Background: The increasing rates of mental health challenges among young people highlight an urgent need for accessible and effective treatment. However, current mental health systems face unprecedented demand, leaving most young people globally with unmet mental health needs. Smartphones present a promising solution to this issue by offering in-the-moment support through innovative just-in-time adaptive interventions, which provide support based on real-time data. Objective: This study explores young people's experiences with Mello, a just-in-time adaptive intervention that focuses on the transdiagnostic mechanism of repetitive negative thinking (RNT), a significant factor contributing to youth depression and anxiety. Methods: Semistructured qualitative interviews were conducted with 15 participants aged 16 to 25 years, all of whom had previously participated in a pilot randomized controlled trial of Mello. Of the 15 participants, 9 (60%) identified as women, 4 (27%) as men (including 1 transgender man), and 2 (13%) as nonbinary. Interviews focused on participants' experiences with the Mello app, factors influencing engagement, perceived benefits and limitations, and suggestions for future improvements. Thematic analysis was used to analyze the data. Results: The analysis identified three superordinate themes: Mello as a tool for intentional reflection; doing therapy your own way; and barriers to engagement during low mood, anxiety, and RNT. Theme 1 explored young people's experiences of how the app facilitated active management of negative thoughts and supported the development of reflective habits, contrasting with their typical strategies of avoidance or resistance. Theme 2 highlighted the value of the app's self-guided nature, with a particular focus on its flexibility and accessibility, particularly when compared to traditional face-to-face therapy. Finally, theme 3 addressed barriers to engagement, particularly during emotionally difficult times, with participants reporting feeling "stuck" in their negative thoughts. To mitigate these challenges, participants suggested incorporating gamification elements, such as progress-tracking visuals, to enhance motivation and increase engagement with the app. Conclusions: Our findings underscored the value of Mello in promoting intentional engagement and reflection with RNT, consistent with prior research that emphasizes the effectiveness of tailored interventions. Although some users valued the self-guided nature of the application, others encountered difficulties with motivation. Future research should explore strategies to enhance engagement for young people with low mood and motivation, such as co-design methodologies, advanced personalization features, and gamification techniques. Five- to 10-year outcomes of a modular head-neck adapter system with ceramic heads in revision hip arthroplasty on large populations are still lacking. A registry study about modular adapter system with Delta ceramic head in revisions was designed, aiming to assess (1) the survival rates of the device, (2) the reasons for re-revisions of the device, and (3) a comparison to factory assembled titanium sleeve and Delta head cohort in revision hips. Using a regional arthroplasty registry, we investigated the modular adapter system with a ceramic head in revision hips. Demographics, implant features, and reasons for revision were recorded. Survival rates and reasons for re-revision were assessed. The modular adapter system with the ceramic ball was compared to a factory-assembled ceramic titanium-sleeved head in revisions, acting as a control group. There were 354 revisions included at a mean follow-up of 5 years (range, 0 to 13). The 5- and 7-year survival rates were 87.9% and 86.9%, respectively. Dislocations (2.8%) and cup aseptic loosening (4.2%) were the 2 most frequent reasons for re-revision. No breakage of the adapter system or the ceramic head occurred. A femoral neck failed (0.3%). No implant features, offset (P =.088) or skirted (P =.870) tapers, impacted on failures. No differences between the 2 cohorts were found regarding survival rates (P =.696) and reasons for re-revision (dislocations, P =.983; cup aseptic loosening, P =.296). The modular head neck adapter system with a ceramic head seems to be a valid option in revisions at 5 and 7 years, without additional risk of implant breakage in this registry. Background: Repetitive negative thinking (RNT) is a key transdiagnostic mechanism underpinning depression and anxiety. Using "just-in-time adaptive interventions" via smartphones may disrupt RNT in real time, providing targeted and personalized intervention. Objective: This pilot randomized controlled trial evaluates the feasibility, acceptability, and preliminary clinical outcomes and mechanisms of Mello—a fully automated, personalized, transdiagnostic, and mechanistic smartphone intervention targeting RNT in young people with depression and anxiety. Methods: Participants with heightened depression, anxiety, and RNT were recruited via social media and randomized to receive Mello or a nonactive control over a 6-week intervention period. Assessments were completed via Zoom sessions at baseline and at 3 and 6 weeks after baseline. Results: The findings supported feasibility and acceptability, with high rates of recruitment (N=55), uptake (55/64, 86% of eligible participants), and retention (52/55, 95% at 6 weeks). Engagement was high, with 90% (26/29) and 59% (17/29) of the participants in the Mello condition still using the app during the third and sixth weeks, respectively. Greater reductions in depression (Cohen d =0.50), anxiety (Cohen d =0.61), and RNT (Cohen d =0.87) were observed for Mello users versus controls. Mediation analyses suggested that changes in depression and anxiety were accounted for by changes in RNT. Conclusions: The results indicate that mechanistic, targeted, and real-time technology-based solutions may provide scalable and effective interventions that advance the treatment of youth mental ill health. Background: The influence of the reasons for revision on the outcomes of revision hip arthroplasties is controversial and poorly described. A registry study was designed to compare the revision hips performed for periprosthetic hip infection (PHI) to the revision hips performed for aseptic loosening. The aims of this study were the long-term assessment and comparison of survival rates, reasons for rerevisions, and mortality rates between these two cohorts.Methods: By using an arthroplasty registry, revision hips were stratified according to the reasons for revision (aseptic loosening and PHI). Preoperative and intraoperative, demographic and implant-related features were recorded. Survival rates, reasons for revision and mortality rates were assessed and compared.Results: The two cohorts were homogenous for preoperative and intraoperative parameters, apart from age at revision, time elapsed between THA and revision, bearing surfaces, and head size. At 10 years, the survival rates were similar between the two groups (P = .51). Half of the rerevisions occurred in the first two years in both the groups. The reasons for rerevision were similar: when rerevision for infection was adopted as an end point, the two cohorts showed nonsignificant differences. The mortality rate was statistically higher in the PHI cohort (P .0015, hazard ratio adjusted for age and gender: 1.42).Conclusion: Aseptic and septic revisions achieved similar outcomes at long-terms, rerevisions tended to fail for the same reason for revision. The mortality rate was significantly higher in the septic cohort. These findings are useful to modify and tune the preoperative, intraoperative, and postoperative managements of revisions. Background: Fourth-generation ceramic-on-ceramic couplings were developed to improve ceramic toughness and strength. Encouraging outcomes in primary total hip arthroplasty were reported. This coupling was proposed as an attractive option even in the case of revision procedures, particularly in younger patients with a longer life expectancy. However, the pertinent literature is scarce. Thus, a registry study was conducted to investigate the midterm survival rates and reasons for re-revision of the fourth-generation ceramic-on-ceramic bearing surfaces in revision hip arthroplasties.Methods: A total of 327 revision implants (all the components were exchanged) with BIOLOX Delta couplings were investigated using the regional orthopedic registry RIPO. The demographics, the survival rates, and the reasons for re-revision were assessed. Revisions with bearings other than Delta were compared.Results: Delta bearings achieved a survival rate of 90.5% at 7 years, with stable results. Twenty-six (8%) re-revisions occurred at a mean follow-up of 4.1 years. And 2.8% of the re-revisions were due to recurrent dislocations; 1.5% of the cases were due to cup aseptic loosening. Septic loosening occurred in 1.6% of the cases. No ceramic fractures were reported. When compared to revisions with bearings other than Delta-on-Delta, Delta coupling cohort achieved similar performances at 5 and 7 years. Lower, nonsignificant rates of aseptic and septic loosening were reported in Delta cohort.Conclusion: Delta-on-Delta couplings demonstrated to be reliable bearing surfaces in revision settings at a midterm follow-up. Clinical studies and longer follow-ups are required to investigate potential adverse effects, like squeaking and fractures, and confirm these preliminary findings.Level Of Evidence: Level III, therapeutic study.
