: Foi testar se a ativação do receptor Mas pela 20-hidroxiecdisona (20E) poderia restaurar o equilíbrio do Sistema Renina-Angiotensina-Aldosterona e reduzir a proporção de adultos hospitalizados por COVID-19 que evoluíram para insuficiência respiratória ou morte. Métodos: Estudo de fase 2/3 duplo-cego, randomizado e controlado por placebo. Randomização: 20E oral 1:1 (350 mg, duas vezes ao dia) ou placebo, até 28 dias ou até que um desfecho seja alcançado. Tendo o desfecho primário como: mortalidade ou insuficiência respiratória que requer oxigênio de alto fluxo, ventilação mecânica ou oxigenação por membrana extracorpórea. E o desfecho secundário principal como: alta hospitalar após recuperação. Resultados: 238 pacientes foram randomizados entre 26 de agosto de 2020 e 8 de março de 2022. Uma redução no risco de morte precoce ou insuficiência respiratória foi observado no dia 28. 43,8% (p = 0,0426), HR 0,44, na população ITT e HR 0,41, (p = 0,037) na população PP. No dia 28, a proporção de pacientes que receberam alta após a recuperação foi de 80,1% e 70,9% nos grupos 20E e placebo, respectivamente (diferença ajustada 11,0%, IC 95% [-0,4%, 22,4%], p = 0,0586). Eventos adversos emergentes do tratamento de insuficiência respiratória foram mais frequentes no grupo placebo (22,7% vs. 31,7%), respectivamente. Conclusão: 20E reduziu significativamente o risco de morte ou insuficiência respiratória. Esta descoberta sugere que o 20E é um importante candidato a medicamento para tratar adultos hospitalizados com sintomas respiratórios graves devido à COVID-19. Summary: Evidence is lacking on alignment of current guidance from the Region of the Americas (AMR) countries with the new guidelines for people with hepatitis B virus (HBV) infection published by the World Health Organization (WHO) in March 2024. We gathered the most updated guidance on HBV infection from organisations/societies and seven countries from AMR. Most guidelines were aligned with the new WHO recommendation to treat persons with elevated ALT and HBV-DNA levels ≥2,000 IU/ml or with HIV-coinfection, hepatocellular carcinoma family history, extra-hepatic manifestations, or immunosuppression. The new WHO 2024 guidelines introduced treatment for persistently abnormal ALT in the absence of HBV-DNA, with TDF and/or entecavir as first-line therapy. TDF in pregnant women with high HBV-DNA levels was recommended to prevent mother-to-child transmission (MTCT). These guidelines advised prophylaxis to pregnant women with positive HBsAg where HBV-DNA is unavailable. WHO 2024 and updated guidelines from most AMR countries had simplified and expanded criteria for HBV treatment and MTCT prevention.

Abstract Background Since the beginning of the COVID-19 pandemic, therapeutic options for treating COVID-19 have been investigated at different stages of clinical manifestations. Considering the particular impact of COVID-19 in the Americas, this document aims to present recommendations for the pharmacological treatment of COVID-19 specific to this population. Methods Fifteen experts, members of the Brazilian Society of Infectious Diseases (SBI) and the Pan-American Association of Infectious Diseases (API) make up the panel responsible for developing this guideline. Questions were formulated regarding prophylaxis and treatment of COVID-19 in outpatient and inpatient settings. The outcomes considered in decision-making were mortality, hospitalisation, need for mechanical ventilation, symptomatic COVID-19 episodes, and adverse events. In addition, a systematic review of randomised controlled trials was conducted. The quality of evidence assessment and guideline development process followed the GRADE system. Results Nine technologies were evaluated, and ten recommendations were made, including the use of tixagevimab + cilgavimab in the prophylaxis of COVID-19, tixagevimab + cilgavimab, molnupiravir, nirmatrelvir + ritonavir, and remdesivir in the treatment of outpatients, and remdesivir, baricitinib, and tocilizumab in the treatment of hospitalised patients with severe COVID-19. The use of hydroxychloroquine or chloroquine and ivermectin was discouraged. Conclusion This guideline provides recommendations for treating patients in the Americas following the principles of evidence-based medicine. The recommendations present a set of drugs that have proven effective in the prophylaxis and treatment of COVID-19, emphasising the strong recommendation for the use of nirmatrelvir/ritonavir in outpatients as the lack of benefit from the use of hydroxychloroquine and ivermectin.

Summary: Background: By October 30, 2022, 76,871 cases of mpox were reported worldwide, with 20,614 cases in Latin America. This study reports characteristics of a case series of suspected and confirmed mpox cases at a referral infectious diseases center in Rio de Janeiro, Brazil. Methods: This was a single-center, prospective, observational cohort study that enrolled all patients with suspected mpox between June 12 and August 19, 2022. Mpox was confirmed by a PCR test. We compared characteristics of confirmed and non-confirmed cases, and among confirmed cases according to HIV status using distribution tests. Kernel estimation was used for exploratory spatial analysis. Findings: Of 342 individuals with suspected mpox, 208 (60.8%) were confirmed cases. HIV-infected patients are at particular risk for invasive pneumococcal disease (IPD). We describe cases of IPD in people living with HIV/AIDS (PLWHA) and find associated risk factors for infection and death. Methods: A retrospective case-control study, nested in a cohort, including PLWHA with and without IPD, conducted in Brazil, 2005–2020. Controls were of the same gender/age and seen at the same time/place as cases. Results: We identified 55 episodes of IPD (cases) in 45 patients and 108 controls. The incidence of IPD was 964/100,000 person-years. A total of 42 of 55 (76.4%) IPD episodes presented with pneumonia and 11 (20%) with bacteremia without a focus and 38/45 (84.4%) were hospitalized. Blood cultures were positive in 54/55 (98.2%). Liver cirrhosis and COPD were the only factors associated with IPD in PLWHA in univariate analysis, although no associated factors were found in multivariate analysis. Penicillin resistance was found in 4/45 (8.9%). Regarding antiretroviral therapy (ART), 40/45 (88.9%) cases vs. 80/102 controls (74.1%) were in use (p = 0.07). Patients with HIV and IPD had a higher CD4 count of 267 cells/mm3 compared with the control group, in which it was 140 cells/mm3 (p = 0.027). Pneumococcal vaccination was documented in 19%. Alcoholism (p = 0.018), hepatic cirrhosis (p = 0.003), and lower nadir CD4 count (p = 0.033) were associated with the risk of death in patients with IPD. In-hospital mortality among PLWHA and IPD was 21.1%, and it was associated with thrombocytopenia and hypoalbuminemia, elevated band forms, creatinine, and aspartate aminotransferase (AST). Conclusions: The incidence of IPD in PLWHA remained high despite ART. The vaccination rate was low. Liver cirrhosis was associated with IPD and death.

Summary: Background: We evaluated in-hospital mortality and outcomes incidence after hospital discharge due to COVID-19 in a Brazilian multicenter cohort. Methods: This prospective multicenter study (RECOVER-SUS, NCT04807699) included COVID-19 patients hospitalized in public tertiary hospitals in Brazil from June 2020 to March 2021. Clinical assessment and blood samples were performed at hospital admission, with post-hospital discharge remote visits. Hospitalized participants were followed-up until March 31, 2021. The outcomes were in-hospital mortality and incidence of rehospitalization or death after hospital discharge. Kaplan–Meier curves and Cox proportional-hazard models were performed. Findings: 1589 participants [54.5% male, age=62 (IQR 50-70) years; BMI=28.4 (IQR,24.9–32.9) Kg/m² and 51.9% with diabetes] were included. The use of antiretroviral therapy has drastically improved the life quality and prognosis of people living with the human immunodeficiency virus (HIV). The risk of acute myeloid leukemia (AML) currently does not appear to be significantly increased compared to the general population. Acute promyelocytic leukemia (APL), infrequent in people with HIV, is a distinct subtype of AML with unique molecular pathogenesis, clinical manifestations, and treatment. Herein we describe a fatal case of APL hypogranular variant in an HIV-positive patient presenting with hyperleukocytosis. Also, we conducted a literature review of the ten cases reported so far.

Introdução: Pessoas com AIDS estão vulneráveis a infecções oportunistas oculares resultando em sequelas visuais e apresentações atípicas desafiadoras. Descrição do caso: Homem, 33 anos, com HIV há 9 anos em abandono de terapia antirretroviral (TAR) há 6 anos, apresentou em março/2021 cefaleia e redução súbita da acuidade visual (AV) em ambos os olhos (AO). Relatou episódio de herpes-zoster dorsal três meses antes, tratado com aciclovir. Os exames realizados à internação evidenciaram na ressonância magnética do crânio aumento do diâmetro do nervo óptico direito, carga viral (CV-HIV) igual a 87.118 cópias/mL, CD4 igual a 28 células/mm3 e antígeno criptocócico (CrAg) positivo no soro. As sorologias revelaram IgG reagente para varicela-zoster (VZV), herpes simplex vírus e citomegalovírus (CMV). O exame do líquido cefalorraquiadiano (LCR) revelou proteinorraquia de 73,7 mg/dL, glicorraquia de 53 mg%, 14 células (98% mononucleares) e CrAg negativo. A fundoscopia foi normal em AO. Foi aventada a hipótese de neurite retrobulbar por criptococose, tratada com anfotericina B deoxicolato e fluconazol por 14 dias, com melhora parcial da AV e dos parâmetros liquóricos. Recebeu alta em tratamento de consolidação com fluconazol e introdução de TAR (esquema de primeira linha). Porém, reinternou em maio/2021 com queda do estado geral, febre, cefaleia e redução da AV. À ocasião, o LCR revelou celularidade de 5 (100% mononucleares), proteinorraquia de 109,3 mg/dL, CV-HIV sérica de 180 cópias/mL e CD4 de 100 células/mm3 (6,48%). Exame oftalmológico revelou midríase fixa no olho direito e fundoscopia sugestiva de retinite por CMV. O exame neurológico foi compatível com meningoencefalite herpética e após introdução do ganciclovir, ocorreu melhora parcial da midríase e AV, possivelmente por melhora da encefalite. No entanto, o PCR multiplex no LCR foi positivo para VZV, confirmando necrose retiniana aguda por VZV, com modificação da terapia para aciclovir. Em setembro/2021, mantém supressão viral em recuperação imunológica e déficit visual grave. Comentários: Múltiplas infecções oportunistas impactam a qualidade de vida dos pacientes. Neste caso, o paciente apresentou uma forma grave de retinite por VZV, possivelmente associada a um componente de reconstituição imunológica, precedida por neurite retrobulbar criptocócica. A investigação por técnicas moleculares associada ao exame oftalmológico podem antecipar o diagnóstico e evitar a progressão da perda visual.

Both multicentric Castleman disease and Kaposi sarcoma are more frequently observed in HIV infected patients. The coexistence of these Human herpesvirus 8 related lesions, in the same tissue, has been observed, but literature reports are scant. On the other hand, the expression of HHV-8-LANA-1 is easily demonstrable by immunohistochemistry. This has been shown to be a powerful tool for the diagnosis of these entities. Cat scratch disease (CSD) is a self limited condition characterized by fever, lymph node enlargement and less often eye involvement. Central nervous system involvement by Bartonella henselae infection is possibly an important cause of morbidity; its role as an agent of aseptic meningitis is unknown. We report a case of a 40 years-old man with CSD accompanied by aseptic meningitis and neuroretinitis. Serum indirect immmunofluorescence (IFI) assays for B. henselae were positive and the cerebrospinal fluid (CSF) analysis showed mononuclear pleocytosis and increased level of protein. Serological tests for other etiologies were negative. The patient responded well to antibiotic therapy with oral doxycicline plus rifampin and in the 12th day of hospitalization evolved to total regression of the headache and partial regression of the visual loss. Clinicians should consider CSD as a differential diagnosis when assessing previously healthy patients with aseptic meningitis associated with regional lymphadenopathy and epidemiological history of feline contact.

Introdução/objetivo: O Brasil foi o segundo país mais afetado pelo surto de mpox em 2022, com maior frequência de casos entre homens cisgêneros que fazem sexo com homens. Neste surto, observou-se predomínio de lesões mucocutâneas-anogenitais e baixa letalidade (0,15%). Cerca de 38-50% dos casos ocorreram em pessoas vivendo com HIV(PVHA). A imunossupressão pelo HIV pode impactar na gravidade e na duração do quadro de mpox, o que determina a duração do isolamento. Esse estudo objetiva identificar fatores associados ao tempo até resolução das lesões entre pessoas diagnosticadas com mpox no Rio de Janeiro, Brasil. Métodos: Coorte prospectiva de casos confirmados de mpox acompanhados em centro de referência no Rio de Janeiro (jun-2022 a fev-2023). Os perfis sociodemográfico e clínico foram descritos de acordo com o status sorológico para o HIV e o grau de imunossupressão. As variáveis associadas à resolução das lesões foram identificadas por meio de modelos quartis univariados (T = 75%). Resultados: Foram acompanhados 236 casos de mpox até resolução das lesões, entre os quais 49,6% eram PVHA. PVHA reportaram menos relações sexuais 30 dias anteriores(87.3% vs 93.2%), apresentaram mais frequentemente úlceras anais (49.6% vs 23.7%) e genitais (83.8% vs 73.1%), proctite (31.6% vs 17.6%) e coinfecção com outras ISTs (42.7% vs 23.4%). A mediana de tempo até resolução das lesões foi de 24 dias, sem diferença de acordo com status para HIV (p = 0.28). PVHA com imunossupressão severa (CD4+ < 200 céls/mm3) apresentaram maior tempo para resolução das lesões quando comparadas a PVHA com CD4>200céls/mm3 e pessoas negativas para HIV, diferindo em até 79 dias (p < 0,001). Independentemente do status de HIV, infecção bacteriana secundária e acometimento de tecidos profundos estiveram associados a maior tempo de resolução das lesões de mpox, acrescentando 20 dias (p = 0.05) e 76 dias (p < 0.001), respectivamente. Conclusão: Nossos achados indicam a imunossupressão avançada pelo HIV como fator associado a cursos clínicos da mpox mais longos, podendo estender o período de transmissibilidade viral, refletindo no tempo de isolamento. Isso pode agravar questões biopsicossociais, impactando na qualidade de vida do paciente e nas medidas de saúde pública. PVHA e com imunossupressão mais severa apresentam maior vulnerabilidade na evolução da mpox, devendo ser priorizados nas estratégias profiláticas e terapêuticas.
