Paroxysmal nocturnal haemoglobinuria (PNH) is a clonal haematopoietic stem cell disease that presents with haemolytic anaemia, thrombosis and bone marrow failure. We report a case of a 51-year-old male with a history of PNH in treatment with Eculizumab admitted to our Hospital for acute chest pain and dyspnoea. The diagnosis was a triple vessel disease and patient was scheduled for coronary artery bypass grafting surgery. To balance the risk between thrombosis and bleeding in this particular clinical setting, we decided to use thromboelastography (TEG) as point of care solution and we used the R parameter as the target of our anticoagulant therapy. The R parameter between 11 and 14 sec can be used as a target value to balance the risk; in addition, there was no evidence of acute hemolysis during the surgery and supplemental dose of Eculizumab was administered in order to minimize any potential exacerbation of intravascular hemolysis.

IntroductionIn immunocompromised patients, SARS-CoV-2 mRNA vaccine has been used in Italy from the beginning of the vaccination campaign, but several studies have shown that the serological response of onco-hematological patients was reduced compared to healthy subjects, due to the state of immunosuppression because of both underlying disease and administered therapy.MethodsWe evaluated the association of anti-SARS-CoV-2 spike IgG titers in 215 hematological patients with clinical and demographic variables to verify if it was possible to identify predictive parameters of serological response, as well as using a control group, consisting of healthy health workers of San Carlo Hospital in Potenza. Anti-SARS-CoV2 IgG titers were evaluated after 30–45 days post second dose vaccine using chemiluminescent microparticle immunoassay technology.ResultsPatients with hematological malignancies, compared with the control arm, had both a mean concentration of anti-SARS-CoV-2 IgG significantly lower and a seroconversion rate numerically lower. All chronic lymphatic leukemia patients showed levels of antibody titer below the mean concentration, also in only clinical surveillance patients. Comparing serological response in hematological malignancies, only acute leukemia patients who were off therapy had the highest seroconversion rate among the patients’ cohorts and a mean antibody concentration greater than the control arm. Patients treated with steroids and rituximab showed a lower level of anti-SARS-CoV-2 spike IgG. Differences in anti-spike IgG levels among chronic myeloid leukemia patients stratified according to tyrosine kinase inhibitor therapy and molecular response were observed, and they could have interesting implications on the evaluation of the effects of these drugs on the immune system, but having not reached statistical significance at the moment. The cohort of patients who received a stem cell transplant was very heterogeneous because it included different hematological malignancies and different types of transplant; however, a mean concentration of anti-SARS-CoV2 IgG greater than the control arm was reported. Indeed, among patients who performed a transplant for over 6 months only one had a spike IgG concentration below the cutoff.ConclusionsOur data confirm reduced serological response in hematological patients after anti-SARS-CoV-2 vaccination. However, we found a great diversity of SARS-CoV-2 antibody response according to types of pathologies and therapies.

Hematopoietic cells express ATP binding cassette (ABC) transporters in relation to different degrees of differentiation. One of the known multidrug resistance mechanisms in acute myeloid leukemia (AML) is the overexpression of efflux pumps belonging to the superfamily of ABC transporters such as ABCB1, ABCG2 and ABCC1. Although several studies were carried out to correlate ABC transporters expression with drug resistance, little is known about their role as markers of diagnosis and progression of the disease. For this purpose we investigated the expression, by real-time PCR, of some ABC genes in bone marrow samples of AML patients at diagnosis and after induction therapy. At diagnosis, ABCG2 was always down-regulated, while an up regulated trend for ABCC1 was observed. After therapy the examined genes showed a different expression trend and approached the values of healthy subjects suggesting that this event could be considered as a marker of AML regression. The expression levels of some ABC transporters such as ABCC6, seems to be related to gender, age and to the presence of FLT3/ITD gene mutation.

This paper deals with the concepts of usage, authority and stance in the field of lexicography, with special reference to the first edition of Webster's Third New International Dictionary of the English Language (W3), the Oxford English Dictionary (OED), and different editions of learner's dictionaries up to the latest ones. The main aim is to compare and assess different dictionary-making policies and their outcomes on the basis of the more or less overt role played by the interpretations and implementations of the above-mentioned concepts of usage, authority and stance and their interconnections with descriptivism and prescriptivism. Scholarly approaches to the concepts of usage, authority, and stance are presented in section 1 to outline the scientific background to their pervasiveness in dictionary-making policies. The definitions of the words usage, authority and stance given in W3 and in the OED, whose outstanding positions in the lexicographical field are undisputed, are then analysed in section 2. The related definitions of descriptivism and prescriptivism are compared and their application and instantiation are exemplified by the treatment of woman question (as a lexical item) in the OED, as representative of its historical perspective, and of ain't in W3, as the symbol of its lexicographic breakthrough (section 3). Section 4 focuses on Learner's Dictionaries and their specific pedagogical role with reference to usage, authority and stance, and on their definitions of ain't in different editions, to see if that headword has undergone socio-cultural changes in time. Historically, the word phraseology has been connected with lexicography (Knappe 2004), especially with bilingual dictionaries (Moon 2000) and their norm-setting role associated with foreign-language teaching and learning. Nineteenth-century specialised bilingual lexicography seems to depart from the then usual norm-based approaches and to favour the inclusion of use-related phraseological combinations. This paper analyses the treatment of word combinations which can be referred to asante-litteramcollocations in two rather different dictionaries, namely Tarver’sRoyal Phraseological English–French, French–English Dictionary(1845–53) andNutt’s English–Italian Conversation Dictionary(1894), to see to what extent nineteenth-century specialised bilingual lexicography has marked a move away from prescriptivism and affected later studies on the lexicographical treatment of collocation and changes in the use of the word phraseology. This article is situated at the crossroads between contrastive linguistics and phraseology. Its objective is to show how some characteristic elements of the co-text of supposedly true friends in English and Italian form different phraseological patterns which account for (un)suspected semantic and pragmatic divergences in their use. Some pairs of true friends, which are supposed to share the same meaning and use in the two languages, have been analysed in two broadly comparable corpora. Findings shed new light on the web of linguistic, cultural and pragmatic relations occurring beneath the lexical level: they ultimately explain why the cognate words analysed often turn out not to be translational equivalents of each other. The same corpus-based approach could prove helpful in analysing similar phenomena in other pairs of languages. [ABSTRACT FROM AUTHOR]
