Objective: Unusual setting of medical care Background: Anal squamous cell carcinoma accounts for about 2-4% of all lower gastrointestinal malignancies, with a distant disease reported in less than 5%. Although surgical treatment is rarely necessary, this often involve large dissections and difficult reconstructive procedures. Case Report: We present a complex but successful case of double-flap reconstruction after abdominoperineal resection and groin dissection for anal squamous cell carcinoma (cT3N3M0) with metastatic right inguinal lymph nodes and ipsilateral threatening of femoral vessels. A multi-specialty team was involved in the operation. A vascular and plastic surgeon performed the inguinal dissection with en bloc excision of the saphenous magna and a cuff of the femoral vein, while colorectal surgeons carried out the abdominoperineal excision. The 2 large tissue gaps at the groin and perineum were covered with an oblique rectus abdominis myocutaneous flap and a gluteal lotus flap, respectively. A partially absorbable mesh was placed at the level of the anterior sheath in order to reinforce the abdominal wall, whereas an absorbable mesh was used as a bridge for the dissected pelvic floor muscles. The post-operative period was uneventful and the follow-up at 5 months showed good results. Conclusions: An early diagnosis along with new techniques of radiochemotherapy allow patients to preserve their sphincter function. However, a persistent or recurrent disease needs major operations, which often involve a complex reconstruction. Good team-work and experience in specialized fields give the opportunity to make the best choices to perform critical steps during the management of complex cases. [ABSTRACT FROM AUTHOR]

Several recent papers have criticized the scientific robustness of the fisheries management system used by the Commission for the Conservation of Antarctic Marine Living Resources (CCAMLR), including that for Ross Sea toothfish. Here we present a response from the wider CCAMLR community to address concerns and to correct some apparent misconceptions about how CCAMLR acts to promote conservation whilst allowing safe exploitation in all of its fisheries. A key aspect of CCAMLR's approach is its adaptive feedback nature; regular monitoring and analysis allows for adjustments to be made, as necessary, to provide a robust management system despite the statistical uncertainties inherent in any single assessment. Within the Ross Sea, application of CCAMLR's precautionary approach has allowed the toothfish fishery to develop in a steady fashion with an associated accumulation of data and greater scientific understanding. Regular stock assessments of the fishery have been carried out since 2005, and the 2013 stock assessment estimated current spawning stock biomass to be at 75% of the pre-exploitation level. There will always be additional uncertainties which need to be addressed, but where information is lacking the CCAMLR approach to management ensures exploitation rates are at a level commensurate with a precautionary approach. Abstract: The performance of the EU long-term management plan for cod stocks, in force since 2009, is analysed focusing on the human and institutional factors. The plan operates through landings quotas (TACs) and effort restrictions following a Harvest Control Rule, and deploys a novel instrument allowing Member States to ‘buy back’ or increase fishing effort for fleet segments engaged in cod-avoidance measures. The stipulated fishing mortality reductions have not been achieved. On the positive side, the ‘buy-back’ instrument has led to increased uptake of selective gear and implementation of permanent and real-time temporary closures. On the negative side, ignoring the dimension of fishers as reactive agents in the design, the impact assessment, and the annual implementation of the measures has contributed to the failure to adequately implement the plan and achieve its objectives. The main problem is that the landings quotas taken in a mixed fishery did not limit catches because fishers were incentivised to continue fishing and discard overquota catch while quota for other species was available. The effort limitations intended to reduce this effect were insufficient to adequately limit fishing mortality in targeted fisheries, although fishers experienced them as prohibiting the full uptake of other quotas. Recommendations for future plans include (i) management through catch rather than landings quotas, (ii) the internalisation of the costs of exceeding quotas, (iii) use of more selective gear types, (iv) the development of appropriate metrics as a basis for regulatory measures and for evaluations, (v) participatory governance, (vi) fishery-based management, (vii) flexibility in fishing strategy at vessel level. Recovery of depleted marine, demersal, commercial fish stocks has proved elusive worldwide. As yet, just a few shared or highly migratory stocks have been restored. Here we review the current status of the depleted North Sea cod (Gadus morhua), the scientific advice to managers, and the recovery measures in place. Monitoring the progress of North Sea cod recovery is now hampered by considerable uncertainties in stock assessments associated with low stock size, variable survey indices, and inaccurate catch data. In addition, questions arise as to whether recovery targets are achievable in a changing natural environment. We show that current targets are achievable with fishing mortality rates that are compatible with international agreements even if recruitment levels remain at the current low levels. Furthermore, recent collations of data on international fishing effort have allowed estimation of the cuts in fishing mortality achieved by restrictions on North Sea effort. By the beginning of 2005, these restrictions are estimated to have reduced fishing mortality rates by about 37%. This is insufficient to ensure recovery of North Sea cod within the next decade. Abstract A variety of tools are available to quantify uncertainty in age-structured fish stock assessments and in management forecasts. These tools are based on particular choices for the underlying population dynamics model, the aspects of the assessment considered uncertain, and the approach for assessing uncertainty (Bayes, frequentist or likelihood). The current state of the art is advancing rapidly as a consequence of the availability of increased computational power, but there remains little consistency in the choices made for assessments and forecasts. This can be explained by several factors including the specifics of the species under consideration, the purpose for which the analysis is conducted and the institutional framework within which the methods are developed and used, including the availability and customary usage of software tools. Little testing of either the methods or their assumptions has yet been done. Thus, it is not possible to argue either that the methods perform well or perform poorly or that any particular conditioning choices are more appropriate in general terms than others. Despite much recent progress, fisheries science has yet to identify a means for identifying appropriate conditioning choices such that the probability distributions which are calculated for management purposes do adequately represent the probabilities of eventual real outcomes. Therefore, we conclude that increased focus should be placed on testing and carefully examining the choices made when conducting these analyses, and that more attention must be given to examining the sensitivity to alternative assumptions and model structures. Provision of advice concerning uncertainty in stock assessments should include consideration of such sensitivities, and should use model-averaging methods, decision tables or management procedure simulations in cases where advice is strongly sensitive to model assumptions.
