Résumé L’activité de télémédecine d’un centre de référence maladies rares (CNR) est un des indicateurs de son expertise et de sa capacité de recours. Les microangiopathies thrombotiques (MAT) sont des maladies dont la faible incidence et le niveau d’expertise requis pour leur prise en charge a justifié la mise en place d’un centre de référence, le CNR-MAT. En effet, la gravité des MAT met en jeu le pronostic vital des malades à court terme ; cependant, prises en charge de manière optimale, ces maladies peuvent être de pronostic excellent. Il est donc nécessaire de savoir les reconnaître afin d’en établir le diagnostic rapidement et d’instituer un traitement adapté en urgence. Dans ce travail, nous décrivons l’activité de télémédecine du CNR-MAT depuis sa création. Sa particularité est que le niveau d’urgence requis par les maladies relevant de son expertise implique souvent une réponse en temps réel, ce qui a eu pour conséquence la nécessité de mettre en place une organisation spécifique fonctionnelle 24/7. Cette activité, amenée à se développer dans les années à venir, nécessite d’être évaluée, quantifiée avec précision et valorisée. Thrombotic microangiopathies (TMA) represent rare diseases requiring a high skill for their management that deserved in France the identification of a dedicated National reference center. TMA are short-term life-threatening diseases; however, with an adapted management, their prognosis can be excellent. It is therefore mandatory to recognize and treat them rapidly according to standard guidelines. Telemedicine is a specialized hub consisting of highly skilled staff trained in a specific domain of medicine. The telemedicine activity of a reference center is an important representative indicator of its expertise and recourse ability. It requires to be accurately evaluated and promoted. In this work, we report the French reference center for TMA telemedicine activity since its setting-up. TMA represent an interesting model of diseases that required a specific organization of telemedicine activity to adapt to clinicians demand, which includes particularly the need to answer resorts in real time 24/7. We have previously shown that tissue cultures derived from various explants of the wheat variety Chinese Spring exhibit organ/tissue-specific changes in the organization of their mitochondrial genome. The aim of this work was to study the influence of passage out of in-vitro culture, and subsequent plant regeneration, on the in vitro 'induced' reorganization of this genome. In all cases but one, subgenomic configurations present in both the donor parent and the tissue culture were evident, in corresponding regenerated plants. The presence, in regenerated plants, of subgenomic configurations found in tissue culture but undetectable in the donor parent appeared to be both timeand organ/tissue-dependent. Moreover, when present, these novel organizations were not systematically found in all regenerated plants. Finally, novel subgenomic configurations were specifically detected after passage out of in-vitro culture. As these results were obtained from a single plant variety, they clearly confirm the extreme plasticity of mitochondrial genome structure in response to in-vitro culture. [ABSTRACT FROM AUTHOR]

Abstract Background The deep-sea hydrothermal vent mussel Bathymodiolus azoricus harbors thiotrophic and methanotrophic symbiotic bacteria in its gills. While the symbiotic relationship between this hydrothermal mussel and these chemoautotrophic bacteria has been described, the molecular processes involved in the cross-talking between symbionts and host, in the maintenance of the symbiois, in the influence of environmental parameters on gene expression, and in transcriptome variation across individuals remain poorly understood. In an attempt to understand how, and to what extent, this double symbiosis affects host gene expression, we used a transcriptomic approach to identify genes potentially regulated by symbiont characteristics, environmental conditions or both. This study was done on mussels from two contrasting populations. Results Subtractive libraries allowed the identification of about 1000 genes putatively regulated by symbiosis and/or environmental factors. Microarray analysis showed that 120 genes (3.5% of all genes) were differentially expressed between the Menez Gwen (MG) and Rainbow (Rb) vent fields. The total number of regulated genes in mussels harboring a high versus a low symbiont content did not differ significantly. With regard to the impact of symbiont content, only 1% of all genes were regulated by thiotrophic (SOX) and methanotrophic (MOX) bacteria content in MG mussels whereas 5.6% were regulated in mussels collected at Rb. MOX symbionts also impacted a higher proportion of genes than SOX in both vent fields. When host transcriptome expression was analyzed with respect to symbiont gene expression, it was related to symbiont quantity in each field. Conclusions Our study has produced a preliminary description of a transcriptomic response in a hydrothermal vent mussel host of both thiotrophic and methanotrophic symbiotic bacteria. This model can help to identify genes involved in the maintenance of symbiosis or regulated by environmental parameters. Our results provide evidence of symbiont effect on transcriptome regulation, with differences related to type of symbiont, even though the relative percentage of genes involved remains limited. Differences observed between the vent site indicate that environment strongly influences transcriptome regulation and impacts both activity and relative abundance of each symbiont. Among all these genes, those participating in recognition, the immune system, oxidative stress, and energy metabolism constitute new promising targets for extended studies on symbiosis and the effect of environmental parameters on the symbiotic relationships in B. azoricus.

Abstract Background Heterokont algae, together with cryptophytes, haptophytes and some alveolates, possess red-algal derived plastids. The chromalveolate hypothesis proposes that the red-algal derived plastids of all four groups have a monophyletic origin resulting from a single secondary endosymbiotic event. However, due to incongruence between nuclear and plastid phylogenies, this controversial hypothesis remains under debate. Large-scale genomic analyses have shown to be a powerful tool for phylogenetic reconstruction but insufficient sequence data have been available for red-algal derived plastid genomes. Results The chloroplast genomes of two brown algae, Ectocarpus siliculosus and Fucus vesiculosus, have been fully sequenced. These species represent two distinct orders of the Phaeophyceae, which is a major group within the heterokont lineage. The sizes of the circular plastid genomes are 139,954 and 124,986 base pairs, respectively, the size difference being due principally to the presence of longer inverted repeat and intergenic regions in E. siliculosus. Gene contents of the two plastids are similar with 139-148 protein-coding genes, 28-31 tRNA genes, and 3 ribosomal RNA genes. The two genomes also exhibit very similar rearrangements compared to other sequenced plastid genomes. The tRNA-Leu gene of E. siliculosus lacks an intron, in contrast to the F. vesiculosus and other heterokont plastid homologues, suggesting its recent loss in the Ectocarpales. Most of the brown algal plastid genes are shared with other red-algal derived plastid genomes, but a few are absent from raphidophyte or diatom plastid genomes. One of these regions is most similar to an apicomplexan nuclear sequence. The phylogenetic relationship between heterokonts, cryptophytes and haptophytes (collectively referred to as chromists) plastids was investigated using several datasets of concatenated proteins from two cyanobacterial genomes and 18 plastid genomes, including most of the available red algal and chromist plastid genomes. Conclusion The phylogenetic studies using concatenated plastid proteins still do not resolve the question of the monophyly of all chromist plastids. However, these results support both the monophyly of heterokont plastids and that of cryptophyte and haptophyte plastids, in agreement with nuclear phylogenies.

Abstract Background AVE9633 is a new immunoconjugate comprising a humanized monoclonal antibody, anti-CD33 antigen, linked through a disulfide bond to the maytansine derivative DM4, a cytotoxic agent and potent tubulin inhibitor. It is undergoing a phase I clinical trial. Chemoresistance to anti-mitotic agents has been shown to be related, in part, to overexpression of ABC proteins. The aim of the present study was to investigate the potential roles of P-gp, MRP1 and BCRP in cytotoxicity in AVE9633-induced acute myeloid leukaemia (AML). Methods This study used AML cell lines expressing different levels of P-gp, MRP1 or BCRP proteins and twenty-five samples from AML patients. Expression and functionality of the transporter protein were analyzed by flow cytometry. The cytotoxicity of the drug was evaluated by MTT and apoptosis assays. Results P-gp activity, but not MRP1 and BCRP, attenuated AVE9633 and DM4 cytotoxicity in myeloid cell lines. Zosuquidar, a potent specific P-gp inhibitor, restored the sensitivity of cells expressing P-gp to both AVE9633 and DM4. However, the data from AML patients show that 10/25 samples of AML cells (40%) were resistant to AVE9633 or DM4 (IC50 > 500 nM), and this was not related to P-gp activity (p-Value: 0.7). Zosuquidar also failed to re-establish drug sensitivity. Furthermore, this resistance was not correlated with CD33 expression (p-Value: 0.6) in those cells. Conclusion P-gp activity is not a crucial mechanism of chemoresistance to AVE9633. For patients whose resistance to conventional anthracycline AML regimens is related to ABC protein expression, a combination with AVE9633 could be beneficial. Other mechanisms such as microtubule alteration could play an important role in chemoresistance to AVE9633.

Objectifs Évaluer l’efficacité et la tolérance de la tomothérapie hélicoïdale des méningiomes intracrâniens de grade II ou III. Patients et méthodes Entre mai 2011 et janvier 2015, 19 patients, 14 hommes et 5 femmes, d’âge médian 63 ans [41–76], atteints d’un méningiome histologiquement prouvé (deux à cellules claires, quatre anaplasiques, sept atypiques, un myxoïde, cinq sans précision), ont reçu une tomothérapie hélicoïdale, dont neuf exclusivement après une récidive, huit après une résection incomplète et deux après une résection complète. Les doses délivrées aux volumes cibles prévisionnels de bas et haut risques étaient respectivement de 59,4 Gy et 68,4 Gy. La toxicité aiguë a été relevée selon la Common Terminology Criteria for Adverse Events vV4.0. Résultats La durée médiane de suivi était de 16 mois [1,6–39,7]. Le délai moyen entre la dernière chirurgie et la radiothérapie était de 4,5 mois [1,4–98,4]. Quatre patients (21 %) ont été atteints d’une récidive locale, en médiane à 19 mois. Les taux de contrôle local à 1 et 2 ans étaient respectivement de 88,9 % et 77,8 %. Un patient est décédé de sa tumeur à 18,1 mois. La probabilité de survie globale à 2 ans était de 88,9 %. Dix-huit patients ont souffert d’une toxicité aiguë, 18 d’une alopécie de grade II-III, 12 d’une dermite de grade I-II, cinq de céphalées et nausées de grade I-II. Dix patients ont reçu une corticothérapie. L’indice de Karnofsky médian était à la fin du traitement était de 80. Conclusions La tomothérapie hélicoïdale des méningiomes de grade II et III semble associée à une bonne tolérance et à un bon taux de contrôle tumoral. Cependant, un nombre de patients et un suivi plus importants sont indispensables pour préciser la place de la tomothérapie hélicoïdale. [ABSTRACT FROM AUTHOR]

Objectif de l’étude La lymphopénie sévère est une complication fréquente de la chimioradiothérapie des cancers du pancréas. Des données récentes ont suggéré un impact défavorable de la lymphopénie sévère (inférieure ou égale à 0,5 × 109/L) sur la survie, avec une association directe entre la dose d’irradiation délivrée à la rate et le risque de lymphopénie sévère. Cependant, aucune contrainte dosimétrique dans la rate n’a été validée pour la chimioradiothérapie des cancers du pancréas. L’objectif de cette étude était de déterminer les contraintes dosimétriques dans la rate susceptible de diminuer le risque de lymphopénie sévère et d’améliorer la probabilité de survie sans progression. Matériel et méthode Une analyse rétrospective des dossiers de 27 patients pris en charge par chimioradiothérapie à la Pitié-Salpêtrière entre 2009 et 2017 pour un cancer du pancréas localement évolué a été faite. La dose de prescription dans le volume cible prévisionnel était de 54 Gy en 30 fractions, en association à une chimiothérapie concomitante par capécitabine. La majorité des patients (63 %) a été prise en charge par radiothérapie conformationnelle avec modulation d’intensité. Les contraintes dosimétriques analysés étaient la dose moyenne (DM rate), le V5 Gy, le V10 Gy et le V20 Gy (V × Gy : volume recevant × Gy). Les numérations-formules hebdomadaires ont été recueillies pendant la chimioradiothérapie et jusqu’à 12 semaines après. Le test t de Student a été utilisé pour l’analyse de la corrélation entre les contraintes dosimétriques et la survenue d’une lymphopénie sévère. L’analyse de la survie sans progression était réalisée avec le test du Log-rank et le modèle de Cox. Résultats La survenue d’une lymphopénie sévère était fréquente : 74 % (n = 20/27). La DM rate (p = 0,03), le V10 Gy = 0,002 et le V20 Gy (p = 0,02) étaient corrélées avec un risque de lymphopénie sévère, contrairement au V5 Gy (p = 0,09). En analyse unifactorielle, les paramètres influençant la survie sans progression étaient la DM rate < 3,3 Gy (hazard ratio [HR] : 0,31 ; intervalle de confiance à 95 % [IC 95 %] : 0,10–0,91 ; p = 0,03), la chirurgie (p = 0,02) et l’âge au moment du diagnostic (p = 0,05), avec une tendance retrouvée pour le volume cible prévisionnel (p = 0,10). L’analyse multifactorielle avec les paramètres précédents retrouvait une amélioration de la probabilité de survie sans progression avec une DM rate < 3,3 Gy (HR : 0,15 ; IC 95 % : 0,034–0,64 ; p = 0,01). Conclusion La DM rate , le V10 Gy et le V20 Gy étaient corrélées avec le risque de lymphopénie sévère. Une meilleure probabilité de survie sans progression a été observée avec une DM rate < 3,3 Gy. Des données prospectives complémentaires sont nécessaires afin de confirmer ces résultats. [ABSTRACT FROM AUTHOR]
