Abstract

RESUMO O reconhecimento da autodeterminação tem vindo a ser evidente também no campo da Dificuldade Intelectual e Desenvolvimental (DID), visto que a pessoa deve assumir-se como agente ativo e decisor na própria vida. A aprendizagem das competências autodeterminadas é fundamental dada a sua transversalidade contextual, não se devendo restringir apenas à adolescência. Em Portugal, as evidências são escassas e baseadas no anterior modelo. Assim sendo, o objetivo deste estudo foi identificar o perfil de autodeterminação, à luz da teoria do agente causal, de adultos com DID, no intuito de analisar as variáveis que o influenciam. A versão portuguesa do Inventário de Autodeterminação foi aplicada a 44 participantes entre os 21-73 anos (40.77±12.07), 20 do género feminino e 24 do masculino. Metade da amostra eram participantes com DID, todos institucionalizados. A consistência interna foi aceitável (α=.65). A análise comparativa pelo diagnóstico, género, idade, habilitações académicas, situação profissional e local de residência apontou a tendência para perfis semelhantes (p>.05). As diferenças encontradas estão associadas a perfis mais autodeterminados de adultos com DID, inferindo-se o maior peso das características envolvimentais do que individuais (diagnóstico). As pessoas com DID reconhecem a autodeterminação como relevante e apresentam as habilidades para a decisão. Recomendações para a investigação e para a prática serão avançadas.

Abstract

Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

3. Subversion as a Resistance Strategy in Artistic Activism

Abstract

The contemporary art world is characterized by precarious and intermittent forms of employment such as subcontracting and freelancing. Non-paid work is also common in the arts sector either in the form of internships or uncompensated exhibiting, writing and curating. In this article we analyse subversion in the context of artistic activism as a resistance strategy against labour relations within contemporary capitalism. As a case study, we present a critical analysis of two works by artist Joshua Schwebel, Subsidy (2015) and Médiation Culturelle (2017-2018), which aimed to take advantage of the institutional exhibition context to not only raise awareness about hidden aspects of labour practices common among art institutions but also to create real change and empower workers. Taking into account capitalism’s adaptability and its undermining of traditional forms of resistance, we argue that artistic activism plays a crucial role in pursuing counter-hegemonic struggle. By analysing and comparing both projects we have concluded that subversion is used as the broader strategy, whereas disruption, dialogue and displacement are used as tactics. This research also establishes context as a fundamental element for artistic activism works and the necessity to adapt strategies to maximize possibilities of achieving social transformation.

Abstract

Background The impact of the COVID-19 pandemic on paediatric populations varied between high-income countries (HICs) versus low-income to middle-income countries (LMICs). We sought to investigate differences in paediatric clinical outcomes and identify factors contributing to disparity between countries.Methods The International Severe Acute Respiratory and Emerging Infections Consortium (ISARIC) COVID-19 database was queried to include children under 19 years of age admitted to hospital from January 2020 to April 2021 with suspected or confirmed COVID-19 diagnosis. Univariate and multivariable analysis of contributing factors for mortality were assessed by country group (HICs vs LMICs) as defined by the World Bank criteria.Results A total of 12 860 children (3819 from 21 HICs and 9041 from 15 LMICs) participated in this study. Of these, 8961 were laboratory-confirmed and 3899 suspected COVID-19 cases. About 52% of LMICs children were black, and more than 40% were infants and adolescent. Overall in-hospital mortality rate (95% CI) was 3.3% [=(3.0% to 3.6%), higher in LMICs than HICs (4.0% (3.6% to 4.4%) and 1.7% (1.3% to 2.1%), respectively). There were significant differences between country income groups in intervention profile, with higher use of antibiotics, antivirals, corticosteroids, prone positioning, high flow nasal cannula, non-invasive and invasive mechanical ventilation in HICs. Out of the 439 mechanically ventilated children, mortality occurred in 106 (24.1%) subjects, which was higher in LMICs than HICs (89 (43.6%) vs 17 (7.2%) respectively). Pre-existing infectious comorbidities (tuberculosis and HIV) and some complications (bacterial pneumonia, acute respiratory distress syndrome and myocarditis) were significantly higher in LMICs compared with HICs. On multivariable analysis, LMIC as country income group was associated with increased risk of mortality (adjusted HR 4.73 (3.16 to 7.10)).Conclusion Mortality and morbidities were higher in LMICs than HICs, and it may be attributable to differences in patient demographics, complications and access to supportive and treatment modalities.

Abstract

Uncertainty is ubiquitous and happens in every single prediction of Machine Learning models. The ability to estimate and quantify the uncertainty of individual predictions is arguably relevant, all the more in safety-critical applications. Real-world recognition poses multiple challenges since a model’s knowledge about physical phenomenon is not complete, and observations are incomplete by definition. However, Machine Learning algorithms often assume that train and test data distributions are the same and that all testing classes are present during training. A more realistic scenario is the Open Set Recognition, where unknown classes can be submitted to an algorithm during testing. In this paper, we propose a Knowledge Uncertainty Estimation (KUE) method to quantify knowledge uncertainty and reject out-of-distribution inputs. Additionally, we quantify and distinguish aleatoric and epistemic uncertainty with the classical information-theoretical measures of entropy by means of ensemble techniques. We performed experiments on four datasets with different data modalities and compared our results with distance-based classifiers, SVM-based approaches and ensemble techniques using entropy measures. Overall, the effectiveness of KUE in distinguishing in- and out-distribution inputs obtained better results in most cases and was at least comparable in others. Furthermore, a classification with rejection option based on a proposed combination strategy between different measures of uncertainty is an application of uncertainty with proven results.

Objective: Psychometric properties of Raven's Standard Progressive Matrices in a Portuguese community sample were investigated. Method: The sample consists of 522 people (250 men and 272 women), aged between 12 and 95 years. All participants completed an informed consent form and a battery of neuropsychological tests, including Raven's Standard Progressive Matrices (RSPM), Rey 15-Item Memory Test, Zung Self-Rating Anxiety Scale, and Rey Complex Figure Test. Results: The average in RSPM was 41.18 (SD = 12.03). The results showed that all sociodemographic variables (age, sex, education, profession, regions, and place of residence) significantly influenced RSPM scores. The reliability and temporal stability of RSPM were adequate. Conclusions: This study suggests that RSPM is an instrument with potential for use among the Portuguese population.

Context: Zinc-oxide eugenol (ZOE) is frequently used due to its satisfactory biological response, sedative effect on the pulp, and easy removal. However, literature is very controversial about the influence of the temporary cement-containing eugenol on the bond strength properties. Aims: This study aims to clarify the literature controversy by evaluating the bond strength of ZOE or ZOE-free applied before bonding procedures and the 7-day resting period after the first session. Settings and Designs: Twelve recently extracted third molars were randomly divided into three groups: Group 1: Control (without treatment), Group 2: Temp-Bond NE; Group 3: Temp-Bond. Subjects and Methods: After temporary restorations, the teeth were immersed in distilled water and stored for 7 days at 37°C. The temporary cement was mechanically removed, and prophylaxis was performed. The adhesive procedures were performed, and a 6-mm-high composite resin block was built. The teeth were immediately stored at 100% relative humidity at 37°C for 24 hours. Specimens with 0.7 (±0.2) mm2 of cross-sectional area were obtained and subjected to a microtensile bond strength (μTBS) test at 1 mm/min until failure. Statistical Analysis Used: Analysis of variance (ANOVA) and Tukey's post hoc test were used for statistical analysis. Results: ZOE or ZOE-free cement did not interfere in the μTBS between resin composite and dentine when used with a two-step-etch-rinse adhesive material, considering 7 days of resting period after the first session, by the ANOVA and Tukey's post hoc test (P < 0.05). Conclusions: The authors recommend the removal of the temporary cementation at least seven days after the first session and suggest that the clinicians follow-up further studies to use the cement in case its removal is recommended within a longer period.

3-Hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (statins) are generally safe and well-tolerated drugs that are extensively used for the primary and secondary prevention of atherosclerotic cardiovascular events. Muscle and liver adverse reactions are the best recognized, while cutaneous side effects are exceedingly rare. We present the case of a 65-year-old woman with severe hypercholesterolemia, who developed generalized erythematous cutaneous lesions with pruritus, resembling lichen planus, months after starting treatment with simvastatin. The symptoms disappeared on withdrawal of simvastatin and reappeared within 3 months upon rechallenge with rosuvastatin. In addition to describing a rare adverse effect of statins, the authors also discuss the nutraceutical approach to the management of a statin-intolerant patient.

A number of new porphyrin-based photosensitizers have been developed for Photodynamic Therapy (PDT) in recent years. Chlorins, which are a reduced form of porphyrins, show better potential of application since they have a stronger absorption band on the red region of the visible spectrum and, hence, a deeper penetration into tissues. We found that by using heterogeneous catalytic transfer reduction (CTR), meso-tetraphenylporphyrin (TPP) could be hydrogenated, although in modest yields, to meso-tetraphenylchlorin (TPC) in a single reaction step. Best reaction conditions were attained using formic acid or sodium phosphinate/water as hydrogen donors, tetrahydrofuran (THF) or toluene as solvent and 10% palladium on charcoal as catalyst.

Este estudo pretende abordar a importância da variável intensidade da distribuição como estratégia de marketing, bem como as interações com outras variáveis relevantes para as empresas. Tal análise revela-se extremamente importante, já que, para produtos para os quais não faz sentido adotar estratégias de diferenciação, as empresas devem compreender bem as implicações e determinantes da intensidade da distribuição. Procura-se avaliar a importância da competitividade da força de vendas, enquanto força interna. O estudo demonstra e justifica a relação positiva que existe entre a competitividade da força de vendas e as variáveis intensidade da distribuição e performance da força de vendas. Como o mercado em estudo é extremamente competitivo, evidencia-se a relevância da intensidade da competição. Verifica-se que um aumento dessa variável tem um impacto positivo na intensidade da distribuição e na performance da força de vendas. Na maioria dos casos confirma-se que a quota de mercado, enquanto medida de performance do ano anterior, influencia positivamente a nossa variável estratégica. Por sua vez, o aumento da intensidade da distribuição melhora a performance da força de vendas e, conseqüentemente, o desempenho da empresa.

Abstract

Nausea and vomiting are frequent complications of anesthesia in the postoperative period. Acupuncture at the pericardium point 6 (PC6) is known to be effective in preventing postoperative nausea and vomiting (PONV).The objective of the present study is to investigate the effects of acupuncture performed at the PC6 point in the prevention of PONV in women undergoing elective open hysterectomy under general inhalational anesthesia and to assess its association with plasma serotonin levels.97 patients undergoing elective open hysterectomy were randomly divided into two groups: acupuncture group (bilateral acupuncture at PC6, n = 49), and a control group (no acupuncture, n = 48). All patients prophylactically received ondansetron and dexamethasone and, as rescue medication, metoclopramide in case of occurrence of PONV. The primary outcome evaluated was occurrence of nausea and vomiting within 24 hours after surgery. Serotonin plasma levels were measured before and after acupuncture prior to anesthesia induction. For the control group, the repeat measurement was performed 30 minutes after admission to the preoperative unit.Acupuncture at PC6 significantly reduced the incidence of nausea (29.2% vs. 6.1%;This study shows that acupuncture at PC6 resulted in a lower incidence of postoperative nausea in patients undergoing hysterectomy.

Abstract

Pleomorphic adenoma, considered the most frequent benign mixed neoplasm of the minor salivary glands, occurs mainly in the region of the hard palate, with slight predilection in females and peak of incidence between the third and fifth decades of life. An increase in recurrence rates has been associated with the histopathological variants of the tumor, cellular characteristics, stroma, and capsule rupture during surgical removal of the lesion. The present case report aims to describe the surgical approach performed on the patient, a 45-year-old woman with a recurrent Pleomorphic Adenoma (PA) in the region of the hard palate on the right side, 10 years after initial enucleation of the lesion; her main complaint was an increase in volume in the palatal region. After extensive local surgical excision of the tumor and 2 years of follow-up, there were no signs of recurrence. Computed tomography and a correct histopathological diagnosis are essential to enable the establishment of an appropriate surgical treatment, with the purpose of achieving complete removal of the lesion, with wide surgical margins, including the lining mucosa and the underlying periosteum, as described in the present case.

Abstract

Background: Cervical cancer is the fourth most common cancer in the world. A raised incidence and mortality parallel a low participation rate in screening, namely screening in poor countries and among specific populations of developed countries. Cervical or vaginal self-sampling may increase adherence to screening due to its low costs and elevated sensibility and specificity. Our main goal was to compare self-sampling with physician sampling and to evaluate the participation rate of women formerly non-adherent to cervical cancer screening. Study Design: We identified relevant studies from PubMed. Studies were eligible for inclusion if they fulfilled the following criteria: women aged between 16 and 80 years, poor and developed countries that use self-sampling, women with a low participation rate in screening and studies published since 2013. After fully reading the articles, data were extracted to an Excel sheet to ­display all relevant information in an organized manner. ­Results: We identified 18 studies, which altogether enrolled 22,118 women. Self-sampling and physician sampling are quite similar regarding the HPV detection rate. Women have a low participation rate mainly because of limited access to health services, religious and culture beliefs, and lack of time due to several demanding tasks in daily life. Self-sampling shows a significant increase in acceptability and preference compared to physician sampling. Conclusion: Self-sampling is a reliable method to involve women in opportunistic or organized screening programs for cervical cancer prevention.

16. Results from PSIPROSPER: A multicenter retrospective study to analyze the impact of treatment with paliperidone palmitate 1-month on clinical outcomes and hospital resource utilization in adult patients with schizophrenia in Portugal

BackgroundSchizophrenia is a chronic psychiatric disorder with a significant impact worldwide. The early onset and its relapsing nature pose a significant challenge to patients and caregivers. The PSIPROSPER study aimed to characterize the real-world context of schizophrenia treatment in Portugal and to measure the impact of including paliperidone palmitate 1-month formulation (PP1M) in the clinical outcomes (relapses and hospitalizations) and healthcare resource utilization, in a context in which payment scheme in Portugal allows for patients to receive free antipsychotics if prescribed at public hospitals.MethodsThis was a multicenter, retrospective, observational study. Male and female adults with a diagnosis of schizophrenia who initiated treatment with PP1M after a minimum of 12 months on an Oral Antipsychotic (OAP), and with complete medical charts, were consecutively included. A mirror-image design over 24 months allowed the comparison of outcomes before and after the PP1M introduction.ResultsOut of the 51 patients included, 80.4% were male, with a mean age of 34 (±9.8) years. Around 92% of patients were being treated with PP1M at inclusion. Lack of adherence to previous OAP was the main driver for PP1M initiation. Only 9.8% of patients were hospitalized during the PP1M period vs. 64.7% during the OAP period (p < 0.0001). The mean number of hospitalizations (0.1) was significantly lower during the PP1M period (p < 0.0001). Type of treatment was the only variable found to be significant in predicting a lower hospitalization rate and a lower risk of hospitalization. Relapses were significantly lower (p < 0.0001) in PP1M (21.6%) vs. OAP (83.7%). Similarly, the mean change in the number of relapses (p < 0.0001) showed significantly better outcomes in PP1M.ConclusionThis study supports PP1M as part of schizophrenia treatment in Portugal. Given the lower number of relapses and hospitalizations observed in schizophrenia patients treated with PP1M when compared to OAP-treated patients, this real-world study seems to provide further evidence to support the use of PP1M to treat this condition, in line with previous research. In the context of scarce public resources, these benefits should be carefully considered by healthcare decision-makers to ensure optimal value-based treatment strategies.

Abstract

Este artigo tem como objetivo relatar a experiência de acadêmicos e as contribuições da atuação em equipe multiprofissional durante projeto de extensão com adolescentes. O público alvo foi composto por jovens de 14 a 18 anos de uma escola estadual em Belo Horizonte. O objetivo foi promover educação em saúde para adolescentes com equipe multidisciplinar, composta por acadêmicos da área de Enfermagem, Fisioterapia, Medicina e Psicologia. Diversos temas foram abordados, todos com grande relevância na vida dos jovens, como por exemplo: “Gravidez na adolescência” e “Direitos e deveres dos adolescentes”. Todos os temas foram abordados através de “Rodas de Conversa”, buscando sempre estimular a conversa e a participação por parte dos jovens. Esse projeto foi extremamente enriquecedor do ponto de vista profissional e pessoal para cada uma das pessoas envolvidas nele e proporcionou aos acadêmicos extensionistas maior capacidade para atuar em questões da sociedade.
