Liposuction for treatment of lymphedema is an effective and time-tested treatment. However, as there is a fear regarding further lymphatic damage caused by liposuction, we objectively compared lymphatic function pre- and post-liposuction. All patients with solid-predominant lymphedema who were treated during the study period of June 2014 and November 2018 were included. Patients were assessed using patient-reported baselines/outcomes, lymphedema-specific quality of life scale (LYMQOL), limb circumference/volume measurements, and indocyanine green lymphography (ICGL) preoperatively and at predefined postoperative time intervals. Fifty-seven limbs from 41 patients were included. Mean lipoaspirate volumes were 2035 mL, 5385 mL, and 3106 mL for the arm, thigh, and leg, respectively with a mean adipose fraction of the lipoaspirate of 71%. All patients underwent redundant skin excision with the "flying squirrel" technique. The mean follow-up was 10.7 months (range 3 - 48 months) with a mean limb volume reduction of 32.2% and all patients reporting satisfactory relief of symptoms. All showed statistically significant improvement in LYMQOL in symptoms, appearance, and function. On ICGL, none showed worsened lymphatic drainage, rather, all showed improved lymph drainage. Furthermore, the improved lymph drainage was found to be progressive during the study period in all patients. Our study results demonstrate that treating extremity lymphedema with liposuction does not worsen lymphatic function and in fact, paradoxically, it induces progressive improvement in lymph drainage. Two japonica rice cultivars with different panicle trait index (PTI), HP917 (a high-PTI cultivar) and DP128 (a low-PTI cultivar) were used to investigate the effects of the nitrogen (N) rate and plant density on the grain distribution of secondary branches on the panicle axis, leaf photosynthetic characteristics, and grain yield by a split plot design. The main plots were assigned to four N rates (0, 140, 200, and 260 kg ha-1), and the subplots were assigned to two plant densities: (D20, 15 plants m-2; D10, 30 plants m-2). Results showed that the grain yield was increased by increasing N rate and plant density, reaching a peak at N200 with D10. Compared with N0 treatment, the PTIs of HP917 and DP128 increased with an increase in the N rate, respectively. The PTIs of HP917 and DP128 increased by 4% with increasing plant density from D20 to D10. The leaf capacity was significantly affected by N rate and plant density. The grain distribution characteristics of secondary branches on the panicle axis was closely related to yield. Correlation analysis showed the PTI was positively correlated with grain yield and net photosynthetic rate. These results suggested the improvement in PTI from 0.15 to 0.52 was beneficial to increase the grain yield, which might contribute to the increased grain number of secondary branches of the middle and bottom panicle. The physiological responses of peanut seedlings exposed to low (5 µM) or high (200 µM) cadmium (Cd) concentration and the ability of sodium nitroprusside (SNP, a donor of NO) to reverse the harmful effects of Cd on peanut (Arachis hypogaea L.) were studied. Changes in plant growth parameters, chlorophyll content, antioxidant system, nutrient contents and Cd accumulation were investigated. The results showed that SNP and 5 µM Cd improved plant growth and chlorophyll content. Furthermore, antioxidative system was up-regulated, and as a result, the production rate of superoxide radical (O2•−) was reduced. Moreover, the absorption of nutrient elements was not impacted, and Cd toxicity was not observed. However, 200 µM Cd had negative effects on the above measured parameters and dramatically increased the accumulation of Cd in all the plant organs. In the 200 µM Cd treatment, addition of 250 µM SNP stimulated plant growth and increased chlorophyll content. It also enhanced the regulation of antioxidative system and reduced the production rate of O2•− and malondialdehyde (MDA) content. The global stability behavior of bird-shaped girders during construction is investigated. The critical torsional buckling moment is derived by the energy method and Galerkin method. Stability design criteria for bird-shaped girders are proposed for practical use. The proposed criteria have been used to evaluate the buckling capacities of several newly constructed bird-shaped roof structures and full-scale specimens. A good conservative estimation is generally observed.

This paper presents a derivation of exact expressions of moment-curvature relations of a circular tubular section. On the basis of these expressions and the observed behavior of full-scale specimens, an approximate method of second-order inelastic analysis applicable to pinned supported columns and modular falsework frames is presented. The approximate method is used to obtain load-deflection curves, and in the solution procedure, a set of simultaneous nonlinear equations is solved using MATHCAD. The load-deflection curves compare well with experimental results, and their application is helpful for verifying the quality of the modular tubular falsework units used in construction.

Abstract: Aims: The aim of the study was to survey rhizobial biogeography and to inoculate soybean with selected rhizobia in China to enhance symbiotic nitrogen fixation (SNF). Methods and Results: Biogeography, genetic diversity and phylogeny of soybean rhizobia were surveyed. Inocula were prepared and applied to soybean. Results showed that Bradyrhizobium elkanii and Ensifer fredii were widely distributed in acid and alkaline soils respectively. Available iron was detected as the first determinant for distribution of the two rhizobia and the soybean varieties did not greatly affect the rhizobial compatibility. Geographical latitude and precipitation in June were the main geographical and climatic factors affecting the rhizobial distribution. Inoculation with selected rhizobia increased the nodule number, fresh weight, occupation ratio, seed protein content and soybean yields. Conclusions: Selection and application of effective soybean rhizobia across China according to biogeography were clarified to promote the SNF, thereby improving soybean yield. Significance and Impact of the Study: Rhizobial diversity and biogeography were evaluated systematically in six sites across China. Available iron and soil pH are found to be the most important determinants for the distribution of soybean rhizobia. Basalt discriminant diagrams have been used to identify the tectonic setting of basaltic magmatism since the 1970s and have played an important role in reconstructing paleotectonic environments. However, the significant increase in the availability of geochemical data has led to a reassessment of these diagrams, suggesting that some of the tectonic settings indicated by these diagrams are not accurate. Here, we use a database of global ocean island basalt (OIB), mid-ocean ridge basalt (MORB), and island arc basalt (IAB) geochemistry to propose a series of new tectonic discriminant diagrams based on the ratios of large-ion lithophile elements (LILEs) to high field strength elements (HFSEs). These new diagrams indicate that the LILE can be used to differentiate OIB, MORB, and IAB samples, meaning that LILE/HFSE ratios can discriminate between these basalts that form in different tectonic settings. Our new diagrams can correctly assign samples to OIB, MORB, and IAB categories more than 85% of the time, with the discrimination between OIB and MORB having an accuracy of slightly less than 85%. Highly-transparent V-doped TiO2 thin films were prepared by sol-gel processing, spin coated on fused silica glass substrates, and annealed in air at 450 °C for 2 h. All of the films were comprised of anatase as the sole crystalline phase. The XPS data suggested that V formed an interstitial solid solution with anatase. The formation of an unsaturated solid solution at the lowest doping level of 0.05 mol% resulted in the formation of lattice defect sites, which enhanced anatase recrystallisation, grain growth, and crystallinity. In contrast, the formation of supersaturated/oversaturated solid solutions at the higher doping levels of 0.10-1.00 mol% resulted in lattice deformation, partial amorphisation, precipitation, and liquid formation. The coexistence of multiple valence states, viz., Ti4+/3+, V4+/3+, and possibly V5+ in the doped TiO2 films was attributed to intervalence charge transfer. Dye degradation tests revealed that the 0.05 mol% V-doped films showed the best photocatalyic performance and this was attributed to the combined effects of greater anatase crystallinity and reduced band gap. In contrast, increasing V-doping levels resulted in gradual decreases in the photocatalytic performance, which are attributed principally to the decrease in areal density of the surface-active sites owing to liquid-phase densification. The greater degradation of MB solution relative to that of RhB solution is attributed principally to the cationic nature of MB, which allows greater adsorption on the photocatalyst surface in comparison to the anionic RhB. OBJECTIVE: The present study aimed to clarify the expression pattern and prognostic role of miR-409-3p in breast cancer patients. MATERIALS AND METHODS: miR-409-3p levels in breast cancer tissues were tested by qRTPCR. The chi-square test and Fishers exact tests were used to examine the associations between miR-409-3p expression and the clinicopathological characters. Overall survival (OS) was estimated using the Kaplan-Meier method. The univariable and the multivariable Cox regression analyses were used to evaluate independent prognostic factors. RESULTS: miR-409-3p expression in breast cancer specimens was observed to be decreased compared with matched normal breast tissues (p < 0.01). Additionally, low miR-409-3p expression in breast cancer tissues was significantly associated with the advanced TNM stage (p = 0.004), lymph node metastasis (p = 0.001), and poorer pathological differentiation (p = 0.026). The patients in the low miR-409-3p group had a shorter overall survival than those in the high miR-409-3p group (p < 0.001). Furthermore, the univariate and the multivariate analyses showed that miR-409-3p expression was an independent predictor of overall survival (p < 0.05). BACKGROUND AND AIMS: In recent years, submucosal tunneling endoscopic resection (STER) was applied more and more often for single gastrointestinal (GI) submucosal tumor (SMT). However, little is known about this technique for treating multiple SMTs in GI tract. In the present study, we investigated the feasibility and outcome of STER for upper GI multiple SMTs originating from the muscularis propria (MP) layer. PATIENTS AND METHODS: A feasibility study was carried out including a consecutive cohort of 23 patients with multiple SMTs from MP layer in esophagus, cardia, and upper corpus who were treated by STER from June 2011 to June 2014. Clinicopathological, demographic, and endoscopic data were collected and analyzed. RESULTS: All of the 49 SMTs were resected completely by STER technique. Furthermore, only one tunnel was built for multiple SMTs of each patient in this study. En bloc resection was achieved in all 49 tumors. The median size of all the resected tumors was 1.5 cm (range 0.8-3.5 cm). The pathological results showed that all the tumors were leiomyoma, and the margins of the resected specimens were negative. The median procedure time was 40 min (range: 20-75 min). Gas-related complications were of the main complications, the rates of subcutaneous emphysema and pneumomediastinum, pneumothorax, and pneumoperitoneum were 13.0%, 8.7% and 4.3%. Another common complication was thoracic effusion that occurred in 2 cases (8.7%), among which only 1 case (4.3%) with low-grade fever got the drainage. Delayed bleeding, esophageal fistula or hematocele, and infection in tunnel were not detected after the operation there were no treatment-related deaths. The median hospital stay was 4 days (range, 2-9 days). No residual or recurrent lesion was found during the follow-up period (median 18, ranging 3-36 months). CONCLUSION: Submucosal tunneling endoscopic resection is a safe and efficient technique for treating multiple esophageal SMTs originating from MP layer, which can avoid patients suffering repeated resections. BACKGROUND: Several comparison studies have demonstrated that endoscopic sphincterotomy (EST) combined with large-balloon dilation (LBD) may be a better option than EST alone to manage large bile duct stones. However, limited data were available to compare this combination method with LBD alone in removal of large bile duct stones. OBJECTIVE: To compare EST plus LBD and LBD alone for the management of large bile duct stones, and analyze the outcomes of each method. PATIENTS AND METHODS: Sixty-one patients were included in the EST plus LBD group, and 48 patients were included in the LBD alone group retrospectively. The therapeutic success, clinical characteristics, procedure-related parameters and adverse events were compared. RESULTS: Compared with EST plus LBD, LBD alone was more frequently performed in patients with potential bleeding diathesis or anatomical changes (P = 0.021). The procedure time from successful cannulating to complete stone removal was shorter in the LBD alone group significantly (21.5 vs. 17.3 min, P = 0.041). The EST plus LBD group and the LBD alone group had similar outcomes in terms of overall complete stone removal (90.2% vs. 91.7%, P = 1.000) and complete stone removal without the need for mechanical lithotripsy (78.7% vs. 83.3%, P = 0.542). Massive bleeding occurred in one patient of the EST plus LBD group, and successfully coagulated. Postoperative pancreatitis did not differ significantly between the EST plus LBD group and the LBD alone group (4.9% vs. 6.3%; P = 1.000). CONCLUSION: Endoscopic sphincterotomy combined with LBD offers no significant advantage over LBD alone for the removal of large bile duct stones. TiO2 thin films of varying thicknesses (~50-430 nm) were fabricated by depositing solgel (prepared using titanium tetraisopropoxide and isopropanol) on soda-lime silica glass substrates by spin coating, followed by annealing at 450°C for 2 h. Mineralogical analyses using glancing angle X-ray diffraction showed that the films were comprised of anatase, the peak intensities of which increased with increasing thickness. Ultra-high resolution field emission scanning electron microscopy revealed that the grain size was approximately constant at ~5 nm but that the agglomerate size increased from ~20 nm to ~30 nm with increasing film thickness. Optical analyses by UV-VIS spectrophotometry showed that the absorption edge exhibited a red shift with increasing film thickness while the optical indirect band gap decreased. Photocatalytic testing using methylene blue decomposition showed increasing photoactivity with increasing film thickness. Since the particle size was constant, the films were porous (thus allowing limited dye solution and UV radiation penetration), the agglomerate size increased, and the impurity concentrations were assumed to decrease at the film surface, these data allow the conclusion that the optical and photocatalytic properties of the films were altered principally through an increase in the charge carrier diffusion distance that was determined by the agglomerate size. Summary: Objective: To study the effect of intra-articular injection of meloxicam (Mobic) on the development of osteoarthritis (OA) in rats and examine concomitant changes in nociceptive behavior and the expression of mitogen-activated protein kinases (MAPKs) in articular cartilage chondrocytes. Methods: OA was induced in Wistar rats by right anterior cruciate ligament transection (ACLT); the left knee was not treated. The OA + meloxicam (1.0 mg) group was injected intra-articularly in the ACLT knee with 1.0 mg of meloxicam once a week for 5 consecutive weeks starting 5 weeks after ACLT. The OA + meloxicam (0.25 mg) group was treated similarly with 0.25 mg meloxicam. The sham group underwent arthrotomy only and received vehicle of 0.1 mL sterile 0.9% saline injections, whereas the naive rats in meloxicam-only groups were treated similarly with 1.0- and 0.25-mg meloxicam. Nociception was measured as secondary mechanical allodynia and hind paw weight-bearing distribution at before (pre-) and 5, 10, 15, and 20 weeks post-ACLT. Histopathology of the cartilage and synovia was examined 20 weeks after ACLT. Immunohistochemical analysis was performed to examine the effect of meloxicam on MAPKs (p38, c-Jun N-terminal kinase (JNK), and extracellular signal-regulated kinase (ERK)) expression in the articular cartilage chondrocytes. Results: OA rats receiving intra-articular meloxicam treatment showed significantly less cartilage degeneration and synovitis than saline-treated controls. Nociception were improved in the OA + meloxicam groups compared with the OA group. Moreover, meloxicam attenuated p38 and JNK but enhanced ERK expression in OA-affected cartilage. Conclusions: Intra-articular injection of meloxicam (1) attenuates the development of OA, (2) concomitantly reduces nociception, and (3) modulates chondrocyte metabolism, possibly through inhibition of cellular p38 and JNK, but enhances ERK expression. In this paper, a new approach of multi-modality image registration is represented with not only image intensity, but also features describing image structure. There are two novelties in the proposed method. First, instead of standard mutual information based on joint intensity histogram, a graph-based implementation of multi-dimensional regional mutual information is employed, which allows neighbourhood information to be taken into account. Second, a new feature image is obtained by means of phase congruency, which is invariant to brightness or contrast changes. By incorporating these features and intensity into regional mutual information, we can combine aspects of both structural and neighbourhood information together, which offers a more robust and a high level of registration accuracy that is essential in application to the medical domain. Background and study aims: Recurrence/persistence of symptoms occurs in approximately 20% of patients after Heller myotomy for achalasia. Controversy exists regarding the therapy for patients in whom Heller myotomy has failed. The aim of the current study was to evaluate the efficacy and feasibility of peroral endoscopic myotomy (POEM), a new endoscopic myotomy technique, for patients with failed Heller myotomy. Patients and methods: A total of 12 patients with recurrence/persistence of symptoms after Heller myotomy, as diagnosed by established methods and an Eckardt score of ⩾4, were prospectively included. The primary outcome was symptom relief during follow-up, defined as an Eckardt score of ⩽3. Secondary outcomes were procedure-related adverse events, lower esophageal sphincter (LES) pressure on manometry, reflux symptoms, and medication use before and after POEM. Results: All 12 patients underwent successful POEM after a mean of 11.9 years (range 2-38 years) from the time of the primary Heller myotomy. No serious complications related to POEM were encountered. During amean follow-up period of 10.4 months (range 5-14 months), treatment success was achieved in 11/12 patients (91.7 %; mean score pre- vs. post-treatment 9.2 vs. 1.3; P<0.001). Mean LES pressure was 29.4 mmHg pre-treatment and 13.5mmHg post-treatment (P<0.001). One patient developed mild reflux symptoms and required intermittent medication with proton pump inhibitors. Conclusions: POEM seems to be a promising new treatment for failed Heller myotomy resulting in short-term symptom relief in >90% of cases. Cationic surfactants were employed to modify granular activated carbon (GAC) for the purpose of improving GAC's adsorption of arsenate from water. Rapid small-scale column tests (RSSCTs) were set up to test the adsorption capacities. Results indicated that surfactant cetyltrimethylammonium chloride (CTAC) modification manifested the highest improvement in GAC's adsorption for arsenate. GAC modified by of CTAC was able to operate approximately 64,000 bed volumes before of arsenate breakthrough as compared with 2,100 bed volumes for virgin GAC. GAC also performed much better than zeolite and bentonite both in surfactant retention and arsenic removal. Arsenate adsorption was affected by influent concentration, pH, and presence of other anions. The effect of competitive anions on arsenate adsorption was in the order of . Breakthrough tests on tap water spiked with arsenate indicated that the modified GAC had high selectivity for arsenate and considerable amount of arsenate was removed though the influent contained interfering anions and total organic carbon (TOC) in concentrations of magnitude higher than that of arsenate.
