8. Influencing Factors for the Effectiveness of Family Doctor Contract Services for Elderly Patients with Multimorbidity in Communities

Abstract

Background With the aging of the population and changes in the disease spectrum, the incidence of multimorbidity among the elderly remained high, and have become the focus of the family doctor contracting service at the primary level. At present, there are many studies promoting the signing of family doctors from the perspective of increasing "quantity", but there is still a lack of studies on the factors affecting the effectiveness of family doctor contract services for elderly multimorbidity patients after signing from the perspective of improving "quality". Objective To explore the important influencing factors of satisfaction and usefulness evaluation of elderly multimorbidity patients with family doctor contract services in urban communities of guangdong province, and to provide a basis for future improvement of multimorbidity management services at the grassroots level. Methods From September to December 2022, elderly patients with multiple chronic conditions in the community of Guangdong Province (Shenzhen, Zhanjiang and Meizhou) were selected by multi-stage stratified whole cluster random sampling method and surveyed by self-administered questionnaire. Multifactorial Logistic regression analysis was used to explore the influencing factors of satisfaction and usefulness evaluation of family doctor contracting services for elderly multimorbidity patients. Results A total of 636 valid questionnaires were collected, with effective recovery rate of 99.69% (636/638). There were 624 family doctors who were relatively satisfied/very satisfied, accounting for 98.11%; The service usefulness evaluation of family doctors was slightly helpful/greatly helpful to 625 people, accounting for 98.27%. The results of multivariate Logistic regression analysis showed that patients with general satisfaction of family doctor contract service were taken as reference. Abstract

Hyperentangled states can outperform their classical counterparts on solving certain tasks. Here we present a simplified scheme for completely distinguishing two-photon hyperentangled Bell states in polarization, spatial, and time-bin degrees of freedom (DOFs). Unity fidelity can be achieved in principle without strong couple limitation between photon and quantum dot (QD), and the incomplete and imperfect QD-cavity interactions are prevented by single-photon detectors. Abstract

With the continuous development of China’s sports competition business, the problem of structural imbalance in China’s sports competition has gradually appeared from the bottom. Starting from the extracurricular sports competitions of primary and secondary school students, this paper makes a preliminary sorting out of the structural problems of primary and secondary school students’ sports competitions. It puts forward a system reform method for the structural imbalance problem of sports competitions. In addition, the study randomly selects the teacher-student system of primary and secondary schools in District M as a research sample. It combines the principal component analysis model and the factor analysis model to conduct a more in-depth study of the factors affecting the structural imbalance of sports competitions. Among them, the cumulative explanatory rate of five factors, namely, policy support, environmental factors, sports industry, economic inputs, and competition activities, is 100%. The correlation coefficient values between the five factors are 0.632, 0.542, 0.371, and 0.675, respectively, with significant correlation. 7.3-15.5 points improve the scores of the indicators of the improved structure compared with those of the pre-improved structure. The structural imbalance problem was effectively resolved through system improvement in this study, resulting in a more sound and perfect sports competition system for primary and secondary school students.

Abstract

Enhancing the capabilities of quantum computing relies heavily on harnessing the power of qudit‐based high‐dimensional quantum gates. In the study, single‐qudit 4D X$ X$, X2$ X^{2}$, and X†$ X^{\dagger }$ gates tailored for a two‐photon system in polarization states are presented. Furthermore, a two‐qudit 4×4$4\times 4$‐dimensional controlled‐not (CNOT) gate designed for a four‐photon system is introduced. These high‐dimensional gates can offer versatile and straightforward optical implementations, ensuring them to fulfill in a deterministic way. To facilitate these processes, an auxiliary system in the form of a Λ$\Lambda$‐type atom residing in a cavity is employed. Remarkably, the auxiliary system retains its original state after the operation process ends, so it is not required to measure and plays a pivotal role in promoting effective interactions among distinct photons in its extended coherence time. Importantly, the in‐depth analysis of the fidelities and efficiencies of these quantum gates showcase remarkable outcomes, affirming the superiority of the proposed protocols. Therefore, these high‐dimensional gates not only amplify quantum parallelism, but also bolster the speed of quantum computations, fortify resilience against errors, and foster scalability for executing intricate quantum operations. Abstract

The interconversion of different types of entangled states not only can realize the information transmission but also play a significant role in quantum information technologies, including increasing scalability and computational power, and reducing error rates. Here, two protocols for achieving a complete interconversion between W state and Knill–Laflamme–Milburn state assisted by the quantum dot (QD)‐cavity systems and common quantum control gates are proposed. In particular, the protocols employ a heralded approach strategically designed to predict potential failures and facilitate seamless interaction between the QD‐cavity system and photons with the help of a single photon detectors, enhancing experimental accessibility. Through extensive analyzes and evaluations of two protocols, the proposed two protocols achieve remarkable utilization rates of photons (i.e., unit in principle) and achieve near‐unit fidelities and high efficiencies in principle.
