Introduction: The development of fracture gap during intramedullary nailing in tibial fractures is associated with poor fracture fragment contact and increased time to union and complications. This study aimed to evaluate the effect of the fracture level in the development of the fracture gap and the effect of the fracture gap on pain, radiologic and functional outcomes, and complication rate. Material and method: A total of 45 patients who underwent reamed intramedullary nailing due to closed transverse or short oblique tibial shaft fractures were divided into the proximal fracture group and the distal fracture group. The correlations between the visual analog scale (VAS) score, modified radiograph union score for tibias (RUST), and postoperative 1-year lower extremity functional scale scores, residual fracture gap, and time to union were evaluated. Knowing that cure cycle has a significant effect on the mechanical properties of the composite materials, determining the effects of cure cycle parameters – such as heating rate, maximum temperature, and dwell time, has gain importance. This study addresses the optimum cure cycle of an infrared cured carbon fiber pre‐preg material. An infrared oven equipped with 1 kW halogen infrared heating unit and vacuum system was used. Different cure cycles determined by means of Taguchi experiment design approach and a cure cycle was found maximizing the tensile strength of the material. Results showed that infrared curing is a strong alternative to the autoclave despite its shortcomings regarding the product geometry. The optimized cure cycle showed 23% increase in the tensile strength comparing the tested cure cycle which is resulted with the minimum tensile strength. Besides, comparing the specimens cured with the optimum cure cycle, the infrared cured specimens showed slightly higher tensile strength than the autoclave cured counterparts. According to Taguchi optimization, optimum values for maximum tensile strength were found as 10°C/min, 130°C for heating rate and 60 min for plateau time. Objective: To investigate the optimal level for femoral shortening osteotomy in patients with dislocated hips undergoing total hip arthroplasty (THA) and make recommendations to avoid union problems. Methods: We retrospectively reviewed 65 hips of 55 patients who underwent total hip arthroplasty for Crowe type IV developmental dysplasia of the hip. Osteotomy level is defined as the distance between the lesser trochanter and osteotomy line. The role of osteotomy level and osteotomy level-stem end distance on non-union rates, radiological union time, unsupported loading time, and Harris Hip Scores were investigated with Pearson correlation analyses. Results: The mean follow-up period was 75.9 ± 32.0 months. The mean radiological union time was 6.5 ± 2.9 months. There was a positive and linear correlation between osteotomy level and radiological union time (r = 0.385; P = .003). Harris Hip Scores were not correlated with osteotomy level (P = .503). The osteotomy performed at a distance of more than 30 mm had higher radiological union times (P = .002). Conclusion: According to the results of the current study, the optimal osteotomy level should be within the 20-30 mm range from the lesser trochanter. Early revision should be avoided for patients with union problems. To prevent non-union, the cable should be well controlled so as not to enter into the osteotomy line. [ABSTRACT FROM AUTHOR]

In this study, the failure behavior of laminated composite plates subjected to anticlastic bending is investigated. Anticlastic bending is a special case of bi-axial out-of-plane loading that produces predominantly twisting moment. A test fixture is designed to actualize anticlastic loading condition. Carbon-fiber-reinforced epoxy composite is selected as the material of the specimens. Totally nine different layup configurations are chosen and at least four specimens are tested for each configuration. Acoustic emission monitoring (AEM) is utilized to detect the first-ply-failure load and the accumulation of damage in the laminates. In this method, elastic waves resulting from initiation and progression of damage are detected, and then AE signals are processed to identify the failure modes and determine the first-ply-failure load. A finite element model is developed to simulate the anticlastic bending test. A code is developed using ANSYS parametric design language to predict the first-ply-failure loads using various failure criteria like Tsai-Wu, Hashin, and Puck. The experimental and numerical results are then compared. Relative strengths and weaknesses of the failure criteria in estimating failure of laminated composite plates under anticlastic bending are discussed. [ABSTRACT FROM AUTHOR]

To evaluate the relationship between radiological and clinical evaluation instruments used in children with flexible pes planus (FPP) with quality of life and functional scores. The study group consisted of sixteen children with FPP (mean age: 9.8 1.1 years). The patients were examined in detail using static pedobarography with the Win- Track system. A physical examination was also undertaken using the Foot Posture Index-6 (FPI-6). The four angles were measured on foot radiographs. For functional evaluation, the Pediatric Outcomes Data Collection Instrument (PODCI) score was employed. A single experienced rehabilitation specialist performed Beighton scoring. No correlation was found between the FPI-6 clinical rating scale and Beighton scores, radiographic data, and pedobarographic evaluation. The PODCI and Beighton scores, on the other hand, had a negative connection (P=0.000, r=-0.781). The radiological, clinical, and pedobarographic values were not correlated with the PODCI functional score. In the management of patients with FPP, the commonly used FPI-6, radiographic and pedobarographic measurements were not correlated with the PODCI functional score. However, children with extensive joint laxity are more likely to have functional limitations. Therefore, it is important to evaluate children with FPP in terms of common joint laxity before treatment planning. [ABSTRACT FROM AUTHOR]

Fiber-reinforced plastics are known as advanced composite materials thanks to their high strength and lightweight features. Carbon fiber reinforced polymers (CFRPs) are one of the high-performance and high-cost fiber-reinforced polymer (FRPs) materials. They are used in several high-performance engineering applications such as motorsports, marine, aviation, energy and defense industry. The cost of carbon fiber is higher compared to many other materials, more competitive and cost-effective productions will spur the demand for composite parts exponentially. Thus, hybrid laminate composite containing carbon and glass fiber materials were manufactured as an alternative for CFRP materials. Because using glass fiber prepreg instead of carbon fiber prepreg will lead the material to become cheaper. However, machining of the FRP materials is still an important issue. Hip dislocation remains a significant complication following total hip arthroplasty, even though its incidence has decreased. While closed reduction is typically performed for early dislocations, delayed or chronic dislocations often necessitate acetabular or femoral component revision. This document describes the treatment of hip dislocation in a 56-year-old patient through pelvic osteotomy without component revision. An acetabular component malposition was identified, exhibiting an 80-degree inclination and 20-degree cup anteversion. Owing to limited bone stock, a modified Stoppa approach was used for pelvic osteotomy to reduce acetabular inclination. The patient displayed remarkable clinical improvement, achieving a Harris Hip Score of 85 at the two-year check-up with no signs of dislocation. Recurrent hip dislocation is difficult to manage. It frequently necessitates component revision, presenting a challenge due to issues with cup extraction and limited bone stock. Preoperative detection of loosened components is crucial. If it goes undetected, the extraction process can result in bone loss, potentially leading to pelvic insufficiency. Successful revisions of hip arthroplasties can be achieved with geometric modifications to the pelvis. • Despite numerous technological advances, hip dislocation remains an essential sequela of THA. • The most challenging aspect of surgical treatment is removing the acetabular cup or femoral stem. • The patient presented to us due to a fourth hip dislocation that had occurred one month previously • We assessed our patient with insufficient acetabular walls and whether ring and cage reconstruction would be needed. • Pelvic osteotomy serves as a geometrical alteration for restoring normal biomechanics without component loosening. Background: To investigate changes in lower extremity coronal alignment in patients with unilateral Crowe type IV developmental dysplasia of the hip who underwent total hip arthroplasty with transverse femoral shortening osteotomy.Methods: We reviewed the preoperative and 1-year postoperative full-length lower extremity radiographs of 25 patients. Femoral offset (FO), mechanical hip-knee-ankle angle, anatomical axis, mechanical axis deviation (MAD), mechanical lateral proximal femoral angle, anatomical medial proximal femoral angle, mechanical lateral distal femoral angle, anatomical lateral distal femoral angle, knee joint line congruency angle, mechanical medial proximal tibial angle, mechanical lateral distal tibial angle, ankle joint line orientation angle, tibial plafond talus angle, extremity length, and pelvic obliquity were measured on both the operative and nonoperative sides.Results: Postoperatively, there were significant changes in FO (P = .001), hip-knee-ankle angle (P = .004), MAD (P = .016), mechanical lateral proximal femoral angle (P = .001), anatomical medial proximal femoral angle (P = .012), mechanical lateral distal femoral angle (P = .043), and ankle joint line orientation angle (P = .012) on the operative side. Only MAD (P = .035) changed significantly on the nonoperative side.Conclusion: Modification of FO and reconstruction of hip joint anatomy led to neutralization of knee and ankle valgus alignment.
