Background: During and after the COVID-19 pandemic, there was a suspicion of varying rates of respiratory tract infections (RTIs), particularly pneumonia (PN). Methods: This research evaluated epidemiological indicators of community-acquired pneumonia (CAP) and hospital-acquired pneumonia (HAP) in the COVID-19 pandemic and post-pandemic period, including pathogens, ventilator-associated pneumonia (VAP), selected risk factors, and PN mortality. Results: At 1740 patients, throughout the 22,774 patient-days (Pt-D) and 18,039 ventilation days (Vt-D), there were 681 PN cases (39.14%): CAP 336 (19.31%) and HAP 345 (19.83%). CAP caused by SARS-CoV-2 was diagnosed in 257/336 (76.49%) patients. The clinical manifestations of PNs were CAP with 336/681 (49.34%), VAP with 232/681 (34.07%), and non-ventilator HAP (NV-HAP) with 113/681 cases (16.59%). The incidence rate of CAP/1000 Pt-D has been over 3 times higher in the pandemic period of 2020–2021 (20.25) than in the post-pandemic period of 2022 (5.86), p = 0.000. Similarly, higher incidence rates of VAP/1000 Pt-D were found in the pandemic period (p = 0.050). For NV-HAP, this difference was not statistically significant (p = 0.585). VAP occurred more frequently in the group of patients with PN in the course of COVID-19 compared to patients without COVID-19 (52/234 [22.2%] vs. 180/1506 [11.95%]); (p = 0.000). The most common CAP pathogen (during the pandemic) was SARS CoV-2 234/291 (80.4%), followed by MSSA/MRSA 8/291 (2.75%), whereas the most common VAP/NV-HAP pathogen was Acinetobacter baumannii XDR/MDR. The highest PN mortality was found in the patients with CAP caused by SARS-CoV-2 159/257 (61.87%). Conclusions: Pneumonias were diagnosed in nearly 40% of Intensive Care Unit (ICU) patients. Surveillance of pneumonias during the specific observation period was beneficial in the epidemiological and microbiological analysis of the ICU patients. [ABSTRACT FROM AUTHOR]

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is often characterized by a life-threatening interstitial pneumonia requiring hospitalization. The aim of this retrospective cohort study is to identify hallmarks of in-hospital mortality in patients affected by Coronavirus Disease 19 (COVID-19). A total of 150 patients admitted for COVID-19 from March to June 2021 to "F. Perinei" Murgia Hospital in Altamura, Italy, were divided into survivors (n = 100) and non-survivors groups (n = 50). Blood counts, inflammation-related biomarkers and lymphocyte subsets were analyzed into two groups in the first 24 h after admission and compared by Student's t-test. A multivariable logistic analysis was performed to identify independent risk factors associated with in-hospital mortality. Total lymphocyte count and CD3+ and CD4+ CD8+ T lymphocyte subsets were significantly lower in non-survivors. Serum levels of interleukin-6 (IL-6), lactate dehydrogenase (LDH), C-reactive protein (CRP) and procalcitonin (PCT) were significantly higher in non-survivors. Age > 65 years and presence of comorbidities were identified as independent risk factors associated with in-hospital mortality, while IL-6 and LDH showed a borderline significance. Introduction: This study aimed to investigate the relationship between community-acquired pneumonia severity and D-dimer levels in individuals older than 65. We also investigated the relationship between D-dimer levels and the adverse outcomes in patients with community-acquired pneumonia after excluding all other potential causes of high D-dimer levels. Materials and Methods: Patients older than 65 who were admitted to the emergency service of a tertiary chest diseases training and research hospital between January 1, 2019, and October 1, 2020, were evaluated. Patients who met the diagnostic criteria for community-acquired pneumonia were included. In clinically questionable cases of coexistence of pulmonary embolism and community-acquired pneumonia, D-dimer levels and pulmonary computerize tomography angiography or ventilation-perfusion scintigraphy were examined. Confirmed pulmonary embolism patients were excluded. Of 4,608 patients evaluated, 82 had a diagnosis of community-acquired pneumonia with no comorbidity. The severity of these cases was determined with the CURB-65 score and pneumonia severity index score. Results: The mean age of the cases was 73.83±6.67 years, while their gender was predominantly male (n=51, 62.2%). A statistically significant correlation was found between D-dimer levels and both the CURB-65 and pneumonia severity index high-risk groups (p=0.001 and p=0.001, respectively). The adverse outcomes were statistically higher in both the CURB-65 and pneumonia severity index high-risk groups (p<0.001). Conclusions: D-Dimer is an easy-to-interpret, fast, inexpensive, highly sensitive, and simple test widely used in clinics. We found that high levels of D-dimer can predict the need for intensive care unit care, disease severity, and mortality of elderly community-acquired pneumonia patients. [ABSTRACT FROM AUTHOR]
