Ovine and dromedary Echinococcus granulosus isolates from Tunisia were identified as G1 and G6 strains based on polymorphism of the mitochondrial cytochrome C oxydase CO1. Single strand conformation polymorphism (SSCP) was used in order to examine the genetic variation within and between Tunisian G1 and G6 strains and to estimate the extent of selfing. The dromedary isolates are genetically distinct from sheep isolates (high value of genetic variation between populations: Fst = 0.46). No significant deficiency in heterozygotes was found in sheep isolates, whereas heterozygote deficiency (suggesting selfing) was found in a limited number of camel isolates.

Mesenteric cysts are documented as a rare entity in pediatric population. They are considered as benign intra-abdominal tumors with an unknown etiology. Symptoms are not specific and knowledge of such condition is essential in order to establish a proper management. We report three pediatrics cases of mesenteric cysts managed between 2000 and 2009 in the pediatric surgery Department of Monastir College Hospital. We described the clinical, radiological and operative findings. Two males and a female were managed (age range: 10 days-5years, mean age: 6,3years). Two patients were presented with an intestinal obstruction. A preoperative diagnosis was made basing on imaging. Thus, abdominal ultrasonography was performed in all of our reported cases and showed a cystic mass in all cases. The cystic nature of the mass, its margins and its extension were better described on tomographic images. The mesenteric cyst was completely and successfully removed in all cases. The histopathological report confirmed the diagnosis and showed a multiloculated cyst with columnar mesothelial lining, without any defined muscular layer or cellular atypia and without any evidence of malignancy. The children were evaluated post-operatively with a mean follow-up of 2 years and a half. No recurrence was noted in our patients during the follow-up period. It is known that clinical features are not specific of such anomaly but once the diagnosis is made, the complete surgical removal of the cyst remains the treatment of choice with excellent outcomes.

Résumé Les lymphangiomes kystiques sont des tumeurs malformatives bénignes des vaisseaux lymphatiques, rares mais potentiellement graves, le plus souvent localisées dans la région cervicale. Le diagnostic est généralement facile mais le traitement est parfois difficile en raison du siège de la tumeur et de son extension dans les tissus environnants. Nous rapportons un cas de lymphangiome kystique géant de la paroi thoraco-axillaire droite, diagnostiqué par l’échographie prénatale à 20 semaines de gestation. Il s’agit d’un garçon né à 37 semaines de gestation avec une volumineuse tuméfaction pariétale latéro-thoracique droite. L’échographie postnatale des parties molles et le scanner thoracique ont confirmé le diagnostic. Une exérèse chirurgicale incomplète a été réalisée à l’âge de 21 jours et les suites opératoires ont été simples. L’échographie des parties molles réalisée à distance a montré la persistance de multiples structures kystiques dans le creux axillaire. Le diagnostic prénatal de ces tumeurs est fondamental pour la planification d’une prise en charge multidisciplinaire postnatale précoce. Summary Cystic lymphangiomas are benign malformative tumors of the lymphatic vessels, rare but potentially serious and usually located in the cervical region. The diagnosis is usually easy, but the treatment is sometimes difficult because of their location and extension into the surrounding tissue. We report a case of giant cystic lymphangioma of the right axillary-thoracic wall diagnosed by prenatal ultrasound at 20 weeks gestation. The male infant was born at 37 weeks gestation with a large, swelling, lateral parietal right chest. Ultrasound soft tissue and chest CT scan confirmed the diagnosis of giant cystic lymphangioma of the right axillary-thoracic wall. Surgical excision was performed on the 21st day after birth and the mass was excised incompletely. The postoperative course was uncomplicated. Ultrasound soft tissue follow-up showed the persistence of multiple cystic structures in the axilla. Prenatal diagnosis of these tumors is essential for planning multidisciplinary management in early postnatal care. [ABSTRACT FROM AUTHOR]

Summary: Esophageal stricture is a rare but often severe complication of recessive dystrophic epidermolysis bullosa in children. The purpose of the study was to review this digestive complication with emphasis on diagnostic modalities and therapeutic management. Patients and methods: This was a retrospective study of two pediatric cases of esophageal stenosis that occurred during generalized recessive dystrophic epidermolysis bullosa of the Hallopeau-Siemens type. Results: The 2 patients were aged 8years 8months and 11years 5months, respectively. Dysphagia was of early onset, before the age of 10years in both cases. Esophageal opacification led to the diagnosis of esophageal stenosis located in the upper 1/3 of the esophagus in 1 case and at the junction between the middle and the lower 1/3 of the esophagus in the other case. None of the 2 patients received medical treatment, and pneumatic dilation was the treatment method that was advocated. Esophageal endoscopy showed the stenosis and helped guide the positioning of the balloon catheter. These patients underwent 2 and 3 sessions of dilation, respectively, at intervals of 2months and 1year. Balloon dilation has allowed the patients to have a more comfortable life with decreased dysphagia and a substantial improvement in nutritional status. However, this improvement was transient (1 patient had symptomatic recurrence of stenosis after 3years), which shows that monitoring of the patients and the resumption of dilatation sessions may be necessary. Conclusion: Esophageal strictures in dystrophic epidermolysis bullosa of the Hallopeau-Simens type are severe and difficult to support. Pneumatic dilatation is the treatment of choice for the fragile esophagus. It gives satisfactory results and can be repeated without significant risk. [ABSTRACT FROM AUTHOR]

Buts Evaluer les résultats des dilatations pneumatiques dans le traitement des sténoses caustiques et préciser les facteurs prédictifs d’échec Méthodes Etude prospective de 46 sténoses caustiques chez l’enfant colligés dans notre service entre 1997 et 2007 Résultats L’âgé moyen était de 4,3ans. La soude était ingérée dans 34cas. Une œsophagite grave été retrouvée chez 29 patients,32 ont eu une corticothérapie à forte dose. Toutes les œsophagites, quelque soit le stade, ont évolué vers la sténose. Celle-ci était symptomatique dans 39cas et de découverte systématique dans 7cas. Tous nos patients ont eu des dilatations pneumatiques de première intention avec un total de 395 séances, une moyenne de 8,58 séances par malade. Le reflux gastro-oesophagien (RGO) associé a été retrouvé dans 4cas. Le taux de succès était de 67,4 %. Les résultats moyens et les échecs ont représenté respectivement 26,1% et 6,5 %. Une perforation était survenue dans 7,1% Les facteurs d’échec sont l’étendue de la sténose et le RGO associé. Conclusion La dilatation pneumatique est la méthode thérapeutique de choix à entreprendre de première intention pour le traitement des sténoses caustiques de l’œsophage chez l’enfant en raison de son innocuité et de sa grande efficacité [ABSTRACT FROM AUTHOR]

Objectif discuter certains problèmes diagnostique et thérapeutique des malformations adénomatoides kystiques des poumons (MAKP) et prévenir une exérèse pulmonaire inutile. Le diagnostic différentiel se pose avec d’autres malformations broncho-pulmoaires ainsi que certaines affections aqcuises. Patients et Méthode Etude rétrospective de 49 enfants opérés pour suspicion de MAKP dans notre service depuis 1996. Analyse d’une expérience personnelle de 13 cas d’erreurs diagnostique et thérapeutique. Resultats Le diagnostic pré-opératoire a été établi sur la radiologie. 43 enfants ont eu une lobectomie. Le diagnostic de MAKP a été confirmé par l’histologie dans 38 cas. Ailleurs, l’examen histologique a conclu à: emphsème interstitial (1cas), infarctus pulmonaire anté-natalet post-natal (2cas), hémangiomatose pulmoanire (1cas), lésion hybride (1cas), abcès du poumon, lésions non classées (7cas) et dilatation des bronches (1cas). Conclusion L’attitude diagnostique et thérapeutique des MAKP repose sur une collaboration multidisciplinaire. Le diagnostic évoqué sur la clinique et la radiologie, nécéssite une confirmation histologique. Certains problèmes diagnostiques nécéssite une confrontation avec des anatomopathologistes avertis pour éviter une éxèrse pulmonaire inutile. [ABSTRACT FROM AUTHOR]
