Objective: Dental treatment for Down's syndrome is often carried out under general anesthesia due to the difficulty of cooperation, excessive dental procedures, and accompanying systemic diseases. The risk of endocarditis can be avoided by maintaining good oral hygiene, preventing tooth decay and administering prophylactic antibiotics before some dental treatments. Material and Method: Down Syndrome patients underwent dental treatment under general anesthesia were evaluated retrospectively. General anesthesia method, demographic variables, ASA scores, Oral Hygiene Index, Frankl scale, Houpt Scale, systemic diseases of the patients, antibiotic prophylaxis and dental treatments were examined as data. Results: Female/male ratio was 62/51, mean age was 14.14±10.28. Most patients were ASA II with 67.3%. The oral hygiene index was poor (%38). While 84.81% of the patients were sedated, 15.2% were given general anesthesia. Tooth extraction with tooth filling was the most performed dental treatment. The most common systemic disease was of cardiac origin. Antibiotic prophylaxis was administered to 82 of the patients. Conclusion: To treat patients with Down Syndrome, frequent use of general anesthesia and sedation, as well as frequent antibiotic prophylaxis, is required. Aim. Infective endocarditis has a poor prognosis and a high mortality rate, meaning that the implementation of prophylactic antibiotic treatment is important. The present study investigates the risk of infective endocarditis and prophylactic antibiotic use in pediatric patients who underwent invasive dental interventions. Materials and methods. The demographic characteristics, the clinical interventions, comorbidities and prophylactic antibiotic use rates of the patients who underwent interventions under general anesthesia sedation were analyzed retrospectively. Results and discussion. Among the 6,718 pediatric patients, 52.4% were males and 47.6% were females, the mean age being of 10.5±4.4 years. Diabetes mellitus and congenital heart disease were the most common comorbidities. Antibiotic prophylaxis was performed in 1,023 (15.2%) patients before the operation. High-risk patients who developed infective endocarditis were identified as 5.0% (n=504). Out of the patients, 1.4 % (n=18) were allergic to penicillin or ampicillin. Conclusions. The specific health problems of the patients should be kept in mind, their involvement and informing before dental interventions in the event of clinical suspicion being crucial. Adequate and effective tissue concentration should be ensured at the time of incision, while an appropriate timing, the mode, dose and duration of antibiotic treatment are also important. Abstract: Study Objectives: To investigate the effects of different doses of oral caffeine-paracetamol combinations in postdural puncture headache (PDPH) prophylaxis. Design: Prospective, randomized, placebo-controlled, blinded study. Setting: University hospital. Patients: A total of 210 ASA physical status I and II patients undergoing lower extremity surgery. Interventions: Patients were randomly divided into 3 groups. One hour before the spinal anesthesia, the first group (n = 70) received placebo, the second group (n = 70) received 500-mg paracetamol + 75-mg caffeine, and the third group (n = 70) received 500-mg paracetamol + 125-mg caffeine orally. The same doses were repeated every 6 hours for 3 days. Patients were then interviewed on days 1, 2, 3, 4, and 7 to inquire about any PDPH. The interviewer was unaware of the PDPH prophylaxis group members. Patients who were discharged early were interviewed by telephone. Measurements and Main Results: Postdural puncture headache occurred in 11 patients (15.7%) in group 1, 10 patients (% 14.28) in group 2, and 10 patients (% 14.28) in group 3. The differences between the groups were insignificant (P > .05). The complications due to spinal anesthesia were similar in the 3 groups. Side effects of caffeine such as lack of sleep, tachycardia, and hypertension were not observed in groups 2 or 3. Conclusions: Prophylactic administration of paracetamol-caffeine combinations at the stated doses does not prevent PDPH. [Copyright &y& Elsevier]

Subcorneal pustular dermatosis (SPD), or Sneddon–Wilkinson disease, is a rare, chronic, recurrent, pustular eruption. Association with several diseases is well known, mainly IgA and IgG gammopathies or myelomas. Although dapsone is often considered to be the first-line treatment, some patients fail to respond or cannot tolerate the side effects. For cases that do not respond well to this treatment, acitretin, an excellent second-line treatment, may be used. Herein, a 55-year-old woman with SPD associated with monoclonal IgA gammopathy refractory to dapsone is presented, who was successfully treated with acitretin in a short period. [ABSTRACT FROM AUTHOR]
