Risikofaktoren für eine gastrointestinale Ulkusblutung Die Helicobacter-pylori-Gastritis (H. pylori-Gastritis) und die Therapie mit nicht steroidalen Antirheumatika (NSAR) sind die wichtigsten Risikofaktoren für die Entstehung eines peptischen Ulkus (PUD) und einer Ulkusblutung. Eine Infektion mit H. pylori erhöht das Risiko einer Ulkusblutung bei Patienten mit PUD, die NSAR, Aspirin oder einen anderen Plättchen-Aggregationshemmer einnehmen. H. pylori-positive Patienten unter kombinierter Plättchen-Aggregationshemmung tragen das höchste Blutungsrisiko. Evidenzbasierte interdisziplinäre Behandlungsempfehlungen zur sicheren NSAR-Anwendung aus dem Jahr 2020 berücksichtigen sowohl das kardiovaskuläre als auch das gastrointestinale Risikoprofil der einzelnen Patienten. Für Patienten mit einem moderaten Risiko für die Entstehung eines PUD wird die Therapiekombination aus NSAR und Protonen-Pumpen-Inhibitoren (PPI) oder eine Monotherapie mit selektiven COX-2-Inhibitoren (COX-2: Cyclooxygenase-2) empfohlen. Patienten mit einem hohen Blutungsrisiko dagegen sollten eine Kombinationstherapie aus selektivem COX-2-Inhibitor und PPI erhalten. Bei Patienten mit einem hohen kardiovaskulären Risiko, bei denen auf eine NSAR-Therapie nicht verzichtet werden kann, werden Naproxen oder Celecoxib empfohlen. Die Behandlung der gastrointestinalen Ulkusblutung Die Ulkusblutung geht mit einer erhöhten Letalität einher. Bei hämodynamisch stabilen Patienten mit Zeichen einer oberen gastrointestinalen Blutung und erhöhtem Sterberisiko (Glasgow-Blatchford Score ≥ 12) ist die Durchführung einer Ösophago-Gastro-Duodenoskopie (ÖGD) nach 6–24 Stunden ausreichend, eine Notfall-ÖGD (innerhalb der ersten 6 Stunden) verbessert vergleichsweise die 30-Tage-Letalität nicht. Nach erfolgreicher endoskopischer Hämostase reduziert eine zusätzliche prophylaktische angiografische Embolisation das Risiko einer erneuten Blutung nicht. Die Helicobacter-pylori-Eradikation Eine erfolgreiche H. pylori-Eradikation reduziert das Risiko für die Entwicklung eines Magenkarzinoms bei Verwandten ersten Grades von Patienten mit Magenkarzinom um 73 %. Bei Patienten mit erfolgreicher endoskopischer Behandlung eines Magenfrühkarzinoms ist eine H. pylori-Testung mit anschließender Eradikation ebenfalls sinnvoll, da hierdurch die Rate an metachronen Magenkarzinomen halbiert werden konnte. Die Standard-Triple-Therapie zu der H. pylori-Eradikation zeigt aufgrund zunehmender Antibiotikaresistenzen, insbesondere gegen Makrolide, eine nachlassende Wirksamkeit. Dementsprechend wird die Bismut-haltige Quadrupel-Therapie als empirische Standard-Erstlinientherapie empfohlen.

We study the regularity of solutions of the Poisson equation with Dirichlet, Neumann and mixed boundary values in polyhedral cones [Formula: see text] in the specific scale [Formula: see text] of Besov spaces. The regularity of the solution in these spaces determines theorder of approximation that can be achieved by adaptive and nonlinear numerical schemes. We aim for a thorough discussion of homogeneous and inhomogeneous boundary data in all settings studied and show that the solutions are much smoother in this specific Besov scale compared to the fractional Sobolev scale [Formula: see text] in all cases,which justifies the use of adaptive schemes.

"An infinite number of fitness apps are available on various app stores. However, hardly any of them are fitted to the needs and requirements of care-dependent peo-ple. This paper investigates the effectiveness of a customised fitness-app prototype for increasing physical activity in home care service users. Home care service users from Austria and Italy were randomly assigned to two groups. In total, 216 partici-pants were involved in the field trial, 104 received a tablet with the fitness app and an activity tracker (treatment group), 112 did not (control group). Regularity of physical activity, frequency of fitness exercises and walking behaviour were self- reported by participants at baseline, after 4 months and after 8 months. In addition, the frequency of using the prototype was assessed based on the fitness app's logged usage data. We estimated multilevel mixed-effects ordered logistic models to examine the effects of the intervention. After 4 months, the intervention increased the home care users’ probability of agreeing strongly with being physically active on a regular basis by 28 percentage points (p< 0.001; 95% CI: 0.20, 0.36) and their probability of reporting to exercise more than once a week by 45 percentage points (p< 0.001; 95% CI: 0.32, 0.57). Walking behaviour was not affected on group- level but improved for frequent users of the activity tracker. Frequent and regular users of the fitness app benefited most and effects persisted until the end of the 8 months controlled trial. Tailoring a fitness-app prototype to the needs of care-dependent people has the potential to support people with functional limitations to engage in a more active lifestyle. Future research is encouraged to seek further insights into how new technologies can sup-port physical activities in people with long-term care needs."

To support the increasing number of older people, new (assistive) technologies are constantly being developed. For these technologies to be used successfully, future users need to be trained. Due to demographic change, this will become difficult in the future, as the resources for training will no longer be available. In this respect, coaching robots could have great potential to support younger seniors in particular. However, there is little evidence in the literature about the perceptions and potential impact of this technology on the well-being of older people. This paper provides insights into the use of a robot coach (robo-coach) to train younger seniors in the use of a new technology. The study was carried out in Austria in autumn 2020, involving 34 participants equally distributed among employees in their last three years of service and retirees in their first three years of retirement (23 female; 11 male). The aim was to assess participants’ expectations and perceptions by examining the perceived ease of use and user experience of the robot in providing assistance during a learning session. The findings reveal a positive impression of the participants and promising results for using the robot as a coaching assistant in daily tasks.

We study approximation classes for adaptive time-stepping finite element methods for time-dependent partial differential equations. We measure the approximation error in $L_2([0,T)\times \varOmega )$ and consider the approximation with discontinuous finite elements in time and continuous finite elements in space, of any degree. As a by-product we define anisotropic Besov spaces for Banach-space-valued functions on an interval and derive some embeddings, as well as Jackson- and Whitney-type estimates.

This paper is concerned with the regularity of solutions to linear and nonlinear evolution equations extending our findings in [22] to domains of polyhedral type. In particular, we study the smoothness in the specific scale $B^r_{\tau,\tau}$, $\frac{1}{\tau}=\frac rd+\frac 1p$ of Besov spaces. The regularity in these spaces determines the approximation order that can be achieved by adaptive and other nonlinear approximation schemes. Preemptive pharmacogenetic testing is reimbursed by health care insurance for only seven drugs (abacavir, carbamazepine, 6-mercaptopurine, azathioprine, 5-fluorouracil, capecitabine, and irinotecan) although, it is proposed to be a cost-effective approach to personalized medicine. The aim of this study was to describe the use of PGx drugs and their corresponding genes in Switzerland.Methods: We identified 90 drugs with dosing recommendations from the Pharmacogenetic Knowledgebase involving 24 genes. We assessed the utilization of those drugs between 2016 and 2020, using claims data from a large Swiss insurance company (Helsana).Results: Of 841 491 persons with drug claims during the whole study period, 78.7% were exposed to PGx drugs. Ibuprofen, pantoprazole, and tramadol had the highest number of users. Seven genes (CYP2C19, CYP2C9, CYP2D6, SLCO1B1, HLA-B, MT-RNR1, and VKORC1) were responsible for over 95% of all potential drug-gene interactions.Conclusion: The prevalence of PGx drug prescriptions is high in the Swiss population. Therefore, intensified preemptive testing may be a useful option as a substantial amount of the Swiss population might benefit.Keywords: PGx, drug use, claims data, pharmacoepidemiology

Objectives To evaluate the potential of blood glucose levels and weight change before the onset of diabetes as predictors of pancreatic cancer among subjects with new-onset diabetes, that is, cancer-related diabetes versus normal type 2 diabetes. Methods We conducted a case-control study among subjects with new diabetes in the United Kingdom-based Clinical Practice Research Datalink. Cases were pancreatic cancer subjects with diabetes for ≤2 years before the cancer diagnosis (i.e., cancer-related diabetes). Controls were cancer-free, type 2 diabetic subjects matched to cases on age, sex, and diabetes duration. We calculated adjusted odds ratios (aORs) for pancreatic cancer as a function of both weight change and blood glucose before the onset of diabetes. Results Weight loss of 10.0%–14.9% at diabetes onset was associated with an aOR for pancreatic cancer of 3.58 (95% CI 2.31–5.54), loss of ≥15.0%, with an aOR of 4.56 (95% CI 2.82–7.36), compared with stable weight. Blood glucose levels of ≤5.1 mmol/L or 5.2–5.6 mmol/L before diabetes onset were associated with an increased risk of a pancreatic cancer diagnosis, with aORs of 2.42 (95% CI 1.60–3.66) and 2.20 (95% CI 1.45–3.35), respectively, when compared with blood glucose levels ≥6.3 mmol/L within >2–3 years before cancer detection. Conclusions Weight loss as well as blood glucose levels in the normal range (and thus rapid development of hyperglycemia) before diabetes onset may be predictive of pancreatic cancer-related diabetes and may help target which subjects with new diabetes to refer for pancreatic cancer screening examinations.

We study traces of certain subspaces of shearlet coorbit spaces on smooth domains in R d with d = 2 , 3 . Our results are based on embedding theorems into Besov spaces which enable us to establish embedding relations of traces on the boundary of these domains.

We study traces on the boundary of generalized smoothness Morrey spaces on C k domains Ω . These spaces are equipped with three parameters s , p , q and a function parameter φ . Our results remain valid for the usual Besov–Morrey spaces N u , p , q s ( Ω ) , Triebel–Lizorkin–Morrey spaces E u , p , q s ( Ω ) , and Triebel–Lizorkin type spaces F p , q s , τ ( Ω ) , which are all included in our scales as special cases. Moreover, to complete our investigation, we also study traces of Besov-type spaces B p , q s , τ ( Ω ) , which are not covered by our approach and give some applications in terms of a priori estimates generalizing results from El Baraka (2005).

Based on socio-economic and medical improvements as well as healthier lifestyles, a majority of baby boomer pensioners still feel fit and active and thus may struggle with transitioning into retirement. They could benefit from an ICT system to ease their transition into retirement and support general well- being.The aim of this paper was to gain insights into the requirements for a digital coach to support their transition into retirement.Two rounds of focus groups with older employees close to retirement, people just retired and relatives of retired were conducted. Outcomes were then identified through a framework analysis and integrated into the design of a digital coach.Four functional groups (i) physical activity, (ii) cognitive support, (iii) emotional support and (iv) social support were identified.

Unexplained iron-deficiency anemia is an important marker for colorectal cancer (CRC). Our objectives were as follows: (a) to assess whether the association between anemia and CRC can be detected on the 'Clinical Practice Research Datalink', (b) to evaluate the timing between laboratory changes and CRC detection, and (c) to analyze its association with survival. We conducted a case-control study on patients with an incident CRC diagnosis during 2008-2012 and a 1 : 1-matched control group. We compared anemia markers serum ferritin (SF), hemoglobin (Hb), mean corpuscular volume (MCV), and red blood cell count between cases and controls using conditional logistic regression. We assessed survival in CRC cases. SF values up to 20 ng/ml were associated with an odds ratio [OR (95% confidence interval)] of 10.66 (6.88-16.51) compared with SF values of 101-300 ng/ml when restricted to measurements up to 180 days before the CRC diagnosis. For measurements taken at 1 year or earlier before the diagnosis, the OR was 2.02 (1.57-2.61). For Hb values less than 9 g/dl compared with Hb values of 13.0-15.9 g/dl the corresponding ORs were 74.25 (34.69-158.91) and 2.19 (1.31-3.67), respectively. The corresponding ORs for MCV values up to 80 fl compared with MCV values of 86-95 fl were 13.94 (10.31-18.85) and 1.89 (1.51-2.36), respectively. Low levels of these markers were only weakly associated with survival. Hb, MCV, and SF levels substantially dropped only shortly before the CRC diagnosis. Although slightly more cases had anemia markers compared with controls at 1 year or earlier before the diagnosis, most cases still had normal values. The Clinical Practice Research Datalink is well-suited to detect associations between low Hb, MCV, and SF levels and CRC.

Background: Retirement is recognized as a factor influencing the ageing process. Today, virtual health coaching systems can play a pivotal role in supporting older adults’ active and healthy ageing. This study wants to answer two research questions: (1) What are the user requirements of a virtual coach (VC) based on an Embodied Conversational Agent (ECA) for motivating older adults in transition to retirement to adopt a healthy lifestyle? (2) How could a VC address the active and healthy ageing dimensions, even during COVID-19 times? Methods: Two-wave focus-groups with 60 end-users aged 55 and over and 27 follow-up telephone interviews were carried out in Austria, Italy and the Netherlands in 2019–2020. Qualitative data were analysed by way of framework analysis. Results: End-users suggest the VC should motivate older workers and retirees to practice physical activity, maintain social contacts and emotional well-being. The ECA should be reactive, customizable, expressive, sympathetic, not directive nor patronizing, with a pleasant and motivating language. The COVID-19 outbreak increased the users’ need for functions boosting community relationships and promoting emotional well-being. Conclusions: the VC can address the active and healthy ageing paradigm by increasing the chances of doing low-cost healthy activities at any time and in any place.

We study unboundedness of smoothness Morrey spaces on bounded domains Ω ⊂ Rn in terms of growth envelopes. It turns out that in this situation the growth envelope function is finite—in contrast to the results obtained by Haroske et al. (2016) for corresponding spaces defined on Rn. A similar effect was already observed by Haroske et al. (2017), where classical Morrey spaces Mu,p(Ω) were investigated. We deal with all cases where the concept is reasonable and also include the tricky limiting cases. Our results can be reformulated in terms of optimal embeddings into the scale of Lorentz spaces Lp,q(Ω).

In recent years, assistant systems have come to widespread use and support people in various situations e.g. in getting from A to B. For quite a time also assistant systems with special attention to older people have been developed. For example, in case of cognitive impairments where autonomous living indoors as well as outdoors is affected, assistant systems can be valuable aids. First attempts for outdoor assistance with GPS-based location systems offering the possibility to define geo-fences for raising an alert if a known area is left have been made. The quality of these systems is largely dependent on the precision of localisation which among others is influenced by the sampling rate. This paper reports on an empirical study under real world conditions to determine a suitable GPS sampling rate for movement analysis of cognitively impaired pedestrians. The work considers GPS measurement and interpolation errors as well as track losses as a result of low sampling rates. For the study, GPS data for different environmental settings and movement scenarios for sampling rates of 1, 2, 3, 4, 5, 10, 15, 20 and 25 s has been collected. The impact of sampling rates on movement parameters like track length and speed has been empirically measured. Additionally, the influence of smoothing approaches on data quality and whether downsampling of data has the same effect as recording with corresponding lower sampling rate has been studied. Results show that across all tested scenarios a sampling rate of 3–5 s seems to be appropriate with respect to speed and track length. Additionally, it can be argued that smoothing improves data quality of highly sampled data up to 4 s. With downsampling, outliers are less in comparison to data sampled at the corresponding sampling rate.

This article explores the social lives of young adults with disabilities as they were transitioning from school to adult life. The young adults were former participants of the Access to Community Employment and Education Program (ACEE) in Halifax, Nova Scotia, Canada, a program, which focuses on the transition of young people with disabilities from school and youth to adulthood, employment, and independent living. The data in the article were drawn from a mixed method research project that explored, through a survey and follow-up focus groups and interviews, the impact of the ACEE program and the lived experiences of its alumni in the years after having completed the program. The study highlights the alumni's social lives, their leisure activities and their relationships and the different ways the participants are living their lives in the community several years after having completed the ACEE program. The data shows that the program is playing an important role in creating lasting friendships and a sense of belonging amongst its peers.

Als Antwort auf die Veranderungen der Arbeitswelt und den ­demografischen Wandel spielt in vielen Unternehmen das ­Betriebliche Gesundheitsmanagement (BGM) eine zentrale Rolle. Bei der Suche nach externer Unterstutzung sind die spezifischen Kompetenzen von Psychotherapeuten sehr gefragt.

Objective When questionnaires are used for pain assessment, the order effect must be taken into consideration; that is, whether the answer to a pain item is influenced by answers given to previous items. This study aimed to evaluate possible order effects in children and adolescents when answering pain items. Furthermore, the possible influence of sex on order effects was analyzed. Method Three pain items (worst, at movement, and at rest) were given in 6 different orders. Two hundred and 46 postoperative hospitalized children and adolescents (age range of 11-18 years, mean of 14.4 years; female: 41%, duration of surgery range of 2-274 min) participated in this study. Each item order was answered by 40-43 children and adolescents. Results While there were no general order effects, we observed a sex-specific order effect. The position of pain at rest (p = .034) and pain on movement (p = .036) items had different influences on worst pain values in female patients compared to male patients. A sex-specific influence on pain at rest values was only caused by the position of pain on movement items (p = .036). Conclusion The fact that male and female pediatric patients are differently influenced by the order of pain items has to be considered in planning a questionnaire. As more specific items (pain on movement) are less influenced by the item order effect, they should be preferred.

Existing full-body tracking systems, which use Inertial Measurement Units (IMUs) as sensing unit, require expert knowledge for setup and data collection. Thus, the daily application for human body tracking is difficult. In particular, in the field of active and assisted living (AAL), tracking human movements would enable novel insights not only into the quantity but also into the quality of human movement, for example by monitoring functional training. While the current market offers a wide range of products with vastly different properties, literature lacks guidelines for choosing IMUs for body tracking applications. Therefore, this paper introduces developments towards an IMU evaluation framework for human body tracking which compares IMUs against five requirement areas that consider device features and data quality. The data quality is assessed by conducting a static and a dynamic error analysis. In a first application to four IMUs of different component consumption, the IMU evaluation framework convinced as promising tool for IMU selection.

Natural extracellular matrix-derived biomaterials from decellularized allogenic tissues are of increasing interest for tissue engineering because their structure and composition provide a complexity that is not achievable with current manufacturing techniques. The prerequisite to bring allogenic tissue from bench to bedside as a functional biomaterial is the full removal of cells while preserving most of its native characteristics such as structure and composition. The exceptionally dense structure of articular cartilage, however, poses a special challenge for decellularization, scaffold preparation, and reseeding. Therefore, we tested 24 different protocols aiming to remove cells and glycosaminoglycans (GAG) while preserving the collagen backbone and ultrastructure. The resulting matrices were analyzed for cell removal (DNA quantification, haematoxylin and eosin staining), GAG content (dimethyl methylene blue assay, Alcian blue staining and micro-computed tomography), collagen integrity (immunohistochemistry and ultrastructure), and biomechanics (compression test). Furthermore, seeding tests were conducted to evaluate cell viability and attachment to the scaffolds. Sodium dodecyl sulfate-based protocols yielded satisfactory reduction of DNA content, yet had negative effects on cell viability and attachment. Hydrochloric acid efficiently decellularized the scaffold and pepsin emerged as best option for GAG depletion. Combining these two reagents led to our final protocol, most efficient in DNA and GAG depletion while preserving the collagen architecture. The compressive modulus decreased in the absence of GAG to ∼1/3 of native cartilage, which is significantly higher than that by commercially available scaffolds tested as a reference (ranging from 1/25 to 1/100 of native cartilage). Cytocompatibility tests showed that human adipose-derived stromal cells readily adhered to the scaffold. In this study, we established a protocol combining freeze-thaw cycles, osmotic shock, and treatment with hydrochloric acid followed by a pepsin digestion step, achieving successful decellularization and GAG depletion within 1 week, resulting in a cytocompatible material with intact collagen structure. The protocol provides a basis for the generation of allogeneic scaffolds, potentially substituting manufactured scaffolds currently used in clinical articular cartilage treatment.

This article proposes a reflection on the issue of the terminology of inclusive education that has been dominating the national and international debates. Reviewing some of the relevant literature and drawing from observations in the education system, the author poses the question if the terms of inclusion and inclusive education represent the most operational terminology when it comes to education of children with disabilities and the different contexts in which their education and schooling occurs. Using examples from four different research studies in France, Germany and Canada, the author demonstrates that inclusion, under the lens of social participation, needs to be reconsidered by listening to people with disabilities, and also by accommodating the need for designated spaces that are offering a reprieve and safety from an ableist society.

Since 2012 six AAL pilot regions were launched in Austria. The main goal of these pilot regions is to evaluate the impact of AAL technologies in daily use considering the entire value chain. Additionally, go-to market strategies for assistive technologies based on an involvement of all relevant stakeholders are developed. Within this paper an overview of the specific objectives, approaches and the status of all Austrian AAL pilot regions is given. Taking into account the different experiences of the different pilot regions, specific challenges in establishing, implementing and sustaining pilot region projects are discussed and lessons-learned are presented. Results show that a careful planning of all project phases taking into account available resources is crucial for the successful implementation of an AAL pilot region. In particular, this applies to all activities related to the active involvement of end-users.

We study necessary and sufficient conditions for embeddings of Besov spaces of generalized smoothness B p , q ? , N ( R n ) into generalized Holder spaces ? ∞ , r µ ( ? ) ( R n ) when s ? ( N ? - 1 ) 0 and ? - 1 ? ? q ' , where ? = ? N - n / p . A borderline situation, corresponding to the limiting situation in the classical case, is included and give new results. In particular, we characterize optimal embeddings for B -spaces.As immediate applications of our results we obtain continuity envelopes and give upper and lower estimates for approximation numbers for the related embeddings.We also consider the analogous results for the Triebel-Lizorkin spaces of generalized smoothness F p , q ? , N ( R n ) .

Summary Background Ocular toxicity was described in the late 1950s for some anti-malarial drugs, but only limited information is available on the comparison of ocular toxicity of different anti-malarials. Methods We conducted a follow-up study with a nested case-control analysis using the General Practice Research Database to compare the risk of developing a first-time diagnosis of an eye disorder during exposure of mefloquine, chloroquine and/or proguanil or atovaquone/proguanil use to non-users. We calculated incidence rates with 95% confidence intervals (CI) and odds ratios using multivariate conditional logistic regression analyses. Results We included 83,148 patients and identified 652 cases with an incident eye disorder. The incidence rates with 95% CI of all eye disorders combined in users of mefloquine, chloroquine and/or proguanil, atovaquone/proguanil or travellers not using anti-malarials were 5.3 (4.3–6.5), 7.1 (5.0–9.9), 6.3 (5.6–7.2) and 5.1 (4.6–5.7), per 1000 person-years, respectively. As compared to non-users of anti-malarials, the adjusted odds ratio with 95% CI in the nested case-control analysis for users of mefloquine, chloroquine and/or proguanil, or atovaquone/proguanil were 1.33 (1.01–1.75), 1.61 (1.06–2.45), and 1.25 (1.03–1.52), respectively. Conclusions The study provides evidence that there was an increased risk of eye disorders in users of all anti-malarials compared to non-users of anti-malarials.

We study fractional Sobolev and Besov spaces on noncompact Riemannian manifolds with bounded geometry. Usually, these spaces are defined via geodesic normal coordinates which, depending on the problem at hand, may often not be the best choice. We consider a more general definition subject to different local coordinates and give sufficient conditions on the corresponding coordinates resulting in equivalent norms. Our main application is the computation of traces on submanifolds with the help of Fermi coordinates. Our results also hold for corresponding spaces defined on vector bundles of bounded geometry and, moreover, can be generalized to Triebel-Lizorkin spaces on manifolds, improving [11].

We study traces in 2-microlocal Besov spaces with variable integrability on hyperplanes using atomic decompositions. Our results cover spaces of variable smoothness and also weighted spaces studied extensively in recent years.

Two general philosophical positions regarding inclusive education have moved to the forefront of inclusive education discourse in Western countries since the debate about inclusive education has begun. One is a moral doctrinal position advocating full inclusion and contending that integration is necessary to maintain universal norms of non-discrimination. The other is an ethical position advocating partial inclusion. This latter position argues that exceptional students should receive accommodations specific to their individual needs through a combination of general classroom instruction and specialized instruction within segregated settings. Taken to their individually reasoned ends, these two philosophical approaches represent opposing ideological views and suggest a largely irresolvable debate regarding how exceptional students are best served. This paper presents a comparative analysis examining the underpinnings of these dichotomies within Canada and France.

First we compute the trace space of Besov spaces – characterized via atomic decompositions – on fractals Γ, for parameters 0 < p < ∞, 0 < q ≤ min(1, p) and s = (n – d)/p. New Besov spaces on fractals are defined via traces for 0 < p, q ≤ ∞, s ≥ (n – d)/p and some embedding assertions are established. We conclude by studying the compactness of the trace operator TrΓ by giving sharp estimates for entropy and approximation numbers of compact embeddings between Besov spaces. Our results on Besov spaces remain valid considering the classical spaces defined via differences. The trace results are used to study traces in Triebel–Lizorkin spaces as well.

We determine the trace of Besov spaces Bsp,q(Ω) and Triebel-Lizorkin spaces F s p,q(Ω), characterized via atomic decompositions, on the boundary of C domains Ω for parameters 0 1 p . The limiting case s = 1 p is investigated as well. In terms of Besov spaces our results remain valid for the classical spaces Bsp,q(Ω) defined via differences. Previous large epidemiological studies reporting on the association between chronic obstructive pulmonary disease (COPD) and cardiovascular diseases mainly focussed on prevalent diseases rather than on the incidence of newly diagnosed cardiovascular outcomes. We used the UK-based General Practice Research Database (GPRD) to assess the prevalence and incidence of cardiovascular diseases in COPD patients aged 40-79 between 1995 and 2005, and we randomly matched COPD-free comparison patients to COPD patients. In nested-case control analyses, we compared the risks of developing an incident diagnosis of cardiac arrhythmias, venous thromboembolism, myocardial infarction, or stroke between patients with and without COPD, stratifying the analyses by COPD-severity, using COPD-treatment as proxy for disease severity. We identified 35,772 patients with COPD and the same number of COPD-free patients. Most cardiovascular diseases were more prevalent among COPD patients than among the comparison group of COPD-free patients. The relative risk estimates of developing an incident diagnosis of cardiac arrhythmia (OR 1.19, 95% CI 0.98-1.43), deep vein thrombosis (OR 1.35, 95% CI 0.97-1.89), pulmonary embolism (OR 2.51, 95% CI 1.62-3.87), myocardial infarction (OR 1.40, 95% CI 1.13-1.73), or stroke (OR 1.13, 95% CI 0.92-1.38), tended to be increased for patients with COPD as compared to COPD-free controls. The findings of this large observational study provide further evidence that patients with COPD are at increased risk for most cardiovascular diseases.

Purpose: Roatan, a small island in Honduras, is home to six ethnic groups. Due to financial constraints, many children have limited access to schooling. This article is a study on teachers’ perceptions of disabilities and students with disabilities and inclusive education on the island. Method: Twenty seven teachers working in public and private schools, and schools funded by the World Bank, were interviewed in March-April of 2014 in order to explore cultural and social representations of disabilities on the island. Results: The findings show that many of the teachers’ representations can be analysed under the lens of different models of disability - the medical model, the social model, and a religious-moral model. Inclusive education is perceived less as a means of including children with disabilities in the regular classroom, and more as a method of creating institutions to take care of their needs. Conclusion: There is a strong intersection of poverty, post-colonialism and disability which makes working under an inclusive lens very difficult for teachers. The cultural norms influence ideas of normalcy and disabilities, and the blame is on parents for having children with disabilities.

Summary Background Case reports and epidemiological studies have associated the use of mefloquine with neuropsychiatric adverse events. Methods We used the General Practice Research Database to conduct a follow-up study with a nested case–control analysis. We assessed the risk of developing first-time anxiety, stress-related disorders/psychosis, depression, epilepsy or peripheral neuropathies in patients using mefloquine, chloroquine and/or proguanil, or atovaquone/proguanil for malaria chemoprophylaxis, as compared to unexposed travelers. Results Compared to non-users of antimalarials, the adjusted odds ratio in the nested case–control analysis for users of mefloquine, chloroquine and/or proguanil, or atovaquone/proguanil were 0.71 (95% CI 0.56–0.90), 1.04 (95% CI 0.74–1.46), and 0.73 (95% CI 0.61–0.86) for anxiety or stress-related disorders combined, 0.54 (95% CI 0.41–0.71), 1.06 (95% CI 0.71–1.59), and 0.75 (95% CI 0.62–0.91) for depression, 0.69 (95% CI 0.35–1.36), 1.41 (95% CI 0.54–3.67), and 0.75 (95% CI 0.42–1.36) for epilepsy, and 1.22 (95% CI 0.50–2.99), 1.59 (95% CI 0.41–6.15), and 1.05 (95% CI 0.54–2.03) for neuropathies, respectively. The risk of all outcomes was higher in females than in males across all exposure categories. Conclusions The risk of neuropsychiatric disorders was similar for users and for non-users of anti-malarial chemoprophylaxis, with evidence for elevated risks in some subgroups.

Objective To study the function of miR-145, known to be dysregulated in endometriosis, and to identify its target genes in an in vitro endometriosis model. Design Experimental laboratory study. Setting University medical centers. Patient(s) Primary endometrial stroma cells were derived from eutopic endometrium of three American Society for Reproductive Medicine stage III endometriosis patients and from ectopic lesions of four patients with deep infiltrating endometriosis. Intervention(s) The human endometriotic cell line 12Z and primary eutopic and ectopic endometrial stroma cells were transiently transfected with miR-145 precursors or anti–miR-145 inhibitors and investigated for posttranscriptional regulation of predicted target genes and changes in cell behavior. Main Outcome Measure(s) Predicted target expression was measured by quantitative reverse transcription–polymerase chain reaction and Western blotting, and altered cell behavior was monitored by cell proliferation assays. The 12Z cells were additionally investigated by Matrigel invasion assays, cell cycle analysis, side population analysis, and aldehyde dehydrogenase activity assays. Result(s) In all cells investigated, miR-145 overexpression inhibited cell proliferation and induced down-regulation of FASCIN-1, SOX2, and MSI2 . In 12Z cells miR-145 upregulation increased Matrigel invasiveness and reduced side population and aldehyde dehydrogenase-1 activity. Additional down-regulated targets in 12Z cells included OCT4, KLF4, PODXL, JAM-A , and SERPINE1/PAI-1. ACTG2 and TAGLN were up-regulated upon pre–miR-145 transfection. JAM-A, FASCIN-1, and PAI-I down-regulation in 12Z cells were confirmed by Western blotting. Conclusion(s) miR-145 inhibits endometriotic cell proliferation, invasiveness, and stemness by targeting multiple pluripotency factors, cytoskeletal elements, and protease inhibitors.
