هدفت هذه الدراسة إلى بحث مفهوم الاندماج الاستراتيجي بشكل عام ودراسة تطبيقه في المؤسسة السورية للحبوب ومدى تحقيق الاندماج للأهداف التي قام من أجلها، وذلك في ظل التوجه الحكومي لدمج وإعادة هيكلة المؤسسات من أجل رفع كفاءة عملها وتحسين أدائها. تم إجراء الدراسة في المؤسسة السورية للحبوب- فرع اللاذقية على عينة قصدية من المديرين في الشعب والأقسام الإدارية، حيث تم توزيع 52 استبانة استرد منها 48 استبانة صالحة للتحليل الاحصائي تتضمن بنوداً تقيس استراتيجياً فعالية الاندماج الذي تم من خلال مدى تحقيق أهدافه وذلك من خلال الأبعاد الأربعة لبطاقة الأداء المتوازن: المالي، العمليات الداخلية، التعلم والنمو والعملاء. تم اتباع المنهج الوصفي التحليلي واستخدام الإحصاء الاستدلالي من خلال البرنامج الاحصائي 27 SPSS للوصول إلى النتائج. توصلت الدراسة إلى نتائج أهمها: أن تحقيق المؤسسة لبعض الأهداف المالية المخططة كتخفيض النفقات، وهناك تحسن في أداء العمليات الداخلية، إلا أن الاندماج الثقافي للموارد البشرية بين المؤسسات الثلاثة المندمجة بقي ضعيفاً ولا يشعر العاملون بالرضا. وقد أوصت الدراسة بضرورة العمل على تحسين كفاءة إدارة الموجودات ورفع معدل العائد على الاستثمار، وتحديث نظم المعلومات المستخدمة، الاهتمام بإيجاد تكامل واندماج ثقافي بين الموارد البشرية للمؤسسة السورية للحبوب.

Abstract Stimulation of the inflammatory reflex (IR) is a promising strategy for treating systemic inflammatory disorders. Recent studies suggest oral sodium bicarbonate (NaHCO3) as a potential activator of the IR, offering a safe and cost-effective treatment approach. However, the mechanisms underlying NaHCO3-induced anti-inflammatory effects remain unclear. We investigated whether oral NaHCO3’s immunomodulatory effects are mediated by the splenic nerve. Female rats received NaHCO3 or water (H2O) for four days, and splenic immune markers were assessed using flow cytometry. NaHCO3 led to a significant increase (p 0.06) in anti-inflammatory markers, including CD11bc + CD206 + (M2-like) macrophages, CD3 + CD4 + FoxP3 + cells (Tregs), and Tregs/M1-like ratio. Conversely, proinflammatory markers, such as CD11bc + CD38 + TNFα + (M1-like) macrophages, M1-like/M2-like ratio, and SSChigh/SSClow ratio of FSChighCD11bc + cells, decreased in the spleen following NaHCO3 administration. These effects were abolished in spleen-denervated rats, suggesting the necessity of the splenic nerve in mediating NaHCO3-induced immunomodulation. Artificial neural networks accurately classified NaHCO3 and H2O treatment in sham rats but failed in spleen-denervated rats, highlighting the splenic nerve's critical role. Additionally, spleen denervation independently influenced Tregs, M2-like macrophages, Tregs/M1-like ratio, and CD11bc + CD38 + cells, indicating distinct effects from both surgery and treatment. Principal component analysis (PCA) further supported the separate effects. Our findings suggest that the splenic nerve transmits oral NaHCO3-induced immunomodulatory changes to the spleen, emphasizing NaHCO3’s potential as an IR activator with therapeutic implications for a wide spectrum of systemic inflammatory conditions. Abstract Introduction People living with HIV (PLWH) have higher rates of non‐infectious comorbid diseases (NCDs) than individuals without HIV. We characterized the risk of NCDs among PLWH with undetectable viral load and persistent low‐level viraemia (pLLV) in the African Cohort Study (AFRICOS). We secondarily quantified the role of immune activation in the association between LLV and NCDs. Methods AFRICOS enrols participants in 12 clinics in Uganda, Kenya, Tanzania and Nigeria. تم في هذا البحث برمجة ومحاكاة اهتزازات الفتل المؤثرة على عمود نقل القدرة الخاص بمحطة الدفع للسفينة وتحليل تأثيرها عليه سواء في مراحل التصميم المبكرة أو أثناء الاستثمار. تم في البداية حساب ترددات الفتل الطبيعية والقسرية ورسم أنماط الاهتزاز ومخططات كامبل وتحديد سرعة الدوران الحرجة التي يحدث عندها الطنين، بهدف تحديد المواضع الحرجة المعرضة لأقصى إجهاد. تم إجراء هذه الحسابات من خلال نمذجة محطة الدفع بنظام الكتل الدوارة باستخدام برنامج الماتلاب. يسمح التنبؤ بأشكال الاهتزاز وسرع الدوران الحرجة بتحديد مجال الطنين وبالتالي اقتراح حلول تصميمية وأخرى استثمارية للتقليل من أسباب نشوء اهتزازات الفتل وتخفيف أضرارها الكارثية على نظام الدفع. لقد تم استخدام عدة طرق تحليلية لإجراء الحسابات ومقارنة هذه الطرق مع بعضها البعض. تم افتراض أن نظام الدفع نظام خطي والقوى التحريضية المؤثرة عليه هي قوى توافقية، في النهاية تم تطبيق خوارزمية الحل المقترحة على نظام دفع لسفينة تجارية حقيقية بخصائص وبارامترات معلومة، وتم دراسة وتحليل النتائج.

Objectives: HIV and malaria coinfection impacts disease management and clinical outcomes. This study investigated hematologic abnormalities in malaria-asymptomatic people living with HIV (PLHIV) in regions with differing malaria transmission. Methods: Study participants were enrolled in the African Cohort Study: two sites in Kenya, one in Uganda, and one in Nigeria. Data was collected at enrollment and every 6 months. Logistic regression estimated odds ratios for associations between HIV/malaria status and anemia, thrombocytopenia, and leucopenia. هدف هذا البحث إلى تحديد تأثير عمليات إدارة المعرفة (توليد المعرفة، تخزين المعرفة، توزيع المعرفة، تطبيق المعرفة) في الأداء الوظيفي في المصارف التجارية الخاصة في الساحل السوري. واعتمد الباحث على المنهج الوصفي التحليلي كمنهج عام للبحث، إذ تم تصميم استبانة وتوزيعها على عينة من الموظفين في جميع المستويات الإدارية في عدد من المصارف التجارية الخاصة في الساحل السورية وقد بلغ عدد أفرادها 117 موظفاً، واسترد منها 105 نسخة صالحة للتحليل الإحصائي، واستخدم الباحث البرنامج الإحصائي (SPSS) في إجراء التوصيف الإحصائي واختبار الفرضيات. وتوصل البحث إلى عدة نتائج أهمها: أن درجة تطبيق المصارف التجارية الخاصة في الساحل السوري لإدارة المعرفة متوسطة وأن مستوى الأداء الوظيفي للعاملين متوسط، وهناك تأثير معنوي إيجابي لإدارة المعرفة في الأداء الوظيفي.

هدفت الدراسة بشكل رئيس إِلى بيان تأثير نظام معالجة المعاملات بأبعاده (الأفراد، الأجهزة، البرمجيات، البيانات) في تحسين الكفاءة التشغيلية إحدى أبعاد الأداء التشغيلي في سوق الهال في محافظة اللَّاذقيَّة. قام الباحث بالاعتماد على المنهج الوصفي التحليلي لتوصيف متغيِّرات الدِّراسة وتحليلها اعتماداً على البيانات الأَولية الَّتي تم جمعها من خلال القيام بدراسةٍ ميدانية في سوق الهال في اللاذقية، وذلك بالاعتماد على استبانة تم تصميمها من خلال اطِّلاع الباحث على الدِّراسات والأَدبيَّات السَّابقة، ومن أَجل اختبار الفرضيَّات قام الباحث بتفريغ البيانات الأَوليَّة في قاعدة بيانات تم تحليلَها باستخدام برنامج SPSSإِصدار /21/. توصلت الدراسة إلى وجود علاقة معنوية بين نظم معالجة المعاملات وتحسين الكفاءة التشغيلية إحدى أبعاد الأداء التشغيلي في سوق الهال في اللاذقية؛ حيث بلغت قيمة معامل الارتباط بيرسون 0.648 وهو يدلّ على ارتباط مقبول بين نظم معالجة المعاملات والكفاءة التشغيلية. أوصت الدراسة بضرورة الاهتمام بنظم معالجة المعاملات من خلال الاهتمام بمكوناته (الأفراد، الأجهزة، البرمجيات، البيانات) نظراً لدورها المهم في تحسين الكفاءة التشغيلية.

Background Due to digitization in the medical sector, many healthcare interactions are switched to online services. This study assessed the acceptance of video consultations (VCs) in cancer care, and determined drivers and barriers of acceptance. Methods A cross-sectional online-based survey study was conducted in Germany from February 2022 to February 2023. Recruitment took place at oncology outpatient clinics, general practitioners, oncology practices and via cancer-related social media channels. Inclusion criteria were a cancer diagnosis, cancer treatment and internet access. Sociodemographic, medical data, eHealth-related data were acquired via an online assessment. The Unified Theory of Acceptance and Use of Technology (UTAUT) model was used to determine the acceptance of VC and its predictors. Results Of N = 350 cancer patients, 56.0% ( n = 196) reported high acceptance of VC, 28.0% ( n = 98) stated moderate acceptance and 16.0% ( n = 56) indicated low acceptance. عندما لا تكون الموازنة العامة مجرد بيان مالي محاسبي يعرض خطة الحكومة ونتائج أعمالها, ذلك يعني أنها تطورت لتصبح أداة مالية تقوم بإحداث أثار اقتصادية واجتماعية ترغبها الحكومة. تسعى هذه الدراسة إلى التحقق من ذل عملياً في سورية, من خلال القيام بعرض تحليلي للموازنة العامة السورية وتقويم نفقاتها وإيراداتها وإجراءا حسابات وما يُحدثه من أثر على الاستثمار. لتحقيق ذلك اتبعت الدراسة منهجية علمية تندرج في سياق المنهج الوصفي التحليلي, فاتبعت الأسلوب الإحصائي: لبرهنة الفرضيات المتعلقة بعلاقة عجز الموازنة العامة بالاستثمار. توصلت الدراسة إلى أن العجز حالة ملازمة للموازنة العامة, وأنه يترك أثراً على الاستثمار الخاص من خلال مكاملة الإنفاق العام للاستثمار الخاص وتأثير الضرائب السلبي عليه. كذلك الأمر كانت العلاقة معنوية مع الاستثمار العام. قدمت الدراسة مقترحات من شأنها تفعيل عجز الموازنة العامة كأداة مالية لتحفيز الاستثمار.

تتناول الدراسة موضوع العلاقات التركية-الأمريكية ومستقبلها في ظل إدارة الرئيس الأمريكي جو بايدن (2021-2024)، حيث ناقشت الدراسة نشأة وتاريخ العلاقات التركية-الأمريكية منذ عهد الإمبراطورية العثمانية، كما تناولت أبرز محطات تطور العلاقات بين الطرفين خلال الفترة 2000-2022، حيث شهدت تلك الفترة تطورات جيوسياسية عديدة على الصعيد الدولي والإقليمي، وتوصلت الدراسة إلى نتائج هامة متعلقة بمدى حالة التعقيد التي تشهدها العلاقات الأمريكية-التركية في ظل إدارة جو بايدن، فالخلافات التكتيكية المؤقتة تشكل سمة مميزة في العلاقات بين الطرفين، ولكن تلك الخلافات المؤقتة لم تؤدي إلى فك التحالف الاستراتيجي بين أنقرة وواشنطن، حيث شكلت تداعيات الحرب الروسية-الأوكرانية فرصة لتطوير وتقارب العلاقات بين الطرفين، وتطرح الدراسة في ختامها ثلاث سيناريوهات أساسية تشرح مستقبل العلاقات الأمريكية-التركية في ظل إدارة جو بايدن.

Prior immune responses to coronaviruses might affect human SARS-CoV-2 response. We screened 2,565 serum and plasma samples collected from 2013 through early 2020, before the COVID-19 pandemic began, from 2,250 persons in 4 countries in Africa (Kenya, Nigeria, Tanzania, and Uganda) and in Thailand, including persons living with HIV-1. We detected IgG responses to SARS-CoV-2 spike (S) subunit 2 protein in 1.8% of participants. Incidence of hepatitis B virus (HBV) infection has declined dramatically since the introduction of the HBV vaccine, but gaps remain in coverage. We characterized factors associated with vaccination among individuals with sexual risk of HBV infection. The BRAHMS observational cohort enrolled men and transgender women, without HIV, aged 18–55 years, with behavioral vulnerability to sexually transmitted infections at 10 German outpatient clinics. HBV surface antigen, surface antibody, and core antibody were assessed at screening for cohort eligibility to determine HBV vaccination status. Modified Poisson regression with robust standard errors was used to calculate adjusted prevalence ratios (aPR) and 95% confidence intervals (95% CI) for factors potentially associated with prior HBV vaccination. A total of 1,042 participants were included in these cross-sectional analyses, with 831 participants (79.7%) immune to HBV due to vaccination. Knowledge around HBV vaccination recommendations (aPR: 0.87; 95% CI: 0.78–0.98). and age (aPR 40–49 vs 18–29 years: 0.88; 95% CI: 0.79–0.97) were significantly associated with a history of HBV vaccination in both the unadjusted and adjusted models. Education about the availability of vaccines and recommendations for vaccinations may improve coverage.

Future efficacy testing of interventions to prevent HIV or other infections will require engagement of vulnerable populations. We characterized willingness to participate in a future HIV vaccine trial and barriers to participation among men who have sex with men in a 12-month German cohort study. Among 1015 participants at enrollment, 604 (60%) reported willingness, 60 (6%) were unwilling, 351 (35%) were unsure or refused to answer. Among those unwilling, the primary reason was fear of getting HIV. Among those willing, reasons included protection against HIV and furthering scientific knowledge. In a multivariable logistic regression model, higher odds of willingness to participate were seen among participants at the 12-month visit (aOR: 1.09, 95% CI: 1.04–1.15) and with prior knowledge of HIV vaccine research (aOR: 1.14, 95% CI: 1.06–1.23). Educating potential participants about vaccine research may facilitate recruitment and participation in future trials of HIV vaccine candidates and other prevention interventions.

هدف البحث إلى دراسة وتقويم مدى الالتزام بمعايير رقابة الأداء وفقاً للمعيار /3000/ الذي تصدره المنظمة الدولية للأجهزة العليا للرقابة المالية العامة والمحاسبية ( INTOSAI) وذلك من قبل أهم المؤسسات الرقابية في سورية وهو الجهاز المركزي للرقابة المالية، ومن أجل تحقيق هدف البحث تم وضع فرضية رئيسة تفرع عنها ثلاث فرضيات فرعية، كما تم اتباع المنهج الوصفي التحليلي، وتوزيع استبانة على عينة من العاملين الفنيين في فرع الجهاز المركزي للرقابة المالية في اللاذقية، وبلغ عدد الاستبانات الصالحة للتحليل 43 استبانة تم تحليلها بالاعتماد على البرنامج الاحصائي SPSS 23 واختبار المتوسطات واختبار t-Student. وتم التوصل إلى نتائج، أهمها: يقوم مفتشو الجهاز المركزي للرقابة المالية فرع اللاذقية بالتدقيق وفقاً لمعايير (INTOSAI) 3000 المتعلقة برقابة الأداء حيث بلغ متوسط إجابات أفراد العينة المدروسة 3.2379، ويلتزم مفتشو الجهاز إلى حدًّ كبير بأبعاد الاقتصاد والكفاءة والفعالية. قدم البحث توصيات، منها: ضرورة التحسين المستمر للعمل الرقابي الذي يقوم به الجهاز المركزي للرقابة المالية وذلك من خلال تحديث المعايير المعتمدة، وإعداد مفتشين مؤهلين قادرين على الإحاطة بجميع معايير رقابة الأداء، ضرورة إجراء دورات تدريبية متخصصة لا سيما حول مفهوم المقارنة المرجعية Benchmarking.

هدف البحث إلى تقييم مدى توافر التخطيط الاستراتيجي للموارد البشرية في الشركة السّورية للاتصالات بالسويداء، وذلك من خلال تناول أربعة أبعاد أساسية(تحليل العمل-التنبؤ بالطلب-تحليل العرض-تحليل الفجوة). اعتمد الباحث على أسلوب الحصر الشامل بالنسبة لمجتمع الدراسة، من خلال توزيع(180) استبانة على مدراء الإدارة العليا والوسطى ورؤساء الأقسام في الشركة المدروسة، حيث تم استرجاعها وتحليلها بشكل كامل. واستخدم الباحث اختبار T ستيودنت كاختبار أساسي لهذة الدراسة بهدف تقييم مدى توافر أبعاد التخطيط الاستراتيجي للموارد البشرية. توصل البحث إلى ارتفاع مستوى كل من تحليل العمل والتنبؤ بالطلب من جهة، وانخفاض مستوى كل من تحليل العرض وتحليل الفجوة من جهة أخرى، كما خلص البحث إلى مجموعة من التوصيات المتعلقة بضرورة إيجاد نظام متكامل للتخطيط الاستراتيجي للموارد البشرية، والموازنة بين الطلب والعرض من الموارد البشرية، مع إقامة دورات تدريبية متخصصة لرفع كفاءة العاملين في إدارة الموارد البشرية.

تتناول الدراسة موضوع التوجهات الأساسية للسياسة الخارجية التركية والأمريكية تجاه منطقة استراتيجية، وهي منطقة الشرق الأوسط، وخاصةً بعد عام 2011 والتي تميزت بحدة المنافسات والأزمات الجيوسياسية المعقدة في تلك المنطقة مثل أزمات دول (ليبيا، اليمن، تونس، سوريا)، وركزت الدراسة على تحليل التغييرات في التوجهات الأساسية لسياسة كل من البلدين خلال أعوام 2011-2022، وتأثيراتها ونتائجها المحتملة، وكما تطرقت الدراسة بشكل تفصيلي حول أهم توجهات الإدارات الأمريكية الثلاث (أوباما، ترامب، بايدن) واستراتيجيات تلك الإدارات حول وضع الخطوط العامة نحو منطقة الشرق الأوسط، وأما بالنسبة للتوجهات التركية تجاه منطقة الشرق الأوسط فقد ركزت الدراسة على أهم خطط وبرامج واستراتيجيات حزب العدالة والتنمية بقيادة أردوغان، وكما عالجت الدراسة أهم نقاط التقارب والتباعد بين التوجهات الأساسية للسياسات الأمريكية والتركية تجاه منطقة الشرق الأوسط، وتوصلت الدراسة إلى مجموعة نتائج أهمها متعلقة بطبيعة تأثير المتغيرات الإقليمية والدولية في منطقة الشرق الأوسط، وأيضاً تأثير تداعيات الحرب الروسية-الأوكرانية، وتأثير تغير الإدارات الأمريكية الثلاث (أوباما، ترامب، بايدن) على توجهات السياسة الخارجية التركية والأمريكية تجاه منطقة الشرق الأوسط.

يهدف البحث الى تحديد أثر ممارسات إدارة الموارد البشرية (التدريب والتأهيل – الأجر والتعويضات) ومدى تطبيقها في الإدارة المركزية بجامعة تشرين وأثرها على الرضا الوظيفي للعاملين. والى توضيح أهمية هذه الممارسات في تعزيز ورفع رضا العاملين. كما هدف البحث الى لفت الانتباه الى أهمية رضا العاملين في العمل ورفع سوية المؤسسة وأدائها. ومن أجل تحقيق أهداف البحث صممت الباحثتان استبانة من أجل جمع البيانات الأولية من عينات الدراسة، وهي 57 استبانة يتم تحليل الاستبانات باستخدام برنامج الحزمة الإحصائية للعلوم الاجتماعية SPSS باستخدام عدد من التحاليل، وخلُصت الدراسة إلى عدة نتيجة مفادها أن إشراك جميع العاملين في الإدارة المركزية بدورات تدريبية لاستخدام الحاسوب في أعمالهم كان بالمستوى المطلوب. كما يوجد ضعف في إنتاجية قاعدة بيانات الإدارة المركزية فيما يخص مهارات العاملين الحالية، وأنشطة تدريب العاملين. وخلص البحث الى عدد من التوصيات من أهمها: العمل على إشراك جميع العاملين بدورات تدريبية لاستخدام الحاسوب في أعمالهم. وإيجاد قاعدة بيانات الإدارة المركزية فيما يخص مهارات العاملين الحالية، وأنشطة تدريب العاملين وتعزيز اهتمام الجامعة بأهمية شهادة الأيزو والمتعلق بتدريب الموظفين.

Summary Preeclampsia is associated with an increased risk of long‐term cardiometabolic disease; however, little is known regarding metabolic factors in the early postpartum years potentially contributing to these health disparities. This study aimed to compare body composition, serum biochemical parameters, energy balance and diet 6 months and 2 years after normotensive pregnancy versus preeclampsia. This is the longitudinal metabolic sub‐study of the Postpartum Physiology, Psychology and Paediatric cohort study. Women were assessed 6 months and 2 years after normotensive pregnancy (n = 118) and preeclampsia (n = 47). Metabolic measures included anthropometry, body composition via bioelectrical impedance analysis, serum biochemical parameters, diet via a food recall diary, and 24‐h energy expenditure using SenseWear Armbands. Two years postpartum, women after preeclampsia continued to have significantly higher weight (median 67.1 kg vs. 63.1 kg, p = .04) compared to normotensive pregnancies, in addition to higher LDL cholesterol levels (2.7 ± 0.8 mmol/L vs. 2.4 ± 0.6 mmol/L, p = .03). These women were also more likely to have an elevated HOMA‐IR score ≥2.08 (44% vs. 19%, p = .01). For all women in our study, waist‐to‐hip ratio, percent fat mass and activity‐associated energy expenditure improved overtime. However, HDL cholesterol levels deteriorated, and excess saturated fat and sodium intake persisted from 6 months postpartum. Therefore, two years after preeclampsia, women remain at greater metabolic risk than their normotensive counterparts, with greater weight, LDL cholesterol and markers of insulin resistance, potentially contributing to long‐term cardiovascular morbidity and requiring early intervention. As the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) continues to spread globally, it became evident that the SARS-CoV-2 virus infects multiple organs including the brain. Several clinical studies revealed that patients with COVID-19 infection experience an array of neurological signs ranging in severity from headaches to life-threatening strokes. Although the exact mechanism by which the SARS-CoV-2 virus directly impacts the brain is not fully understood, several theories have been suggested including direct and indirect pathways induced by the virus. One possible theory is the invasion of SARS-CoV-2 to the brain occurs either through the bloodstream or via the nerve endings which is considered to be the direct route. Such findings are based on studies reporting the presence of viral material in the cerebrospinal fluid and brain cells. Nevertheless, the indirect mechanisms, including blood-clotting abnormalities and prolonged activation of the immune system, can result in further tissue and organ damages seen during the course of the disease. This overview attempts to give a thorough insight into SARS-CoV-2 coronavirus neurological infection and highlights the possible mechanisms leading to the neurological manifestations observed in infected patients.

Abstract Background Retention in clinical care is important for people living with HIV (PLWH). Evidence suggests that missed clinic visits are associated with interruptions in antiretroviral therapy (ART), lower CD4 counts, virologic failure, and overlooked coinfections. We identified factors associated with missed routine clinic visits in the African Cohort Study (AFRICOS). Methods In 2013, AFRICOS began enrolling people with and without HIV in Uganda, Kenya, Tanzania, and Nigeria. At enrollment and every 6 months thereafter, sociodemographic questionnaires are administered and clinical outcomes assessed. Missed clinic visits were measured as the self-reported number of clinic visits missed in the past 6 months and dichotomized into none or one or more visits missed. Background The coronavirus disease 2019 pandemic has led to an increase in remote consultations in health care. This study aimed to assess the acceptance of video consultation as an alternative to face-to-face in-office visits in general practice (GP) and to investigate its drivers and barriers. Methods A cross-sectional study was conducted in Germany during the coronavirus disease 2019 pandemic from December 2020 to April 2021. Participants were recruited among patients in 16 GP surgeries. Assessed were sociodemographic and medical data as well as information and communications technology related data. Acceptance of video consultation and its predictors were determined using a modified questionnaire based on a short version of the renowned unified theory of acceptance and use of technology model. Results In total, 371 participants were included in the data analysis. Acceptance of video consultation was moderate. A hierarchical regression revealed acceptance was significantly predicted by the PHQ-2, taking no regular medication, computer proficiency, knowledge about digital health care solutions, no prior use of video consultation, and the unified theory of acceptance and use of technology predictors performance expectancy, effort expectancy, and social influence. The extended unified theory of acceptance and use of technology model explained significantly more variance than the restricted unified theory of acceptance and use of technology model in acceptance of video consultation. Conclusions In this study computer proficiency, existing knowledge about digital health care solutions and depressive symptoms functioned as drivers to acceptance, no prior use of video consultation could be identified as a potential barrier. Patients with regular medication have been particularly receptive to video consultation. The study confirmed the validity of the unified theory of acceptance and use of technology model in determining acceptance of video consultation. Considering that there is growing demand and acceptance for different approaches to engage with health care providers, additional steps should be taken to establish video consultation as a genuine alternative.

Abstract Background Conditions such as genital prolapse and hernia are known to be related to connective tissue dysfunction. In this report on cases of the rare simultaneous finding of large genital prolapse and post-prolapse repair female inguinal bladder hernia, we aim to contribute to the discussion of a possible clinical definition of connective tissue weakness, for its clinical assessment and preoperative patient counselling. Case presentation Three cases of medial third-grade (MIII, Aachen classification) inguinal bladder hernia developing or enlarging after successful stage-IV pelvic organ prolapse (POP) repair at a university pelvic floor centre are presented. All patients were aged ≥ 80 years with long-standing postmenopausal status. One patient was followed for 5 years and two patients were followed for 6 months. In all patients, ultrasound revealed that the hernia sac contained the urinary bladder, which had herniated through the inguinal hernia orifice. A literature search revealed only one case report of direct female inguinal bladder hernia and few investigations of the simultaneous occurrence of POP and hernia in general. Conclusion The simultaneous occurrence of inguinal hernia and female POP can lead to bladder herniation following prolapse surgery in the sense of a “locus minoris resistentiae”. Clinical examination for simultaneous signs of connective tissue weakness and counselling prior to pelvic reconstructive surgery could help to increase patients’ compliance with further surgical treatment for hernia.

Abstract Background Increased availability of HIV care over the past decade has dramatically reduced morbidity and mortality among people living with HIV (PLWH) in sub-Saharan Africa. However, perceived and experienced barriers to care, including dissatisfaction with services, may impact adherence and viral suppression. Abstract Background Each year, 5.6 million new syphilis cases are diagnosed globally. Guidelines for people living with HIV (PLWH) in low-income countries (LIC) recommend STI testing for symptomatic persons and those newly diagnosed with HIV; routine STI testing is less clear. Here we provide updated syphilis prevalence and identify co-infection risk factors in PLWH in the African Cohort Study (AFRICOS) to understand these rates as they relate to syndromic treatment. Methods AFRICOS is a study enrolling PLWH and HIV-uninfected individuals in four African countries. Participant study enrollment information was used to determine syphilis prevalence and co-infection risk factors. Inclusion criteria consisted of adults 18 years or older receiving care at a participating clinic as a long-term resident who consented to data and specimen collection. Exclusion criteria consisted of pregnancy and/or imprisonment. Screen-positive syphilis was defined as a reactive rapid plasma regain (RPR) upon study enrollment whereas confirmed syphilis included a reactive RPR followed by reactive treponemal test. Multivariate analyses was performed to determine HIV and syphilis co-infection risk factors. Results Between 2013 and March 1, 2020, 2939 PLWH enrolled and 2818 were included for analysis. Screen-positive and confirmed syphilis prevalence were 5.3% (151/2818) and 3.1% (87/2818), respectively. When the analysis was restricted to PLWH with an RPR titer of greater than, or equal to, 1:8, 11/87 (12.6%) participants were included. No PLWH and confirmed syphilis had documented genital ulcers. In the multivariate model, participants with confirmed syphilis co-infection were more likely to have none or some primary education [aOR 3.29 (1.60, 6.74)] and consume alcohol [aOR 1.87 (1.16, 3.03)] compared to those without syphilis. Antiretroviral therapy (ART) with suppressed viral load (VL) was protective in the unadjusted model but not adjusted multivariate model. Conclusions Our findings show that syphilis rates in sub-Saharan Africa remain elevated where diagnosis remains challenging, and that both lower education level and alcohol consumption are significantly associated with HIV/syphilis co-infection in AFRICOS. Based on our analysis, current STI guidelines targeting testing for African individuals with either new HIV diagnosis or syndromic symptoms may be inadequate, highlighting the need for increased testing and treatment strategies in resource-limited settings.
