Abstract Qualitative observer-based and quantitative radiomics-based analyses of T1w contrast-enhanced magnetic resonance imaging (T1w-CE MRI) have both been shown to predict the outcomes of brain metastasis (BM) stereotactic radiosurgery (SRS). Comparison of these methods and interpretation of radiomics-based machine learning (ML) models remains limited. To address this need, we collected a dataset of n = 123 BMs from 99 patients including 12 clinical features, 107 pre-treatment T1w-CE MRI radiomic features, and BM post-SRS progression scores. A previously published outcome model using SRS dose prescription and five-way BM qualitative appearance scoring was evaluated. We found high qualitative scoring interobserver variability across five observers that negatively impacted the model’s risk stratification. Radiomics-based ML models trained to replicate the qualitative scoring did so with high accuracy (bootstrap-corrected AUC = 0.84–0.94), but risk stratification using these replicated qualitative scores remained poor. Radiomics-based ML models trained to directly predict post-SRS progression offered enhanced risk stratification (Kaplan–Meier rank-sum p = 0.0003) compared to using qualitative appearance. The qualitative appearance scoring enabled interpretation of the progression radiomics-based ML model, with necrotic BMs and a subset of heterogeneous BMs predicted as being at high-risk of post-SRS progression, in agreement with current radiobiological understanding. Our study’s results show that while radiomics-based SRS outcome models out-perform qualitative appearance analysis, qualitative appearance still provides critical insight into ML model operation.

Hemoglobinopathies are the commonest monogenic disorder worldwide, with approximately seven percent of the world population being carriers of hemoglobinopathies. The healthcare utilization impact of thalassemia has resulted in global public health initiatives to screen for hemoglobinopathies, especially sickle cell disease (SCD). The Iowa Newborn Screening Program (INSP) has been in place for more than 50 years with a primary focus on detecting SCD. Recent changes in migration patterns have led to a global distribution of hemoglobinopathies in the western world, which has translated to an increase in the diagnosis of SCD and the incidental detection of non-sickling hemoglobinopathies within the INSP. This study documents the birth prevalence of hemoglobinopathies diagnosed in newborns through the INSP and highlights the need for newborn screening programs to evolve to meet the healthcare needs of underserved, minority populations.

Student-faculty partnerships are a growing practice in scholarship of teaching & learning (SoTL) projects. They can foster greater student engagement in higher education and help advance teaching & learning experiences. For graduate students, in particular those pursuing academic careers, such partnerships can offer opportunities for development of their professional identities as emerging SoTL scholars. In this article, we expand upon previous theorizations of partnerships to include the unique attributes of graduate student partnerships, such as in terms of longer timeframes, increased complexity, and long-term goals. Drawing on a two-year SoTL study, we present a three-layer framework characterizing key attributes for a successful graduate student-faculty partnership: 1) individual attributes in a partnership, 2) collective attributes for a partnership, and 3) outcomes of a partnership. The framework is grounded in literature and illustrative examples from our experiences as graduate students and faculty members working together in partnership with a SoTL project. This framework offers a structured mechanism to inform, create, and enhance the capacity of student-faculty partnerships in SoTL research.

Abstract Introduction The aim of this study was to investigate the dosimetric differences between surface mould high‐dose‐rate (HDR) brachytherapy and external beam volumetric‐modulated arc therapy (VMAT) for two treatment sites. Methods Previously treated HDR brachytherapy surface mould scalp (n = 4) and lower leg (n = 3) treatments were retrospectively analysed. The VMAT plans were optimised using an additional 3‐mm setup margin on the clinical target volume (CTV) of the previously treated HDR plans. The HDR plans were calculated and normalised using the TG‐43 formalism and recalculated with Acuros BV (AC). Results On average, the mean brain and normal tissue doses were reduced by 44.8% and 27.4% for scalp and lower leg VMAT cases, respectively, when compared to AC calculated HDR plans. For VMAT plans, the average dose to a 1‐mm thick skin structure deep to the target volume was not any lower than that in AC HDR plans. On average, the CTV coverage was 13.8% and 9.6% lower for scalp cases with AC dose calculation than with TG‐43 and 8.3% and 5.3% lower for lower leg cases if 0‐ or 1‐cm backscatter material was applied above the catheters, respectively. Conclusions VMAT is a feasible treatment option in the case of extensive skin malignancies of the scalp and lower leg. Uncertainties related to delivered dose with HDR brachytherapy when using the TG‐43 dose calculation model or possible air gaps between the mould and skin favour the use of VMAT. The potential soft tissue deformation needs to be considered if VMAT is used.

Newborn screening (NBS) is an essential public health service that performs screening to identify those newborns at increased risk for a panel of disorders, most of which are genetic. The goal of screening is to link those newborns at the highest risk to timely intervention and potentially life-saving treatment. The global COVID-19 pandemic led to disruptions within the United States public health system, revealing implications for the continuity of newborn screening laboratories and follow-up operations. The impacts of COVID-19 across different states at various time points meant that NBS programs impacted by the pandemic later could benefit from the immediate experiences of the earlier impacted programs. This article will review the collection, analysis, and dissemination of information during the COVID-19 pandemic facilitated by a national, centralized technical assistance and resource center for NBS programs.

Newborn screening (NBS) follow-up programs in the United States are managed at the state level, leaving limited opportunities for collaboration across programs and coordinated resource sharing. The Newborn Screening Technical assistance and Evaluation Program (NewSTEPs), a program of the Association of Public Health Laboratories (APHL), has established a national community of practice for NBS follow-up by creating a network of follow-up staff and stakeholders through education and engagement opportunities. The activities of NewSTEPs in support of NBS follow-up have strengthened information dissemination, collaboration, data collection and technical assistance-driven mentorship across the national system.

Purpose: Nylon 6/6 interstitial brachytherapy catheters may stretch when exposed to moisture, mechanical tension, and body temperature. The purpose of this study is to evaluate the magnitude of catheter stretch during a course of multi-catheter interstitial breast brachytherapy for nylon 6/6 treatment catheters, and to assess the impact this has on treatment plan dosimetry. Material and methods : Catheters were exposed to water at 37°C for six days and the internal catheter length change (∆L_W) was measured daily. Additionally, the measured internal catheter length change (∆L_P), performed prior to each treatment fraction, for seven interstitial breast brachytherapy patients (total of 76 catheters) were retrospectively analyzed. The maximum length change seen in ∆L_P and ∆L_W were introduced as a source positional error to retrospective treatment plans, and treatment plan dosimetry was analyzed. Results : ∆L_W and ∆L_P were on average +4.0% and +1.6% of the catheter lengths in water or tissue after 48 hours. Weak correlation was seen between the average ∆L_P per catheter and both the catheter length within tissue (ρ = 0.36, p = 0.0007), and the mid-catheter depth in tissue (ρ = 0.42, p < 0.0001). The D90CTV decreased 1.5% (p < 0.05) and 8.2% (p < 0.05) when the ∆L_P and ∆L_W were introduced to the initial plans. Conclusions : Nylon 6/6 catheters stretch during a course of multi-catheter interstitial breast brachytherapy treatment. The observed stretch may affect treatment plan dosimetry, if the catheter internal length is only measured immediately after the insertion. Additional catheter length checks are recommended to verify the actual catheter internal length during the treatment.

Writing involves more than attention to form (e.g., orthography, grammar), since it requires attention to text type, content, and genre. However, most students of English as a second language (L2) tend to prioritize linguistic accuracy in their writing, to the detriment of the content of their texts. Automatic speech recognition (ASR) has the potential to mitigate this, as it reduces the cognitive burden of writing by facilitating the text input process (using a skill most humans possess--speaking), offering assistance in spelling, and allowing a focus on other aspects of the task (e.g., cohesion, content). Automatic speech recognition is not only accessible and free, but it also fulfills Chapelle's (2001) criteria of an effective computer-assisted language learning tool (e.g., authenticity, learner fit). Despite these affordances, there is a dearth of studies examining the possible affordances of ASR for writing. This mixed methods, one-shot study examines L2 writers' perceptions of using ASR to write using the technology acceptance model (TAM). Seventeen (N = 17) undergraduate students at a Canadian university were provided with training on Google Voice Typing (Google Docs) and carried out a series of ASR-based writing tasks over a two-hour period. In order to measure their perceptions of the target criteria, participants filled in a TAM-informed survey consisting of statements about their experience with ASR scored on a 7-point Likert scale. To further explore the participants' perceptions, semi-structured interviews followed. Findings indicate positive perceptions of ASR's usefulness in terms of language learning and its ease of use due to the user-friendly voice commands. This suggests that ASR has pedagogical potential, thus requiring further examination to determine its optimal use for L2 writing.

Today’s business is dependent on information; information about an individual’s financial wealth, education, purchasing preferences and even health conditions. How companies treat the information, or data, they accumulate from individuals is governed by the laws in which they are incorporated and operate. Unfortunately, these laws often conflict especially when an American business is operating in Europe. This conflict led the European Commission to develop data privacy principles known as the Safe Harbor Directive, which if followed allowed US companies to store and use EU customer data. However, a lawsuit challenging the legitimacy of the Safe Harbor’s privacy protections for EU citizens resulted in a 2015 ruling by the European Court of Justice (ECJ) that invalidated the Safe Harbor Directive. The chaos that resulted from this ruling sent businesses on both sides of the Atlantic scrambling for an alternative. Unfortunately, Brexit has made an extremely complex legal and business situation even more complicated. Brexit raises two important questions concerning the EU’s recent invalidation of the Safe Harbor Directive that this paper will address: 1) what impact will the UK’s decision to leave the EU have on the newly enacted Privacy Shield and 2) what data privacy measures will the UK implement when it is no longer part of the EU?

From a design perspective, the intentionality of students to engage in surface or deep learning is often experienced through prescribed activities and learning tasks. Educators understand that meaningful learning can be furthered through the structural and organizational design of the online environment that motivates the student towards task completion. However, learning engagement is unique for each student. It is dependent on both how students learn and their intentions for learning. Based on this challenge, the design of online discussions becomes a pedagogical means in developing students’ intentionality for the adoption of strategies leading to deep learning. Through a Design-Based Research (DBR) approach, iterative design of online learning components for undergraduate field experience courses were studied. For this paper, the focus of the research is on examining factors that influenced deep and surface levels of learning in online discussion forums. The results indicate that design factors (i.e., student engagement, group structures, and organization) influence the nature and degree of deep learning. From the findings, two implications for practice are shared to inform the design and scaffolding of online discussion forums to foster deep approaches to student learning.

Newborn screening is a process-based public health service. Newborn screening staff and families alike are essential to maintaining the timeliness of the screening process. Newborn screening education must be accurate and accessible. Past newborn screening conferences have highlighted gaps in best practice and evidence-based guidance on newborn screening education. Sharing successful strategies across programs mitigates the scarcity of resources by cutting costs and reducing the burden of work. These factors illustrate the need for an education framework to guide newborn screening education efforts. The Newborn Screening Education Best Practices Framework responds to these issues by outlining guidance for newborn screening education approaches. Experts in the fields of newborn screening, genetics, and bioethics as well as previous research on best practice guidelines have contributed to the development of this framework. The framework outlines a process for users to evaluate newborn screening education approaches as best practices. This framework reviews best practices using a two-step approach, looking at guiding questions, implementation of the newborn screening issue, and evaluation. The framework helps the user define the characteristics of the newborn screening issue, intended audience, and practical steps to implementation, and then decide whether or not it can be used as a best practice.

Erythropoietin (Epo) is a cytokine that binds and activates an Epo receptor (EpoR) expressed on the surface of erythroid progenitor cells to promote erythropoiesis. While early studies suggested EpoR transcripts were expressed exclusively in the erythroid compartment, low-level EpoR transcripts were detected in nonhematopoietic tissues and tumor cell lines using sensitive RT-PCR methods. However due to the widespread use of nonspecific anti-EpoR antibodies there are conflicting data on EpoR protein expression. In tumor cell lines and normal human tissues examined with a specific and sensitive monoclonal antibody to human EpoR (A82), little/no EpoR protein was detected and it was not functional. In contrast, EpoR protein was reportedly detectable in a breast tumor cell line (MCF-7) and breast cancer tissues with an anti-EpoR polyclonal antibody (M-20), and functional responses to rHuEpo were reported with MCF-7 cells. In another study, a functional response was reported with the lung tumor cell line (NCI-H838) at physiological levels of rHuEpo. However, the specificity of M-20 is in question and the absence of appropriate negative controls raise questions about possible false-positive effects. Here we show that with A82, no EpoR protein was detectable in normal human and matching cancer tissues from breast, lung, colon, ovary and skin with little/no EpoR in MCF-7 and most other breast and lung tumor cell lines. We show further that M-20 provides false positive staining with tissues and it binds to a non-EpoR protein that migrates at the same size as EpoR with MCF-7 lysates. EpoR protein was detectable with NCI-H838 cells, but no rHuEpo-induced phosphorylation of AKT, STAT3, pS6RP or STAT5 was observed suggesting the EpoR was not functional. Taken together these results raise questions about the hypothesis that most tumors express high levels of functional EpoR protein.

This article examines the views of British political leaders Blair, Hague and Duncan Smith. It argues that a comparison with Australian political debates over the construction of 'Anglo-Celtic' and national identity can be particularly helpful. The Australian experience draws attention to the need to consider diasporic constructions of 'Britishness'; reinforces the need to address the colonial implications of mainstream British identity; and emphasizes the dangers posed by conservative mobilizations of traditional ethnic identities in response to 'modernizing' projects by Labour governments. This paper reviews the state of the art of protocols for measurement of water-leaving radiance in the context of fiducial reference measurements (FRM) of water reflectance for satellite validation. Measurement of water reflectance requires the measurement of water-leaving radiance and downwelling irradiance just above water. For the former there are four generic families of method, based on: (1) underwater radiometry at fixed depths; or (2) underwater radiometry with vertical profiling; or (3) above-water radiometry with skyglint correction; or (4) on-water radiometry with skylight blocked. Each method is described generically in the FRM context with reference to the measurement equation, documented implementations and the intra-method diversity of deployment platform and practice. Ideal measurement conditions are stated, practical recommendations are provided on best practice and guidelines for estimating the measurement uncertainty are provided for each protocol-related component of the measurement uncertainty budget. The state of the art for measurement of water-leaving radiance is summarized, future perspectives are outlined, and the question of which method is best adapted to various circumstances (water type, wavelength) is discussed. This review is based on practice and papers of the aquatic optics community for the validation of water reflectance estimated from satellite data but can be relevant also for other applications such as the development or validation of algorithms for remote-sensing estimation of water constituents including chlorophyll a concentration, inherent optical properties and related products.

This paper reviews the state of the art of protocols for the measurement of downwelling irradiance in the context of Fiducial Reference Measurements (FRM) of water reflectance for satellite validation. The measurement of water reflectance requires the measurement of water-leaving radiance and downwelling irradiance just above water. For the latter, there are four generic families of method, using: (1) an above-water upward-pointing irradiance sensor; (2) an above-water downward-pointing radiance sensor and a reflective plaque; (3) a Sun-pointing radiance sensor (sunphotometer); or (4) an underwater upward-pointing irradiance sensor deployed at different depths. Each method—except for the fourth, which is considered obsolete for the measurement of above-water downwelling irradiance—is described generically in the FRM context with reference to the measurement equation, documented implementations, and the intra-method diversity of deployment platform and practice. Ideal measurement conditions are stated, practical recommendations are provided on best practice, and guidelines for estimating the measurement uncertainty are provided for each protocol-related component of the measurement uncertainty budget. The state of the art for the measurement of downwelling irradiance is summarized, future perspectives are outlined, and key debates such as the use of reflectance plaques with calibrated or uncalibrated radiometers are presented. This review is based on the practice and studies of the aquatic optics community and the validation of water reflectance, but is also relevant to land radiation monitoring and the validation of satellite-derived land surface reflectance.
