Background Conventional MRI sequences are often affected in neuropediatric imaging by unavoidable movements. Therefore, children younger than 6 years usually have to be examined under sedation/anesthesia. A new real-time MRI technique with automatic slice advancement allows for motion-robust T2-weighted volume coverage of the whole brain within a few seconds in adults. Objective To evaluate to which extent the new volume coverage method can be used to visualize cerebrospinal fluid and reduce the need for anesthesia in children. Materials and methods We assessed 30 children ages 6 years and younger with suspected or proven hydrocephalus, hygroma or macrocephalus using volume coverage sequences with 20 slices per second in three planes. If necessary, a parent was placed in the bore together with the child for calming and gentle immobilization. We compared visualization of cerebrospinal fluid spaces and course of the shunt catheter in volume coverage sequences vs. fast spin-echo sequences. Results The clinical issue could be sufficiently assessed in all children with use of volume coverage sequences, whereas conventional fast spin-echo sequences performed moderately to poorly. Visualization of the tip of a shunt failed in 16% of volume coverage scans and 27% of turbo spin-echo scans. A subsequent examination under anesthesia was never necessary. None of the examinations had to be stopped prematurely. Conclusion The motion-robust volume coverage sequences with T2-type contrast can be used to avoid sedation of children in the evaluation of cerebrospinal fluid spaces, even in the presence of vigorous motion. For other indications and contrasts, the technique must still be evaluated.

Background The aim of this study was to evaluate the reproducibility and clinical value of the novel single-shot T1 mapping method for rapid and accurate multi-slice coverage of the whole brain, described by Wang et al. 2015. Methods At a field strength of 3 Tesla, T1 mappings of 139 patients (51 of them without pathologic findings) and two repeats of five volunteers were performed at 0.5 mm in-plane resolution. Mean T1 values were determined in 18 manually segmented regions-of-interest without pathologic findings. Reproducibility of the repeated scans was calculated using mean coefficient of variations. Pathologies were grouped and separately evaluated. Results The mean age of the cohort was 49 (range 1–95 years). T1 relaxation times for ordinary brain and pathologies were in accordance with the literature values. Intra- and inter-subject reproducibility was excellent, and mean coefficient of variations were 2.4% and 3.8%, respectively. Discussion The novel rapid T1 mapping method is a reliable magnetic resonance imaging technique for identifying and quantifying normal brain structures and may thus serve as a basis for assessing pathologies. The fast and parallel online calculation enables a comfortable use in everyday clinical practice. We see a possible clinical value in a large spectrum of diseases, which should be investigated in further studies.

Purpose To develop a rapid and motion-robust technique for volumetric MRI, which is based on cross-sectional real-time MRI acquisitions with automatic advancement of the slice position. Methods Real-time MRI with a frame-by-frame moving cross-section is performed with use of highly undersampled radial gradient-echo sequences offering spin density, T1 , or T2 /T1 contrast. Joint reconstructions of serial images and coil sensitivity maps from spatially overlapping sections are accomplished by nonlinear inversion with regularization to the preceding section-formally identical to dynamic real-time MRI. Shifting each frame by 20% to 25% of the section thickness ensures 75% to 80% overlap of successive sections. Acquisition times of 40 to 67 ms allow for rates of 15 to 25 sections per second, while volumes are defined by the number of cross-sections times the section shift. Results Preliminary realizations at 3T comprise studies of the human brain, carotid arteries, liver, and prostate. Typically, coverage of a 90- to 180-mm volume at 0.8- to 1.2-mm in-plane resolution, 4- to 6-mm section thickness, and 0.8- to 1.5-mm section shift is accomplished within total measuring times of 4 to 6 seconds and a section speed of 15 to 37.5 mm per second. However, spatiotemporal resolution, contrast including options such as fat saturation and total measuring time are highly variable and may be adjusted to clinical needs. Promising volumetric applications range from fetal MRI to dynamic contrast-enhanced MRI. Conclusion The proposed method allows for rapid and motion-robust volume coverage in a variety of imaging scenarios.

OBJECTIVE: Movements inside the oral cavity during lip trilling in horn-playing are poorly understood and controversial, particularly with respect to pedagogy. Developments in real-time magnetic resonance imaging (RT-MRI) allow representations of oral cavity movement during lip trill performance on a MRI-compatible horn to be recorded and quantified. METHODS: We present RT-MRI data on 11 highly skilled horn players obtained from serial images acquired at acquisition times of 33.3, 18.2, and 10.0 ms (i.e., at 30, 55 and 100 frames/sec) as they performed sixteenth note, whole-step trills between Eb4 and F4 (concert pitch) at two tempos, ~60 bpm and as fast as possible. RESULTS: For fast trilling (mean speed 178.3±24.7 bpm), 7 of 11 subjects exclusively utilized a tongue movement strategy, 3 used both a tongue and jaw strategy, and 1 exclusively used a jaw strategy. For trilling at ~60 bpm, all 11 subjects used a tongue movement strategy. CONCLUSIONS: We suggest using these movement strategies in teaching whole-step trills.

Recent MRI studies of blood flow in the inferior vena cava (IVC) resulted in findings which are inconsistent with earlier observations by invasive procedures - most likely because ECG-gated MRI techniques are unable to resolve dynamic adjustments due to respiration. The purpose of this work was to apply real-time phase-contrast MRI at 50 ms resolution to re-evaluate IVC flow in response to normal and deep breathing as well as breath holding and Valsalva maneuver (11 young healthy subjects). Real-time flow MRI relied on highly undersampled radial gradient-echo sequences and a model-based nonlinear inverse reconstruction. A frequency analysis of the predominant pulsatility classified IVC flow in individual subjects as "cardiac", "respiratory" or "mixed" type. Peak flow velocities during free breathing ranged from 30 to 58 cm s(-1), while flow rates varied from 15 to 37 ml s(-1). The subject-specific IVC flow pattern persists during deep breathing although the enhanced respiratory influence may shift subjects form "cardiac" to "mixed" or from "mixed" to "respiratory" type. Peak velocities increased relative to normal breathing but led to similar flow rates of 16 to 34 ml s(-1). Inspiration during deep breathing elicited brief periods of flow reversal in all subjects with mean peak velocities of -21 cm s(-1). The observation of only mildly flattened parabolic velocity distributions within the IVC indicated mostly laminar flow. Breath holding reduced blood flow velocities and rates by more than 40% on average, while Valsalva maneuvers completely abolished venous return. In conclusion, IVC blood flow is dominated by the acquired respiratory behavior of individual subjects and its pressure-induced alterations relative to cardiac pulsation. The responses to breath holding and Valsalva maneuver are in full agreement with previous invasive observations of reduced or even ceased flow, respectively.

Objective The aim of this study is to evaluate a real-time magnetic resonance imaging (MRI) method that not only promises high spatiotemporal resolution but also practical robustness in a wide range of scientific and clinical applications. Materials and methods The proposed method relies on highly undersampled gradient-echo sequences with radial encoding schemes. The serial image reconstruction process solves the true mathematical task that emerges as a nonlinear inverse problem with the complex image and all coil sensitivity maps as unknowns. Extensions to model-based reconstructions for quantitative parametric mapping further increase the number of unknowns, for example, by adding parameters for phase-contrast flow or T1 relaxation. In all cases, an iterative numerical solution that minimizes a respective cost function is achieved with use of the iteratively regularized Gauss-Newton method. Convergence is supported by regularization, for example, to the preceding frame, whereas temporal fidelity is ensured by downsizing the regularization strength in comparison to the data consistency term in each iterative step. Practical implementations of highly parallelized algorithms are realized on a computer with multiple graphical processing units. It is "invisibly" integrated into a commercial 3-T MRI system to allow for conventional usage and to provide online reconstruction, display, and storage of regular DICOM image series. Results Depending on the application, the proposed method offers serial imaging, that is, the recording of MRI movies, with variable spatial resolution and up to 100 frames per second (fps)-corresponding to 10 milliseconds image acquisition times. For example, movements of the temporomandibular joint during opening and closing of the mouth are visualized with use of simultaneous dual-slice movies of both joints at 2 × 10 fps (50 milliseconds per frame). Cardiac function may be studied at 30 to 50 fps (33.3 to 20 milliseconds), whereas articulation processes typically require 50 fps (20 milliseconds) or orthogonal dual-slice acquisitions at 2 × 25 fps (20 milliseconds). Methodological extensions to model-based reconstructions achieve improved quantitative mapping of flow velocities and T1 relaxation times in a variety of clinical scenarios. Conclusions Real-time gradient-echo MRI with extreme radial undersampling and nonlinear inverse reconstruction allows for direct monitoring of arbitrary physiological processes and body functions. In many cases, pertinent applications offer hitherto impossible clinical studies (eg, of high-resolution swallowing dynamics) or bear the potential to replace existing MRI procedures (eg, electrocardiogram-gated cardiac examinations). As a consequence, many novel opportunities will require a change of paradigm in MRI-based radiology. At this stage, extended clinical trials are needed.

Purpose To achieve dynamic water/fat separation and B 0 field inhomogeneity mapping via model-based reconstructions of undersampled triple-echo multi-spoke radial FLASH acquisitions. Methods This work introduces an undersampled triple-echo multi-spoke radial FLASH sequence, which uses (i) complementary radial spokes per echo train for faster spatial encoding, (ii) asymmetric echoes for flexible and nonuniform echo spacing, and (iii) a golden angle increment across frames for optimal k-space coverage. Joint estimation of water, fat, B 0 inhomogeneity, and coil sensitivity maps from undersampled triple-echo data poses a nonlinear and non-convex inverse problem which is solved by a model-based reconstruction with suitable regularization. The developed methods are validated using phantom experiments with different degrees of undersampling. Real-time MRI studies of the knee, liver, and heart are conducted without prospective gating or retrospective data sorting at temporal resolutions of 70, 158, and 40 ms, respectively. Results Up to 18-fold undersampling is achieved in this work. Even in the presence of rapid physiological motion, large B 0 field inhomogeneities, and phase wrapping, the model-based reconstruction yields reliably separated water/fat maps in conjunction with spatially smooth inhomogeneity maps. Conclusions The combination of a triple-echo acquisition and joint reconstruction technique provides a practical solution to time-resolved and motion robust water/fat separation at high spatial and temporal resolution.

Quantitative parameter mapping in MRI is typically performed as a two-step procedure where serial imaging is followed by pixelwise model fitting. In contrast, model-based reconstructions directly reconstruct parameter maps from raw data without explicit image reconstruction. Here, we propose a method that determines T1 maps directly from multi-channel raw data as obtained by a single-shot inversion-recovery radial FLASH acquisition with a Golden Angle view order. Joint reconstruction of a T1, spin-density and flip-angle map is formulated as a nonlinear inverse problem and solved by the iteratively regularized Gauss-Newton method. Coil sensitivity profiles are determined from the same data in a preparatory step of the reconstruction. Validations included numerical simulations, in vitro MRI studies of an experimental T1 phantom, and in vivo studies of brain and abdomen of healthy subjects at a field strength of 3 T. The results obtained for a numerical and experimental phantom demonstrated excellent accuracy and precision of model-based T1 mapping. In vivo studies allowed for high-resolution T1 mapping of human brain 0.5-0.75 mm in-plane, 4 mm section thickness and liver 1.0 mm, 5 mm section within 3.6-5 s. In conclusion, the proposed method for model-based T1 mapping may become an alternative to two-step techniques, which rely on model fitting after serial image reconstruction. Objective: To develop a novel method for rapid myocardial T1 mapping at high spatial resolution. Methods: The proposed strategy represents a single-shot inversion recovery experiment triggered to early diastole during a brief breath-hold. The measurement combines an adiabatic inversion pulse with a real-time readout by highly undersampled radial FLASH, iterative image reconstruction and T1 fitting with automatic deletion of systolic frames. The method was implemented on a 3-T MRI system using a graphics processing unit-equipped bypass computer for online application. Validations employed a T1 reference phantom including analyses at simulated heart rates from 40 to 100 beats per minute. In vivo applications involved myocardial T1 mapping in short-axis views of healthy young volunteers. Results: At 1-mm in-plane resolution and 6-mm section thickness, the inversion recovery measurement could be shortened to 3 s without compromising T1 quantitation. Phantom studies demonstrated T1 accuracy and high precision for values ranging from 300 to 1500 ms and up to a heart rate of 100 beats per minute. Similar results were obtained in vivo yielding septal T1 values of 1246 ± 24 ms (base), 1256 ± 33 ms (mid-ventricular) and 1288 ± 30 ms (apex), respectively (mean ± standard deviation, n = 6). Conclusion: Diastolic myocardial T1 mapping with use of single-shot inversion recovery FLASH offers high spatial resolution, T1 accuracy and precision, and practical robustness and speed. Advances in knowledge: The proposed method will be beneficial for clinical applications relying on native and post-contrast T1 quantitation.

Velocity-encodedphase-contrast MRI ofcardiovascularblood flowcommonlyreliesonelectrocardiogram-synchronized cine acquisitions of multiple heartbeats to quantitatively determine the flow of an averaged cardiac cycle. Here, we present a new method for real-time phase-contrast MRI that combines flow-encoding gradients with highly undersampled radial fast low-angle shot acquisitions and phase-sensitive image reconstructions by regularized nonlinear inversion. Apart from calibration studies using steady and pulsatile flow, preliminary in vivo applications focused on through-plane flow in the ascending aorta of healthy subjects. With bipolar velocity-encoding gradients of alternating polarity that overlap the slice-refocusing gradient, the method yields flow-encoded images with an in-plane resolution of 1.8mm, section thickness of 6mm and measuring time at 3T of 24ms (TR/TE=3.44/2.76ms; flip angle, 10o; seven radial spokes per image). Accordingly, phase-contrast maps and corresponding velocity profiles achieve a temporal resolution of 48ms. The evaluated peak velocities, stroke volumes, flow rates and respective variances over at least 20 consecutive heartbeats are in general agreement with literature data. In particular, the situation applies to 3D mapping of curved 2D structures which requires only two projection images with different spatial phase-encoding gradients. These phase-contrast gradients define the field of view and mean spin-density positions of the object in the perpendicular dimension by respective phase differences. When combined with highly undersampled radial fast low angle shot (FLASH) and image reconstruction by regularized nonlinear inversion, spatial phase-contrast MRI allows for dynamic 3D mapping of 2D structures in real time. First examples include 3D MRI movies of the acting human hand at a temporal resolution of 50 ms. With an even simpler technique, 3D maps of curved 1D structures may be obtained from only three acquisitions of a frequency-encoded MRI signal with two perpendicular phase encodings. Here, 3D MRI movies of a rapidly rotating banana were obtained at 5 ms resolution or 200 frames per second. The desire to visualize noninvasively physiological processes at high temporal resolution has been a driving force for the development of MRI since its inception in 1973. In this article, we describe a unique method for real-time MRI that reduces image acquisition times to only 20 ms. Although approaching the ultimate limit of MRI technology, the method yields high image quality in terms of spatial resolution, signal-to-noise ratio and the absence of artifacts. As proposed previously, a fast low-angle shot (FLASH) gradient-echo MRI technique (which allows for rapid and continuous image acquisitions) is combined with a radial encoding scheme (which offers motion robustness and moderate tolerance to data undersampling) and, most importantly, an iterative image reconstruction by regularized nonlinear inversion (which exploits the advantages of parallel imaging with multiple receiver coils). In this article, the extension of regularization and filtering to the temporal domain exploits consistencies in successive data acquisitions and thereby enhances the degree of radial undersampling in a hitherto unexpected manner by one order of magnitude. The results obtained for turbulent flow, human speech production and human heart function demonstrate considerable potential for real-time MRI studies of dynamic processes in a wide range of scientific and clinical settings. This paper describes the development of a novel technique for functional MRI of the human brain at 0.135 microL resolution for a whole brain section. In comparison with conventional studies at 3 mm isotropic resolution or 27 microL voxel size, the method yields an improvement by a factor of 200. To achieve optimum image quality, the approach is based on a multi-echo fast low-angle shot (FLASH) sequence with unipolar traversals of k-space in the frequency-encoding dimension and echo train shifting to avoid amplitude discontinuities in the phase-encoding dimension. These strategies ensure a smooth point-spread function and eliminate image ghosting artifacts without the need for any phase correction or other post-processing. Signal-to-noise losses due to the considerably reduced voxel sizes are compensated for by single slice acquisitions, optimized bandwidths and an experimental four-channel shoulder coil matched to the posterior portion of the head. Multi-echo FLASH studies of the human brain (2.9 T, seven echoes, 200 Hz/pixel bandwidth, effective echo time 36 ms, acquisition time 6 s) at 300 microm resolution (no interpolation) and 1.5 mm slice thickness revealed robust activations in primary visual areas in response to binocular stimulation. The new method holds promise for refined studies of the columnar organization of specific brain systems and for functional assessments of the gray matter at laminar resolution.

Background Functional assessments of the heart by dynamic cardiovascular magnetic resonance (CMR) commonly rely on (i) electrocardiographic (ECG) gating yielding pseudo real-time cine representations, (ii) balanced gradient-echo sequences referred to as steady-state free precession (SSFP), and (iii) breath holding or respiratory gating. Problems may therefore be due to the need for a robust ECG signal, the occurrence of arrhythmia and beat to beat variations, technical instabilities (e.g., SSFP "banding" artefacts), and limited patient compliance and comfort. Here we describe a new approach providing true real-time CMR with image acquisition times as short as 20 to 30 ms or rates of 30 to 50 frames per second. Methods The approach relies on a previously developed real-time MR method, which combines a strongly undersampled radial FLASH CMR sequence with image reconstruction by regularized nonlinear inversion. While iterative reconstructions are currently performed offline due to limited computer speed, online monitoring during scanning is accomplished using gridding reconstructions with a sliding window at the same frame rate but with lower image quality. Results Scans of healthy young subjects were performed at 3 T without ECG gating and during free breathing. The resulting images yield T1 contrast (depending on flip angle) with an opposed-phase or in-phase condition for water and fat signals (depending on echo time). They completely avoid (i) susceptibility-induced artefacts due to the very short echo times, (ii) radiofrequency power limitations due to excitations with flip angles of 10° or less, and (iii) the risk of peripheral nerve stimulation due to the use of normal gradient switching modes. For a section thickness of 8 mm, real-time images offer a spatial resolution and total acquisition time of 1.5 mm at 30 ms and 2.0 mm at 22 ms, respectively. Conclusions Though awaiting thorough clinical evaluation, this work describes a robust and flexible acquisition and reconstruction technique for real-time CMR at the ultimate limit of this technology.
